AIM： To investigate the role of major non-protein and protein sulfhydryls and disulfides in chemically induced gastric hemorrhagic mucosal lesions （HML） and the mechanism of gastroprotective effect of sucralfate.METHODS： Rats were given 1 mL of 75% ethanol, 25%NaCl, 0.6 mol/L HCI, 0.2 mol/L NaOH or 1% ammonia solutions intragastrically （i.g.） and sacrificed 1, 3, 6 or 12 min later. Total （reduced and oxidized） glutathione （GSH ＋ GSSG）, glutathione disulfide （GSSG）, protein free sulfhydryls （PSH）, protein-glutathione mixed disulfides （PSSG） and protein cystine disulfides （PSSP） were measured in gastric mucosa and liver.RESULTS： Reduced glutathione （GSH） was depleted in the gastric mucosa after ethanol, HCI or NaCl exposure,while oxidized glutathione （GSSG） concentrations increased, except by HCI and NaOH exposure. Decreased levels of PSH after exposure to ethanol were observed,NaCl or NaOH while the total protein disulfides were increased. Ratios of reduced to oxidized glutathione or sulfhydrils to disulfides were decreased by all *** changes in thiol homeostasis were detected in the liver after i.g. abbreviation should be spelled out the first time here administration of ethanol. Sucralfate increased the concentrations of GSH and PSH and prevented the ethanol-induced changes in gastric mucosal thiol concentrations.CONCLUSION： Our modified methods are now suitable for direct measurements of major protein and nonprotein thiols/disulfides in the gastric mucosa or liver.A common element in the pathogenesis of chemically induced HML and in the mechanism of gastroprotective drugs seems to be the decreased ratios of reduced and oxidized glutathione as well as protein sulfhydryls and disulfides.

Structural and molecular features of intestinal strictures in rats with Crohn's-like disease

AIM: To develop a new rat model we wanted to gain a better understanding of stricture formation in Crohn’s disease (CD).METHODS: Chronic colitis was induced locally by the administration of 2,4,6-trinitrobenzenesulfonic acid (TNBS). The relapsing inflammation characteristic to CD was mimicked by repeated TNBS treatments. Animals were randomly divided into control, once, twice and three times TNBS-treated groups. Control animals received an enema of saline. Tissue samples were taken from the strictured colonic segments and also adjacent proximally and distally to its 60, 90 or 120 d after the last TNBS or saline administrations. The frequency and macroscopic extent of the strictures were measured on digital photographs. The structural features of strictured gut wall were studied by light- and electron microscopy. Inflammation related alterations in TGF-beta 2 and 3, matrix metalloproteinases 9 (MMP9) and TIMP1 mRNA and protein expression were determined by quantitative real-time PCR and western blot analysis. The quantitative distribution of caspase 9 was determined by post-embedding ***: Intestinal strictures first appeared 60 d after TNBS treatments and the frequency of them increased up to day 120. From day 90 an intact lamina epithelialis, reversible thickening of lamina muscularis mucosae and irreversible thickening of the muscularis externa were demonstrated in the strictured colonic segments. Nevertheless the morphological signs of apoptosis were frequently seen and excess extracellular matrix deposition was recorded between smooth muscle cells (SMCs). Enhanced caspase 9 expression on day 90 in the SMCs and on day 120 also in myenteric neurons indicated the induction of apoptosis. The mRNA expression profile of TGF-betas after repeated TNBS doses was characteristic to CD, TGF-beta 2, but not TGF-beta 3 was up-regulated. In the manufacturing processes of high value-added products in the pharmaceutical, fine chemical polymer and food industry, insufficient control might produce off-grade products. This can cause significant financial losses, or in the pharmaceutical industry, it can result in an unusable batch. In these industries, batch reactors are commonly used, the control of which is essentially a problem of temperature control. In the industry, an increasing number of heating-cooling systems utilising three different temperature levels can be found, which are advantageous from an economic point of view. However, it makes the control more complicated. This paper presents a split-range designing technique using the model of the controlled system with the aim to design a split-range algorithm more specific to the actual sys- tem. The algorithm described provides high control performance when using it with classical PID-based cascade temperature control of jacketed batch reactors;however, it can be used with or as part of other types of controllers, for ex- ample, model-based temperature controllers. The algorithm can be used in the case of systems where only two as well as where three temperature levels are used for temperature control. Besides the switching between the modes of opera- tion and calculating the value of the manipulated variable, one of the most important functions of the split-range algo- rithm is to keep the sign of the gain of the controlled system unchanged. However, with a more system-specific split-range solution, not only can the sign of the gain be kept unchanged, but the gain can also be constant or less de- pendent on the state of the system. Using this solution, the design of the PID controller becomes simpler and can be implemented in existing systems without serious changes.

