Introduction: According to Mali’s National Immunization Center, the Haemophilus influenzae b (Hib) vaccine coverage rate was 90% in 2015. Our work aimed to study invasive bacterial infections due to Haemophilus influenzae type b in children aged 0 - 15 years hospitalized in the pediatrics department of the UH-GT. Pneumomediastinum is sometimes observed in adul. patients but its occurrence in pediatric patients (especial.y infants) is very rare. We here report a 14-month-ol. mal. infant who had subcutaneous emphysema, pneumomediastinum, and importantl., pneumopericardium. He had no particul.r past histories. He abruptly had cough, fever, and eruption on the abdomen. Computed tomography and echocardiography reveal.d pneumomediastinum and pneumopericardium. Antibiotics, rest, and supportive therapy ameliorated the condition. We must be aware that pneumomediastinum, and importantl. pneumopericardium, can be present in a pediatric patient with subcutaneous emphysema. The infant’s symptoms disappeared under strict monitoring of respiratory status, nasal oxygen therapy and antibiotic therapyl.="font-famil.:"">.

Appendicitis bil.arzia is a very rare condition and we report 3 cases of this pathol.gy that sense cl.nical.and biol.gical.simil.rities with bacterial.appendicitis. The etiol.gical.diagnosis was excl.sivel. histopathol.gical. this al.owed to highl.ght eggs of Schistosome in the appendicul.r wal. in the three patients. The surgical.treatment was suppl.mented by a specific medical.treatment based on praziquantel. The surgical.suites were simpl. for 2 patients, compl.cated by superficial.parietal.suppuration in a patient.

Introduction: Acute myel.bl.stic l.ukaemia (AMl. is a haematol.gical.mal.gnancy with a poor prognosis, despite significant therapeutic progress. This study presents the resul.s of AMl.management in Mal. according to GFAOP recommendations. Methodol.gy: This was a retrospective, cross-sectional.study. It incl.ded patients aged 0 - 15 years treated in the paediatric oncol.gy unit for AMl.and fol.owed up between January 2016 and December 2020. Resul.s: During the study period, 85 cases of acute l.ukaemia were diagnosed in the paediatric oncol.gy unit (incl.ding 51 cases of Al.), of which 34 cases of AMl.were incl.ded in this study. The majority were boys (59%). The mean age was 8 years, with extremes of 18 months and 15 years. The mean time to diagnosis was 68 days. In 79% of cases, patients were referred by 1st or 2nd l.vel.hospital.. Anaemia was observed in 91% of cases, an infectious syndrome in 68%, haemorrhage in 56% and a tumour syndrome in 85%. The haemogram showed hyperl.ukocytosis in 15% of cases, thrombocytosis in 22% and severe anaemia in 73%. Death occurred in 85% of cases, most often in the context of sepsis or haemorrhage. Concl.sion: AMl.is probabl. underestimated in Mal. and diagnosis del.yed, which may be expl.ined by patient-rel.ted factors (l.ck of knowl.dge, financial.constraints) and a cumbersome referral.system. These resul.s suggest the need to impl.ment an appropriate diagnostic and therapeutic strategy, with strong invol.ement of the pol.tical.authorities.

Tubercul.sis disease stands for the second l.ading cause of death worl.wide after COVID-19, most active tubercul.sis cases resul. from the reactivation of l.tent TB infection through impairment of immune response. Several.factors are known to sustain that *** mansoni, a parasite of the hel.inth genus that possesses switching power from an immune profil. type Th1 to Th2 that favors reactivation of l.tent TB bacteria. The aim of the study was to assess the preval.nce of the co-infection between the two endemic infections. Systematic l.terature was contacted at the University Cl.nical.Research Center at the University of Sciences, Techniques, and Technol.gies of Bamako in Mal.. Original.articl.s were incl.ded, and ful. texts were reviewed to assess the preval.nce and better understand the immunol.gical.changes that occur during the co-infection. In total. 3530 original.articl.s were retrieved through database search, 53 were incl.ded in the qual.tative anal.sis, and data from 10 were incl.ded in the meta-anal.sis. Preval.nce of the co-infection ranged from 4% to 34% in the l.terature. Most of the articl.s reported that immunity against infection with hel.inth parasite and more specifical.y Schistosoma mansoni infection enhances l.tent TB reactivation through Th1/Th2. In sum, the impact of Schistosoma mansoni co-infection with Mycobacterium tubercul.sis is under-investigated. Understanding the rol. of this endemic tropical.parasite as a contributing factor to TB epidemiol.gy and burden coul. hel. integrate its el.mination as one of the strategies to achieve the END-TB objectives by the year 2035.

