We consider the problem of viscosity solution of integro-partial differential equation( IPDE in short) with one obstacle via the solution of reflected backward stochastic dif ferential equations(RBSDE in short) with jumps. We show the existence and uniqueness of a continuous viscosity solution of equation with non local terms, if the generator is not monotonous and Levy's measure is infinite.

本文主要研究Conakry和它周边的两个城市Coyah和Duberka的土地利用情况,像大多数其它的海岸带一样,这个地区在在过去的几十年内土地利用/覆盖变化已经发生了很大的变化。政府机构和科研人员都开始关注。本文主要是对该地区的土地利用覆盖变化做出评价,并推求变化的原因。由于人口的增长和国家城市发展的战略和计划,有力的推动了这三个城市之间的城市化进程。农业生产力低下,畜牧业、手工艺术活动、交通和工业的发展,这些经济活动都对土地利用覆盖产生很大的影响。基于遥感技术和地理信息技术(GIS),对ETM+数据进行解译和地图数字化,采用人口增长模型和地理信息系统的叠加分析研究土地里覆盖变化状况。本文主要研究1941年和2000年土地利用覆盖的空间格局和在这个时段内的变化情况。这两个时期的土地覆盖空间格局主要表现为:1941年土地利用景观格局为原始状态,而2000年则受到人类的严重干扰。所以在这59年中,土地利用覆盖发生了变化,这种变化与人口增长、气候变化、城市化、森林覆盖率降低以及农业和工业的增长等方面有关。因此对变化结果的分析有助于我们对今后社会经济的发展中采取有效的措施制止土地变化向不利的方向发展。对于本研究区,研究结果显示,土地覆盖变化已经使得整体的生态环境恶化,在今后的经济发展过程中,要求的政府部门制定一系列严厉的、持续的政策,来合理地开发和管理土地覆盖资源,并且能够对热带地区土地利用覆盖变化过程提供参考价值。

Prevalence and Associated Factors of Postpartum Depression: A Study Conducted in Health Centres in Dakar (Senegal)

Over the years there has been many well documented cases of major oil pollution from leaked offshore oil rigs, ships in distress and from coastal oil facilities. In response to concerns about the possibility of a major oil spillage occurring along the coast of Guinea, the Guinean government drew up a plan and organisation. The purpose of this research was to develop Guinean Marine Oil Spill Response Information System. To do this, we simulated an oil spill concentration transport. The results show that the numerical model in oil spill accident simulation is important in allowing setting up of possible response and contingency planning. From the results we conclude that, the search strategies for futures responses and contingency planning to possible oil spill case in Guinean coasts has been clarified. Implications of research findings allow international community to consider reasonable estimation of spilled oil trajectory with limited environmental input data.

Introduction: Postpartum depression is the most common psychiatric disorder affecting women in the perinatal period. According to the WHO, it affects 10% - 15% of women who have just given birth. In Africa, and particularly in Senegal, epidemiological studies focusing exclusively on postpartum depression are rare or non-existent. For this reason, we conducted this screening study on postpartum depression to assess the socio-demographic characteristics and associated factors. Methods: This was a prospective cross- sectional study with descriptive and analytical aims. It was carried out in the twelve Dakar referral health centres during the period August 2021 to June 2022. Results: At the end of our survey we had recruited one hundred and twenty-two (122) participants with a PPD prevalence rate of 25.41%. The most common age group was 24 - 29 years. 40% of the participants were of Fulani ethnicity, followed by Wolof. Married women made up 93% of the sample. The school enrolment rate was 89%. Most participants were not in paid employment. Domestic violence was present in 14% of couples. 28% of mothers reported experiencing a stressful life event during their pregnancy. 22% of the women had a chronic medical condition, of which 55% had high blood pressure. 1% of the women reported a personal psychiatric history and 5% a family psychiatric history. Most women had between two and four healthy living children. The pregnancy was unwanted in 17% of cases and 12% of participants had a complication. 6% were dissatisfied with the quality of their antenatal care. The caesarean section rate was 18%. In terms of associated factors, eight factors were identified in a bivariate analysis: single marital status, lack of support from partner, domestic violence, conflict with in-laws, and the occurrence of a stressful life event during pregnancy, the presence of a personal medical history, lack of satisfaction with pregnancy follow-up, and mode of delivery by caesarean section. Introduction: Oesophageal mycosis (OM) is one of the most common opportunistic infections in patients infected with HIV (Human Immunodeficiency Virus). However, this condition is increasingly observed in immunocompetent subjects. The aim of this study was to determine the endoscopic prevalence, clinical characteristics and risk factors for the occurrence of oesophageal mycosis in our department. Patients and Method: This was a prospective cross-sectional study of all patients who underwent oeso-gastroduodenal fibroscopy during the period from 1st January to 31st December 2022, i.e. one year, at the digestive endoscopy unit of the hepato-gastroenterology department of the Donka CHU national hospital in Conakry. All patients found to have oesophageal mycosis by FOGD were included. The endoscopy was performed using appropriate equipment: A Fujinon 4400 video endoscopy column;Three Fujinon EG 590 video gastroscopes;A hoover;Data were collected using a pre-established survey form and analysed using Epi info software version 6.0.4;Pearson’s Chi2 test as a test of independence and the exact 5% threshold ficher test. Results: Out of 1343 upper gastrointestinal endoscopies performed, 107 cases of oesophageal mycosis were found, representing a prevalence of 7.96%. The mean age was 40 years, with a male predominance of 55.42%. The sex ratio M/F was 1.24. The 45 and over age group was the most affected, with a prevalence of 40.43%, followed by the [35 - 45] age group, with a prevalence of 22.43%. Clinical symptoms were dominated by epigastralgia in 74.76% of cases, followed by odynophagia in 37.38% of cases, nausea and vomiting in 28.03% of cases, and pyrosis in 26.16% of cases. Oesophageal mycosis without oesophagitis was the most common endoscopic finding in 70% of cases. The main associated endoscopic lesions were erythemato-erosive and congestive gastropathy in 28.03% of cases, peptic oesophagitis (9.34%) and gastric ulcer (5.60%). The main risk factor

