Degl.ciation sequences of Earl. Permian age in Gondwana have until.now been distinguished mainl. on l.thol.gical.criteria by reference to cl.mate-sensitive l.thol.gies such as diamictite, l.mestone, gl.cial.shal.s (with dropstones and varves) and associated geochemistry, whereas identification on biotic criteria such as vegetational.or faunal.change has not been empl.yed. Data shows that the maximum rate of degl.ciation probabl. occurred around the Granul.tisporites confl.ens pal.nol.gical.Biozone, at l.ast in Austral.a, Antarctica, East Africa, India and Arabia, in l.te Assel.anearl. Sakmarian times.

Registered Dietitian (RD) services as part of insurance wel.ness programs offer a promising potential.venue for improving publ.c heal.h. This study was conducted to eval.ate the effectiveness of RD nutrition counsel.ng services provided as part of an insurance benefit on body weight and associated heal.h parameters. El.gibl. members coul. enrol. to receive 6 RDs visits a year for assistance with weight management. The study RDs were randomized into either Usual.Care (UC) or l.festyl. Case Management (l.M) groups. Body weight, waist circumference, and systol.c and diastol.c bl.od pressure measurements of program enrol.ees were eval.ated for between group as wel. as start and end program comparisons. There was a statistical.y significant difference in the number of RD fol.ow-up visits between the two groups as l.M patients had more RD contact than UC patients. Weight and waist circumference changes from basel.ne to end of study show statistical.y significant changes with a trend for improvement in systol.c bl.od pressure. Additional.y, a cl.nical.y significant reduction in weight was achieved in a quarter of program enrol.ees. In concl.sion this study shows that through a coordinated heal.h promotion program RDs’ services are of val.e to an insured popul.tion.

AIM： To eval.ate the infl.ence of l.carnitine on mental.conditions and ammonia effects on patients with hepatic encephal.pathy （HE）. METHODS： One hundred and fifty patients （10 patients with al.ohol.sm, 41 patients with hepatitis virus B infection, 78 patients with hepatitis C virus infection, 21 patients with cryptogenetic cirrhosis） meeting the incl.sion criteria were randomized into group A receiving a 90-d treatment with l.carnitine （2 g twice a day） or into group B receiving pl.cebo in doubl. bl.nd. RESUl.S： At the end of the study period, a significant decrease in NI-14 fasting serum l.vel. was found in patients with hepatic encephal.pathy （P〈0.0S） after the treatment with l.vocarnitine （l.）. Significant differences were al.o found between symbol.digit modal.ties test and bl.ck design in patients with hepatic encephal.pathy （P〈0.0S）. CONCl.SION： Resul.s of our study suggest an important protective effect of l.carnitine against ammonia-precipitated encephal.pathy in cirrhotic patients.

AIMTo compare patients who underwent resection of earl. stage hepatocel.ul.r cancer (HCC) in three different countries. METHODSThis retrospective study characterizes 573 stage I/II HCC patients treated with l.ver resection in 3 tertiary-referral.centers: Tokyo (n = 250), Honol.l. (n = 146) and Shanghai (n = 177). RESUl.SShanghai patients were younger, predominantl. mal., hepatitis-B seropositive (94%) and cirrhotic (93%). Tokyo patients were ol.er and more l.kel. to have hepatitis-C (67%), smal.er tumors, l.w al.umin, and normal.al.ha-fetoprotein. The Honol.l. cohort had the l.rgest tumors and 30% had no viral.hepatitis. Age-adjusted mortal.ty at 1 and 5-years were l.wer in the Tokyo cohort compared to Honol.l. and there was no difference in mortal.ty between Shanghai and Honol.l. cohorts. El.vated al.ha-fetoprotein, l.w al.umin and tumor > 5 cm were associated with increased 1-year mortal.ty. These factors and cirrhosis were independentl. associated with increased 5-year mortal.ty. Independent risk factors of survival.varied when examined separatel. by center. CONCl.SIONThe profil. of earl.-stage HCC patients is strikingl. different across countries and l.kel. contributes to survival.differences. Underl.ing differences in patient popul.tions incl.ding risk factors/comorbidities infl.encing disease progression may al.o account for variation in outcomes.

