Minimally Invasive Pericranial Flap for Reconstruction after Endonasal Endoscopic Surgery of 30 Consecutive Patients

Objectives/Hypothesis: The introduction of intranasal pedicled flaps has reduced the incidence of postoperative cerebrospinal fluid (CSF) leaks to less than 5%. Nevertheless, in malignant tumors those flaps are not always available because of nasal septum invasion. Minimally invasive pericranial flaps (PCF) are associated with minimal adverse effects and good cosmetic appearance. In spite of that, there are only a few reports of this reconstructive technic limited to short surgical series and radio-anatomical analysis. Clinical results of a surgical cohort are presented. Study Design: Cohort prospective study. Methods: Clinical data, including age, gender, stage, histopathological findings, rate of complications and appearance of PCF at fifth day and two months postoperative were recorded. Postoperative morbidities were recorded as wound abnormalities, nasosinusal, orbital and central nervous system complications. Chi-squared test was used to correlate qualitative variables and Student-t-test to correlated qualitative and quantitative variables. Items were considered statistically significant with a p value of less than 0.05 (confidence Interval of 95%). Results: Thirty patients (18 males and 12 females) were registered. Mean age was 51.5 years ± 23.0 and range between 20 and 71 years. Most common histologic subtypes were adenocarcinoma, epidermoid carcinoma and squamous cell carcinoma. Complete resection of the tumor was achieved in all patients including surgical margins. Length of the PCF varies between 9.9 cm and 13.9 cm with a mean of 11.8 cm. There was an association between length of the flaps and the covering structure with the nose apex relation. None patient experienced postoperative cerebrospinal fluid (CSF) leak, frontal sinusitis or other complications. Conclusions: Minimally invasive PCF constitute a good and inexpensive reconstructive option in patients with malignant anterior cranial base tumors in whose nasoseptal flap was not a feasible option.

Effect of BA Treatments on Morphology and Physiology of Proliferated Shoots of <i>Bambusa vulgaris</i>Schrad. Ex Wendl in Temporary Immersion

Axillary buds, collected from greenhouse-grown plants of Bambusa vulgaris Schrad. ex Wendl (B. vulgaris), were incubated on a static liquid culture medium, Murashige and Skoog (MS) medium with 2% (w/v) sucrose, and supplemented with 12.0 μM 6-benzyladenine (BA). They were transferred to a temporary immersion system (TIS) using liquid MS medium supplemented with 0 (CK-free medium), 6.0, 12.0, 18.0 μM BA. The morphological and anatomical indicators were measured. The BA influenced in vitro multiplication of B. vulgaris. The best results were achieved in the SIT with a concentration of 6.0 μM of BA, which increased the number of shoots (5.1 shoots/explant) in the absence of hyperhydric shoots. Results demonstrated that the water content in the sprouts increased with 12.0 and 18.0 μM BA every four hours. Furthermore, these high levels of BA contributed to a lower accumulation of phenolic compounds and lignin content. The total chlorophyll significantly increased when using 6.0 uM BA, but decreased both parameters with other treatments. These results favor to increase the number of shoots/explants during in vitro multiplication. They will also optimize the in vitro culture conditions, leading to an improvement of in vitro propagation methods for this species.

Contribution of Soil Lead to Blood Lead in Children: A Study from New Orleans, LA

In recent years, a significant number of environmental studies have been conducted in New Orleans, LA and surrounding Gulf Coast areas due in part to the occurrence of hurricanes Katrina and Rita. Data collected from studies in the New Orleans area indicate that inorganic contaminants including arsenic (As), iron (Fe), lead (Pb), and vanadium (V);high concentration of bioaerosols, particularly Cladosporium and Aspergillus, and several organic pollutants (PAHs, pesticides, and volatiles) may pose a risk to human health in New Orleans. While many of these results resemble historical data, a current quantitative exposure assessment has not been conducted. We engaged in one such assessment for lead (Pb) contamination in surface soils. We used Pb concentrations in surface soils ( μg/day to 102 μg/day for our study area within urbanNew Orleans. These data are concerning because children exposed to >33.5 μg/d Pb may cause their blood-Pb levels to exceed the Centers for Disease Control and Prevention (CDC) threshold for blood-Pb of 10 μg/dL. It has generally been accepted that a more protective blood Pb concentration threshold of 6 - μg/dL is warranted. Using the 6-μg/dL threshold puts children exposed to as little as 20.2 μg/day Pb at risk.

目的探讨转移性肝癌的腹腔镜肝切除术经验。方法对1997年至2004年度布里斯班医院所进行的所有肝切除术患者进行回顾性研究。结果有84例患者进行了腹腔镜肝切除,其中33例(39%)为恶性肿瘤。33例中28例为转移性,其中22例为结直肠癌转移。13例患者进行左肝外侧叶切除,9例患者进行了右半肝切除,其余6例行肝段或不规则切除。67%失去随访,追踪随访12例患者2年存活率和无瘤生存率为75%和67%。结论在高度选择过的恶性肿瘤患者中行腹腔镜肝切除术是可行的。但要求术者有丰富的开腹肝切除术经验和腹腔镜操作技能。

Major Approaches on the Piezoelectric Device, Drills and Saws to Orthognathic Surgery: A Systematic Review

Introduction: The incidence of lesions of sensory and motor nerves associated with oral and maxillofacial surgery is often underestimated, representing around 32.0% of all lesions, and orthognathic surgery contributes 12.3% to these lesions. Thus, the use of piezosurgery (PiZCi) has been indicated in several procedures within oral and maxillofacial surgery due to a number of advantages over conventional drill and saw techniques. Objective: to analyze the main literature on the use of the piezoelectric device in relation to the use of drills and to orthognathic surgery in dentistry. Methods: A total of 41 articles were found involving Orthognathic Surgery, Piezoelectric Device, Drills Device, Saws Device, and Efficiency. Initially, it was held the exclusion existing title and duplications following the interest described this work. After this process, the summaries were evaluated, and a new exclusion was held. 29 articles were included and discussed in this study. Initially, the keywords were determined by searching the DeCS tool (Descriptors in Health Sciences, BIREME base) and later verified and validated by MeSh system (Medical Subject Headings, the US National Library of Medicine). Results and conclusion: It was concluded that the piezoelectric device has excelled with an effective surgical technique, being able to be indicated in the most diverse areas of the oral surgery, providing accurate, clean and minimally traumatic osteotomies for the soft tissues, intraoperative bleeding, when compared to conventional surgery using drills and/or saws.
