Comparing the incidence of major cardiovascular events and severe microvascular complications in patients with type 2 diabetes mellitus:A systematic review and meta-analysis

BACKGROUND Type 2 diabetes mellitus(T2DM)causes both macrovascular and microvascular ***,currently,selection of glycemic measures and their thresholds to diagnose T2DM,and efficacy outcomes in evaluation of anti-diabetic drugs is predominantly informed by the relation of T2DM to microvascular *** can be severely mistaken on T2DM by neglecting macrovascular complications which are generally more severe,if they also occur more commonly than microvascular *** To compare the incidence of major cardiovascular events(MACEs)and severe microvascular complications(SMICs)in T2DM *** MEDliNE,EMBASE,and Cochrane Central Register of Controlled Trials were searched from inception to September *** studies or trials of T2DM patients aged 18 years or older that reported incidence of both MACEs and SMICs were *** were defined as nonfatal myocardial infarction and stroke,and cardiovascular death,while SMICs included serious retinopathy,nephropathy and diabetic *** relative risk(RR)was estimated as the incidence of MACEs divided by that of SMICs in same patients and combined with meta-analysis in a random-effect *** Twelve studies with a total of 16 cohorts and 387376 patients were included,and the combined RR was 2.02(95%CI:1.46–2.79).The higher incidence of MACEs remained in various subgroup and sensitivity *** Patients with T2DM are much more likely to develop MACEs than *** taking more serious consequences and relatively higher incidence into consideration,macrovascular complications deserve more emphasis in developing the diagnostic criteria of T2DM and in evaluating the efficacy of antidiabetic drugs.

Magneto‑Dielectric Synergy and Multiscale Hierarchical Structure Design Enable Flexible Multipurpose Microwave Absorption and Infrared Stealth Compatibility

Developing advanced stealth devices to cope with radar-infrared(IR)fusion detection and diverse application scenarios is increasingly demanded,which faces significant challenges due to conflicting microwave and IR cloaking mechanisms and functional integration ***,we propose a multiscale hierarchical structure design,integrating wrinkled MXene IR shielding layer and flexible Fe_(3)O_(4)@C/PDMS microwave absorption *** top wrinkled MXene layer induces the intensive diffuse reflection effect,shielding IR radiation signals while allowing microwave to pass ***,the permeable microwaves are assimilated into the bottom Fe_(3)O_(4)@C/PDMS layer via strong magneto-electric *** theoretical and experimental optimization,the assembled stealth devices realize a near-perfect stealth capability in both X-band(8–12 GHz)and long-wave infrared(8–14μm)wavelength ***,it delivers a radar cross-section reduction of−20 dB m^(2),a large apparent temperature modulation range(ΔT=70℃),and a low average IR emissivity of ***,the optimal device demonstrates exceptional curved surface conformability,self-cleaning capability(contact angle≈129°),and abrasion resistance(recovery time≈5 s).This design strategy promotes the development of multispectral stealth technology and reinforces its applicability and durability in complex and hostile environments.

Background：At present,China has listed the compound tablet containing a fixed dose ofrosiglitazone and metformin,Avandamet,which may improve patient *** aim of this study was to evaluate the efficacy and safety of Avandamet or uptitrated metformin treatment in patients with type 2 diabetes inadequately controlled with metformin ***：This study was a 48-week,multicenter,randomized,open-labeled,active-controlled *** with inadequate glycaemic control （glycated hemoglobin [HbA1c] 7.5-9.5%） receiving a stable dose of metformin （≥1500 mg） were recruited from 21 centers in China （from 19 November,2009 to 15 March,2011）.The primary objective was to compare the proportion of patients who reached the target of HbA1c ≤7% between Avandamet and metformin ***：At week 48,83.33% of patients reached the target of HbA 1 c ≤7% in Avandamet treatment and 70.00% in uptitrated metformin treatment,with significantly difference between *** target of HbAlc ≤6.5% was reached in 66.03% of patients in Avandamet treatment and 46.88% in uptitrated metformin *** target of fasting plasma glucose （FPG） ≤6.1 mmol/L was reached in 26.97% of patients in Avandamet treatment and 19.33% in uptitrated metformin *** target of FPG ≤7.0 mmol/L was reached in 63.16% of patients in Avandamet treatment and 43.33% in uptitrated metformin *** insulin decreased 3.24 ±0.98 μU/ml from baseline in Avandamet treatment and 0.72 ＆#177; 1.10 μU/ml in uptitrated metformin *** adverse event （AE） rates and serious AE rates were similar between *** occurred rarely in both ***：Compared with uptitrated metformin,Avandamet treatment provided significant improvements in key parameters ofglycemic control and was generally well *** number：ChiCTR-TRC-13003776.

