Prevalence of Charles Bonnet syndrome in low vision:a systematic review and meta-analysis

Comparing corneal outcome between femtosecond laser-assisted cataract surgery and conventional phaco surgery in Fuchs’endothelial dystrophy patients:a randomized pilot study with 6mo follow up

AIM:To compare the corneal outcome in Fuchs’endothelial dystrophy(FED)patients between femtosecond laser-assisted cataract surgery(FLACS)and conventional phaco surgery(CPS).METHODS:This was a randomized controlled study comparing one eye surgery by FLACS and the contralateral eye operated by CPS(stop and chop technique)in FED *** corneal thickness,corneal light backscatter,corneal densitometry,and central corneal endothelial cell count and hexagonality(noncontact endothelial cell microscope),and corrected distance visual acuity(CDVA)were assessed preoperatively and at day 1,40,and 180 ***:Totally 31 patients(16 women)were *** day 40 postoperatively,the mean endothelial cell loss(ECL)was 23.67%by FLACS and 17.30%by CPS(P=0.53).At day 180 postoperatively,ECL was 25.58%in FLACS and 21.32%in CPS(P=0.69).Densitometry data in all layers and all annuli from anterior layer to posterior layer in annuli 0-2,2-6,6-10 and 10-12,total densitometry with all layers and all annuli was performed.A significant difference was found in 6-10(posterior layer)at day 1 with-1.42 grayscale units(GSU;95%CI:-2.66 to-0.19,P=0.02).In 10-12(anterior layer,central layer and all layers)at day 40 were significant different with 7.7(95%CI:1.89 to 13.50,P=0.009),3.97(95%CI:0.23 to 7.71,P=0.03),4.73 GSU(95%CI:0.71 to 8.75,P=0.02),*** the remaining parameters we found no difference between the two groups(P>0.05).Three CPS eyes suffered from corneal ***:There is no significant difference in corneal outcome between FLACS and *** cell density and pentacam corneal outcome may be inadequate as outcome parameters in FED patients.

AIM To investigate whether immune mediated diseases(IMD) are more frequent in patients with inflammatory bowel disease(IBD).METHODS In this population based registry study,a total of 47325 patients with IBD were alive and registered in the Danish National Patient Registry on December 16,2013. Controls were randomly selected from the Danish Civil Registration System(CRS) and matched for sex,age,and municipality. We used ICD 10 codes to identify the diagnoses of the included patients. The IBD population was divided into three subgroups: Ulcerative colitis(UC),Crohn's disease(CD) and Both the latter referring to those registered with both diagnoses. Subsequently,odds-ratios(OR) and 95%CI were obtained separately for each group and their respective controls. The use of Bonferoni post-test correction adjusted the significance level to P < 0.00125. P-values were estimated using Fisher's exact *** There were significantly more women than men in the registry,and a greater percentage of comorbidity in the IBD groups(P < 0.05). Twenty different IMDs were all significantly more frequent in the IBD group. Sixteen were associated with UC versus twelve with CD. In both UC and CD ORs were significantly increased(P < 0.00125) for primary sclerosing cholangitis(PSC),celiac disease,type 1 diabetes(T1D),sarcoidosis,asthma,iridocyclitis,psoriasis,pyoderma gangrenosum,rheumatoid arthritis,and ankylosing spondylitis. Restricted to UC(P < 0.00125) were autoimmune hepatitis,primary biliary cholangitis,Grave's disease,polymyalgia rheumatica,temporal arteritis,and atrophic gastritis. Restricted to CD(P < 0.00125) were psoriatic arthritis and episcleritis. Restricted to women with UC(P < 0.00125) were atrophic gastritis,rheumatoid arthritis,temporal arteritis,and polymyalgia rheumatica. During the late years there has been an ever-increasing focus on the possibilities to change the building process to raise quality on the final building products as well as the activities of actors involved in the building process. One reason for this interest is the new opportunities evolving due to introduction of advanced information and communication technology (ICT). The paper focuses on creative changes of the building process powered by user driven innovation activities. An overview of existing user driven innovation methodologies is given as well experiences from the ongoing Virtual Innovation in Construction (VIC) project. One important driving force for change is the opportunity for users to develop and articulate real needs concerning for example different functionalities of a building and its parts, but also on artifacts supporting the actual needs capture and requirements formulation during building design. A general methodological framework and meta ontology for Virtual Innovation in Construction is presented as well as findings from implementation of the method.

