Association between the elderly frequent attender to the emergency department and 30-day mortality： A retrospective study over 10 years

BACKGROUND: To determine if elderly frequent attenders are associated with increased 30-day mortality, assess resource utilization by the elderly frequent attenders and identify associated characteristics that contribute to mortality. METHODS: Retrospective observational study of electronic clinical records of all emergency department(ED) visits over a 10-year period to an urban tertiary general hospital in singapore. Patients aged 65 years and older, with 3 or more visits within a calendar year were identified. Outcomes measured include 30-day mortality, admission rate, admission diagnosis and duration spent at ED. Chi-square-tests were used to assess categorical factors and Student t-test was used to assess continuous variables on their association with being a frequent attender. Univariate and multivariate logistic regressions were conducted on all significant independent factors on to the outcome variable(30-day mortality), to determine factor independent odds ratios of being a frequent ***: 1.381 million attendance records were analyzed. Elderly patients accounted for 25.5% of all attendances, of which 31.3% are frequent attenders. Their 30-day mortality rate increased from 4.0% in the first visit, to 8.8% in the third visit, peaking at 10.2% in the sixth visit. Factors associated with mortality include patients with neoplasms, ambulance utilization, male gender and having attended the ED the previous ***: Elderly attenders have a higher 30-day mortality risk compared to the overall ED population, with mortality risk more marked for frequent attenders. This study illustrates the importance and need for interventions to address frequent ED visits by the elderly, especially in an aging society.

Packing and film thickness theories for the mix design of high-performance concrete

A high-performance concrete （HPC） is required to have superior performance in various aspects such as workability,strength, durability, dimensional stability, segregation stability, and passing ability. The mix design of HPC is rather complicatedbecause the number of ingredients in HPC is usually more than those in conventional concrete and some of the required propertiesare conflicting with each other in the sense that improvement in one property would at the same time cause impairment of anotherproperty. However, there is still lack of understanding regarding how the various mix parameters should be optimised forachieving best overall performance. Most practitioners are still conducting mix design primarily through trial concrete mixing,which is laborious, ineffective, and often unable to timely respond to fluctuations in the properties of raw materials. To addressthese issues, the authors have been developing the packing and film thickness theories of concrete materials, in order to revamp themix design philosophy of HPC in terms of the water film thickness （WFT）, paste film thickness （PFT）, and mortar film thickness（MFT） in the concrete. Based on the findings from an extensive experimental programme, suitable ranges ofWFT, PFT, and MFThave been recommended.

Monday blues of deceased-donor liver transplantation

BACKGROUND:There is a constant and global shortage of deceased-donor organs for *** to identify areas for securing potential deceased-donor organs may improve the supply and hence benefit more patients in need of transplantation. METHODS:We looked into the disparity of the number of deceased-donor liver transplantation(DDLT)performed at our hospital on different days of the weeks from January 2000 to the end of December 2009(237 DDLTs).The number of DDLT performed on each day was compared with the other days of the week. RESULTS:It was apparent that there were fewer DDLTs on Mondays,as shown by the numbers of DDLT performed on different days of the week in an ascending order:Monday 18(7.6%),Sunday 30(12.7%),Thursday 34(14.3%),Friday 36(15.2%),Wednesday 38(16.0%),Tuesday 40(16.9%), and Saturday 41(17.3%).The difference reached statistical significance when Monday was compared with Tuesday (P=0.019),Wednesday(P=0.010),Friday(P=0.021),and Saturday(P=0.007).It was twice as unlikely a DDLT would be performed on Monday as compared with other *** a trend did not change even with an increase in the number of deceased-donor liver grafts in the last *** consent to donation was obtained from the donor family the day before DDLT,fewer consents were thus obtained on ***:These findings suggested that deceased-donor organ donation activities were less active on Sundays and could be *** further raises the concern of possible wastage of potential cases of organ donation.

Rhizobium bacteria are known as symbionts of legumes for developing nodules on plant roots and fixing N2 for the host plants but unknown for associations with ***,we detected,isolated,and characterized a Rhizobium species from the marine toxic dinoflagellate Gambierdiscus *** 16S rRNA gene(rDNA)is 99%identical to that of Rhizobium rosettiformans,and the affiliation is supported by the phylogenetic placement of its cell wall hydrolase-encoding gene(cwh).Using quantitative PCR of 16S rDNA and cwh,we found that the abundance of this bacterium increased during the late exponential growth phase of Gambierdiscus and under nitrogen limitation,suggesting potential physiological interactions between the dinoflagellate and the *** is the first report of dinoflagellate-associated Rhizobium bacterium,and its prevalence and ecological roles in dinoflagellate-Rhizobium relationships remain to be investigated in the future.

