There are five vital signs that healthcare providers assess: temperature, pulse, respiration, blood pressure, and pain. Normal levels for the five vital signs are published by the American Heart Association, and other specialty organizations, however, the sixth vital sign (resilience) which adopts the measure of immune resilience is suggested in this paper. Resilience is the ability of the immune system to respond to attacks and defend effectively against infections and inflammatory stressors, and psychological resilience is the capacity to resist, adapt, recover, thrive, and grow from a challenge or a stressor. Individuals with better optimal immune resilience had better health outcomes than those with minimal immune resilience. The purpose of this paper is to conceptualize, contextualize, and operationalize all six vital signs. We suggest measuring resilience subj.ctively and obj.ctively. Subj.ctively, use a 5-item guided interview revised from the Connor-Davidson Resilience Scale (CDRC), a scale of 10 items. The revised CDRC scale is a 5-item scale. The scale is rated on a 5-point Likert scale from 0 (not true) to 4 (true all the time). The total score ranges from 0 to 20, with higher total scores indicating greater resilience. The scale demonstrated good construct validity and internal consistency (α = 0.85) during the development of the scale. The CD-RISC had a good Cronbach’s alpha level of 0.85. The Revised CD-RISC can be completed in 2 - 4 minutes. To measure resilience obj.ctively, we suggest using Immune Resilience (IR) levels, the level of resilience to preserve and/or rapidly restore immune resilience functions that promote disease resistance and control inflammation and other inflammatory stress. IR levels are gauged with two peripheral blood metrics that quantify the balance between CD8 and CD4 T-cell levels and gene expression signatures tracking longevity-associated immunocompetence and mortality- or entropy-associated inflammation. Though silanization of aluminum powder is currently used to improve its flow properties,for use as an alternative fuel source,there are a wide range of experimental parameters for the process and not all of them have been thoroughly *** this is complete,it is unknown if the process is optimized in terms of time,efficiency,and ***,we report on a study into the effects of changes in humidity,degree of agitation,reaction temperature,and curing time upon the deposition of phenyl triethoxysilane into 20μm(d50)aluminum *** confirm the deposition of the coating via diffuse reflectance infrared spectroscopy and x-ray photoelectron *** then characterize the coated particles using apparent density measurements and Carney flow *** analysis of variance,we find that,of the parameters explored,only changes in cure time and reaction temperature provide meaningful changes to the apparent density,while none of our parameters produced statistically significant changes in Carney flow ***,we conclude that,when optimizing silanization of aluminum particles,environmental control of humidity is unneeded and that the reaction can be run with minimal *** ability to largely ignore these parameters is a benefit to large-scale processing.

Metabolic syndrome and the urinary microbiome of patients undergoing percutaneous nephrolithotomy

Obj.ctive:To identify possible stone-promoting microbes,we compared the profiles of microbes grown from stones of patients with and without metabolic syndrome(MetS).The association between MetS and urinary stone disease is well established,but the exact pathophysiologic relationship remains *** evidence suggests urinary tract dysbiosis may lead to increased nephrolithiasis ***:At the time of percutaneous nephrolithotomy,bladder urine and stone fragments were collected from patients with and without *** sample types were subj.cted to expanded quantitative urine culture(EQUC)and 16 S ribosomal RNA gene ***:Fifty-seven patients included 12 controls(21.1%)and 45 MetS patients(78.9%).Both cohorts were similar with respect to demographics and non-MetS *** controls had uric acid stone *** EQUC,bacteria were detected more frequently in MetS stones(42.2%)compared to controls(8.3%)(p=0.041).Bacteria also were more abundant in stones of MetS patients compared to *** validate our EQUC results,we performed 16 S ribosomal RNA gene *** 12/16(75.0%)sequence-positive stones,EQUC reliably isolated at least one species of the sequenced *** were detected in both“infectious”and“non-infectious”stone ***:Bacteria are more common and more abundant in MetS stones than control *** findings support a role for bacteria in urinary stone disease for patients with MetS regardless of stone composition.

