Total pancreatectomy with islet cell transplantation vs intrathecal narcotic pump infusion for pain control in chronic pancreatitis

AIM: To evaluate pain control in chronic pancreatitis patients who underwent total pancreatectomy with islet cell transplantation or intrathecal narcotic pump ***: We recognized 13 patients who underwent intrathecal narcotic pump(ITNP) infusion and 57 patients who underwent total pancreatectomy with autologous islet cell transplantation(TP + ICT) for chronic pancreatitis(CP) pain control between 1998 and 2008 at Indiana University Hospital. All patients had already failed multiple other modalities for pain control and the decision to proceed with either intervention was made at the discretion of the patients and their treating physicians. All patients were evaluated retrospectively using a questionnaire inquiring about their pain control(using a 0-10 pain scale), daily narcotic dose usage, and hospital admission days for pain control before each intervention and during their last follow-up. REsUlTs: All 13 ITNP patients and 30 available TP + ICT patients were evaluated. The mean age was approximately 40 years in both groups. The median duration of pain before intervention was 6 years and 7 years in the ITNP and TP + ICT groups, respectively. The median pain score dropped from 8 to 2.5(on a scale of 0-10) in both groups on their last follow up. The median daily dose of narcotics also decreased from 393 mg equivalent of morphine sulfate to 8 mg in the ITNP group and from 300 mg to 40 mg in the TP + ICT group. No patient had diabetes mellitus(DM) before either procedure whereas 85% of those who underwent pancreatectomy were insulin dependent on their last evaluation despite ICT. CONClUsION: ITNP and TP + ICT are comparable for pain control in patients with CP however with high incidence of DM among those who underwent TP + ICT. Prospective comparative studies and longer follow up are needed to better define treatment outcomes.

目的对整体护理在急诊急救护理工作中的应用进行研究分析。方法该次的研究对象是该院2013年2月—2016年2月期间收治的24例急诊急救患者。所有患者均采用动态随机化分法,分为研究组12例和对照组12例。对照组患者使用常规护理,另一组患者给予整体护理干预(研究组),观察对比两组急诊急救患者抢救成功率、不良反应发生率、有效抢救时间及护理前后生活质量情况。结果结果得知,研究组患者抢救成功率(91.67%)、不良反应发生率(8.33%)与对照组对比,明显有较大优势(P<0.05);研究组患者有效抢救时间(46.35±2.54)min优于对照组(P<0.05);研究组患者护理后躯体功能评分(82.19±10.30)分、心理功能评分(84.51±11.22)分、社会功能评分(86.71±9.24)分、物质生活评分(83.47±10.23)分均比对照组更高(P<0.05)。结论采用整体护理对急诊急救患者进行护理效果显著,能有效提高抢救成功率,降低不良反应发生率,有效提高患者生活质量,在临床上可以广泛应用。

目的探究阿奇霉素联合痰热清在支气管扩张症治疗中的疗效。方法选择2009年1月至2011年12月确诊为支气管扩张症而就诊的112例患者。随机分为观察组56例和对照组56例。观察组给予阿奇霉素0.5g加入5%葡萄糖注射液250ml;痰热清注射液20ml加入5%葡萄糖注射液250ml静脉用药,每日1次。对照组给予左氧氟沙星0.4g加入5%葡萄糖注射液250ml;盐酸氨溴索30mg加入生理盐水100ml静脉用药,每日1次。结果观察组在症状改善、咳嗽、发热、肺部症状上比对照组有着更好的临床疗效,两组的治愈率比较差异有统计学意义(P<0.05)。结论阿奇霉素联合痰热清在支气管扩张症的治疗中疗效明显。