Epstein Barr virus （EBV） positive mucocutaneous ulcers（EBVMCU） form part of a spectrum of EBV-associatedlymphoproliferative disease. They have been reportedin the setting of immunosenescence and iatrogenicimmunosuppression, affecting the oropharyngeal mucosa,skin and gastrointestinal tract （GIT）. Case reports andseries to date suggest a benign natural history respondingto conservative management, particularly in the GIT. Wereport an unusual case of EBVMCU in the colon, arisingin the setting of immunosuppression in the treatment ofCrohn＇s disease, with progression to Hodgkin lymphoma18 mo after cessation of infliximab. The patient presentedwith multiple areas of segmental colonic ulceration,histologically showing a polymorphous infiltrate withEBV positive Reed-Sternberg-like cells. A diagnosisof EBVMCU was made. The ulcers failed to regressupon cessation of infliximab and methotrexate for 18mo. Following commencement of prednisolone for herCrohn＇s disease, the patient developed widespreadHodgkin lymphoma which ultimately presented as alife-threatening lower GIT bleed requiring emergencycolectomy. This is the first report of progression ofEBVMCU to Hodgkin lymphoma, in the setting of ongoingiatrogenic immunosuppression and inflammatory boweldisease.

Influences of fiber length and water film thickness on fresh properties of basalt fiber-reinforced mortar

In plain mortar,the water film thickness(WFT)has been found to play a key role in the fresh ***,in fiber-reinforced mortar,the role of WFT has not been investigated *** this research,basalt fibers of different lengths were added to the mortar,and the dynamic and static flowability,cohesiveness,adhesiveness,and packing density were tested to study the effects of fiber length on the packing density and WFT,and the combined effects of fiber length and WFT on the fresh *** results showed that in fiber-reinforced mortar,the WFT also plays a key role,whereas the fiber length exerts its influences through the indirect effects on the packing density and WFT and the direct effect on fiber-mortar ***,an increase in fiber length decreases the packing density and WFT,decreases the dynamic and static flowability needed for placing,increases the cohesiveness needed for avoiding segregation,and,quite unexpectedly,decreases the adhesiveness needed for rendering and spraying *** analysis yielded good correlation of the fresh properties to fiber length and WFT,and best-fit formulas for the mix design for basalt fiber-reinforced mortar were obtained.

Main nutritional contents of 30 Dalian coastal microalgae species

This paper reports results of study on the contents of proteins, amino acids, polysaccharose and uronic acids in 30 species of macroalgae from Shicao, Heishijiao, Shimiao, and Xiaofujiazhuang in the vicinity of Dalian City, ***. The results showed that the protein contents of the 30 algae from highest (112.55μg/ml) to the lowest (0.24 靏/ml) was in the descending order of Dictyopteris ndalata, Gelidium vagum, Gymnogongrus japonican, Ectocarpus confervoides, Tinocladia crassa, Sargassum thunberii. In general, the protein content in red algae was higher than that in brown algae. The content of free amino acids showed no significent differences from 7.44 靏/ml-4.96 靏/ ml in all these algae, in the descending order of Gymnogongrus japonican, Sargassum confusum, Undoria pinnatifida, laminaria japonica and Ectocarpus confervoides. The content of polysaccharose varied from 168.2 靏/ml-22.15 靏/ml in the descending order of Symphyocladia latiuscula, Scytosiphon lomentarius, Desmarestia viridis., Tinocladia crassa, Gracilaria asiatica and Porphyra yezoensis. The content of uronic acids is from 196.24 靏/ml-20.77 靏/ml in the descending order of Ulva lactuca, Symphyoclaldia latiuscula, Scytosiphon lomentarius, Ceramimum kondoi, Gracilaria vemucosa and Porphyra yezoensis. The fatty acids in 30 species of algae belong to Rhodophyta, Chlorophyta and Phaeophyta. Most phaeophytes have many (4-12) types of fatty acids.
