Epidemiology of Admissions in a Pediatric Emergency Department in Albert Royer Hospital Dakar

Background: Pediatric emergencies are a public health priority in Senegal. The objective was to describe the epidemiology of admissions to the emergency department of the Albert Royer National Children’s Hospital in Dakar, Senegal. Methods: Prospective, descriptive and analytical study over a period of 4 months (January-April 2020). The study population was children admitted for vital distress. Results: A total of 300 children met the inclusion criteria and represented the study population (2.83%). The majority (70.67%) were younger than 60 months, compared with 2.00% of newborns. Boys represented 53.8%, sex ratio 1.16. A proportion of 35.1% was referred. More than half of our patients (55.88%) were managed within 30 minutes of arrival. Medicalized transport concerned 21.5% against 78.5% of nonmedicalized. Emergencies were dominated by respiratory distress (56.33%), dehydration (29%) and shock (13.33%). We recorded 16 deaths (5.3%). This mortality was significantly related to the low socio-economic level (p = 0.000), as well as the young age of the children (p = 0.01). Conclusion: Life-threatening emergencies are frequent and responsible for significant lethality. Health policy efforts are still needed to reduce this infant and child morbidity and mortality.

Quantitative Study on Individual Representations in Senegalese Patients Living with HIV (PLHIV)

Context: HIV infection is an infectious, viral, chronic disease described as a public health problem. It has an impact on bio-psycho-social well-being. Thus, the objective of our study is to decrypt the representations of HIV infection in our patients. Methodology: 31 consecutively recruited HIV-infected patients participated in our study. The completed questionnaire included socio-demographic characteristics, association membership, psycho-social aspects of HIV infection, mode of transmission and adherence to antiretroviral treatment. Secondly, we proceeded to an assessment of the psychological state of the patient. Results: Almost 77% of HIV patients do not date peers in an association. At the time of the study, more than half of patients or 19/31 with HIV had a positive image of the disease. Before the diagnosis of the disease, more than 2/3 of patients with HIV, i.e. 25/31, have a negative image of the disease. The 31 PLHIV have been aware of their positive HIV status for an average of 11 years with extremes of 2 years and 21 years. Almost 21/31 of PLHIV have shared their positive serological status. The main barrier in patient communication was the risk of disclosure of HIV status. Most of the patients (45.2%) thought of death when they announced their positive HIV status, and 19.4% thought of their children. In 80.6% of cases, relationships with family, friends and neighbors were normal and remained unchanged for 74% of patients. The patients thought they had been infected through sexual intercourse in 84% of cases. Our study showed that 6 of the 31 PLHIV experienced stigma. Among these 6 patients, 3 had psychological repercussions and a voluntary interruption of treatment. Conclusion: The representations are dynamic. In Senegal, the eradication of discrimination among people living with HIV must imperatively involve a reconstruction of individual, collective and social representation

The World Health Organization declared that corona virus diseases-19 (COVID-19) is a public health emergency. The COVID-19 pandemic is more likely to cause disaster in developing area including West Africa due to limited medical resources. COVID-19 reportedly causes severer conditions in adults with advanced age and in patients with underlying comorbidities including sickle cell anemia. We recently experienced two pediatric patients with sickle cell disease (SS), who had COVID-19. We here highlight the difficulties of management and the severity of COVID-19 infection in children with homozygous sickle cell SS.

Islanding Detection Method for Multi-Inverter Distributed Generation

Islanding detection is an essential function for safety and reliability in grid-connected distributed generation (DG) systems. Several methods for islanding detection are proposed, but most of them may fail under multi-source configurations, or they may produce important power quality degradation which gets worse with increasing DG penetration. This paper presents an active islanding detection algorithm for Voltage Source Inverter (VSI) based multi-source DG systems. The proposed method is based on the Voltage Positive Feedback (VPF) theory to generate a limited active power perturbation. Theoretical analyses were performed and simulations by MATLAB /Simulink /SimPowerSystems were used to evaluate the algorithm’s performance and its advantages concerning the time response and the effects on power quality, which turned out to be negligible. The algorithm performance was tested under critical conditions: load with unity power factor, load with high quality factor, and load matching DER’s powers.

The Magnetic Field as Trigger for Disturbances of a Flow of a Newtonian Fluid through a Cylindrical Pipe with a Horizontal Axis

A Fourrier Petrov Galerkin spectral method is described for high accuracy computation of linearized dynamics for flow in a circular pipe. The code used here is based on solenoidal velocity variables. It is written in FORTRAN. Systematic studies are presented on the dependence of eigenvalues and other quantities on the axial and azimuthal wave number;the reynolds’ number Re and a new none-dimensional number Ne. The flow will be considered stable if all the real parts of the eigenvalues are negative and unstable if only one of them is positive.