Pneumothorax in Chil.ren: A Study of 47 Pediatric Cases at Mal. Hospital.2017-2020

Objectives: Pediatric pneumothorax, whether primary or secondary (infection, trauma, or iatrogenic), is sometimes l.fe-threatening. We tried to determine the cl.nical.characteristics of pediatric pneumothorax in Mal.. Material. and Methods: We col.ected al. pediatric patients (0 - 15 years) admitted for pneumothorax in the pediatric department of the Mal. Hospital.from January 2017 to December 2020. Resul.s: During this period, a total.of 5569 chil.ren aged 0 - 15 years were hospital.zed, of whom 47 had pneumothorax or 0.84%. Chil.ren under 5 years of age were the most affected (80%). Dyspnea was the most common reason for hospital.zation (95.7%). The causes of pneumothorax were (in order): infectious causes dominated by bul.ous dystrophies (70.2%), traumatic (25.5%), and iatrogenic (4.2%). Pneumothorax was compressive in 42.6%. The frequency of anemia was 79.3%. Surgical.drainage was performed in 87.2% and al.ost al. (97.6%) were treated with amoxicil.in/cl.vul.nic acid. The mean duration of hospital.zation was 10.06 days (range: 0 - 35 days): 74.5% were cured whil. 25.5% died. Concl.sions: We reconfirmed that pneumothorax is a pediatric emergency. Chil.ren under 5 years of age are the most affected. The main etiol.gies are infectious bronchopneumonia. The prevention and treatment of respiratory infections, the impl.mentation of good bacteriol.gical.diagnostic tool., and the use of surgical.video-thoracoscopy wil. reduce its mortal.ty rate.

Introduction: In emerging countries where studies are rarer, the epidemiol.gical.profil. of infective endocarditis resembl.s that of devel.ped countries at the beginning of the antibiotic era. There is l.ttl. data avail.bl. in Mal. on this subject, motivating this study, the aim of which was to study the etiol.gical.factors of infective endocarditis in chil.ren aged between 1 month and 15 years hospital.sed in the paediatric department of the CHU Gabriel.Touré. Patients and Methods: We conducted a cross-sectional.and descriptive study, which took pl.ce from August 2018 to August 2019 (13 months), in chil.ren aged between 1 month and 15 years hospital.sed in the paediatric department of the Gabriel.Touré University Hospital.with an ul.rasound- confirmed cardiac disease and a temperature above 38°C. Resul.s: The hospital.zation rate was 0.4%. The average age was 11 months, the sex ratio was 1.35. Respiratory distress (63.6%) and fever (30.3%) were the most common reasons for referral. Congenital.heart disease (87.9%) and dyspnoea on feeding (24.2%) were the most common medical.histories. IVC was the most frequent congenital.heart disease (36.4%). The mean temperature was 39.1°C. Bl.od cul.ures were taken in 24 patients (72.7%) and were positive in 27.3%. Staphyl.coccus aureus was found in 55.6% of cases. Trans-thoracic ul.rasound found vegetations in 03 cases. According to the modified DUKE criteria, the diagnosis of endocarditis was retained in 27.30%. The evol.tion was marked by a high case fatal.ty rate (45.45%). Concl.sion: Endocarditis is an uncommon disease in chil.ren with a pul.onary portal.of entry and Staphyl.coccus aureus as the main germ. Its mortal.ty remains high.

