Giant Cell Tumor of Lumbar Spine Treated with RapidArc Intensity Modulated Arc Therapy: Case Report and Review of Literature

Background: Giant cell tumors of the lumbar spine are rare and complete resection without major functional comprise is challenging despite advancements in spine surgery techniques. Radiation therapy has been an option in such cases;however there are high concerns for associated high small bowel toxicity and lack of dose escalation to achieve local control. With advent of intensity modulated radiation therapy (IMRT) it is now possible to give high radiation dose to tumor with minimal toxicity. Herein we present a rare case of giant cell tumor of fourth lumbar (L4) vertebra treated with RapidArc intensity modulated arc therapy (IMAT) with relevant review of literature. Methods: A 30-year-old female had low back pain for 6 months. She underwent surgical decompression at another hospital as having a L4-5 protruded intervertebral disc 2 months previously, but her back pain progressed with weakness of both legs with restricted movement. Radiological and pathological work-up confirmed the diagnosis of giant cell tumor of L4 vertebra. She refused further surgery and was referred to us for radiotherapy. Treatment plans for prescribed radiation dose of 59.4 Gy in 30 fractions were made by 3DCRT and RapidArc IMAT and comparison was made. Student’s unpaired t test was used to determine the significance of the difference between two plans in terms of dose to the tumor and small bowel. A p value of 0.05 was considered statistically significant. Related literature was searched. Results: In RapidArc IMAT and 3DCRT plans mean doses to planning treatment volume (PTV) were 61.24 Gy (55.98 - 66.23) and 60.71 Gy (49.87 - 63.74) respectively (p 0.04) and mean doses to small bowel were found lesser in RapidArc plan [14.78 Gy (range: 0.39 - 53.15)] as compared to 3DCRT plan. Patient was started on RapidArc IMRT and she completed the course without any major sequelae. Conclusion: Lumbar spine giant cell tumors are rare and complete resection is often not possible. Background: Penile toxicity after preoperative concurrent chemoradiation (CCRT) for rectal cancers is extremely rare and only two cases of phimosis and one case of recto-cavernosalfistula have been reported so far in literature. Preoperative CCRT for rectal cancer is given in prone position and with the support of belly board (BBD) to avoid small bowel toxicity. However, positional errors during rectal radiotherapy can lead to unexpected penile toxicity. Case Presentation: A 50-year-old Saudi male with diagnosed case of rectal adenocarcinoma stage cT3N1M0 was given preoperative CCRT 50.4 Gy in 28 fractions with three-dimensional conformal radiation therapy (3DCRT) in prone position using belly board with concurrent oral capecitabine 825 mg/2 twice a day. After the completion of CCRT, he complained of severe soreness, itching over glans penis and dysuria. Examination revealed grade 3 erythema, skin desquamation over glans penis (balanitis). Portal imaging of treatment revealed glans penis to lie within posterior radiation beam. A patient was assured and he recovered fully after local steroids and short course of antibiotics. Conclusion: Penile toxicity after CCRT for rectal cancer is extremely rare manifestation. Radiation oncologists and therapists must be aware of this rare side effect and must assure proper patient education and positioning during CCRT for rectal cancer.

Dysfunction in circadian rhythms is a common occurrence in patients with Alzheimer’s disease.A predominant function of the retina is circadian synchronization,carrying information to the brain through the retinohypothalamic tract,which projects to the suprachiasmatic ***,Alzheimer’s disease hallmarks,including amyloid-β,are present in the retinas of Alzheimer’s disease patients,followed/associated by structural and functional ***,the mechanistic link between circadian dysfunction and the pathological changes affecting the retina in Alzheimer’s disease is not fully understood,although some studies point to the possibility that retinal dysfunction could be considered an early pathological process that directly modulates the circadian rhythm.

Biodiversity loss is a significant problem at a global scale and may be amplified by climate *** recent years,coniferous forests have had substantial die-back across Europe due to drought and subsequent bark-beetle *** many studies on the consequences of disturbance and subsequent management have focused on natural stands,management implications for managed spruce stands are not well understood,even though such stands are widespread throughout *** this study,beetle taxonomy,conservation value,and community com-position are compared among spruce plantations and four post-disturbance management approaches:standing dead-wood,lying deadwood,clear cuts,and long-term *** and community composition differed significantly among management categories,while different beetle fami-lies responded *** spruce stands harbored the lowest beetle diversity while the highest taxonomic diver-sity and conservation value was on clear cuts and stands with lying or standing *** proportion of forest specialists was highest in successional *** summary,different forest management categories harbored distinct beetle communities at the family-,species-,and ecological guild ***,post-disturbance management should consider the landscape scale and include different management *** enhances landscape heterogeneity and thus overall biodiversity but could also mitigate negative impacts of natural disturbances on ecosystem services.

Nowadays buildings contain innovative materials,materials from local resources,production surpluses and rapidly renewable natural *** Change Materials(PCM)are one such group of novel materials which reduce building energy *** the wider availability of microencapsulated PCM,there is an opportunity to develop a new type of insulating materials,combinate PCM with traditional insulation materials for latent heat energy *** materials are typically flammable and are located on the interior wall finishing yet there has been no detailed assessment of their fire *** this research work prototypes of low-density insulating boards for indoor spaces from hemp shives using carbamide resin binder and cold pressing were ***-scale cone calorimeter tests were conducted to evaluate fire risk,with a focus on assessing material flammability properties and the influence of PCM on the *** this research,the amount of smoke,heat release rate,effective heat of combustion,specific extinction coefficient,mass loss,carbon dioxide yield,specific loss factor,ignition time of hemp straws samples and samples of hemp straws with 10%and without PCM admixture were *** is a risk of flammability for PCM and their fire reaction has not been evaluated when incorporating PCM into interior wall finishing *** obtained results can be used by designers to balance the potential energy savings of using PCM with a more complete understanding and predictability of the associated fire risk when using the proposed *** also allows for appropriate risk mitigation strategies.