Perfluoropentane-based oxygen-loaded nanodroplets reduce microglial activation through metabolic reprogramming

Microglia,the primary immune cells within the brain,have gained recognition as a promising therapeutic target for managing neurodegenerative diseases within the central nervous system,including Parkinson’s *** perfluorocarbon droplets have been reported to not only possess a high oxygen-carrying capacity,but also exhibit remarkable anti-inflammatory ***,the role of perfluoropentane in microglia-mediated central inflammatory reactions remains poorly *** this study,we developed perfluoropentane-based oxygen-loaded nanodroplets(PFP-OLNDs)and found that pretreatment with these droplets suppressed the lipopolysaccharide-induced activation of M1-type microglia in vitro and in vivo,and suppressed microglial activation in a mouse model of Parkinson’s *** suppression led to a reduction in the inflammatory response,oxidative stress,and cell migration capacity in ***,the neurotoxic effects were mitigated,which alleviated neuronal ***,ultrahigh-performance liquid chromatography–tandem mass spectrometry showed that the anti-inflammatory effects of PFP-OLNDs mainly resulted from the modulation of microglial metabolic *** further showed that PFP-OLNDs regulated microglial metabolic reprogramming through the AKT-mTOR-HIF-1α***,our findings suggest that the novel PFP-OLNDs constructed in this study alleviate microglia-mediated central inflammatory reactions through metabolic reprogramming.

Distributed optimal consensus control for multiagent systems based on event-triggered and prioritized experience replay strategies

This study uses event-triggered(ET) and reinforcement learning methods to investigate the optimal consensus control problem for cooperative-competitive multiagent systems. It proposes a novel distributed ET control strategy, which relies on a prioritized experience replay(PER) policy. This strategy not only conserves communication resources but also ensures acceptable system performance. To implement the proposed method, actor-critic(AC) dual-structured neural networks(NNs) are used to approximate the value function and control policy. In the AC NNs, the weight estimates for the NNs are updated at the moment of event triggering, resulting in a nonperiodic weight adjustment pattern. This approach decreases the computational cost in comparison with the traditional ET mechanism. The PER-based ET mechanism makes full use of valid historical data and effectively establishes a balance between system performance and communication resource ***, it does not require the following two conditions in most existing studies:(1) requirement of the system dynamics model to be known, and(2) persistent excitation. In addition, Zeno behavior is excluded from this study. Finally, a simulation is conducted to confirm the validity of the suggested approach.

High-entropy materials represent a new category of high-performance materials,first proposed in 2004 and extensively investigated by researchers over the past two *** definition of high-entropy materials has continuously *** the last ten years,the discovery of an increasing number of high-entropy materials has led to significant advancements in their utilization in energy storage,electrocatalysis,and related domains,accompanied by a rise in techniques for fabricating high-entropy electrode ***,the research emphasis has shifted from solely improving the performance of high-entropy materials toward exploring their reaction mechanisms and adopting cleaner preparation ***,the current definition of high-entropy materials remains relatively vague,and the preparation method of high-entropy materials is based on the preparation method of single metal/low-or medium-entropy *** should be noted that not all methods applicable to single metal/low-or medium-entropy materials can be directly applied to high-entropy *** this review,the definition and development of high-entropy materials are briefly ***,the classification of high-entropy electrode materials is presented,followed by a discussion of their applications in energy storage and catalysis from the perspective of synthesis ***,an evaluation of the advantages and disadvantages of various synthesis methods in the production process of different high-entropy materials is provided,along with a proposal for potential future development directions for high-entropy materials.

