Background： The Food and Drug Administration recently announced that the use of morcellation may cause fibroids or pelvic dissemination and metastasis of uterine sarcoma; therefore, the use of morcellation is limited in the USA. A large sample study is necessary to assess the proportion of uterine malignant tumors found in patients with laparoscopic myomectomy. Methods： A national multicenter study was performed in China. From 2002 to 2014, 33,723 cases were retrospectively selected. We calculated the prevalence and recorded the clinical characteristics of the patients with malignancy after morcellation application. A total of 62 cases were finally pathologically confirmed as malignant postoperatively. Additionally, the medical records of the 62 patients were analyzed in details. Results： The proportion of postoperative malignancy after morcellation application was 0.18% （62/33,723） for patients who underwent laparoscopic myomectomy. Nearly 62.9% （39/62） of patients had demonstrated blood flow signals in the uterine fibroids before surgery. And, 23 （37.1%） patients showed rapid growth at the final preoperative ultrasound. With respect to the pathological types, 38 （61.3%） patients had detectable endometrial stromal sarcoma, 13 （21.0%） had detectable uterine leiomyosarcoma, only 3 （3.2%） had detectable carcinosarcoma, and 5 （8.1%） patients with leiomyoma had an undetermined malignant potential. Conclusions： The proportion of malignancy is low after using moreellation in patients who undergo laparoscopie myomectomy. Patients with fast-growing uterine fibroids and abnormal ultrasonic tumor blood flow should be considered for malignant potential, and morcellation should be avoided.

Inter-and intra-tumoral heterogeneity is a hallmark of glioblastoma(GBM)that facilitates recurrence and treatment *** only gene expression profiles or mutations but also epigenetic states dictate GBM biology.O-methylguanine-DNA methyltransferase(MGMT)promoter methylation is a significant prognostic marker for GBM patients’ survival,however,underlying mechanisms by which MGMT epigenetics impacts patient outcomes are not fully ***,using in vitro cultures and in vivo orthotopic models of IDH1/2-wild-type GBM stem-like cells(GSCs)we show that MGMT promoter methylation status,but not molecular subtypes of GBM,is tightly associated with contrasting roles of ***-40 promotes the RAS-MEK-ERK signaling pathway and stemness,chemoresistance and tumorigenicity in MGMT-unmethylated *** contrast,YKL-40 suppresses the RAS-MEK-ERK signaling pathway and the stem-like properties in MGMT-methylated *** study demonstrates previously underappreciated epigenetic status association with distinct functions of cancer-relevant genes,and supports the need of epigenome characterization into tumor stratification and targeted therapeutic development.

目的:应用微循环阻力指数(IMR)评价非心肌梗死冠状动脉性心脏病(冠心病)患者的心肌微循环状态,并分析影响IMR的相关因素。方法:连续纳入101例非心肌梗死冠心病患者,在血流储备分数(FFR)排除功能性病变或行经皮冠状动脉介入治疗(PCI)后即刻进行IMR检测。以IMR≥25 U定义为冠状动脉微血管疾病,分析IMR异常的发生率和影响因素。结果:共在116支冠状动脉靶血管中测量了IMR,IMR为14.2(11.4,19.9)U,17例(16.8%)患者IMR≥25 U。高血压、服用钙拮抗剂(CCB)的患者IMR较高(P均<0.05);服用他汀类药物和左心室舒张末期内径(LVEDD)增大的患者IMR较低(P均<0.05)。IMR与空腹血糖、高密度脂蛋白胆固醇、FFR呈正相关(r=0.203、0.203、0.207,P均<0.05),而与左心室质量指数(LVMI)呈负相关(r=-0.205,P<0.05)。多重线性回归分析提示,空腹血糖、LVEDD、他汀类药物、CCB为IMR的独立相关因素(β=1.177,P=0.003;β=-7.015,P=0.002;β=-8.568,P<0.001;β=3.250,P=0.017)。结论:近17%的非心肌梗死冠心病患者存在IMR反映的冠状动脉微血管疾病;空腹血糖、LVEDD、应用他汀类药物及CCB可能是影响IMR的独立相关因素。

目的：分析存在冠状动脉病变的患者空腹血糖水平与冠状动脉病变复杂程度的关系。方法回顾性收集2009年1月至2011年1月于北京大学第三医院疑诊为冠心病并行冠状动脉造影（CAG）证实存在冠状动脉病变的929例患者的临床和CAG资料。根据SYNTAX积分对患者进行分组，应用双变量相关分析，多元逐步回归分析和Logistic回归分析空腹血糖水平与冠状动脉SYNTAX积分之间的相关性。结果①929例患者中，以SYNTAX积分分为：低危组（积分＜22分）47例，中危组（22分≤积分＜33分）189例，高危组（积分≥33分）693例。组间比较显示，患者年龄、空腹血糖水平、空腹血糖异常和既往糖尿病患者比例，差异具有统计学意义。②相关分析提示， SYNTAX积分与空腹血糖呈相关性（r=0.167，P=0.000）。亚组分析则显示，仅在无糖尿病史患者中，SYNTAX积分与空腹血糖之间存在相关性（r=0.149，P=0.000）；而在既往糖尿病史患者中，两者不具有相关性。③多元逐步回归分析显示，空腹血糖水平与SYNTAX积分独立相关（β=0.452， P=0.002）。亚组分析显示，仅在既往无糖尿病史亚组中，空腹血糖水平与SYNTAX积分独立相关（β=1.039，P=0.000）。④将SYNTAX中危组和高危组合并为中高危组，以SYNTAX积分作为因变量，以性别、年龄、高血压、糖尿病、高脂血症、吸烟、空腹血糖为自变量，进行Logistic回归分析，在整组和无糖尿病史亚组中均筛选出两个变量即：年龄（整组OR 1.033，95%CI 1.017～1.049， P=0.000；无糖尿病史组OR 1.039，95%CI 1.020～1.059，P=0.000）和空腹血糖水平（整组OR 1.114，95%CI 1.038～1.195，P=0.003；无糖尿病史组OR 1.299，95%CI 1.088～1.387，P=0.001）。结论存在冠状动脉病变的患者空腹血糖水平可能反映冠状动脉病变的复杂程度，具有预测中高SYNTAX积分的作用，尤其适用于无糖尿病史患者。

