Catheter-directed thrombolysis in inflammatory bowel diseases:Report of a case

In patients with inflammatory bowel diseases (IBD) the prevalence of thrombosis is 6.2%, the average incidence of thromboembolism (tE) is 3.6 times higher compared to normal population. the tE is a common extraintestinal complication of IBD, squarely associated with the IBD activity. the application of antico-agulant and thrombolytic therapy in severe IBD is an unresolved issue. Herein we report the first case in literature of an active IBD patient with an upper limb acute arterial occlusion and successful catheter-directed thrombolysis (CDt). A 46-year-old male patient is reported who had Crohn's disease for 10 years. His right hand suddenly became cold and painful. Angiography proved acute occlusion of the brachial and radial artery. Vascular surgery intervention was not applicable. Endoscopy showed extended, severe inflammation of the colon. Despite the severe endoscopic findings, frequent bloody stools and moderate anaemia, CDt with recombinant tissue plasminogen activator was performed. the control angiography proved improvement, the radial artery pulse appeared. No bleeding complication was observed. this case supports that CDt-after careful estimation of the bleeding risk-can be effective and safe in patients with severe or life-threatening tE and active IBD.

AIM： to screen a suspected Hungarian HNPCC family to find specific mutations and to evaluate their effect on the presentation of the disease. MEthODS： the family was identified by applying the Amsterdam and Bethesda Criteria. Immunohistochemistry was performed, and DNA samples isolated from tumor tissue were evaluated for microsatellite instability. the identification of possible mutations was carried out by sequencing the hMlH1 and hMSH2 genes. RESUltS： two different mutations were observed in the index patient and in his family members. the first mutation was located in exon 7, codon 422 of hMSH2, and caused a change from Glu to StOP codon. No other report of such a mutation has been published, as far as we could find in the international databases. the second mutation was found in exon 3 codon 127 of the hMSH2 gene, resulting in Asp→Ser substitution. the second mutation was already published, as a non-pathogenic allelic variation. CONClUSION： the pedigree analysis suggested that the newly detected nonsense mutation in exon 7 of the hMSH2 gene might be responsible for the development of colon cancers. In family members where the exon 7 mutation is not coupled with this missense mutation, colon cancer appears after the age of 40. the association of these two mutations seems to decrease the age of manifestation of the disease into the early thirties.

Microscopic colitis: A retrospective study of clinical presentation in 53 patients

AIM: to evaluate the relationship between symptoms and microscopic colitis (MC) subtypes: to test whether collagenous colitis (CC) and/or lymphocytic colitis (lC)might be related to both constipation and ***: A cohort of patients with independently confirmed typical histopathological changes was investigated. Fifty-three patients with histologically proved MC (46 with CC, 7 with lC) were included. the existence of diarrhea or constipation and the co-existence of autoimmune diseases were also investigated and all data were retrospectively ***: twenty-three (43.39%) of MC patients had chronic constipation (20 in CC, 3 in lC patients). twentyfour(45.28%) of MC patients had autoimmune disease and the diagnosis of autoimmune disease was always prior to MC. Sjogren's syndrome was associated only with the constipation ***: the Janus face of MC resembles the subgroups of irritable bowel syndrome. the co-existence of autoimmune diseases and MC is confirmed in both the constipation and diarrhea subgroups.

to reach an acceptable controller strategy and tuning it is important to state what is considered “good”. to do so one can set up a closed-loop specification or formulate an optimal control problem. It is an interesting question, if the two can be equivalent or not. In this article two controller strategies, model predictive control (MPC) and constrained direct inversion (CDI) are compared in controlling the model of a pilot-scale water heater. Simulation experiments show that the two methods are similar, if the manipulator movements are not punished much in MPC, and they act practically the same when a filtered reference signal is applied. Even if the same model is used, it is still important to choose tuning parameters appropriately to achieve similar results in both strategies. CDI uses an analytic approach, while MPC uses numeric optimization, thus CDI is more computationally efficient, and can be used either as a standalone controller or to supplement numeric optimization.