Introduction: Fibroscan is a recent, non-invasive and non-irradiating diagnostic method. It is based on the principle of ultrasound, which enables liver tissue elasticity to be quantified using a probe, and fibrosis to be assessed. Fibroscan measures both elasticity correlated with hepatic fibrosis and CAP correlated with steatosis. The aim of this study was to evaluate hepatic fibrosis and steatosis using pulse elastometry (Fibroscan/CAP). Methods: This was a descriptive and analytical cross-sectional study in which 170 patients were included. It was conducted from October 1 2021 to December 31 2023, i.e. 27 months, in an outpatient clinic in the hepato-gastroenterology department of the Donka national hospital of the CHU Conakry. Results: Of the 170 patients identified, 87 were male (51%) and 83 female (49%), giving a M/F sex ratio of 1.04. The average age of our patients was 40. The 30 - 50 age group was the most affected, with a frequency of 58.23% (n = 99), followed by the 50 age group with a frequency of 29.41% (n = 50). Hepatomegaly, steatotic liver on ultrasonography, transaminase elevation and obesity were the main indications, respectively: (21.76%), (17.65%), (14.71%), and (13.53%). The examinations were requested by hepatogastroenterologists (47.06%), diabetologists (35.88%) and general practitioners (29%). Of the 170 patients, 100 patients (58.82%) had no significant fibrosis F0F1, 39 (22.94%) had moderate fibrosis F2, 20 patients (11.76%) had severe fibrosis F3 and 11 patients (6.47%) had fibrosis F4. Hepatic steatosis: 62 patients (36.47%) had no S0 steatosis;29.41% had S1 steatosis, 20% had S2 steatosis and 24 patients (14.11%) had S3 steatosis. Abdominal ultrasound revealed a normal liver in 67.05% of patients, hepatic steatosis in 29.41% and non-decompensated cirrhosis in 6 cases. Introduction: Gastric infection by “Helicobacter pylori” remains a topical issue due to the evolving scientific data concerning its pathophysiology, the diseases and pathologies concerned, which now extend beyond the gastric or digestive sphere, and the treatment methods faced with the development of antibiotic resistance. Diagnosis of infection involves two inseparable aspects: identification of the bacterium itself and identification of the endoscopic and histological lesions caused by the bacterium. Objective: To evaluate the rate of eradication of Helicobacter pylori infection by bismuth quadruple therapy and concomitant treatment. Patients and methods: This was a prospective, cross-sectional, analytical study of all outpatients in the hepato-gastroenterology department during the study period from 1 January 2022 to 30 November 2023. All patients had undergone oeso-gastroduodenal fibroscopy and the diagnosis was made by histological examination of the gastric biopsy. Results: Our 113 patients comprised 68 men and 46 women. The sex ratio was 1.48 in favour of men. The mean age of our patients was 40.28 years, with extremes of 13 and 80 years. The clinical signs that prompted oeso-gastroduodenal fibroscopy were as follows: epigastralgia (69.91%), dyspepsia (14.16%), vomiting (7.08%), gastro-oesophageal reflux (6.19%) and altered general condition with weight loss (2.65%). The endoscopic lesions were: gastropathy (antral, fundic and diffuse: antro-fundial) (69.02%), bulbar ulcer (6.19%), gastric ulcer (5.31%). Helicobacter pylori infection was associated with chronic gastritis in all our patients;it was mild in 50%, moderate in 41% and severe in 9%. The eradication rate of Helicobacter pylori according to the treatment protocol used was 92.30% for bismuth quadruple therapy and 94.12% for concomitant quadruple therapy. Introduction: Ureteroscopy is a minimally invasive endoscopic surgery which provides access to the ureter, pyelon and calyceal cavities via the urethra and the bladder. Laser ureteroscopy uses the laser as an energy source to treat the stone and eliminate it naturally. Minimally invasive endoscopic methods are struggling to become popular in sub-Saharan African countries, especially for the upper urinary tract. The objective of our work was to report the results of our first laser ureteroscopy experience in the department. Materials and Methods: This was a prospective and descriptive study running from December 1, 2023 to February 19, 2024. Included in our study was any case of upper urinary tract stone operated by Laser ureteroscopy. The characteristics of the lithiasis were determined by CT scan. Sterilization of urine was verified by carrying out a cytobacteriological examination of urine. Ureteral lithiasis was approached by semi-rigid ureteroscopy. Renal lithiasis was immediately addressed by flexible ureteroscopy. Ureteroscopy was coupled with a Holmium YAG laser. A double J ureteral catheter was placed after the operation. A 230 µm laser fiber was used in each case with a generator with a power of 35 watts (Storz Calculase III type). An access sheath was used in all cases of flexible ureteroscopy. The parameters studied were: sociodemographic characteristics, lithiasis (site, size, number, density, topography), type of anesthesia, duration of laser use, duration of intervention, postoperative outcomes. Data entry and analysis were carried out using the software (Word 2016 and SPSS). Result: We collected 30 cases of laser ureteroscopy. The average age was 37 years with extremes of 9 and 79 years. The male gender was more represented. The most common age group was 24-39 years old. Renal colic was the most frequent reason for admission, 12 patients (40%). On physical examination, lumbar tenderness was present in 47% (14 patients). ECBU was positive in 4 patients

