Ulcerated lesion in Buccal Mucosa in a Patient with Paracoccidioidomycosis——Case Report

Paracoccidioidomycosis is the most prevalent systemic mycosis in latin America. It is a fungal infection caused by the dimorphic fungus Paracoccidioides brasiliensis. This microorganism can penetrate the human body through direct contact and can cause intense lymphadenopathy, ulcerated lesions in the oral cavity and in the skin among other alterations. In this study, we report a case of paracoccidioidomycosis with unusual clinical presentation, as it manifested in the female sex and although it did not present any relevant complaints of respiratory focus, it presented a single ulcerated lesion in the buccal mucosa. A biopsy of the lesion and a mycological examination revealed Paracoccidioidesbrasiliensis infection.

Depending on the treatment and weight of the breasts or abdomen, they may exceed volumes considered giant and morbidly obese. In these cases, and when the patient’s BMI is high above 40 kg/m2, the weight of the breasts or abdomen produces what we consider suffocation when the patient is placed in horizontal position on surgical tables, decreasing his respiratory capacity and increasing the difficulty in treating respiratory or embolic risks. An 8-kg breast on the patient’s chest prevents normal breathing. An abdomen with a volume of 30 kg causes difficulties in all senses, making the physiological expansion of the lungs impossible and even preventing surgical assistance to patients. These patients are almost always customers who sleep in the sitting position to breathe better. The gigantic extirpation of the surgical parts facilitates a better respiratory expansion reducing by a large percentage the risk of death, what we call body relief. This relief does not free the patient from bariatric surgery for a possible weight loss, which is vital for the proper functioning of the organs and decreasing arterial hypertension and diabetes.

Aim: To evaluate data related to mental work functioning of workers with cancer diagnosis in radiotherapy in a private service in the city of Campinas, State of São Paulo, Brazil. Method: It consists in a cross-sectional, observational and analytical study with comparison between cancer patients and healthy people, using the Brazilian version of the Work Role Functioning Questionnaire (WRFQ-Br). Descriptive and statistical analysis is performed by the Mann-Whitney test. Results: There was a significant difference between the groups in the evaluation of mental demand;however, the population diagnosed with malignant neoplasm submitted to radiotherapy had unexpected higher scores, reflecting lower mental exigency. Conclusions: The observed context may occur due to extensive support of family, government and interdisciplinary team of health care for individuals diagnosed with cancer. This data provide support for new research studies related to the importance of the work to promote mental health, considering aspects related to social interaction and productivity as the support system members. The importance of disseminating these results and limitation is based in the necessity of investments to improve research and actions to promote the permanence and early return to work for cancer survivors in Brazil and latin America.

Henoch-Schönlein purpura is a vasculitis of small vessels, characterized by tissue deposition of IgA, affecting predominantly the skin, intestine, joints and kidneys. It mainly affects the pediatric age group, especially from 2 to 11 years old. Most often, it is preceded by episode of upper airway infection from days to weeks. The most common clinical manifestations are purpuric cutaneous lesions located in the lower limbs and buttocks, abdominal pain in colic, nausea, vomiting, diarrhea, in addition to arthralgia/arthritis, usually with spontaneous resolution. Renal impairment usually manifests with hematuria and proteinuria, and renal function may also be impaired. The case described refers to a female patient, 86 years old, with a previous history of infection of the upper airways, followed by purpura in the lower limbs and renal impairment: serum creatinine 3.1 mg/dl (N 0.5 - 1.2 mg/dl);glomerular filtration rate 15 ml/min (N > 60 ml/min);urea 118 mg/dl (N önlein purpura in an atypical age group, with significant renal impairment, improvement only with conservative treatment, thus avoiding the use of immunosuppressants and the risks of immunosuppression and toxicity related to such drugs.

Phenolic compounds from Dipteryx alata Vogel were assayed against the in vitro neurotoxic effect induced by Bothrops jararacussu (Bjssu) venom. Mutagenicity was assessed by the Ames test using Salmonella typhimurium strains TA98, TA97a, TA100, and TA102, in experiments with and without metabolic activation. Anti-bothropic activity was obtained by using mouse phrenic nerve-diaphragm (PND) preparation and myographic technique. Control experiments with physiological Tyrode solution were used for keeping the PND preparations alive (n = 4). Concentrations of phe-nolic compounds were as follow: protocatechuic and vanillic acids (200 μg/ml, n = 4), vanillin (50 μg/ml, n = 4). These compounds were used alone or pre-incubated with the venom (40 μg/ml), 30 min prior the addition to the organ bath (n = 4). Phenolic compounds significantly inhibited the neuromuscular blockade of Bjssu in the following order of potency: vanillic acid > protocatechuic = vanillin. Vanillic acid added 10 min after the Bjssu venom was also able to avoid the venomblockade evolution. The mutagenicity assay indicated that all phytochemicals were unable to in-crease the number of revertants, demonstrating the absence of mutagenic activity. This study demonstrated both the safety and therapeutical potential of the three phenolic compounds as novel complementary anti-bothropic agents.

long-acting glucagon-like peptide-1(GlP-1) analogues marketed for type 2 diabetes(T2D) treatment have been showing positive and protective effects in several different tissues, including pancreas, heart or even brain. This gut secreted hormone plays a potent insulinotropic activity and an important role in maintaining glucose homeostasis. Furthermore, growing evidences suggest the occurrence of several commonalities between T2 D and neurodegenerative diseases, insulin resistance being pointed as a main cause for cognitive decline and increased risk to develop dementia. In this regard, it has also been suggested that stimulation of brain insulin signaling may have a protective role against cognitive deficits. As GlP-1 receptors(GlP-1R) are expressed throughout the central nervous system and GlP-1 may cross the blood-brain-barrier, an emerging hypothesis suggests that they may be promising therapeutic targets against brain dysfunctional insulin signaling-related pathologies. Importantly, GlP-1 actions depend not only on the direct effect mediated by its receptor activation, but also on the gut-brain axis involving an exchange of signals between both tissues via the vagal nerve, thereby regulating numerous physiological functions(e.g., energy homeostasis, glucose-dependent insulin secretion, as well as appetite and weight control). Amongst the incretin/GlP-1 mimetics class of anti-T2 D drugs with an increasingly described neuroprotective potential, the already marketed liraglutide emerged as a GlP-1R agonist highly resistant to dipeptidyl peptidase-4 degradation(thereby having an increased half-life) and whose systemic GlP-1R activity is comparable to that of native GlP-1. Importantly, several preclinical studies showed anti-apoptotic, anti-inflammatory, anti-oxidant and neuroprotective effects of liraglutide against T2 D, stroke and Alzheimer disease(AD), whereas several clinical trials, demonstrated some surprising benefits of liraglutide on weight loss, microglia in