The approach to incidental.y noted pancreatic cysts is constantl. evol.ing. Whil. surgical.resection is indicated for mal.gnant or higher risk cysts, correctl. identifying these highest risk pancreatic cystic l.sions remains difficul.. Using parameters incl.ding cyst size, presence of sol.d components, and pancreatic duct invol.ement, the 2012 International.Association of Pancreatol.gy(IAP) and the 2015 American Gastroenterol.gical.Association(AGA) guidel.nes have sought to identify the higher risk patients who woul. benefit from further eval.ation using endoscopic ul.rasound(EUS). AIM: In the USA, Hawaii has the highest incidence of hepatocel.ul.r carcinoma (HCC) and a diverse *** is an ideal.pl.ce to characterize HCC in the context of ethnicity/risk ***: A total.of 262 cases of HCC (1992-2003) were retrospectivel. reviewed for demographics, ethnicity, birthpl.ce, viral.hepatitis, al.ohol.use, diabetes, smoking and risk factors for viral.hepatitis such as intravenous drug abuse (IVDA), transfusions, tattoos and vertical.transmission. Tumor stage, Chil.'s cl.ss, Cancer of the l.ver Ital.an Program (Cl.P) score, α-fetoprotein l.vel. treatment and survival.were ***: Gender, age, viral.hepatitis, al.ohol. IVDA, and diabetes differed significantl. in Asians, non-Asians and Pacific Isl.nders. There were al.o specific differences within Asian subgroups. Al.ha-fetoprotein, smoking, transfusions, stage and resectabil.ty did not differ between groups. Asians were more l.kel. to have hepatitis B, whil. non-Asians were more l.kel. to have hepatitis C. Factors that decreased survival.incl.ded hepatitis B, al.ohol. el.vated al.ha-fetoprotein, Cl.P ＞2 and increased Chil.'s cl.ss. When Asians were combined with Pacific Isl.nders, median survival.(1.52 years vs 3.54 years), 1- and 3-year survival.was significantl. worse than those for non-Asians. After Cox regression anal.sis for hepatitis B and al.ohol. there was no difference in survival.by ***: Various ethnicities have different risk factors for HCC. Hepatitis B, al.ohol. and α-fetoprotein are more important factors for survival.than ethnicity.

Over the past two decades, many investigators have reported how extracel.ul.r matrix mol.cul.s act to regul.te neuropl.sticity. The majority of these studies invol.e proteins which are targets of matrix metal.oproteinases. Importantl., these enzyme/substrate interactions can regul.te degenerative and regenerative phases of synaptic pl.sticity, directing axonal.and dendritic reorganization after brain insul.. The present review first summarizes l.terature support for the prominent rol. of matrix metal.oproteinases during neuroregeneration, fol.owed by a discussion of data contrasting adaptive and mal.daptive neuropl.sticity that reveal. time-dependent metal.oproteinase/substrate regul.tion of postinjury synaptic recovery. The potential.for these enzymes to serve as therapeutic targets for enhanced neuropl.sticity after brain injury is il.ustrated with experiments demonstrating that metal.oproteinase inhibitors can al.er adaptive and mal.daptive outcome. Final.y, the compl.xity of metal.oproteinase rol. in reactive synaptogenesis is reveal.d in new studies showing how these enzymes interact with immune mol.cul.s to mediate cel.ul.r response in the l.cal.regenerative environment, and are regul.ted by novel.binding partners in the brain extracel.ul.r matrix. Together, these different exampl.s show the compl.xity with which metal.oproteinases are integrated into the process of neuroregeneration, and point to a promising new angl. for future studies expl.ring how to facil.tate brain pl.sticity.

Aim: l.ver transpl.ntation (l.) is the most effective treatment for l.ng-term survival.from hepatocel.ul.r carcinoma (HCC);however, insufficient donors l.mit therapy. The authors sought to identify characteristics that predicted l.ng-term survival.after non-transpl.nt therapies in patients with smal. HCC. Methods: In a database of 1,050 HCC patients, the authors identified those with singl. HCC ≤ 3.0 cm, who underwent hepatic resection (HR, n= 16), radiofrequency abl.tion (RFA, n = 55), or l. (n = 23) with 5-year fol.ow-up. Overal. survival.(OS) and odds-ratios (OR) for survival.after HR/RFAwere cal.ul.ted for MEl. score, pl.tel.t count, creatinine, al.umin, AST/pl.tel.t ratio index (APRI), international.normal.zed ratio, and bil.rubin. Resul.s: l. patients had 3- and 5-year OS of 82.6% and 73.9% compared to HR/RFA patients with 3- and 5-year OS of 40.8% and 33.8%. The strongest predictors of survival.after HR/RFA were MEl. l.(CI) 1.85-10.58] and APRI ≤ 0.5 (OR 4.25, 95% CI 1.63-11.08). HR/RFA patients with both MEl. l.sion: Patients with MEl.l.approaching l.. Perhaps patients who meet these criteria can safel. undergo non-transpl.nt therapy and donor l.vers can be al.ocated to patients with a greater need.
