Objective:Real-world diagnostic and treatment data for pancreatic cancer in China are *** such,the present study investigated the clinical characteristics,diagnosis,and treatment of advanced pancreatic cancer(including locally advanced and metastatic disease)in the Hospital-based Advanced Pancreatic Cancer Cohort in China of the China Pancreas Data Center ***:A total of 5349 Chinese patients with advanced pancreatic cancer were identified from a *** entire course of real-world pancreatic cancer management was ***:The proportion of patients with advanced pancreatic cancer was higher among males than females(62.4%vs 37.6%,respectively).Patients typically had a history of hypertension(30.8%),diabetes(21.6%),and cholangitis(20.2%).Abdominal pain(51.6%),abdominal distension(27.1%),jaundice(20.1%),and weight loss(16.3%)were the main symptoms observed in patients with advanced pancreatic cancer in this *** carbohydrate antigen(CA)19-9 is one of the most common tumor *** the present study,2562 patients underwent first-line *** median progression-free survival(PFS)for patients undergoing first-line therapy was 4.1 *** major options for first-line therapy included gemcitabine(GEM)plus S-1(GS/X)(23.4%),nab-paclitaxel plus GEM(AG)(18.1%),oxaliplatin,irinotecan,and leucovorin-modulated fluorouracil(FOLFIRINOX;11.9%),nab-paclitaxel plus S-1(AS)(8.9%),and GEM combined with oxaliplatin/cisplatin(GEMOX/GP)(7.6%).The AS and GS/X regimens were associated with the highest PFS ***:This is the first study to report multicenter,real-world data regarding advanced pancreatic cancer in *** revealed that real-world treatment options differed from guideline recommendations,and PFS was shorter than that in previously reported *** intelligent follow-up systems and standardizing diagnosis and treatment of pancreatic cancer is recommended.

Background:Current treatment options for human epidermal growth factor receptor 2(HER2)-overexpressing gastric cancer at third-line have shown limited clinical ***,there is no specific treatment for HER2 immunohistochemistry(IHC)2+and fluorescence in-situ hybridization-negative ***,we report the efficacy and safety of a novel anti-HER2 antibody RC48 for patients with HER2-overexpressing,advanced gastric or gastroesophageal junction ***:Patients with HER2-overexpressing(IHC 2+or 3+),locally advanced or metastatic gastric or gastroesophageal junction cancer who were under at least second-line therapy were eligible and received RC482.5 mg/kg alone every 2 *** primary endpoint was the objective response rate(ORR)assessed by an independent review *** endpoints included progressionfree survival(PFS),overall survival(OS),duration of response,time to progression,disease control rate,and ***:Of 179 patients screened,125 were eligible and received RC48 *** ORR was 24.8%(95%confidence interval[CI]:17.5%-33.3%).The median PFS and OS were 4.1 months(95%CI:3.7-4.9 months)and 7.9 months(95%CI:6.7-9.9 months),*** most frequently reported adverse events were decreased white blood cell count(53.6%),asthenia(53.6%),hair loss(53.6%),decreased neutrophil count(52.0%),anemia(49.6%),and increased aspartate aminotransferase level(43.2%).Serious adverse events(SAEs)occurred in 45(36.0%)patients,and RC48-related SAEs were mainly decreased neutrophil count(3.2%).Seven patients had adverse events that led to death were not ***:RC48 showed promising activity with manageable safety,suggesting potential application in patients with HER2-overexpressing,advanced gastric or gastroesophageal junction cancer who have previously received at least two lines of chemotherapy.

胆道癌恶性程度高,预后极差,5年生存率不足5%。手术切除是目前胆道系统肿瘤唯一可能治愈的方法,但存在根治性切除率低、术后易复发等难题。围手术期治疗主要针对局部晚期恶性肿瘤,以放化疗为主要手段,能够提高手术切除率,防止疾病复发,最终达到延长生存期的目的。目前已有多项胆道癌围手术期治疗模式的临床研究结果公布,为局部晚期胆道癌的综合治疗带来了新的选择。本文围绕新辅助治疗、辅助治疗和转化治疗三部分就局部晚期胆道癌的围手术期治疗热点及研究现状进行综述。

目的观察重组人p53腺病毒注射液联合顺铂与紫杉醇治疗晚期或复发宫颈癌的临床疗效。方法 80例晚期宫颈癌患者随机分为对照组和试验组,每组40例。对照组予紫杉醇联合顺铂化疗,试验组予重组人p53腺病毒注射液联合顺铂与紫杉醇治疗。观察2组的客观缓解率(ORR)、疾病控制率(DCR)、无进展生存时间(PFS)和不良反应。结果试验组和对照组的ORR分别为55. 0%和32. 5%,DCR分别为75. 0%和60. 0%,差异均有统计学意义(P 0. 05)。结论重组人p53腺病毒注射液联合紫杉醇和顺铂可改善晚期及复发宫颈癌患者的临床疗效,且未增加不良反应,值得临床推广。

