Objective:Double-J(DJ)ureteric stents are commonly placed perioperatively for semirigid or flexible ureteroscopic renal *** is believed that lesser stent material within the bladder mitigates stent-related *** study aimed to evaluate the J-Fil ureteral stent,a single pigtail suture stent compared with conventional DJ stent in relation to stent symptoms in an Asian population undergoing ureterorenal ***:Based on internal audit committee recommendation approval,the records of 50 patients retrieved,available data of 41 patients who were prospectively enrolled into two groups(Group 1[J-Fil stent group],n=21 and Group 2[DJ stent group],n=20)between August 2020 to January 2021,were *** compared were nature of procedure,stone location and size,ease of deployment or removal,and complications.A modified universal stent symptom questionnaire was used to assess morbidity of stent symptoms within 48 h of insertion and at ***:Both groups had similar median age,distribution in male to female ratio,and stone *** overall median universal stent symptom questionnaire score at insertion was similar for bladder pain,flank or loin pain,and quality of life between Group 1 and 2;however,at removal Group 1 fared significantly better than Group 2,especially for flank or loin pain and pain at *** groups had similar ease in insertion with no hospital ***:Our audit favoured the single pigtail suture stent in Asian ureters in mitigating stent-related *** showed a good safety profile with easy deployment and *** promises a new standard in stenting.

This paper evaluated the daily performance of four precipitation products(MSWEP, ERA5, PERSIANN, and TRMM) for seven regions of the Chinese mainland, using observations from 2462 ground stations across the country as a benchmark. We used four statistical and four classification indicators to describe their spatial and temporal accuracy, and capability to detect precipitation events while analyzing their applicability. The results show that according to the precipitation characteristics and accuracy of different types of precipitation products over the Chinese mainland, MSWEP was the most suitable product over the Chinese mainland, having the lowest root mean square error and mean absolute error, along with the highest coefficient of determination. It was followed by TRMM and ERA5, whereas PERSIANN lagged behind in terms of performance. In terms of different regions, MSWEP still performed well, especially in North China and East China. The accuracy of the four precipitation products was relatively low in the summer months, and they all overestimated in the northwest region. In other months, MSWEP and TRMM were better than PERSIANN and ERA5. Background: Post dural puncture headache (PDPH) is a known and potentially debilitating complication of neuraxial anesthesia that can impede patient recovery. The conventional treatment includes hydration and symptomatic treatment like simple analgesics. Those who have unremitting symptoms following conservative measures are offered an epidural blood patch (EBP). However, EBP, an invasive procedure, is associated with complications in itself. Case: We report a 40-year-old man who experienced PDPH after spinal anesthesia. His symptoms recurred after conservative management was instituted. He was then offered a trans-nasal sphenopalatine ganglion (SPG) block. He had excellent pain relief and did not require an EBP. Conclusion: SPG blocks can be considered early in the treatment of PDPH together with general supportive measures. However, if pain relief is not achieved, an epidural blood patch should still be considered.

Introduction: Acromioclavicular (AC) joint dislocation is a common shoulder injury, comprising 9% - 12% of shoulder girdle injuries. Optimal management remains challenging, with treatment decisions guided by the Rockwood classification system. Controversies surround grade III injuries, necessitating further classification. Non-operative treatment has shown favorable outcomes, while surgical interventions vary. Anatomical coracoclavicular reconstruction (ACCR) has demonstrated biomechanical advantages over traditional methods. Arthroscopic techniques offer advantages, minimizing deltoid detachment and allowing concurrent pathology identification. This study evaluates the outcomes of arthroscopic-assisted ACCR in chronic AC joint dislocation. Surgical Technique: Arthroscopic-assisted ACCR involves meticulous portal placement, tendon graft harvesting, diagnostic arthroscopy, and coracoid exposure. The clavicle tunnels were made to mimic the conoid and trapezoid ligament positions, using FibreTape#2 loop and Dog Bone Button for correct placement against the coracoid base, and passing the semitendinosus graft through to reconstruct the conoid ligament, reduction done and graft follow through for anatomical reconstruction. Methods: A retrospective cohort study at Hospital Kuala Lumpur analyzed 35 patients undergoing arthroscopic-assisted ACCR for Rockwood grade III - V AC joint dislocations. Inclusion criteria encompassed trauma ≥ 3 weeks prior, no prior shoulder injuries, and ≥12-month follow-up. Functional and radiological assessments utilized ASES scores and coracoclavicular distances, respectively. Statistical analysis employed descriptive statistics and logistic regression. Results: The mean age was 38.9 years (SD 11.26), and 34 of 35 patients were male. Grade IV injuries were predominant (37.1%). Waiting time for surgery averaged 234.9 days. The disposal of spent activated carbon(AC) will inevitably create secondary pollution. In overcoming this problem, the spent AC can be regenerated by means of biological approach. Bioregeneration is the phenomenon in which through the action of microorganisms, the adsorbed pollutants on the surface of the AC will be biodegraded and this enables further adsorption of pollutants to occur with time elapse. This review provides the challenges and perspectives for effective bioregeneration to occur in biological activated carbon(BAC)column. Owing to very few reported works on the bioregeneration rate in BAC column, emphasis is put forward on the recently developed models of bioregeneration kinetic in batch system. All in all, providing potential solutions in increasing the lifespan of AC and the enhancement of bioregeneration rate will definitely overcome the bottlenecks in spent AC bioregeneration.

Performance of pulsed plasma thruster at low discharge energy

As the size of satellites scales down, low-power and compact propulsion systems such as the pulsed plasma thruster(PPT) are needed for stabilizing these miniature satellites in orbit. Most PPT systems are operated at 2 J or more of discharge energy. In this work, the performance of a PPT with a side-fed, tongue-flared electrode configuration operated within a lower discharge energy range of 0.5-2.5 J has been investigated. Ablation and charring of the polytetrafluoroethylene propellant surface were analyzed through field-effect scanning electron microscopy imaging and energy-dispersive X-ray spectroscopy. When the discharge energy fell below 2 J, inconsistencies occurred in the specific impulse and the thrust efficiency due to the measurement of the low mass bit. At energy ≥2 J, the performance parameters are compared with other PPT systems of similar configuration and discussed in depth.

AIM： We set to determine factors that determine clinical severity after the development of resistance.METHODS： Thirty-five Asian patients with genotypic lamivudine resistance were analyzed in three groups： 13/35 （37%） were non-cirrhotics with normal pre-treatment ALT （Group IA）, 12/35 （34%） were non-cirrhotics with elevated pre-treatment ALT （Group IB）, and 10/35 （29%） were cirrhotics （Group II）. Patients were followed for a median of 98 wk （range 26-220） after the emergence of genotypic resistance.RESULTS： Group IA patients tended to retain normal ALT. Group IB patients showed initial improvement of ALT with lamivudine but 9/12 patients （75%） developed abnormal ALT subsequently. On follow-up however, this persisted in only 33%. Group II patients also showed improvement while on treatment, but they deteriorated with the emergence of resistance with 30% death from decompensated liver disease. Pretreatment ALT levels and CPT score （in the cirrhotic group） were predictive of clinical resistance and correlated with peak ALT levels and CPT score.CONCLUSION： The phenotype of lamivudine-resistant HBV correlated with the pretreatment phenotype. The clinical course was generally benign in non-cirrhotics. However, cirrhotics had a high risk of progression and death （30%） with the development of lamivudine resistance.
