Progress Toward Implementing the Sendai Framework, the Paris Agreement, and the Sustainable development Goals: Policy from Aotearoa New Zealand

In 2015, Aotearoa New Zealand became a signatory to the Sendai Framework for Disaster Risk Reduction 2015–2030(Sendai Framework), the Paris Climate Change Agreement(Paris Agreement), and the Sustainable development Goals(SDGs). Since 2017 Aotearoa New Zealand has been undergoing governance reform to realign priorities and to improve the management of natural hazards and climate change. The aim of this article is to provide a review of how Aotearoa New Zealand is taking steps to improve consistency of planning across the legislative environment, thereby implementing its commitments to the Sendai Framework, the SDGs, and the Paris Agreement. It provides an overview of the national governance arrangements, with a focus on the key legislative tools;identifies how key terms are defined nationally;and provides an overview of the governance arrangements that contribute to the country’s international obligations. The discussion describes how obligations are applied, and considers two"disruptive and proactive" action examples. Four recommended actions are provided to further implement these international aspirations:(1) take into account these international agreements during the development and implementation of all legislation;(2) build awareness, capability,and capacity within central, regional, and local governments to support implementation;(3) actively evaluate the progress of implementing initiatives designed to reduce vulnerability and strengthen resilience;and(4) ensure that more weight and value are given to indigenous planning documents.

Student self-assessment of strengths and needed improvements during a family medicine clerkship

Objective:There are few reports on how students self-assess their performance on a family medicine *** studied what students perceived as their strengths and areas of needed improvement at the mid-point in our family medicine ***:We introduced a form for family medicine clerkship students to self-assess their strengths and areas of needed improvements using the clerkship objectives as a *** cal-culated the frequency in which each clerkship objective was reported as a strength or an area of needed *** students’open-ended comments,two reviewers independently organ-ized students’comments into themes,then negotiated any initial differences into a set of themes that incorporated both the reviewers’*** performed c2 tests to determine any significant differences in the frequency of responses between male and female ***:During the study period(July 2012 to June 2014),372 students submitted com-pleted self-assessment *** most frequently reported strengths were professional objectives(48.9%)and interpersonal communication objectives(43.0%)The most frequently reported areas of needed improvement were the ability to explain key characteristics of commonly used medica-tions(29.3%)and the ability to develop a management plan(28.5%).There were no significant differences in the frequency of responses between male and female ***:We now have a better understanding of students’perceived strengths and areas of needed improvement in our family medicine *** have shared this information with our community faculty preceptors so that they will be better prepared to work with our *** medicine clerkship preceptors at other institutions may also find these results useful.

Barriers and Facilitators in Interorganizational Disaster Response: Identifying Examples Across Europe

Disaster response actors are facing new challenges, which encompass not only new and ever more complex threats but also the need to collaborate across organizational boundaries and even state *** on scale, these interactions have to work across governance setups, political and legal conditions, organizational cultures, as well as personal preferences and experiences that vary among actors, organizations, and countries. But which concrete measures are taken by crisis management actors at different scales to bridge these challenges and which of these could serve others as example to address comparable challenges in their contexts? This study made attempts to analyze whether certain solutions across organizations and states exist that facilitate effective interorganizational crisis management in the member states of the European Union(EU). It is based on selected expert interviews with representatives of different types of disaster response organizations(health services,police services, fire services, and other crisis management organizations) from seven EU member states(Germany,Netherlands, United Kingdom, Ireland, Italy, Austria, and Greece).

The 7th International Eclogite Conference was held in Seggau, near Leibnitz, Austria, from July 3-9, 2005, and was accompanied by inter-conference and post-conference field trips to the Tauren Window of the Alps. The Conference provided a comprehensive scientific forum in the spirit of previous conferences of this series with extended contributions related to highand ultrahigh-pressure metamorphism, and ultra-deep subduction of continental material to Earth＇s mantle. The first workshop of the new Task Group Ⅱ-10 （UDCCS--Ultra-deep Continental Crust Subduction）, under the auspices of International Lithosphere Program, was held in conjunction with the conference. The Task Group Ⅱ-10 was established in 2005 to be continued to 2009. Crustal Signature of δ^(13)C and Nitrogen Content in Microdiamonds from Erzgebirge, Germany: Ion Microprobe Studies