To reach an acceptable controller strategy and tuning it is important to state what is considered “good”. To do so one can set up a closed-loop specification or formulate an optimal control problem. It is an interesting question, if the two can be equivalent or not. In this article two controller strategies, model predictive control (MPC) and constrained direct inversion (CDI) are compared in controlling the model of a pilot-scale water heater. Simulation experiments show that the two methods are similar, if the manipulator movements are not punished much in MPC, and they act practically the same when a filtered reference signal is applied. Even if the same model is used, it is still important to choose tuning parameters appropriately to achieve similar results in both strategies. CDI uses an analytic approach, while MPC uses numeric optimization, thus CDI is more computationally efficient, and can be used either as a standalone controller or to supplement numeric optimization.

AIM:To analyze the difference in disease course and need for surgery in patients with Crohn’s disease(CD).METHODS:Data of 506 patients with incident CD were analyzed(age at diagnosis:31.5±13.8 years).Both hospital and outpatient records were collected prospectively with a complete clinical follow-up and comprehensively reviewed in the population-based Veszprem province database,which includes incident CD patients diagnosed between January 1,1977 and December 31,***-up data were collected until December 31,*** patients included had at least 1year of follow-up *** with indeterminate colitis at diagnosis were excluded from the ***:Overall,73 patients(14.4%)required resective surgery within 1 year of *** exposure and need for biological therapy were lower in patients with early limited surgery(P<0.001 and P=0.09).In addition,surgery rates during follow-up in patients with and without early surgery differed significantly after matching on propensity scores(P<0.001,HR=0.23).The need for reoperation was also lower in patients with early limited resective surgery(P=0.038,HR=0.42)in a Kaplan-Meier and multivariate Cox regression(P=0.04)***,this advantage was not observed after matching on propensity scores(PLogrank=0.656,PBreslow=0.498).CONCLUSION:Long-term surgery rates and overall exposure to steroids and biological agents were lower in patients with early limited resective surgery,but reoperation rates did not differ.

Pulmonary embolization as primary manifestation of hepatocellular carcinoma with intracardiac penetration: A case report

Intracardiac manifestation of hepatocellular carcinoma (HCC) is a rare condition and an uncommon finding even at autopsy. Pulmonary tumor embolism as a presenting feature of HCC has been published only twice previously. In our case report, a 63-year-old man presented with high fever and six episodes of recurrent pneumonias during the last half year. Echocardiography was performed, a solid mass was found in the right atrium. Transesophageal echocardiography proved a tumor mass in the inferior vena cava (IVC) extending into the right atrium, abdominal ultrasound revealed tumor mass in the IVC and a solid tumor in the liver. Combined liver and heart surgery was attempted in order to remove the tumor mass from both the liver and the right atrium. Acute cor pulmonale occurred during tumor removal from the right atrium and the patient expired. In addition to local factors the possibility of embolization should arise in the background of recurrent pneumonia. Occult carcinoma must be included in possible causes of recurrent pulmonary embolism. Searching for primary malignancy should include HCC as frequent cause of hypercoagulability. In case of HCC, echocardiography is suggested because of the possibility of expansion in IVC or right atrium and tumor-embolization.
