Patients with inflammatory bowel disease have increased risk of autoimmune and inflammatory diseases

AIM To investigate whether immune mediated diseases(IMD) are more frequent in patients with inflammatory bowel disease(IBD).METHODS In this population based registry study,a total of 47325 patients with IBD were alive and registered in the Danish National Patient Registry on December 16,2013. Controls were randomly selected from the Danish Civil Registration System(CRS) and matched for sex,age,and municipality. We used ICD 10 codes to identify the diagnoses of the included patients. The IBD population was divided into three subgroups: Ulcerative colitis(UC),Crohn's disease(CD) and Both the latter referring to those registered with both diagnoses. Subsequently,odds-ratios(OR) and 95%CI were obtained separately for each group and their respective controls. The use of Bonferoni post-test correction adjusted the significance level to P < 0.00125. P-values were estimated using Fisher's exact *** There were significantly more women than men in the registry,and a greater percentage of comorbidity in the IBD groups(P < 0.05). Twenty different IMDs were all significantly more frequent in the IBD group. Sixteen were associated with UC versus twelve with CD. In both UC and CD ORs were significantly increased(P < 0.00125) for primary sclerosing cholangitis(PSC),celiac disease,type 1 diabetes(T1D),sarcoidosis,asthma,iridocyclitis,psoriasis,pyoderma gangrenosum,rheumatoid arthritis,and ankylosing spondylitis. Restricted to UC(P < 0.00125) were autoimmune hepatitis,primary biliary cholangitis,Grave's disease,polymyalgia rheumatica,temporal arteritis,and atrophic gastritis. Restricted to CD(P < 0.00125) were psoriatic arthritis and episcleritis. Restricted to women with UC(P < 0.00125) were atrophic gastritis,rheumatoid arthritis,temporal arteritis,and polymyalgia rheumatica. Sphingosine signaling and atherogenesis

Sphingosine-1-phosphate (S1P) has diverse biological functions acting inside cells as a second messenger toregulate cell proliferation and survival, and extracellularly, as a ligand for a group of G protein-coupled receptors(GPCRs) named the endothelial differentiation gene (EDG) family. Five closely related GPCRs of EDG family(EDG1, EDG3, EDG5, EDG6, and EDG8) have recently been identified as high-affinity S1P receptors. Thesereceptors are coupled via Gi, Gq, G12/13, and Rho. The signaling pathways are linked to vascular cell migration,proliferation, apoptosis, intracellular Ca2+ mobilization, and expression of adhesion molecules. The formation of anatherosclerotic lesion occurs through activation of cellular events that include monocyte adhesion to the endothe-lium and vascular smooth muscle cell (VSMC) migration and proliferation. Thus, S1P signaling may play animportant role in the pathogenesis of atherosclerotic vascular disease. This review highlights S1P signalling invascular cells and its involvement in the formation of atherosclerotic lesions.

Background and Aims:There is an unmet need for new bi-omarkers to improve diagnostics and prognostics in primary biliary cholangitis(PBC)and primary sclerosing cholangitis(PSC).Inter-α-inhibitor heavy chain 4(ITIH4)is an abun-dant,liver-produced protein,and its synthesis may be al-tered in liver *** investigated whether ITIH4 plas-ma concentrations were affected in PBC and PSC ***:We developed an immunoassay specific for ITIH4 and determined ITIH4 plasma concentrations in 66 PBC,126 PSC,92 autoimmune hepatitis(AIH),67 chronic hepatitis C(CHC),33 alcoholic hepatitis(AH)patients and 138 healthy controls(HCs).Hepatic ITIH4 expression was investigated by immunohistochemistry in ***:The mean plas-ma concentration of ITIH4 was almost doubled in PBC[409μg/mL(95%CI:388-431)]and 35%higher in PSC[308μg/mL,(95%CI:296-319)]compared with HCs[226μg/mL(95%CI:221-231);p<0.001].In PBC patients,ITIH4 correlated with IgM(rho=0.49,p<0.001).Responders to ursodeoxycholic acid treatment(UDCA)had lower levels of ITIH4 than incomplete responders[395μg/mL(95%CI:364-425)]vs.460μg/mL(95%CI:421-498);p=0.02].Four weeks of UDCA treatment had no effect(p=0.19).In-creased ITIH4 immunohistochemical staining was seen in a liver biopsy from a PBC ***4 levels in AIH[224μg/mL(95%CI:208-241)]and HCs were similar(p=0.8).ITIH4 levels were lower in AH[199μg/mL(95%CI:175-223)]and CHC[202μg/mL(192-212)]patients than in HCs(p<0.05).Conclusions:The plasma concentration of ITIH4 was highly elevated in patients with PBC and PSC,suggesting that ITIH4 should be further investigated as a biomarker in cholestatic liver disease.