The Scrotoschisis about a Case in the Pediatric Surgery Department of the Donka National Hospital

Introduction: Scrotoschisis is a very rare congenital defect of the scrotum characterized by the exteriorization of one or two testes. We report a case of right scrotoschisis in a newborn as well as a review of the literature for an approach of probable etiology. Patient and Observation: A newborn baby of 8 hours of life, weighing 3200 g was referred to our department for a right scrotal defect with exteriorization of the testis associated with fluid swelling of the left bursa. The 18-year-old mother, primiparous and primigeste followed all the prenatal consultations with eutocic delivery. After clinical investigation the diagnosis of right scrotosisis and left hydrocele was retained. Surgical treatment was carried out by primary closure after orchidopexy and exploration of the contralateral bursa, the content of which was calcified meconium bathed in a yellowish liquid. The post-operative consequences were simple. Conclusion: Scrotoschisis is an extremely rare birth defect whose etiology is poorly understood. However, it could be the cause of meconium peritonitis secondary to a scarred fatal intestinal perforation.

本文研究了复合酶制剂对AA肉仔鸡生长、屠宰、免疫和抗氧化性能以及肉品质的影响。48只体重相近的健康AA肉仔鸡随机分为对照组和试验组。对照组饲喂基础日粮,试验组饲喂基础日粮添加复合酶制剂。第一部分：主要研究复合酶制剂对AA肉仔鸡生长和屠宰性能的影响。结果表明,试验组肉仔鸡的采食量、增重和饲料效率高于对照组,但差异不显著(P>0.05)。与对照组相比,试验组的胸肌率和瘦肉率有极显著提高(P0.05)。第二部分：主要研究复合酶制剂对AA肉仔鸡免疫功能和抗氧化性能的影响。结果表明,复合酶制剂对21日龄肉仔鸡胸腺指数、脾脏指数和法氏囊指数无显著影响(P>0.05)。与对照组相比,试验组21日龄肉仔鸡血清中TNF-α含量极显著减少(P0.05)。同时与对照组相比,试验组42日龄肉仔鸡血清中MDA含量降低了23.55%(P>0.05),而SOD活性、CAT活牲、GSH-Px活性和T-AOC分别提高了16.60%(P>0.05)、7.29%(P>0.05)、13.43%(P>0.05)和20.06%(P0.05)和12.61%(P>0.05),而SOD活性、GSH-Px活性和T-AOC分别比对照组提高了21.18%(P>0.05)、81.18%(P>0.05)和31.90%(P>0.05)。第三部分：主要研究复合酶制剂对AA肉仔鸡肉质的影响。结果表明,复合酶制剂的添加对肉仔鸡胸肉营养成分无显著影响(P>0.05)。与对照组相比,试验组的L*、a*、b*均无显著变化(P>0.05);pH45和pH24均有提高的趋势,但差异也不显著(P>0.05);滴水损失和蒸煮损失分别降低了16.95%和15.14%,但差异也不显著(P>0.05);剪切力显著降低了24.45%(P0.05)。复合酶制剂对鸡胸肉的硬度、内聚性、弹性、胶粘性和咀嚼性均无显著性影响(P>0.05)。与对照组相比,试验组的总蛋白溶解度、肌浆蛋白溶解度和肌原纤维蛋白溶解度均有所提高,但差异不显著(P>0.05);磷脂酶A2含量也有所降低,但差异不显著(P>0.05)。

