Objective： Infra-pyloric artery （IPA） is an important anatomical landmark in treatment of gastric cancer and is the key vessel for pylorus-preserving gastrectomy and subgroup of infra-pyloric lymph nodes. However, its anatomical variation is not thoroughly understood. Our study aimed to clarify the origination of the IPA. Methods： We did this prospective, multicenter, open-label, observational study at gastric surgery departments of 34 hospitals in China. Gastric cancer patients aged 18 years or older and scheduled to undergo elective total or distal gastrectomy were assigned. During the surgery, IPA dissecting and exposing the origination point with photographs or video clips were required. The primary outcome was the origination of the IPA. Analysis of variance, χ2 tests and Fisher＇s tests were used to analyze the differences between groups. The study is registered at *** （No. NCT03071237）. Results： Between May 8 and July 31, 2017, 429 patients were assigned for the study, and 419 （97.7%） patients had the IPA dissected and recorded through photograph or video and were included in the primary outcome analysis. The median age was 62 years old, and 73.7% were male. Among the patients, 78.5% received laparoscopic surgery. Single IPA origination was identified in 398 （95.0%） patients, including gastroduodenal artery （GDA） in 154 （36.8%） patients, anterior superior pancreaticoduodenal artery （ASPDA） in 130 （31.0%） patients, and right gastroepiploic artery （RGEA） in 114 （27.2%） patients. Fifteen （3.6%） patients were identified with multiple IPA and 6 （1.4%） patients were identified as IPA absence. The differences in the distribution of surgical approach （P=0.003） and geographic area （P=0.030） were statistically significant. No difference was shown in sex, age, gastrectomy type, tumor location, and clinical T, N and M stage. Type 1 diabetes mellitus is heterogeneous in many facets. The patients suffered from type 1 diabetes present several levels of islet function as well as variable number and type of islet-specific autoantibodies. This study was to investigate prevalence and heterogeneity of the islet autoantibodies and clinical phenotypes of type 1 diabetes mellitus; and also discussed the process of islet failure and its risk factors in Chinese type 1 diabetic patients. A total of 1,291 type 1 diabetic patients were enrolled in this study. Demographic information was collected. Laboratory tests including mixed-meal tolerance test, human leukocyte antigen alleles, hemoglobin A1 c, lipids, thyroid function and islet autoantibodies were conducted. The frequency of islet-specific autoantibody in newly diagnosed T1 DM patients(duration shorter than half year) was 73% in East China. According to binary logistic regressions, autoantibody positivity, longer duration and lower Body Mass Index were the risk factors of islet failure. As the disease developed, autoantibodies against glutamic acid decarboxylase declined as well as the other two autoantibodies against zinc transporter 8 and islet antigen 2. The decrease of autoantibodies was positively correlated with aggressive beta cell destruction. Autoantibodies can facilitate the identification of classic T1 DM from other subtypes and predict the progression of islet failure. As there were obvious heterogeneity in autoantibodies and clinical manifestation in different phenotypes of the disease, we should take more factors into consideration when identifying type 1 diabetes mellitus.

Background:Clinical outcomes of undifferentiated arthritis(UA)are diverse,and only 40%of patients with UA develop rheumatoid arthritis(RA)after 3 *** predictive markers at disease onset for further intervention is ***,our objective was to analyze the clinical outcomes of UA and ascertain the predictors for RA ***:We performed a prospective,multi-center study from January 2013 to October 2016 among Chinese patients diagnosed with UA in 22 tertiary-care *** and serological parameters were obtained at ***-up was undertaken in all patients every 12 weeks for 2 *** factors of disease progression were identified using multivariate Cox proportional hazards ***:A total of 234 patients were recruited in this study,and 17(7.3%)patients failed to follow up during the *** the 217 patients who completed the study,83(38.2%)patients went into *** patients who developed RA had a higher rheumatoid factor(RF)-positivity(42.9%vs.16.8%,χ^2=8.228,P=0.008),anti-cyclic citrullinated peptide(CCP)antibodypositivity(66.7%vs.10.7%,χ^2=43.897,P<0.001),and double-positivity rate of RF and anti-CCP antibody(38.1%vs.4.1%,χ^2=32.131,P<0.001)than those who did ***-CCP antibody but not RF was an independent predictor for RA development(hazard ratio 18.017,95%confidence interval:5.803–55.938;P<0.001).Conclusion:As an independent predictor of RA,anti-CCP antibody should be tested at disease onset in all patients with UA.