COVID-19 Pandemic: Lessons from Peak Years of Influenza &Suicide Deaths by Age in Ten Western Countries 1979-2016: An Alert for Psychiatry and Children’s Services

Background: The IMF fears the COVID-19 pandemic will create an economic down-turn matching the Great Depression. More recent recessions have led to physical and mental health problems including suicide deaths of young adults (15 - 34). We aim to identify risk patterns of mortality by age from influenza and suicide mortality in peak years from 1979 to 2016 to see if there are lessons to be learned for policy makers and psychiatric services. Method: Using WHO mortality data for 1979-2016 peak years of influenza deaths and suicides are identified in ten Western countries. Death rates per million in each age-band are calculated for both sexes and the percentage of the total deaths accruing in each of five age-bands for influenza and suicides. Ratios of influenza to suicide by age in regard to rates and percentages of deaths indicate differential risk mortality and morbidity patterns. Results: Of the ten country’s average Influenza deaths, 95% occurred in people over 55, including 80% - 85% for the over 75’s. Conversely it was 59% of suicides occurred in peoples aged Discussion: IMF predicts major unemployment, which is likely to create mental health problems, including suicide as well as worsening child health outcomes linked to relative poverty. These wider inter-related factors should be considered by policy-makers and mental health services. The age patterns of risk of mortality point towards a cumulative summation of morbidity and mortality risks of the socio-economic consequences of the COVID pandemic.

Spatial variability in sea-ice algal biomass: an under-ice remote sensing perspective

Sea-ice algae are a paramount feature of polar marine ecosystems and ice algal standing stocks are characterized by a high spatio-temporal variability. Traditional sampling techniques, e.g., ice coring, are laborintensive, spatially limited and invasive, thereby limiting our understanding of ice algal biomass variability patterns. Thishas consequences for quantifying ice-associated algal biomass distribution, primary production, and detecting responses to changing environmental conditions. Close-range under-ice optical remote sensing techniques have emerged as a capable alternative providing non-invasive estimates of ice algal biomass and its spatial variability. In this review we first summarize observational studies, using both classical and new methods that aim to capture biomass variability at multiple spatial scales and identify the environmental drivers. We introduce the complex multi-disciplinary nature of under-ice spectral radiation profiling techniquesand discuss relevant concepts of sea-ice radiative transfer and bio-optics. In addition, we tabulate and discuss advances and limitations of different statistical approaches used to correlate biomass and under-ice light spectral composition. We also explore theoretical and technical aspects of using Unmanned Underwater Vehicles （UUV）, and Hyperspectral Imaging （HI） technology in an under-ice remote sensing context. The review concludes with an outlook and way forward to combine platforms and optical sensors to quantify ice algal spatial variability and establish relationships with its environmental drivers.

Sports activity patterns and cardio-metabolic health over time among adults in Germany: Results of a nationwide 12-year follow-up study

Purpose: Androgen deprivation therapy (ADT) is a cornerstone in prostate cancer (PCa) management that prolongs PCa-free and overall survival, but effects of ADT on human cardiac function have not been investigated. We used echocardiography to examine cardiac structure and function in patients with prostate cancer receiving ADT and to determine whether an exercise intervention can elicit cardiac adaptations in these subjects. Methods: Forty-three patients with prostate cancer receiving ADT were randomized to 12 weeks football training (ST group;n = 20) or usual care (control [CO] group;n = 23). Cardiac function was assessed at baseline and after 12 weeks by comprehensive echocardiography. Peak oxygen consumption, blood pressure and peripheral microvascular function was also measured. Results: At baseline, no considerable echocardiographic abnormalities were observed. In the ST group, increases in left ventricular diastolic function variables including E/A ratio (P = 0.03), E’ (P = 0.016), E’TDIcolor (P = 0.040) and in left atrial diameter (P = 0.001) were observed after 12 weeks. In addition, diastolic blood pressure (P = 0.027) and resting heart rate (P 0.001) were reduced after ST. In the CO group, no significant changes were observed in the examined variables after 12 weeks. Despite within group changes in the ST group, no significant differences were observed after 12 weeks between groups in echocardiographic variables, peak oxygen consumption, blood pressure and peripheral microvascular function. Conclusion: In men with prostate cancer receiving ADT, echocardiography showed no abnormalities in cardiac structure and function. Twelve weeks of ST failed to elicit significant cardiovascular adaptations and ADT may blunt cardiovascular adaptations to short-term exercise training.