水稻是我国最主要的粮食作物之一,60%以上的人口以稻米为主食。稻田是农业环境中CH4排放的重要来源,约占全球人为排放量的1/4。如何在持续提升水稻产量的同时,减少农田温室气体的排放,实现高产低碳稻作已经成为亟待解决的重要科学问题和生产技术。已有研究表明,土壤耕作措施和秸秆还田均可以影响稻田温室气体排放,但对其交互效应尚不明确。因此,本文以我国主要的水稻生产区域-双季稻区为研究对象,开展了不同耕作方式和秸秆还田方式对双季稻区水稻生产力和稻田温室气体排放影响的研究,同时在100年的尺度上评价不同处理下稻田的周年温室效应。主要研究结论如下:(1)与翻耕相比,旋耕条件下早稻和晚稻的产量均较高,但差异不显著;秸秆还田显著提高早稻和晚稻的产量。水稻产量的高低主要取决于有效穗数和穗粒数。其中,晚稻产量主要受穗粒数的影响,与翻耕秸秆全量还田相比,翻耕根茬还田、旋耕根茬还田和旋耕秸秆全量还田处理下的穗粒数分别高出7.9%、23.9%和10.6%;早稻产量则主要受有效穗数的影响,与旋耕秸秆全量还田相比,翻耕根茬还田、翻耕秸秆全量还田和旋耕根茬还田处理有效穗数分别高出约13.7%、6.2%和21.6%。(2)秸秆还田显著增加了早稻和晚稻稻田CH4的排放,但降低了冬闲期CH4的排放。与旋耕相比,翻耕显著降低了两季水稻稻田CH4的排放,但对N2O的影响不显著。早稻和晚稻稻田CH4排放均呈现出双峰的趋势,但第一个峰值出现时间有所不同,晚稻峰值出现在分蘖盛期,而早稻峰值出现略晚。与根茬还田相比,翻耕方式下晚稻季、冬闲和早稻季秸秆换全量还田下CH4排放量分别增加了47.3%、26.1%、5.4%,旋耕方式下则分别增加了39.7%、7.1%、6.1%。与旋耕相比,根茬还田下晚稻季、冬闲、早稻季翻耕处理的CH4排放分别降低了96.1%、45.4%、30.0%;而秸秆全量还田下则分别降低了约86.0%、82.7%和30.8%。(3)在100年尺度上,CH4的排放量决定全球增温潜势(GWP)。本研究中,GWP呈现出和CH4累积排放量一样的趋势,即秸秆全量还田大于根茬还田,旋耕大于翻耕,且差异均达到显著水平。稻田温室气体排放强度(GHGI)由产量和GWP共同决定。GHGI在晚稻季趋势与GWP一致,而早稻季差异不显著。周年效应上,由于冬闲期CH4排放量较低,故GWP与CH4排放趋势一致,即秸秆全量还田显著大于根茬还田,旋耕显著大于翻耕。由于翻耕+根插还田处理中晚稻产量低,所以在翻耕秸秆全量还田处理中周年GHGI最低。因此,对于双季稻区而言,在翻耕方式下进行秸秆全量还田是一种较好的高产低碳稻作措施。

Introduction: Central serous chorioretinopathy (CSCR) is associated with serous sensorineural retinal detachment in the macular region. The condition that affects the male gender more than their female counterpart results from focal leakage disturbance of retinal pigment epithelial cells and choroidal abnormalities. The corticosteroid is highlighted as one of the major factors that promote the occurrence of CSCR. Stress and pregnancy, especially in the third trimester, have also been reported by some authors. Central serous chorioretinopathy associated with pregnancy complicated by retroplacental hematoma is a rare event. Observation: We report the case of a pregnant woman at approximately 34 weeks of amenorrhea who presented a sudden and severe drop in visual acuity on a pregnancy complicated by retroplacental hematoma and delivered by cesarean section. The diagnosis was based on clinical arguments associated with photos of the retina made by the MII Ret Cam device (Made In India, Retinoscope Camera) highlighting the lifting of the macular region. Ophthalmological monitoring without medical or physical treatment was instituted. At two months of progression without treatment, bilateral visual acuity was 10/10. Conclusion: Central serous chorioretinopathy and pregnancy is not unusual;however, its bilateral and brutal form in a context of pregnancy complicated by retroplacental hematoma is rare. Despite this favorable evolution, monitoring remains necessary in order to prevent recurrences and complications.

Dynamic Detection of Reinforced Concrete Bridge Damage by Finite Element Model Updating

The present work consists of dynamic detection of damages in reinforced concrete bridges by using a MMUM （mathematical model updating method） from incomplete test data. A well suited finite element model of a repaired bridge is carried out. The diagnosis enables us to locate and detect the damage in a reinforced concrete bridge. Thus, developments of analytical predictions have been checked by modal testing techniques. Besides, the FTCS （finite time centered space） scheme is developed to solve the set of equations which can easily handle finite element matrices of a bridge model. It is shown in this study that the method is applied to detect damages as well as existing cracks in real time of a repaired bridge. To check the efficiency of the method, the repaired bridge of OuedOumazer in Algeria has been selected. It is proven that identification methods have been able to detect the exact location of damage areas to be corrected avoiding the inaccuracy from the finite element model for the mass, stiffness and loading.