The Circular Electron Positron Collider(CEPC)is a large scientific project initiated and hosted by China,fostered through extensive collaboration with international *** complex comprises four accelerators:a 30 GeV linac,a 1.1 GeV Damping Ring,a Booster capable of achieving energies up to 180 GeV,and a Collider operating at varying energy modes(Z,W,H,and tt).The linac and Damping Ring are situated on the surface,while the subterranean Booster and Collider are housed in a 100 km circumference underground tunnel,strategically accommodating future expansion with provisions for a potential Super Proton Proton Collider(SPPC).The CEPC primarily serves as a Higgs *** its baseline design with synchrotron radiation(SR)power of 30 MW per beam,it can achieve a luminosity of 5×10^(34)cm^(-2)s^(-1)per interaction point(IP),resulting in an integrated luminosity of 13 ab^(-1)for two IPs over a decade,producing 2.6 million Higgs *** the SR power to 50 MW per beam expands the CEPC's capability to generate 4.3 million Higgs bosons,facilitating precise measurements of Higgs coupling at sub-percent levels,exceeding the precision expected from the HL-LHC by an order of *** Technical Design Report(TDR)follows the Preliminary Conceptual Design Report(Pre-CDR,2015)and the Conceptual Design Report(CDR,2018),comprehensively detailing the machine's layout,performance metrics,physical design and analysis,technical systems design,R&D and prototyping efforts,and associated civil engineering ***,it includes a cost estimate and a preliminary construction timeline,establishing a framework for forthcoming engineering design phase and site selection *** is anticipated to begin around 2027-2028,pending government approval,with an estimated duration of 8 *** commencement of experiments and data collection could potentially be initiated in the mid-2030s.

目的探讨紫龙金片药物暴露与肺癌患者生存期之间的相关性,为紫龙金片的临床应用提供真实世界证据。方法以课题组30家参研单位常规收集的医疗健康数据和基于研究目的主动收集的数据构成研究数据库,以是否服用紫龙金片作为暴露划分因素,提取患者人口学、肺癌诊断、治疗、中医辨证、症状、合并治疗等信息,采用Kaplan-Meier法进行单因素分析,采用回归模型控制混杂因素,Cox模型法进行多因素生存分析。结果研究纳入2553例患者资料,暴露队列各访视点总生存率均高于非暴露队列;Kaplan-Meier生存分析显示,暴露队列估计平均生存时间高于非暴露队列(35.9个月vs 25.0个月,P<0.001)。Cox生存分析显示,性别、年龄、临床分期、神疲乏力评分、肺气虚证、原发病灶瘤径是死亡终点事件的独立危险因素(P<0.001),手术治疗和紫龙金暴露为独立保护因素(P<0.001)。结论基于当前数据及研究方法,紫龙金片暴露对于肺癌患者的生存期表现出较为明显的保护作用。

目的探讨静息状态下冠脉狭窄远端压力与主动脉根部或冠脉口部压力的比值(Pd/Pa)与血流储备分数(FFR)之间的关系。方法回顾性分析在北京大学第三医院住院的84例患者中接受冠脉内压力导丝评估的103处病变的数据。多体位选择性冠脉造影,QCA评价病变直径狭窄程度。结果静息Pd/Pa值与FFR具有线性相关性(r=0.78;P<0.0001),以FFR值≤0.75来定义为缺血临界值时,当静息Pd/Pa≤0.86时,其预测缺血的阳性预测值可达95%,而当Pd/Pa值≥0.93时,其预测缺血阴性的预测值也达95%。结论静息Pd/Pa值与FFR具有明显的相关性,通过静息Pd/Pa值预测FFR缺血界值具有较高的阳性和阴性预测值,这提示在压力-病变功能评估中部分患者能够避免使用腺苷等微循环扩张剂,达到减少药物相关反应和手术过程的目的。

目的评价瞬时无波形比值(iFR)与血流储备分数(FFR)的相关性及影响诊断一致性的因素。方法回顾性收集北京大学第三医院2015年5月至2019年4月接受iFR和FFR检测的冠心病患者,探讨二者的相关性及影响其诊断一致性的因素。结果收集60例患者80支冠状动脉的109组FFR和iFR数据,FFR和iFR中位数分别为0.80(0.74,0.85)和0.90(0.87,0.94),二者呈线性相关(r=0.780,P0.89且FFR≤0.80组的参考血管直径[(3.06±0.75)mm比(2.54±0.67)mm]、最小管腔直径[1.84(1.42,2.21)mm比1.35(1.17,1.52)mm]和最小管腔面积[2.65(1.58,3.83)mm2比1.42(1.08,1.83)mm2]均较大(均P<0.05)。结论iFR与FFR具有良好的相关性,病变局部解剖特点可能对二者的诊断一致性产生一定影响。