High-quality Bi2-xPbxSr2 CaCu2O8+δ(Bi2212) single crystals have been successfully grown by the traveling solvent floating zone technique with a wide range of Pb substitution(x = 0–0.8).The samples are characterized by transmission electron microscope(TEM) and measured by high resolution laser-based angle-resolved photoemission spectroscopy(ARPES) with different photon energies.A systematic evolution of the electronic structure and superstructure with Pb substitution has been revealed for the first *** superstructure shows a significant change with Pb substitution and the incommensurate modulation vector(Q) decreases with increasing Pb *** the meantime, the superstructure intensity from ARPES measurements also decreases dramatically with increasing Pb *** superstructure in Bi2212 can be effectively suppressed by Pb substitution and it nearly disappears with a Pb substitution of x = *** also find that the superstructure bands in ARPES measurements depend sensitively on the photon energy of lasers used;they can become even stronger than the main band when using a laser photon energy of 10.897 *** results provide important information on the origin of the incommensurate superstructure and its control and suppression in bismuth-based high temperature superconductors.

To the Editor:Gastric cancer is one of the most common cancers in *** is mainly found in the middle/lower part of the stomach,[1]where it commonly metastasizes from the infrapyloric lymph node(No.6).Previous studies reported that the risk of No.6 lymph node metastasis was different based on tumor *** lower gastric cancer patients,the metastatic rate of No.6 lymph node can reach up to 18.7%,and in upper gastric cancer,the rate is merely 1.9%.[2]The distance between the primary tumor and the pylorus was proved to be related to No.6 lymph node ***,most of the studies were retrospective and their lymph nodes dissection’s quality control was controversial.

Effects of glycine on plasma and liver tissue changes of TNF-α,ET-1 and nitric oxide contents in rats with obstructive jaundice

AIM: To evaluate the effect of glycine on plasma and liver tissue changes of tumor necrosis factor-α (TNF-α),endothelin-1 (ET-1) and nitric oxide (NO) contents in rats with obstructive ***: Ninety healthy Wistar rats of both sexes weighing 275±25 g were randomly divided into shamoperated, bile duct-ligated, and bile duct-ligated plus glycine-treated groups, the latter was performed with 5 %glycine solution substituting for tap water drunk ad libium for 5 days before and 6 days after operation. Blood and liver tissue were sampled at the time of sacrifice on the 8th day post operation. Plasma total bilirubin, endotoxin, levels,as well as TNF-α, ET-1 and NO contents in liver tissue were ***: Plasma endotoxin and total bilirubin levels were significantly higher in both bile duct-ligated and bile ductligated plus glycine-treated rats than in sham-operated animals (P=0.000613, 0.00921 and 0.00737, 0.00841respectively), whereas they did not display any statistically significant difference between the former groups (P=0.417and 0.374 respectively). likewise, TNF-α, ET-1 and NO contents in both plasma and liver tissue were significantly increased in both bile duct-ligated and bile duct-ligated plus glycine-treated rats compared with sham-operated animals (P=-0.00813, 0.00793, 0.00671, 0.00804, 0.00872, and 0.00947 in plasma and 0.00531, 0.00785, 0.00912, 0.00981and 0.00635 in liver tissue respectively). However, these inflammatory mediators in both plasma and liver tissue were significantly reduced in bile duct-ligated rats fed on 5 %glycine solution compared with that without (P=0.00953,0.00891, 0.0795, 0.00867, 0.0697 and 0.00907 in plasma and liver tissue respectively).CONCLUSION: Reduction of TNF-α, ET-1 and NO contents in plasma and liver tissue of rats fed on glycine may be helpful to alleviate pathological lesions in obstructive jaundice.

Quantum technology establishes a foundation for secure communication via quantum key distribution （QKD）. In the last two decades, the rapid development of QKD makes a global quantum communication network feasible. In order to construct this network, it is economical to consider small-sized and low-cost QKD payloads, which can be assembled on satellites with different sizes, such as space stations. Here we report an experimental demonstration of space-to-ground QKD using a small-sized payload, from Tiangong-2 space lab to Nanshan ground station. The 57.9-kg payload integrates a tracking system, a QKD transmitter along with modules for synchronization, and a laser communication transmitter. In the space lab, a 50MHz vacuum＋weak decoy-state optical source is sent through a reflective telescope with an aperture of 200mm. On the ground station, a telescope with an aperture of 1200mm collects the signal photons. A stable and high-transmittance communication channel is set up with a high-precision bidirectional tracking system, a polarization compensation module, and a synchronization system. When the quantum link is successfully established, we obtain a key rate over 100bps with a communication distance up to 719km. Together with our recent development of QKD in daylight, the present demonstration paves the way towards a practical satellite-constellation-based global quantum secure network with small-sized QKD payloads.
