Body Contour in the Post-Bariatric Male Patient

Background: Obesity is currently considered a public health problem. Bariatric procedures have become an important part of obesity management and, consequently, the number of male patients seeking post-bariatric reconstructive procedures have increased. Therefore, the clinical approach and understanding of the body contour of this population have become more relevant. The goal of post-bariatric reconstruction is to enhance the male silhouette through removal of skin and adipose tissue excess, and abdominal rectus diastasis repair. Material and Methods: We conducted a retrospective study at the Plastic and Reconstructive Surgery Department of the national Medical Center “20 de noviembre”. All male patients referred to our department to start a post-bariatric reconstruction protocol from January 2018 to December 2022 were included in this study. Results: In total, 15 patients who underwent corporal contouring procedures were included;median age was 49.2 years with minimum of 33 years, and a maximum of 57 years. Median Body mass index was 28.4 kg/m2 with minimum of 22 kg/m2 and maximum of 38 kg/m2. All patients were treated 18 months after their bariatric surgery. All patients underwent an abdominoplasty as a body contouring procedure. 4 (26.7%) patients presented complications related to the surgery. Conclusion: We described a comprehensive and systematic approach to massive weight loss for male patients, suggesting an abdominal marking based on the patient’s clinical features and the expected results avoiding feminization of the abdominal body contour.

Obesity is one of the diseases with severe health consequences and rapidly increasing worldwide *** the complex network of food intake and energy balance regulation is an essential prerequisite for pharmacological intervention with obesity.G protein-coupled receptors(GPCRs)are among the main modulators of metabolism and energy ***,for instance,regulate appetite and satiety in certain hypothalamic neurons,as well as glucose and lipid metabolism and hormone secretion from *** in some GPCRs,such as the melanocortin receptor type 4(MC4R),have been associated with early-onset ***,we identified the adhesion GPCR latrophilin 1(ADGRL1/LPHn1)as a member of the regulating network governing food intake and the maintenance of energy *** of the highly conserved receptor in mice results in increased food consumption and severe obesity,accompanied by dysregulation of glucose ***,we identified a partially inactivating mutation in human ADGRL1/LPHn1 in a patient suffering from ***,we propose that LPHn1 dysfunction is a risk factor for obesity development.

Piezo proteins are mechanically activated ion channels,which are required for mechanosensing functions in a variety of cell *** we and others have previously demonstrated that the expression of Piezo1 in osteoblast lineage cells is essential for boneanabolic processes,there was only suggestive evidence indicating a role of Piezo1 and/or Piezo2 in *** we addressed the question if and how chondrocyte expression of the mechanosensitive proteins Piezo1 or Piezo2 controls physiological endochondral ossification and pathological osteoarthritis(OA)*** with chondrocyte-specific inactivation of Piezo1(Piezo1^(Col2a1Cre)),but not of Piezo2,developed a near absence of trabecular bone below the chondrogenic growth plate ***,all Piezo1^(Col2a1Cre) animals displayed multiple fractures of rib bones at 7 days of age,which were located close to the growth *** skeletal growth was only mildly affected in these mice,OA pathologies were markedly less pronounced compared to littermate controls at 60 weeks of ***,when OA was induced by anterior cruciate ligament transection,only the chondrocyte inactivation of Piezo1,not of Piezo2,resulted in attenuated articular cartilage ***,osteophyte formation and maturation were also reduced in Piezo1^(Col2a1Cre) *** further observed increased Piezo1 protein abundance in cartilaginous zones of human ***,we identified Ptgs2 and Ccn2 as potentially relevant Piezo1 downstream genes in ***,our data do not only demonstrate that Piezo1 is a critical regulator of physiological and pathological endochondral ossification processes,but also suggest that Piezo1 antagonists may be established as a novel approach to limit osteophyte formation in OA.

Outlining key inflammation-associated parameters during early phase of an experimental gram-negative sepsis model in rhesus macaques(Macaca mulatta)

The aim of this study was to identify inflammation-associated markers during the early phase of sepsis in rhesus macaque. Four rhesus macaques were given an intravenous dose of 1010 CFU/kg of E. coli. Blood samples were collected before, or 30 minutes, 2, 4, 6 and 8 hours after E. coli infusion. Physiological parameters, bacteremia, endotoxemia, C-reactive protein(CRP), procalcitonin(PCT), and plasma cytokines/chemokines were determined for each animal. Bacteremia was present in all animals from 30 minutes to 3 hours after E. coli infusion whereas endotoxin was detected during the full-time course. CRP and PCT levels remained at detectable levels during the whole experimental window suggesting an ongoing inflammatory process. Signature cytokines and chemokines such as TnF-α, MIP-1α, and MIP-1β peaked about 2 hours after E. coli infusion and decreased thereafter. Plasma IL-6, IL-12 p40, IFn-γ, and IL-1 Ra, as well as I-TAC, MIG, IP-10 and MCP-1, remained at detectable levels after 4 hours of E. coli infusion. This nonhuman primate model could be useful for the assessment of new therapeutics aiming to suppress key inflammatory markers throughout sepsis early phases.

