BACKGROUND:Remimazolam is a novel ultra-short-acting sedative,but its safety and adverse events(AEs)in high-risk patients in the intensive care unit(ICU)setting remain ***:This was a single-center,retrospective study that compared remimazolam to propofol and midazolam in patients undergoing upper gastrointestinal *** primary outcome was the incidence of treatment-related *** secondary outcomes were the time to extubation,the length of ICU stay,and the average cost of sedative per ***:Of the 88 patients analyzed,47 were treated with remimazolam(mean dose,7.90±4.84mg),and 41 were treated with propofol(21.19±17.98 mg)or midazolam(3.08±2.17 mg).There was no statistically significant difference in the average duration of the endoscopic procedure(35.89±13.37 min vs.44.51±21.68 min,P=0.133)or the time to extubation(15.00±9.75 h vs.20.59±18.71 h,P=0.211)in the remimazolam group(group I)compared to the propofol or midazolam group(group II).ICU stays(5.40±2.93 d vs.4.63±3.31 d,P=0.072)and treatment-related AEs(48.61%vs.51.38%,P=0.056)were similar between *** average cost of sedative per case was significantly lower in the group I than in the group II(RMB 16.07±10.58 yuan *** 24.37±15.46 yuan,P=0.016).CONCLUSION:Remimazolam-based sedation was noninferior to the classic sedatives and had lower average cost per case,indicating that it may be used as a promising sedative for high-risk patients during endoscopic procedures in the ICU setting.

This paper presents an energy resolution study of the JUNO experiment,incorporating the latest knowledge acquired during the detector construction *** determination of neutrino mass ordering in JUNO requires an exceptional energy resolution better than 3% at 1 *** achieve this ambitious goal,significant efforts have been undertaken in the design and production of the key components of the JUNO *** factors affecting the detection of inverse beta decay signals have an impact on the energy resolution,extending beyond the statistical fluctuations of the detected number of photons,such as the properties of the liquid scintillator,performance of photomultiplier tubes,and the energy reconstruction *** account for these effects,a full JUNO simulation and reconstruction approach is *** enables the modeling of all relevant effects and the evaluation of associated inputs to accurately estimate the energy *** results of this study reveal an energy resolution of 2.95% at 1 ***,this study assesses the contribution of major effects to the overall energy resolution *** analysis serves as a reference for interpreting future measurements of energy resolution during JUNO data ***,it provides a guideline for comprehending the energy resolution characteristics of liquid scintillator-based detectors.

Continual evolution of the severe acute respiratory syndrome coronavirus(SARS-CoV-2)virus has allowed for its gradual evasion of neutralizing antibodies(nAbs)produced in response to natural infection or *** rapid nature of these changes has incited a need for the development of superior broad nAbs(bnAbs)and/or the rational design of an antibody cocktail that can protect against the mutated virus ***,we report two angiotensin-converting enzyme 2 competing nAbs—8H12 and 3E2—with synergistic neutralization but evaded by some Omicron ***-electron microscopy reveals the two nAbs synergistic neutralizing virus through a rigorous pairing permitted by rearrangement of the 472-489 loop in the receptor-binding domain to avoid steric *** antibodies based on these two nAbs tremendously extend the neutralizing breadth and restore neutralization against recent variants including currently dominant ***,these findings expand our understanding of the potential strategies for the neutralization of SARS-CoV-2 variants toward the design of broad-acting antibody therapeutics and vaccines.

It's a challenge for detecting the therapeutic targets of a polypharmacological drug from variations in the responsed networks in thedfferentiated populations with complex diseases,as stable coronary heart ***,in an adaptive,31-center,randomized,double-blind trial invoving 920 patients with moderate symptomatic stable angina treated by 14-day Danhong injection(DHI),a kind of polyphamacological drug with high quality control,or placebo(0.9%saline),with 76-day following-up,we firstly confrmed that DHl couldincrease the proportion of patients with clinically significant changes on angina-frequency assessed by Seattle Angina Questionnaire(ASAQ-AF220)(12.78%at Day 30,95%confidence interval[C]5.86-19.71%,P=0.0003,13.82%at Day 6C0,95%CI 6.82-20.82%,P=0.0001and 8.95%at Day 90,95%CI 2.06-15.85%,P=0.01).We also found that there were no significant differences in new-onset major vascularevents(P=0.8502)and serious adverse events(P=0.9105)between DHl and *** performing the RNA sequencing in 62 selectedpatients,we developed a systemic modular approach tp identfy differentilly expressed modules(DEMs)of DHI with the Z_(summay)valueless than 0 compared with the control group,calculated by weighted gene co-expression network analysis(WGCNA),and sketched out thebasic framework on a modular map with 25 functional modules targeted by ***,the effective therapeutic module(ETM),defined as the highest corelation value with the phenotype alteration(SAQ-AF,the change in SAQ-AF at Day 30 from baseline)calculatedby WGCNA,was identifed in the population with the best effect(ASAQ-AF240),which is related to anticoagulation and regulation ofcholesterol *** assessed the modular flexbility of this ETM using the global topological D value based on Euclidean distance,which is corelated with phenotype alteration(r^(2):0.8204,P=0.019)by linear *** study identified the ant-angina therapeuticmodule in the effective population treated by the multi-target

The ultra-high-energy（UHE） gamma-ray source 1LHAASO J0007+7303u is positionally associated with the composite SNR CTA1 that is located at high Galactic Latitude b ≈ 10.5°. This provides a rare opportunity to spatially resolve the component of the pulsar wind nebula（PWN） and supernova remnant（SNR） at UHE. This paper conducted a dedicated data analysis of 1LHAASO J0007+7303u using the data collected from December 2019 to July 2023. This source is well detected with significances of 21σ and 17σ at 8-100 TeV and >100 TeV, respectively. The corresponding extensions are determined to be 0.23°±0.03°and 0.17°±0.03°. The emission is proposed to originate from the relativistic electrons accelerated within the PWN of PSR J0007+7303. The energy spectrum is well described by a power-law with an exponential cutoff function dN/dE =（42.4 ± 4.1）(E/（20 TeV）)-2.31±0.11exp(-E/（110±25 TeV）) TeV-1cm-2s-1in the energy range from 8 to 300 TeV, implying a steady-state parent electron spectrum dNe/dEe∝(Ee/（100 TeV）)-3.13±0.16exp[(-Ee/（373±70 TeV）)2] at energies above ≈ 50 TeV. The cutoff energy of the electron spectrum is roughly equal to the expected current maximum energy of particles accelerated at the PWN terminal shock. Combining the X-ray and gamma-ray emission, the current space-averaged magnetic field can be limited to≈ 4.5 μG. To satisfy the multi-wavelength spectrum and the γ-ray extensions, the transport of relativistic particles within the PWN is likely dominated by the advection process under the free-expansion phase assumption.