Adenosquamous carcinoma of the pancreas(ASCP)is a rare histological subtype of pancreatic cancer with a poor prognosis and a high metastasis ***,little is known about its genomic landscape and prognostic biomarkers.A total of 48 ASCP specimens and 98 pancreatic ductal adenocarcinoma(PDAC)tumour specimens were sequenced to explore the genomic landscape and prognostic *** homozygous deletion of the 9p21.3 region(including CDKN2A,CDKN2B,and MTAP)(9p21 loss)occurred in both ASCP and PDAC,and a higher frequency of 9p21 loss was observed in ASCP(12.5%vs 2.0%,P=0.022).Notably,9p21 loss was significantly associated with poor disease-free survival(DFS)in ASCP patients(mDFS(Median DFS)=4.17 vs 7.33 months,HR(Hazard Ratio)=3.70,P=0.009).The most common gene alterations in patients with ASCP were KRAS(96%),TP53(81%),CDKN2A(42%),SMAD4(21%),CDKN2B(13%),and FAT3(13%).The mutation rates of ACVR2A(6.25%vs 0%),FANCA(6.25%vs 0%),RBM10(6.25%vs 0%),and SPTA1(8.33%vs 1.02%)were significantly higher in ASCP than in *** conclusion,we have comprehensively described the genomic landscape of the largest cohort of ASCP patients to date and highlight that 9p21 loss may be a promising prognostic biomarker for ASCP,which provides a molecular basis for prognosis prediction and new therapeutic strategies for ASCP.

目的:在肿瘤学的临床教学中引入多学科综合治疗(multi-disciplinary treatment,MDT)模式,探讨其相对于传统教学模式的优势。方法:选取46名在肿瘤科实习的肿瘤学专业研究生、规范化培训学生或进修医生,随机分组后分别采取MDT教学模式和传统教学模式进行培养,待实习结束后对两种教学模式的教学质量和教学满意度进行比较。结果:MDT教学组无论理论课考试成绩还是临床综合能力成绩均明显高于传统教学对照组(P<0.05);MDT教学组的学生在提高临床思维能力、激发自身学习兴趣、增强自主学习能力、增加交流与合作能力、提高表达能力和加深知识的理解和记忆等方面的教学满意度也显著高于传统教学组(P<0.05)。结论:在肿瘤学的临床教学中逐步推广MDT模式,可提高肿瘤学的教学质量和教学满意度。

目的:研究同源异形盒基因D10(homeobox D10,HOXD 10)在人胆管癌组织中的启动子甲基化状态,并探讨HOXD 10基因启动子甲基化与胆管癌患者预后的关系。方法:采用甲基化特异性PCR法检测87例人胆管癌组织与30例癌旁组织中HOXD 10基因启动子的甲基化情况,并进一步采用重亚硫酸盐测序法检测HOXD 10基因启动子中27个胞嘧啶鸟嘌呤二核苷酸(cytosinephosphateguanosine,CpG)位点的甲基化程度。然后,应用Spearman相关检验分析HOXD 10基因启动子甲基化与其蛋白表达的相关性,用cutpoint生存分析和Kaplan-Meier法检验HOXD 10基因启动子区CpG位点甲基化数目与胆管癌患者预后的关系。结果:胆管癌组织中HOXD 10基因启动子甲基化率明显高于癌旁组织(P<0.05),而且癌旁组织中HOXD 10启动子区域的CpG位点数目少于胆管癌组织。HOXD10蛋白表达和HOXD 10基因启动子甲基化在胆管癌中呈显著负相关性(r=-0.82,P<0.000 1)。HOXD 10基因启动子区有7个CpG位点甲基化是胆管癌患者生存分析的最佳"截点"(P<0.001),CpG位点数目为0～6个的胆管癌患者生存时间明显长于CpG位点数目≥7个的患者(P<0.01)。结论:基因启动子甲基化与HOXD10蛋白的表达失活密切相关,HOXD 10基因启动子区存在≥7个CpG位点甲基化可作为胆管癌患者预后不良的预测指标之一。

目的:研究非甾体类抗炎药印甲薪对人乳腺癌细胞株MCF-7体外侵袭能力的影响。方法:通过细胞增殖抑制实验确定可应用于后续实验的印甲薪药物浓度,应用Transwell细胞体外侵袭实验研究印甲薪对MCF-7细胞移行及浸润能力的影响。结果:在安全浓度范围内,随着印甲薪浓度的增加,MCF-7细胞的移行能力无明显变化,但浸润能力明显下降,与对照组相比有明显的统计学意义(P<0.05),并且浸润能力与印甲薪浓度呈负相关(r=-0.985)。结论:印甲薪可以明显抑制MCF-7细胞的体外侵袭能力,并且该抑制作用呈现浓度依赖性。

目的:探究五味子酯甲(SA)对肺癌细胞生长和转移的潜在作用机制。方法:本研究以非小细胞肺癌细胞系A549和H1299作为实验对象,将SA溶解在二甲基亚砜中,用不同剂量的SA(0μmol/L、5μmol/L、10μmol/L、20μmol/L、50μmol/L)孵育A549和H1299细胞24 h、48 h和72 h。采用细胞计数盒-8检测细胞增殖,采用FC500流式细胞仪分析细胞凋亡,采用伤口愈合实验评价细胞迁移,采用Transwell检测细胞侵袭。通过实时定量逆转录聚合酶链反应(qRT-PCR)检测细胞中CCAT1 mRNA水平;通过Western Blotting检测细胞中p-PI3K、PI3K、p-AKT、AKT、PTEN、E-cadherin、MMP2和MMP9的蛋白表达水平。结果:与未处理的细胞比较,SA以剂量和时间依赖性方式降低了A549和H1299的细胞活力(P0.05)。结论:SA通过抑制CCAT1和PI3K/AKT信号通路抑制肺癌细胞的生长和转移。