Government Data Openness and Coverage. How do They Affect Trust in European Countries?

Purpose: This paper aims to assess if the extent of openness and the coverage of data sets released by European governments have a significant impact on citizen trust in public ***/methodology/approach: Data for openness and coverage have been collected from the Open Data Inventory 2018(ODIn), by Open Data Watch;institutional trust is built up as a formative construct based on the European Social Survey(ESS), Round 9. The relations between the open government data features and trust have been tested on the basis of structural equation modelling(SEM).Findings: The paper reveals that as European governments improve data openness, disaggregation, and time coverage, people tend to trust them more. However, the size of the effect is still small and, comparatively, data coverage effect on citizens' confidence is more than twice than the impact of *** limitations: This paper analyzes the causal effect of Open Government Data(OGD) features captured in a certain moment of time. In upcoming years, as OGD is implemented and a more consistent effect on people is expected, time series analysis will provide with a deeper *** implications: Public officers should continue working in the development of a technological framework that contributes to make OGD truly open. They should improve the added value of the increasing amount of open data currently available in order to boost internal and external innovations valuable both for public agencies and ***/value: In a field of knowledge with little quantitative empirical evidence, this paper provides updated support for the positive effect of OGD strategies and it also points out areas of improvement in terms of the value that citizens can get from OGD coverage and openness.

Using music to reduce anxiety among older adults in the emergency department:a randomized pilot study

BACKGROUnD： An emergency department （ED） visit may be distressing and anxiety-provoking for older adults （age 〉 65 years）. no studies have specifically evaluated the effect of music listening on anxiety in older adults in the ED. OBJECTIVE： The objective of this pilot study was to evaluate the effect of music listening on anxiety levels in older ED patients. DESIGn, SETTInG, PARTICIPAnTS AnD InTERVEnTIOnS： This was a randomized pilot study in the geriatric ED of an urban academic tertiary medical center. This was a sample of English-speaking adults （age 〉 65 years） who were not deaf （n = 35）. Subjects consented to participate and were randomized to receive up to 60 min of music listening with routine care, while the control group received routine care with no music. Subjects in the music treatment group received headphones and an electronic tablet with pre-downloaded music, and were allowed to choose from 5 selections. MAIn OUTCOME MEASURES： The primary outcome was change in anxiety levels, measured by the state- trait anxiety inventory （STAI）, at enrollment and 1 h later. RESULTS： A total of 35 participants were enrolled： 74% were female, 40% were white, and 40% were black; of these, 32 subjects completed the study protocol. When comparing control （n = 18） against intervention subjects （n = 17）, there were no significant differences in enrollment STAI scores （43.00 ± 15.00 vs. 40.30± 12.80, P = 0.57）. STAI scores 1 hour after enrollment （after the music intervention） were significantly reduced in the intervention subjects compared to the control subjects （with reduction of 10.00± 12.29 vs. 1.88 ± 7.97, P = 0.03）. COnCLUSIOn： These pilot results suggest that music listening may be an effective tool for reducing anxiety among older adults in the ED.

The importance of MYC function in cancer was discovered in the late 1970s when the sequence of the avian retrovirus that causes myelocytic leukemia was *** then,over 40 years of unceasing research have highlighted the significance of this protein in malignant transformation,especially in hematologic ***,some of the earliest connections among the higher expression of proto-oncogenes(such as MYC),genetic rearrangements and their relation to cancer development were made in Burkitt lymphoma,chronic myeloid leukemia and mouse *** myeloma(MM),in particular,is a plasma cell malignancy strictly associated with MYC deregulation,suggesting that therapeutic strategies against it would be beneficial in treating this ***,targeting MYC was-and,somehow,still is-challenging due to its unique properties:lack of defined three-dimensional structure,nuclear localization and absence of a targetable enzymatic *** these difficulties,however,many studies have shown the potential therapeutic impact of direct or indirect MYC *** molecules have been tested,in fact,in the context of *** this review,we summarize the current status of the different compounds,including the results of their clinical testing,and propose to continue with the efforts to identify,repurpose,redesign or improve drug candidates to combine them with standard of care therapies to overcome resistance and enable better management of myeloma treatment.