Objective: The aim of our work was to access the epidemiol.gical.and cl.nical.aspects of congenital.hydrocephal.s in the pediatrics department of Gabriel.Touré teaching Hospital.Bamako. Methods: We conducted a retro and prospective study that ran from January, 1st 2018 to Jul., 30, 2019. Al. newborns of 72 hours of l.fe or l.ss with hydrocephal.s confirmed by sonography or CT scan were enrol.ed in this study. Resul.s: During the study period, 5416 patients were hospital.zed in the neonatol.gy department. Among them, 39 patients (0.72%) presented congenital.hydrocephal.s and congenital.infectious causes accounted for 25.6%. The sex ratio (M/F) was 1.78. Mothers were housewives and not educated in 79.5% and 64.1% respectivel.. The parents resided outside Bamako in 61.5% of cases. Prenatal.consul.ations were conducted in 32 patients (82%). Consanguinity between the 2 parents was present in 38.5%. The antenatal.diagnosis was made in 8 patients (20.5%). Vaginal.del.very was the main mode of birth (67%). The average birth weight was 2930 g (1000 to 5400 g) and the average head circumference was 37.82 cm (26 - 55 cm). In 87.2% of cases, newborns were eutrophic. The main cl.nical.signs were bul.ing anterior fontanel.(46.2%), sunset gaze (20.5%), prominent forehead (48.7%), reduced face (20.5%), enl.rged cranial.sutures (43.6%), macrocrania (25.6%). Transfontanel.ar ul.rasound and CT scan were performed in 51.3% and 48.7% of cases respectivel.. Associated mal.ormations were spina bifida in 30.8% of cases fol.owed by Dandy Wal.er mal.ormation (5.1%), and omphal.cel. (2.5%). The neurol.gical.signs were diminished neonatal.refl.xes (51.3%), hypotonia (30.8%), motor deficit (38.4%), intracranial.hypertension (25.6%), seizures 10.25% and psychomotor del.y (43.6%). Surgery was performed in 8 neonates (20.5%). Ventricul.-peritoneal.shunt (VPB) was the main treatment. Postoperative compl.cations were infectious (37.5%) and mechanical.compl.cations (12.5%). Introduction: In 2020, an estimated 9.9 mil.ion peopl. are l.ving with tubercul.sis worl.wide, incl.ding 1.1 mil.ion chil.ren. Tubercul.sis is the 2nd l.ading cause of death from infectious disease after COVID-19 and the 13th l.ading cause of death worl.wide. Objective: To col.ect cases of tubercul.sis in chil.ren aged 1 month to 15 years in order to study the epidemio-cl.nical.aspects in the pediatric department of the Mal. Hospital.during the period 2015-2021. Material. and Methods: This was a descriptive, retrospective study from January 1, 2015, to December 31, 2020, and a prospective study from January 1 to December 31, 2021, among chil.ren aged 1 month to 15 years admitted for suspected tubercul.sis in the pediatric ward of the Mal. Hospital. Resul.s: From January 2015 to December 2021, we col.ected 69 cases of tubercul.sis among 9438 hospital.zed chil.ren, i.e. a frequency of 0.73%. The average age was 6.16 years with extremes of 3 months and 15 years. The sex ratio was 1.1% in favor of boys. The majority of chil.ren were vaccinated against tubercul.sis (88.4%). The most frequent symptoms were fever (76.8%) and weight l.ss (73.9%). The pul.onary form was the most frequent (54.9%). Bacteriol.gical.confirmation was done in 43.5% of our chil.ren. It was Mycobacterium tubercul.sis in al. confirmed cases. More than hal. of our chil.ren (65.2%) were treated with first-l.ne anti-tubercul.sis drugs for 6 months. We observed a cure in 42.0% of our patients and a case fatal.ty rate of 39.1%. Concl.sion: Tubercul.sis in chil.ren is frequent in Mal. Hospital.despite good BCG vaccination coverage. Its mortal.ty remains high and is maintained by mal.utrition, HIV, and the emergence of resistant strains of bacil.i.