Using motion capture to assess colonoscopy experience level

AIM: To study technical skills of colonoscopists using a Microsoft Kinect? for motion analysis to develop a tool to guide colonoscopy ***: Ten experienced endoscopists(gastroenterologists, n = 2; colorectal surgeons, n = 8) and 11 novices participated in the study. A Microsoft Kinect? recorded the movements of the participants during the insertion of the colonoscope. We used a modified script from Microsoft to record skeletal data. Data were saved and later transferred to MatLab for analysis and the calculation of statistics. The test was performed on a physical model, specifically the "Kagaku Colonoscope Training Model"(Kyoto Kagaku Co. Ltd, Kyoto, Japan). After the introduction to the scope and colonoscopy model, the test was performed. Seven metrics were analyzed to find discriminative motion patterns between the novice and experienced endoscopists: hand distance from gurney, number of times the right hand wasused to control the small wheel of the colonoscope, angulation of elbows, position of hands in relation to body posture, angulation of body posture in relation to the anus, mean distance between the hands and percentage of time the hands were approximated to each ***: Four of the seven metrics showed discriminatory ability: mean distance between hands [45 cm for experienced endoscopists(SD 2) vs 37 cm for novice endoscopists(SD 6)], percentage of time in which the two hands were within 25 cm of each other [5% for experienced endoscopists(SD 4) vs 12% for novice endoscopists(SD 9)], the level of the right hand below the sighting line(z-axis)(25 cm for experienced endoscopists vs 36 cm for novice endoscopists, P < 0.05) and the level of the left hand below the z-axis(6 cm for experienced endoscopists vs 15 cm for novice endoscopists, P < 0.05). 背景对于肥胖患者，药代动力学参数显示，应按照患者的理想体重计算罗库溴铵给药剂量，但此方生却可能延长罗库溴铵起效时间或不能满足插管条件。本研究按不同的校正体重将患者分成3组，并分别按照校正体重计算罗库溴铵给药剂量，比较3组中罗库溴铵的起效时间、插管条件和药物持续时间。方法将51例择期行腹腔镜胃束带减容术和胃转流术的肥胖患者随机分成3组，平均体重指数为44（34-72）kg／m^2，分别按照理想体重（IBW组，n=17）、理想体重加超标体重的20％[校正体重（CBW）20％组，n=17]、理想阵重加起标体重的40％（CBW40％组，n=17）计算罗库溴铵给药剂量（0．6mg／kg）．所有患者都采用丙泊酚-瑞芬太尼复合麻醉。一次静注丙泊酚200mg并以5mg·kg^-1·h^-1泵注维持麻醉，瑞芬太尼则以1．0μg·kg^-1·min^-1的速度维持输注，2种药物均采用CBW40％组的校正体重计算给药量。采用TOF（4个成串刺激）监测神经肌肉功能，首要观察指标为罗库溴铵持续时间，即4个成串神经刺激中第4个肌颤搐的恢复时间。结果IBW组、CBW20％组和CBW40％组罗车溴铵持续时间分别为32（18～49）分钟、38（25～66）分钟和42（24—66）分钟，IBW组和CBW40%组间差异有统计学差异（P=0．001）：而在各组罗库溴铵的起效时间（分别为85、84和80秒）和注药后90秒内的插管条件差异无统计学意义。结论对于行胃束带减容术和胃转流术的肥胖患者，按照理想体重给予罗库溴铵后其持续时间短，不会明显延长药物的起效时间，也不会影响插管条件。