Introduction: Ametropias or refractive errors are pathologies commonly encountered in ophthalmology characterized by the projection on the retina of a blurry image. There are several types of ametropia and each of them has a specific means of correction. In the literature, numerous data have been reported on the issue through studies that were mostly carried out on infant populations. To better treat ametropia in a more formal way, it is important to know their general characteristics within a larger population, hence the present investigation, the aim of which is to carry out an epidemiological study of ametropia at the Kati University Hospital-BSS. Materials and Methods: The study was carried out at Kati University Hospital-BSS between January 1 and December 31, 2022 and included 663 ametropic patients of all ages. Results: During the study period, 663 cases of ametropia were diagnosed out of 1678 new consultations, representing an incidence of 39.51%. Patients aged between 21 and 40 years were more affected. Depending on the type, these ametropias were dominated by compound astigmatism with 68.75% followed by isolated hyperopia and isolated astigmatism. Visual fatigue was the most mentioned reason for consultation. The female gender was in the majority with 70.9%. Low ametropia (Discussion: The growing visual needs of young people, especially with new information and communication technologies, would push them to consult the ophthalmologist more, and therefore to want optical correction in the event of a refractive anomaly, which would partly explain their high frequency in this age group. Conclusion: It is important to know the general characteristics of ametropia in the general population in order to improve the quality of life of patients affected by it through efficient therapeutic solutions.

Due to the status of the Dakar region as the former capital of AOF (French West Africa) and current capital of the Republic of Senegal, it is home to a very large part of the population and most of the socio-economic, administrative and cultural activities on a very small area of the country (0.28%). This situation makes it a very attractive region and subject to strong land pressure. The objective of this article is to study past and current urban dynamics as well as changes in natural spaces, in order to identify new urban centers. The methodological approach consists in analyzing satellite images to understand the evolution of different forms of spatial occupation in the Dakar region, and to study population movement flows and urban forms in order to reveal new urban centers. The results of the study show that the Dakar urban space is characterized by diverse forms which express a heterogeneous set of spaces. They also revealed the emergence of new clusters between 1986 and 2016, notably Ngor, Grand Yoff, Parcelles Assainies in the North-West, and Keur Massar and the North of the communes of Rufisque in the eastern part of the Dakar region.
