Molecular insights into clinical trials for immune checkpoint inhibitors in colorectal cancer:Unravelling challenges and future directions

Colorectal cancer(CRC)is a complex disease with diverse etiologies and clinical *** considerable progress in development of CRC therapeutics,challenges remain regarding the diagnosis and management of advanced stage metastatic CRC(mCRC).In particular,the five-year survival rate is very low since mCRC is currently rarely *** the past decade,cancer treatment has significantly improved with the introduction of cancer immunotherapies,specifically immune checkpoint *** aimed at blocking immune checkpoints such as PD-1,PD-L1,and CTLA-4 target inhibitory pathways of the immune system,and thereby enhance anti-tumor *** therapies thus have shown promising results in many clinical trials alone or in *** efficacy and safety of immunotherapy,either alone or in combination with CRC,have been investigated in several clinical *** trials,including KEYNOTE-164 and CheckMate 142,have led to Food and Drug Administration approval of the PD-1 inhibitors pembrolizumab and nivolumab,respectively,for the treatment of patients with unresectable or metastatic microsatellite instability-high or deficient mismatch repair ***,these drugs benefit only a small percentage of patients,with the benefits of immunotherapy remaining elusive for the vast majority of CRC *** this end,primary and secondary resistance to immunotherapy remains a significant issue,and further research is necessary to optimize the use of immunotherapy in CRC and identify biomarkers to predict the *** review provides a comprehensive overview of the clinical trials involving immune checkpoint inhibitors in *** underlying rationale,challenges faced,and potential future steps to improve the prognosis and enhance the likelihood of successful trials in this field are discussed.

Obesity-linked type 2 diabetes is one of the paramount causes of morbidity and mortality worldwide,posing a major threat on human health,productivity,and quality of *** great progressmadetowards a better understanding of the molecular basis of diabetes,the available clinical counter-measures against insulin resistance,a defect that is central to obesity-linked type 2 diabetes,remain ***,an abundant adipocyte-secreted factor with a wide-range of biological activities,improves insulin sensitivity in major insulin target tissues,modulates inflammatory responses,and plays a crucial role in the regulation of energy ***,adiponectin as a promising therapeutic approach has not been thoroughly explored in the context of pharmacological intervention,and extensive efforts are being devoted to gain mechanistic understanding of adiponectin signaling and its regulation,and reveal therapeutic ***,we discuss tissue-and cell-specific functions of adiponectin,with an emphasis on the regulation of adiponectin signaling pathways,and the potential crosstalk between the adiponectin and other signaling pathways involved in metabolic *** better just why and how adiponectin and its downstream effector molecules work will be essential,together with empirical trials,to guide us to therapies that target the root cause(s)of type 2 diabetes and insulin resistance.

低收入群体是弱势群体,不仅影响社会稳定,更阻碍共同富裕的实现。2020年底,我国如期完成了新时代的脱贫攻坚任务,解决了绝对贫困问题。然而,刚刚脱离绝对贫困的人群生活仍然较为艰难,依然不同程度存在潜在的返贫风险,生存状况与社会平均水平存在较大差距,相对贫困问题仍将长期存在。“后扶贫时代”如何深化低收入群体综合帮扶,巩固脱贫攻坚成果,提高低收入群体的收入,破解低收入群体相对贫困问题,最终实现共同富裕是时代赋予的新考卷。本文采用了文献研究法、实地调查法,通过阅读和借鉴国内外优秀学者的研究成果,深入龙港市社会事业局、低收入群体家庭进行调研,了解龙港市低收入群体的基本情况,对龙港市低收入群体综合帮扶过程中存在的问题和成因进行分析,揭示现阶段龙港市低收入群体综合帮扶面临的困境与不足,并提出有针对性和可行性的对策建议,从而为龙港市深化低收入群体综合帮扶提供更有效的思路和建议。同时,通过进一步厘清地方在低收入群体综合帮扶过程中的职能定位,丰富学界关于低收入群体综合帮扶方面的理论研究,并对相关实践经验作重要补充,为国内其他地方低收入群体综合帮扶过程中遇到的问题提供具体的借鉴和参考。本论文的创新之处在于打破传统的从各种社会保障的角度来撰写,而是以系统思维,从建立新队伍、完善新标准、提升新能力、整合新资源等四个方面提出详细的对策。一是通过建立新队伍,加大帮扶政策的宣传力度、提高队伍专业化素质、构建低收入群体动态监测指标体系,解决主动发现渠道不健全、队伍建设不规范导致的精准识别不适问题。二是完善新标准,通过优化共享服务平台、探索建立闭环管理的社会救助体系、加强政策智能匹配等方式,解决集成帮扶制度供给不足导致协同合作不力的问题。三是提升新能力,通过拓宽岗位来源渠道、能力提升渠道、资金来源渠道、住房保障渠道和监督管理渠道,帮扶方式单一、帮扶供给遭遇多重局限导致帮扶周期漫长的问题。四是整合新资源,通过集聚社会资源、发展公益组织、打造慈善品牌,推进社会帮扶常态化,解帮扶资源分散、帮扶资源体系约束趋紧的问题。

The Direct and Reposting Effects of Advertorials on Sales

Advertorials have been shown to influence advertising recall and brand evaluation. However, no research has examined their impact on sales. This study uses longitudinal data on durable consumer goods in China to assess the extent to which advertorials influence sales. The study finds that advertorials have dual, positive effects on firms＇ sales： a direct effect through original copy and a reposting effect through online exposure.