Background:Pediatric cardiology patients often experience decreased quality of life(QoL)and higher rates of mental illness,particularly with severe disease,but the relationship between them and comparisons across diagnostic groups are *** mixed-methods cross-sectional study assessed the association between QoL anxiety and behavior problems among children with structural heart disease,arrhythmia,or other cardiac ***:Children(6–14 years,n=76,50%female)and their parents completed measures of QoL(PedsQL),behavior(BASC-2,subset of 19 children)and anxiety(MASC-2,children 8+years).Pearson correlations/regression models examined associations between QoL,behavior and anxiety,controlling for age,sex,diagnosis and condition *** t-tests compared results to published normative ***-structured interviews(15 children,13 parents)examined self-and parent-reported perceptions of the patient’s emotional ***:Child reported social QoL(95%CI difference:−11.0,−2.1),emotional well-being(95%CI difference:−12.5,−3.8)and adj.stment skills(95%CI difference:−13.9,−3.0)were significantly below published norms(p<0.05).Neither age,sex,nor diagnosis were significantly related to questionnaire *** to children with mild conditions,child and parent-reported QoL was significantly lower in children with complex conditions(np2≥0.14,p<0.05).Lower QoL was strongly associated with more internalizing problems based on both child(r=−0.78,p<0.001)and parent(r=−0.67,p<0.001)*** responses suggested emotional distress was attributed to“normal”child problems rather than the child’s cardiac ***:Children with complex cardiac conditions,regardless of diagnosis,had diminished *** the PedsQL in clinical settings may help identify pediatric cardiology patients experiencing emotional distress(i.e.,anxiety and depression).

A method based on regression modeling was developed to discern the contribution of component chemicals to the toxicity of highly complex, environmentally realistic mixtures of disinfection byproducts（DBPs）. Chemical disinfection of drinking water forms DBP *** of concerns about possible reproductive and developmental toxicity, a whole mixture（WM） of DBPs produced by chlorination of a water concentrate was administered as drinking water to Sprague–Dawley（S–D） rats in a multigenerational study. Age of puberty acquisition,i.e., preputial separation（PPS） and vaginal opening（VO）, was examined in male and female offspring, respectively. When compared to controls, a slight, but statistically significant delay in puberty acquisition was observed in females but not in males. WM-induced differences in the age at puberty acquisition were compared to those reported in S–D rats administered either a defined mixture（DM） of nine regulated DBPs or individual DBPs. Regression models were developed using individual animal data on age at PPS or VO from the DM study. Puberty acquisition data reported in the WM and individual DBP studies were then compared with the DM models. The delay in puberty acquisition observed in the WM-treated female rats could not be distinguished from delays predicted by the DM regression model, suggesting that the nine regulated DBPs in the DM might account for much of the delay observed in the WM. This method is applicable to mixtures of other types of chemicals and other endpoints.

Context Hemoglobin-based blood substitutes(HBBSs)are infusible oxygen- carrying liquids that have long shelf lives,have no need for refrigeration or cross- matching,and are ideal for treating hemorrhagic shock in remote *** trials of HBBSs during the last decade have reported increased risks without clinical benefit. Obj.ctive To assess the safety of HBBSs in surgical,stroke,and trauma patients. Data Sources PubMed,EMBASE,and Cochrane Library searches for articles using hemoglobin and blood substitutes from 1980 through March 25,2008;reviews of Food and Drug Administration(FDA)advisory committee meeting materials;and In- ternet searches for company press releases. Study Selection Randomized controlled trials including patients aged 19 years and older receiving HBBSs *** database searches yielded 70 trials of which 13 met these criteria;in addition,data from 2 other trials were reported in 2 press releases,and additional data were included in 1 relevant FDA review. Data Extraction Data on death and myocardial infarction(MI)as outcome variables. Results Sixteen trials involving 5 different products and 3711 patients in varied pa- tient populations were identified.A test for heterogeneity of the results of these trials was not significant for either mortality or MI(for both,I=0%,P(?)60),and data were combined using a fixed-effects ***,there was a statistically significant in- crease in the risk of death(164 deaths in the HBBS-treated groups and 123 deaths in the control groups;relative risk [RR],1.30;95% confidence interval [CI],1.05-1.61) and risk of MI(59 MIs in the HBBS-treated groups and 16 MIs in the control groups;RR,2.71;95% CI,1.67-4.40)with these *** analysis of these trials in- dicated the increased risk was not restricted to a particular HBBS or clinical indication. Conclusion Based on the available data,use of HBBSs is associated with a signifi- cantly increased risk of death and MI.

地球科学研究中数据的准备、分发、访问、处理和分析都依赖于计算机软件。科学结果的重复通常需要复用同样的软件。同样,科学数据的有效分发、访问或者分析也都需要进行软件复用。地球科学软件开发社区已经意识到了软件复用的价值,并且在社区内大力鼓励和培育软件复用。因此,他们成立了 NASA 地球科学数据系统( ESDS) 软件复用工作组( WG) 来支持社区成员之间的软件复用。工作组开发了软件复用门户来发布软件复用相关的信息、资源,并且提供培训来提高和改进整个社区的软件复用实践。工作组还开发了一个复用支持系统( RES) 原型系统来支持可复用软件的访问。此外, 工作组还制定了复用成熟度级别, 从而提高了评估和选择可复用软件的能力。
