Laparoscopic Heller-Dor operation for patients with achalasia

Background Laparoscopic Heller cardiomyotomy and Dor fundoplication is the surgical procedure of choice for esophageal achalasia. The aim of this study was to investigate the clinical outcome of laparoscopic Heller-Dor procedure in our initial series of 25 patients with achalasia. Methods Between October 2003 and January 2006, a total of 25 patients with achalasia underwent laparoscopic Heller-Dor operation. Among them, 9 were male and 16 were female with an average age of （41.5 ± 5.1） years （21- 66）. All the patients received upper gastrointestinal series （barium swallow）, esophagogastroscopy, esophageal manometry to exclude esophageal carcinoma and to confirm the diagnosis, and 21 patients also had 24-hour ambulatory pH studies. All the patients were operated by laparoscopic modified Heller＇s myotomy with Dor fundoplication. In addition, 2 of them had combined laparoscopic cholecystectomy ± excision of hepatic hemangioma and laparoscopic cholecystectomy, respectively. Results The average operating time was （110.6± 12.9） minutes （range, 60-180）, operative blood loss averaged （18.6±7.1） ml （5-50）, the median time to oral feeding was （1.6±0.4） days （1-4） and the median hospital stay was （12.6± 1.2） days （10-20）. There was no conversion to open surgery. Intraoperative mucosal perforation was encountered in six patients and was repaired in all of them by laparoscopic suture. All the patients had an uneventful recovery without postoperative complication. After a median follow-up of （10.6 ± 7.2） months （1 -27）, 24 patients were asymptomatic and 1 had mild postoperative dysphagia. Conclusions Laparoscopic Heller-Dor operation had the advantages of reduced compromise of the cardiopulmonary function, with less disruption of the supporting structures （phrenoesophageal membrane） of the antireflux mechanism, requiting simpler general anesthesia and providing excellent exposure permitting an easy fundoplication, less pain and reduced morbidity, shorter hospitalization and

Simplified Compact Perovskite Solar Cells with Efficiency of 19.6% via Interface Engineering

For the commercialization of perovskite solar cells(PSCs), it is more appealing to develop high-performance simplified PSCs where perovskite films are just sandwiched between the back and front electrodes, in order to simplify the fabrication process and to reduce the cost. However, to date, this kind of devices shows rather low performance, and there are few researches on this ***, we report on a kind of compact PSCs(CPSCs) that are free of independent charge transport layers(CTLs). The devices are realized by the use of organic monolayer-modified effective electrodes, along with the use of [6,6]-phenyl-C61-butyric acid methyl ester(PCBM)-assisted anti-solvent technique to obtain ultra-thin(~10 nm) PCBM-embedded perovskite films. Compared to control devices, CPSCs achieve a promising champion power conversion efficiency of 19.6% with largely reduced hysteresis. Moreover, the unencapsulated CPSC shows good stability under ambient atmosphere, with only 10% efficiency loss after 60 days’ storage. This work indicates that, by delicate design, CPSCs with smaller materials consumption in device architecture can perform competitively as conventional PSCs. Further reduction in the actual usage of costly CTL materials can be expected upon our CPSCs by developing more facile and economic methods to prepare ultra-thin CTLs.

通过实验和数值模拟系统研究了亚毫米气泡鼓泡塔与常规鼓泡塔在流动和传质特性上的区别,并建立了适用于亚毫米气泡气液两相流流动和传质过程的数值模拟方法。研究结果表明,相比常规鼓泡塔,相同操作条件下亚毫米气泡鼓泡塔的气泡尺寸分布更窄,平均尺寸降至前者3%左右,气含率提高2倍以上,比表面积提高2个数量级。另外亚毫米气泡气液两相流中气液径向分布更均匀,轴向返混程度更小。亚毫米气泡鼓泡塔的相界面积是强化传质的关键控制因素,其液相传质系数虽低于常规鼓泡塔,但依靠巨大的相界面积,其体积传质系数是常规鼓泡塔的10倍左右。针对大规模鼓泡塔反应器的模拟结果也表明,亚毫米气泡可使反应器达到更均匀的气含率分布,受初始气液分布的影响小。