Background: COVID-19 is a rapidly transmissible pneumonia-like illness caused by sARs-CoV-2 that out broke in China in 2019 and is currently circulating worldwide. In the current context of the sARs-CoV-2 pandemic, complications are observed in clinical settings for the treatment of severe COVID-19 disease in nosocomial settings, due to cases of fungal co-infections. Objective: To carry out a review on fungal infections associated with respiratory infections caused by COVID-19 (sars-Cov-2) and their aggravation. Methodology: The purpose of this study is to inform the reader about the characteristics of sARs-CoV-2 and the main fungal species that are affecting patients undergoing treatment for severe COVID-19, provoking discussion of the importance of the proposed topic, in relation to co-infections by different fungal microorganisms. Result: 80 scientific studies were selected, resulting from patients with COVID-19 and most commonly observed in patients with a history of comorbidities such as diabetes mellitus, hypertension, kidney disease, severe liver disease, oncological diseases, obesity and with severe COVID-19. These data do not represent the total number of records of the disease in the world, but cases reported by researchers in their series, showing the overlapping of fungal co-infection through the compromised immune status due to the use of therapeutic drugs, dysregulation of the microbiota, age of patients and the severity itself of the severe inflammation caused by COVID-19. Conclusion: The immunosuppression caused by the infection of COVID-19 concomitant with its therapy through corticosteroid therapy and comorbidities of the patients made fungal infections more susceptible, and these interfere in the evolution of the case and in the treatment of COVID-19, being relevant to distinguish secondary infections to therapy and the best possible reconstitution of the clinical picture.

Evolution of heat transport pathways in the Indonesian Archipelago during last deglaciation

The Indonesian Archipelago provides important heat transport pathways of the Western Pacific Warm Pool between the northern Indian Ocean and western equatorial Pacific Ocean, that exert important impacts on global climate change. This study investigated AMs 14C, δ18O, planktonic foraminifer assemblages and sedimentation rates in three piston cores collected in the Indonesian Archipelago. The results indicate that changes in the Indonesian Archipelago heat transport pathways were phase characteristic and in steps during the last deglaciation. In the deglaciation Termination ia, at about 12.5 kaBP, sea level rose rapidly in a short time period, and Makassar and lombok straits widened suddenly for warm and fresh water from the Pacific to pour into the Java sea and eastern Indian Ocean. During the Termination IB, about 9.5 kaBP, sea level rose rapidly again, and the south China sea (sCs) started to connect with the Java sea. Physiological Performance of Common Bean seeds Treated with Bioactivator with and without Moisture Restriction

The commonbean (Phaseolus vulgaris l.) is an important agricultural crop due to its nutritional properties. Insecticides and fungicides employed to control pests and diseases in the common bean may cause as yet unknown effects on the plant’s morphology and metabolism. Bioactivators are complex organic substances, capable of exerting modification upon growth patterns, plant DNA transcription, gene expression, metabolic enzymes and mineral nutrition. Thiamethoxam insecticide has shown positive action on the enhancement of vigor expression, phytomass accretion and the promotion of deeper root systems. The aim of this work was to evaluate the effect of thiamethoxam on the physiological performance of common bean seeds. seeds from cultivars Pérola and iaPAR siriri were treated to thiamethoxam concentrations of 0, 100, 200, 300, 400 and 500 ml·kg-1 seed. Physiological quality was measured through tests for germination value, first count, cold test, accelerated aging and seedling shoot and root lengths, all undertaken in the presence and absence of moisture stress. Thiamethoxam stimulated the physiological performance of common bean seeds under moisture stress or in the absence of it, with levels varying according to cultivar and seed lot. The best physiological seed performance was obtained within the range of 200 ml·kg-1 seed to 400 ml·kg-1 seed.

The first search for the doubly heavy Ωbc0baryon and a search for the Ξbc0baryon are performed using pp collision data collected via the lHCb experiment from 2016 to 2018 at a centre-of-mass energy of 13 TeV,corresponding to an integrated luminosity of 5.2 *** baryons are reconstructed via their decays to ∧c+π-andΞc+π-.No significant excess is found for invariant masses between 6700 and 7300 MeV/c2,in a rapidity range from2.0 to 4.5 and a transverse momentum range from 2 to 20 MeV/*** limits are set on the ratio of the Ωbc0andΞbc0production cross-section times the branching fraction to ∧c+π-（Ξc+π-） relative to that of the ∧b0（Ξb0） baryon,for different lifetime hypotheses,at 95% confidence *** upper limits range from 0.5×10-4to 2.5×10-4for theΩbc0→∧c+π-(Ξbc0→∧c+π-) decay,and from 1.4×10-3to 6.9×10-3for the Ωbc0→Ξc+π-(Ξbc0→Ξc+π-) decay,depending on the considered mass and lifetime of the Ωbc0(Ξbc0) baryon.