CoTCPP integrates with BiOBr microspheres for improved solar-driven CO_(2)reduction performance

CO_(2) photoreduction into carbon-based chemicals has been considered as an appropriate way to alleviate the energy issue and greenhouse ***,the 5,10,15,20-tetra(4-carboxyphenyl)porphyrin cobalt(II)(CoTCPP)has been integrated with BiOBr microspheres and formed the CoTCPP/BiOBr *** as-prepared CoTCPP/BiOBr-2 composite shows optimized photocatalytic performance for CO_(2) conversion into CO and CH_(4) upon irradiation with 300 W Xe lamp,which is 2.03 and 2.58 times compared to that of BiOBr,*** introduced CoTCPP significantly enhanced light absorption properties,promoted rapid separation of photogenerated carriers and boosted the chemisorption of CO_(2) *** metal Co^(2+) at the center of the porphyrin molecules also acts as adsorption center for CO_(2) molecules,accelerating the CO_(2) conversion into CO and *** possible mechanism of CO_(2) photoreduction was explored by in-situ FT-IR *** work offers a new possibility for the preparation of advanced photocatalysts.

To the Editor：Diabetes caused by mitochondrial tRNALeu（UUR） A3243G mutation is one of the most common types ofmitochondrial diabetes mellitus （MDM）.Seventeen years ago,we reported that the prevalence of MDM was 0.4％ in clinically diagnosed type 2 diabetes mellitus （T2DM） patients （n =716）.[1] Recently,we reviewed all the studies reporting MDM cases from grade three and first-class hospitals in China （unpublished） and found that the prevalence of MDM in a pooled randomly selected T2DM population was 0.64％.MDM patients are usually characterized by early age at diagnosis,low beta-cell function,and lack of obesity,insulin resistance,and autoantibodies associated with type 1 diabetes mellitus （T1DM）.

Solar-driven interface evaporation with high solar-to-steam conversion efficiency has shown great potential in seawater ***,due to the influence of latent heat and condensation efficiency,the water yield from solar-driven interface evaporation remains insufficient,posing a significant challenge that requires *** this work,we designed a dual-mode high-flux seawater desalination device that combines solar-driven interface evaporation and capacitive *** utilizing coupled desalination materials exhibiting both photothermal conversion and capacitance activity,the device demonstrated photothermal evaporation rates of 1.41 and 0.97 kg m-2h-1for condensate water yield under one-sun ***,the device exhibited a salt adsorption capacity of up to48 mg g-1and a salt adsorption rate of 2.1 mg *** addition,the salt adsorption capacity increased by approximately 32% under one-sun ***,photo-enhanced capacitive desalination performance was explored through numerical simulations and theoretical *** calculations and characterizations confirmed that the defect energy levels formed by the introduction of sulfur vacancies can effectively widen the light absorption range,improve photothermal conversion performance,and stimulate more photoelectrons to participate in capacitive ***,the electron distribution state of molybdenum disulfide with sulfur vacancies and surface defect sites contributes to ion/electron transport at the solid-liquid *** work provides a novel pathway for integrating solar vapor generation with other low-energy desalination technologies.

Decreased levels of phosphorylated synuclein in plasma are correlated with poststroke cognitive impairment

Poststro ke cognitive impairment is a major secondary effect of ischemic stroke in many patients;however,few options are available for the early diagnosis and treatment of this *** aims of this study were to(1)determine the specific relationship between hypoxic andα-synuclein during the occur of poststroke cognitive impairment and(2)assess whether the serum phosphorylatedα-synuclein level can be used as a biomarker for poststro ke cognitive *** found that the phosphorylatedα-synuclein level was significantly increased and showed pathological aggregation around the cerebral infa rct area in a mouse model of ischemic *** addition,neuronalα-synuclein phosphorylation and aggregation were observed in the brain tissue of mice subjected to chronic hypoxia,suggesting that hypoxia is the underlying cause ofα-synuclein-mediated pathology in the brains of mice with ischemic *** phosphorylatedα-synuclein levels in patients with ischemic stroke were significantly lower than those in healt hy subjects,and were positively correlated with cognition levels in patients with ischemic ***,a decrease in serum high-density lipoprotein levels in stroke patie nts was significantly correlated with a decrease in phosphorylatedα-synuclein *** ischemic stroke mice did not show significant cognitive impairment or disrupted lipid metabolism 14 days after injury,some of them exhibited decreased cognitive function and reduced phosphorylatedα-synuclein *** together,our results suggest that serum phosphorylatedα-synuclein is a potential biomarker for poststroke cognitive impairment.