The study of δ13CCpDB （Pee dee Belemnite） and nitrogen contents in 1 to 5-pm-diameter microdinmonds included in garnets from the qnartz-feldspathic gneisses （Erzgehirge, Germany） was performed in situ with the Nano-scale Secondary Ion Mass Spectrometer. The results revealed that there were two stages of diamond crystallization from a C-O-H supeeritical fluid rich in biogenic carbon and diverse minor elements of crustal origin. The δ13CCpDB Of the Erzgebirge diamond of the first stage falls in the range -17‰ to 19‰, with an average value of -17.8‰; the average content of nitrogen is 820 ppm. Diamonds of the second stage are characterized by δ13CPDB=-21.5‰ to -25.5‰, with an average value of -23.24‰; the average nitrogen content is non-homogeneously scattered from 740 ppm to 3 370 ppm among 6 diamonds situated in garnets within the same polished rock slide. Both diamond of the first stage and diamond of the second stage carbon reservoirs belong to biogenic matter, therefore confirming deep subduction of the continental crust sediments and their subsequent exhumation during the Variscan orogeny.

Cholangiocarcinoma:A 7-year experience at a single center in Greece

AIM： To evaluate survival rate and clinical outcome of cholangiocarcinoma. METHODS： The medical records of 34 patients with cholangiocarcinoma, seen at a single hospital between the years 1999-2006, were retrospectively reviewed. RESULTS： Thirty-four patients with a median age of 75 years were included. Seventeen （50%） had painless jaundice at presentation. Sixteen （47.1%） were perihilar, 15 （44.1%） extrahepatic and three （8.8%） intrahepatic. Endoscopic retrograde cholangiography （ERCP） and/or magnetic resonance cholangiography （MRCP） were used for the diagnosis. Pathologic confirmation was obtained in seven and positive cytological examination in three. Thirteen patients had co-morbidities （38.2%）. Four cases were managed with complete surgical resection. All the rest of the cases （30） were characterized as non-resectable due to advanced stage of the disease. Palliative biliary drainage was performed in 26/30 （86.6%）. The mean follow-up was 32 mo （95% CI, 20-43 too）. Overall median survival was 8.7 mo （95% CI, 2-16 mo）. The probability of 1-year, 2-year and 3-year survival was 46%, 20% and 7%, respectively. The survival was slightly longer in patients who underwent resection compared to those who did not, but this difference failed to reach statistical significance. Patients who underwent biliary drainage had an advantage in survival compared to those who did not （probability of survival 53% vs 0% at 1 year, respectively, P = 0.038）. CONCLUSION： Patients with cholangiocarcinoma were usually elderly with co-morbidities and/oradvanced disease at presentation. Even though a slight amelioration in survival with palliative biliary drainage was observed, patients had dismal outcome without resection of the tumor.

Clinical outcomes after implantation of a new intrastromal corneal ring with 140-degree of arc in patients with corneal ectasia

AIM：To evaluate the clinical and tomographic outcomes after implantation of a new intrastromal corneal ring segment（ICRS） with 140-degrees of arc in eyes with corneal ***：We evaluated patients with corneal ectasia implanted with Ferrara 140° ICRS from April 2010 to February *** measures included preoperative and postoperative corrected distance visual acuity（CDVA）,keratometry simulated（K） reading,tomographic astigmatism and *** patients were evaluated using the Pentacam Scheimpflug ***：The study evaluated 58 *** mean followup was 16.81±10.8 *** CDVA（logM AR） improved from 0.5±0.20（20/60） to 0.3±0.21（20/40）（P〈0.01）.The average K reduced from 49.87±7.01 to 47.34±4.90 D（P〈0.01）.The asphericity changed from-0.60±0.86 to-0.23±0.67 D（P〈0.01）.The mean preoperative tomographic astigmatism decreased from-8.0±3.45 to-4.53±2.52 D（P〈0.01）.CONCLUSION：The new ICRS model with 140-degrees of arc effectively improve the visual acuity and reduce the high astigmatism usually found in patients with corneal ectasia.
