OBJECTIVES: We sought to investigate the effect of cardiac medication on long-term mortality in patients with peripheral arterial disease(PAD). BACKGROUND: Peripheral arterial disease is associated with increased cardiovascular morbidity and mortality. Treatment guidelines recommend aggressive management of risk factors and lifestyle modifications. however, the potential benefit of cardiac medication in patients with PAD remains ill defined. METhODS: In this prospective observational cohort study, 2,420 consecutive patients(age, 64± 11 years, 72% men) with PAD(ankle-brachial index ≤ 0.90) were screened for clinical risk factors and cardiac medication. Follow-up end point was death from any cause. Propensity scores for statins, beta-blockers, aspirin, angiotensin- converting enzyme(ACE) inhibitors, calcium channel blockers, diuretics, nitrates, coumarins, and digoxin were calculated. cox regression models were used to analyze the relation between cardiac medication and long-term mortality. RESULTS: Medical history included diabetes mellitus in 436 patients(18% ), hypercholesterolemia in 581(24% ), smoking in 837(35% ), hypertension in 1,162(48% ), coronary artery disease in 1,065(44% ), and a history of heart failure in 214(9% ). Mean ankle-brachial index was 0.58(± 0.18). During a median follow-up of eight years, 1,067 patients(44% ) died. After adjustment for risk factors and propensity scores, statins(hazard ratio [hR] 0.46, 95% confidence interval [CI] 0.36 to 0.58), beta-blockers(hR 0.68, 95% CI 0.58 to 0.80), aspirins(hR 0.72, 95% CI 0.61 to 0.84), and ACE inhibitors(hR 0.80, 95% CI 0.69 to 0.94)were significantly associated with a reduced risk of long-term mortality. CONCLUSIONS: On the basis of this observational longitudinal study, statins, beta-blockers, aspirins, and ACE inhibitors are associated with a reduction in long-term mortality in patients with PAD.

介绍针对热带气旋的风场、波浪、风暴潮的NOPP(国家海洋合作项目)实时预报系统。这个系统的低分辨率模式曾被用于2004年和2005年的飓风季节预测,国家飓风中心每6 h在内部网上发布飓风路径、极限半径与特征浪高的预测值。这个预测系统通过输入来自h*WIND的高精度初始风场数据来预测以后每半小时的风场预测数据。改进后的风场数据被用于追踪飓风路径以及驱动三阶的波浪模块和风暴潮模块。而波浪模块得出的辐射压力场则被耦合于飓风登陆时的海岸风暴潮位预测中。此系统正在采用低分辨率与高分辨率模式模拟2006年飓风季节情况。

Tumor Volume Associated with Recurrence in Prostate Cancer Patients with Seminal Vesicle Invasion

Objectives: To examine predictors of progression-free survival in men with seminal vesicle invasion (SVI) following radical prostatectomy (RP) for clinically localized prostate cancer. Methods and Materials: Between 1999 and 2009, 1383 men underwent RP at Indiana University. Among them, 115 men were identified with SVI. Disease progression was defined by a rise in PSA ≥ 0.2 ng/ml, receipt of salvage therapy, progression to metastatic disease, or death. After excluding 13 patients for receiving adjuvant therapy, 102 were stratified according to surgical margin (SM) and lymph node (LN) status for Kaplan-Meier analysis of disease progression. cox proportional hazards analyses of biochemical progression-free survival were undertaken with respect to margin status, pre-operative prostate specific antigen (PSA), tumor volume, age, and post-operative Gleason sum. Stem and leaf plot was undertaken for tumor volume by biochemical PFS. Results: Mean age was 61 years, median Gleason sum was 7, mean tumor volume was 9.7 ml, and mean pre-operative PSA was 13.6 ng/ml. Mean time to disease progression was 17 months. Mean follow-up was 37 months. Kaplan-Meier analysis revealed statistically insignificant differences in progression-free survival stratified by SM and LN status (p = 0.12). cox univariate analyses revealed tumor volume to be a statistically significant predictor of progression free survival (p = 0.02). Stem and Leaf plot revealed tumor volume to be statistically significantly larger in patients who experienced biochemical recurrence, compared to those who did not. Conclusion: Tumor volume was associated progression-free survival in this cohort of SVI patients, while pathologic Gleason sum, PSA, margin and nodal status were not.

Background: Systemic inflammation due to radiation exposure has been identified in a biological system by certain metabolic and behavioral disorders. These anarchies mostly mediated under a regulation of cyclooxygenase 2 (cox2) induced production of an inflammatory mediator prostaglandin E2 (PGE2). Aim: This study was undertaken to investigate the anti-inflammatory impact of brown sea weed extract (BSWE) against induction of cox2/PGE2 inflammatory pathway in gamma-irradiated rats. Rats were orally administrated with BSWE (27 mg/kg body weight/day) for 7 consecutive days before exposure to 8 Gy fractionated gamma radiation (2 Gy × 4;every 3 days). Treatment with BSWE was extended along with and in-between irradiation doses for another 14 successive days. Our data demonstrated that the administration of BSWE to rats exposed to gamma radiation, following the regimen suggested, significantly neutralize the changes induced in the inflammatory molecules cox2, PGE2, tumor necrosis alpha (TNF-α), and nitric oxide (NO). In addition, it adjusted significantly the cellular redox tone via regulation of changes induced in malondialdehyde (MDA) reduced glutathione (GSh), superoxide dismutase (SOD) catalase (CAT) and xanthine oxidoreductase system (XOR). Credibly, from the results emerged in this study, it could be suggested that BSWE has substantial anti-inflammatory activities and gamma radiation protection capabilities. It is recommended to include BSWE in the treatment strategy of various inflammatory diseases especially cancer as a safe natural anti-inflammatory agent.

AIM:To investigate whether impaired fasting glucose(IFG)confers cardiovascular ***:A non-diabetic population-based sample representative of middle-aged and elderly Turks was studied at 8.5 years’follow-up for incident diabetes and coronary heart disease(ChD).Metabolic syndrome(MetS)was defined by ATP-Ⅲcriteria modifiedfor male abdominal obesity,and IFG and type 2 diabetes were identified by criteria of the American Diabetes *** by presence of MetS was *** were predicted providing estimates for hazard ratio(hR)obtained by use of cox proportional hazards regression analysis in models that controlled for potential ***:In 3181 adults(aged 52±11.5 years at baseline),analysis stratified by MetS,gender and IFG status distinguished normoglycemic subjects by a"hypertriglyceridemic waist"phenotype consisting of significantly higher waist circumference,fasting triglyceride and lower high-density lipoprotein-cholesterol,regardless of gender and ***,lipoprotein(Lp)(a)tended to be lower in(especially female)participants with *** linear regression in a subset of the sample demonstrated decreased Lp(a)levels to be associated with increased fasting glucose and insulin concentrations,again particularly in *** cox regression analysis,compared with normoglycemia,baseline IFG adjusted for major confounders significantly predicted incident diabetes at a 3-fold hR in men and only women with *** models for developing ChD in 339 individuals,adjusted for conventional risk factors,revealed that IFG status protected against ChD risk[hR=0.37(95%CI:0.14-0.998)]in subjects free of MetS,a protection that attenuated partly in male and fully in female participants with ***:IFG status in non-diabetic people without MetS displays reduced future ChD risk,yet is modulated by MetS,likely due to autoimmune activation linked to serum Lp(a).