R2R3-type Lo MYB21 affects jasmonate-regulated development and dehiscence of anthers in lily(Lilium oriental hybrids)

Lilies are widely cultivated for cut flowers,but their large anthers carry a considerable amount of colored pollen that is dispersed *** the molecular mechanism of anther development and dehiscence could help solve this ***21,encoding a putative R2R3v-myb avian myeloblastosis viral oncogene homolog(MYB)transcription factor,was identified from oriental lilies(Lilium‘Siberia’).Real-time quantitative PCR analysis showed that LoMYB21 was mainly expressed in the anther,filament and stigma and had high expression during the late stages of lily anther ***21 had transactivation activity and was located in the nucleus through yeast one-hybrid assays and transient expression in Nicotiana *** of LoMYB21 by virus-induced gene silencing(VIGS)in Lilium‘Siberia’led to anther indehiscence and reduced the expression of genes related to Jasmonate acid(JA)biosynthesis and signal *** of LoMYB21 in DEX::LoMYB21 transgenic Arabidopsis caused procumbent inflorescences that became infertile,accompanied by higher expression of JA biosynthetic and signaling *** results demonstrated that JA content and signaling were abnormal in silenced lily and transgenic LoMYB21 Arabidopsis,which affected anther *** study indicated that LoMYB21 could regulate lily anther dehiscence through JA biosynthesis and signaling during the late stages of anther development.

背景与目的：患者参与在预防医疗实践相关不良反应和伤害中有举足轻重的作用,积极且成功的患者参与可有效降低不良反应与伤害的发生率.患者安全管理机构及卫生行政部门一致认为,患者参与自我医疗管理可显著预防医疗差错.然而住院患者对这一观点的态度和想法的实践转化情况仍然存争议.本研究旨在探析患者对参与自我医疗管理所持的态度及采取实际行动的情况,以及态度积极的患者是否更愿意将想法转化为实践.方法：本项横断面研究采用便利抽样法从汕头大学医学院两所附属教学医院招募了2052名患者.我们采用了包含7个条目的调查问卷调查研究对象对参与自我医疗活动所持的态度及采取实际行动的情况.本研究采用描述性分析和独立t检验比较分析了持积极和消极态度的患者采取实际行动的差异.结果：调查结果显示,多数患者对参与自我安全管理活动持积极态度,但较少将想法付诸实践.持积极态度的患者更有可能采取实际行动,但持不同态度的患者在诸如“嘱医生或护士洗手”与“嘱护士或医生确认您的身份”等方面的差异无统计学意义.结论：对参与自我安全管理活动持积极态度是提高患者参与自我医疗活动的重要因素.基于本次研究结果,我们认为需要采取针对性的干预措施以改变患者参与自我安全管理的态度,并学会如何实现想法的实践转化.此外,在制定干预策略时,还需要考虑患者的文化背景.

【目的】从土壤理化性状和微生物群落结构及多样性角度,研究蚯蚓联合生防细菌改善百合连作障碍的机理和效果,为湖南百合产业化种植提供技术措施。【方法】采用盆栽试验方法,在湖南长沙连续种植了两季百合(Lilium lancifolium Thunb.)。供试土壤为红壤,生防菌为Bacillales velezensis YFB3-1菌液(菌体浓度为10^(9) CFU/mL),蚯蚓为赤子爱胜蚓(Eisenia fetida)。试验设单施菌液(T1)、只接种蚯蚓(T2)、同时使用菌液和蚯蚓(T3) 3个处理,以不使用菌剂和蚯蚓为对照(CK)。调查了两茬百合产量和枯萎病病情指数,分析了第二茬百合根际土壤理化性质、细菌群落结构和多样性,以及连作百合产量与根际土壤理化性质、细菌群落和枯萎病的相关性。【结果】与第一茬相比,同一处理的第二茬百合产量都显著下降。两年连作百合产量都以同时使用菌液和蚯蚓(T3)处理为最高,分别为16464、15674 kg/hm^(2),都显著高于同年份其他处理。两年连作百合枯萎病发病率和病情指数都以T3为最低,都显著低于同年份其他处理,T3处理百合枯萎病病情指数分别为18.15(2019年)、25.00 (2020年),并且两年的防治效果(T3)都明显优于单施菌液(T1)和接种蚯蚓(T2)。相比于对照(CK),2019、2020年T3处理百合枯萎病防治效果分别为44.55%、37.66%,分别增产15.05%、14.78%。蚯蚓联合生防菌YFB3-1增加了根际土壤Rhizobiales、Flavobacteriales、Pseudomonadales、Bacillales等有益微生物的种群丰度,蚯蚓与生防菌YFB3-1在抑制连作百合枯萎病方面具有显著的互作效应,因而缓解连作百合产量下降的效果最佳。【结论】在施用芽孢杆菌生防菌剂防治连作百合枯萎病的同时,可以增施有机肥(牛粪)来提高土壤中蚯蚓数量,有效预防百合枯萎病发生,减少连作百合产量损失。

东方百合籽球抗逆性差,田间土壤栽培易感染病虫害,种球年生长量小。为提高种球质量,缩短繁育周期,开展籽球无土栽培技术研究,建立东方百合种球无土栽培繁育技术体系。[方法]以东方百合‘Siberia’为试验材料,采用单因素随机区组设计,探讨不同栽培基质配方、栽培密度、水溶性肥对百合籽球生长的影响。[结果]东方百合籽球无土栽培最佳基质配方为进口草炭∶珍珠岩=3∶1,最适宜无土栽培密度为100粒/箱,生长期施花无缺水溶性肥2000 mg/L最有利于籽球的生长。[结论]筛选出东方百合籽球无土栽培最佳基质配方、栽培密度、肥料种类和浓度,为东方百合籽球规模化生产提供参考。