济南市免疫预防工作人员对免疫规划疫苗接种程序的认知现况调查

目的系统了解济南市免疫预防专业人员对《国家免疫规划疫苗儿童免疫程序及说明（2021年版）》中相关接种指导意见的认知情况，为提供更有针对性的技术指导和更好地完善儿童免疫程序和指导原则提供参考。方法于2022年9月对济南市的区县疾病预防控制中心（简称疾控中心）15名免疫预防技术骨干及423名在接种单位（含产科接种室）从事预检、接种的工作人员进行问卷调查，用Excel 2010和SPSS 18.0软件对调查数据进行描述性分析、χ2检验和多因素logistic回归分析。以15名疾控中心人员对免疫程序总知识、特殊健康状态儿童接种知识和疫苗同时接种知识的回答正确率为标准计算调查对象相应知识的知晓率。结果 438名调查对象对免疫程序总知识、特殊健康状态儿童接种知识和疫苗同时接种知识的知晓率分别为29.91%、23.29%和48.63%。多因素logistic回归分析结果显示，文化程度(OR=1.88,95%CI:1.13～3.12）、预防接种工作年限（OR=1.49,95%CI:1.01～2.22）和执业资格类别（OR=0.25,95%CI:0.068～0.88）是调查对象免疫程序总知晓率的影响因素。文化程度（OR=3.01,95%CI:1.63～5.57）和专业（OR=0.24,95%CI:0.075～0.78）是调查对象特殊健康状态儿童接种知晓率的影响因素。仅性别（OR=0.37,95%CI:0.16～0.86）是调查对象疫苗同时接种知识知晓率的影响因素。结论济南市预防接种门诊工作人员对于免疫程序的认知水平，包括对于特殊健康状态儿童接种以及疫苗同时接种知识的把握尚待进一步提高。建议合理增加侧重相关知识的预防接种专业技术培训，建议进一步完善和细化有关特殊健康状态儿童预防接种以及疫苗同时接种相关工作指南或指导性文件。

Comparison of the Immunogenicities of HIV-1 Mutants Based on Structural Modification of env

Eleven env mutants were designed and generated by site-directed mutagenesis of the regions around NAb epitopes and deletions of variable regions in env. The immunogenicities of the generated mutants were evaluated using single-cycle infection neutralization assays with two pseudoviruses and IFN-γ ELISPOT. Overall, five mutants (dWt, M2, M5-2, M5-1 and dM7) induced higher neutralization activities for both pseudoviruses than plasmid Wt, while only two of the mutants (dWt and M5-2) showed significant differences (P<0.05). Two mutants (M2 and dM2) induced more Env-specific T cells than plasmid Wt. Statistically however, significance was only reached for mutant M2. Thus, properly modified HIV-1 Env may have the potential to induce potent cellular and humoral immune responses.

The synthesis of a series of new covalently-connected azoporphyrin derivatives is described and the photochemical properties of the new compounds are discussed. The two chromophores of these derivatives exhibit their absorption spectroscopic properties respectively. In the fluorescence emission spectra, intermolecular fluorescence quenching is detected.

本试验以某冶炼厂产出高铜含镍溶液为原料开展试验研究,试验确定了采用“一次蒸发结晶-电积脱铜-二次蒸发结晶”工艺。试验研究结果表明:一次蒸发结晶铜的结晶率为81.48%,镍的结晶率为5.75%。电积脱铜可将溶液中的铜离子浓度脱除至3g/L,铜的脱除效率为92.97%,电流效率为84.32%。二次蒸发结晶镍的结晶率为82.13%,铜的结晶率为3.65%。本工艺可实现溶液中铜镍的初步分离,为铜镍元素的后续净化提纯创造有利的条件。