AIM: to compare the incidence of intraoperative complications during primary phacoemulsification(phaco) surgery between resident surgeons(residents) and staff surgeons(specialists) and to objectively determine the difficulty of stages in phaco surgery. MEthODS: this retrospective study included cases of phaco cataract surgery performed between January and December 2019. there were no exclusion criteria. For each patient, demographics, clinical history, case complexity, type of surgeon, and operative details were reviewed. Primary outcomes included intraoperative complication rates and the objective measure of difficulty in the steps of the surgery performed by residents and ***: A total of 3272 cases were included;7.4%(n=241) of cases were performed by residents. the overall complication rate was 5.4%(n=177). the intraoperative complication rate was significantly higher(Pts(n=33, 13.7%) than in specialists(n=144, 4.8%). the most frequent complications were posterior capsule tear(n=85, 2.6%), anterior capsule tear(n=50, 1.53%), zonular fiber loss(n=45, 1.38%), and dropped nucleus(n=15, 0.46%). Objectively, the most difficult steps during surgery were phaco in 66(60.0%), capsulorhexis in 21(19.1%), irrigation/aspiration in 13(11.8%), hydrodissection in 9(8.2%), and intraocular lens(IOl) implantation in 1(0.9%) case. CONClUSION: Intraoperative complication rates are higher in residents than in specialists. the order of objective difficulty in phaco surgery steps is in line with the subjective findings of other surveys, revealing that the most challenging parts of phaco surgery are phaco and capsulorhexis.

AIM： to establish the therapeutic potential of proteasome inhibition, we examined the therapeutic effects of MG132 （Z-leu-leu-leu-aldehyde） in an experimental model of acute pancreatitis. MEthODS： Pancreatitis was induced in rats by two hourly intraperitoneal （ip） injections of cholecystokinin octapeptide （CCK; 2×100μg/kg） and the proteasome inhibitor MG132 （10 mg/kg ip） was administered 30 min after the second CCK injection. Animals were sacrificed 4 h after the first injection of CCK. RESUltS： Administering the proteasome inhibitor MG132 （at a dose of 10 mg/kg, ip） 90 min after the onset of pancreatic inflammation induced the expression of cell-protective 72 kDa heat shock protein （HSP72） and decreased DNA-binding of nuclear factor-κB （NF-κB）. Furthermore MG132 treatment resulted in milder inflammatory response and cellular damage, as revealed by improved laboratory and histological parameters of pancreatitis and associated oxidative stress. CONClUSION： Our findings suggest that proteasome inhibition might be beneficial not only for the prevention, but also for the therapy of acute pancreatitis.

For centuries, reclamation of sodic soils has been an essential part of cropping practices in several parts of the world. Parallel to increasing population, the need for new cropland constantly re-evaluates land suitability concepts. therefore, the importance of sodic soils as potential croplands is increasing worldwide. Although theoretically farmers can choose from a wide variety of reclamation options, according to profitability, business plans, and human and financial resources, in practice, few reclamation methods are applied at large scale. this article touches on the early history, 20th Century intensive research, and current trends of sodic soil reclamation. New approaches such as leaching, chemical amendments, addition of organic material, and biological and microbial improvements are discussed, and also brand-new approaches are reviewed. the early history is reviewed using historical books, the achievements of the last hundred years using basic technical literature, mostly books, and the current approaches of our time with fresh publications, mostly papers and two very recent conferences published in English.

Endoscopic transluminal pancreatic necrosectomy using a self-expanding metal stent and high-flow water-jet system

Walled-off pancreatic necrosis and a pancreatic abscess are the most severe complications of acute pancreatitis. Surgery in such critically ill patients is often associated with significant morbidity and mortality within the first few weeks after the onset of symptoms. Minimal invasive approaches with high success and low mortality rates are therefore of considerable interest. Endoscopic therapy has the potential to offer safe and effective alternative treatment. We report here on 3 consecutive patients with infected walled-off pancreatic necrosis and 1 patient with a pancreatic abscess who underwent direct endoscopic necrosectomy 19-21 d after the onset of acute pancreatitis. the infected pancreatic necrosis or abscess was punctured transluminally with a cystostome and, after balloon dilatation, a non-covered self-expanding biliary metal stent was placed into the necrotic cavity. Following stent deployment, a nasobiliary pigtail catheter was placed into the cavity to ensure continuous irrigation. After 5-7 d, the metal stent was removed endoscopically and the necrotic cavity was entered with a therapeutic gastroscope. Endoscopic debridement was performed via the simultaneous application of a high-flow water-jet system; using a flush knife, a Dormia basket, and hot biopsy forceps. the transluminal endotherapy was repeated 2-5 times daily during the next 10 d. Supportive care included parenteral antibiotics and jejunal feeding. All patients improved dramatically and with resolution of their septic conditions; 3 patients were completely cured without any further complications or the need for surgery. One patient died from a complication of prolonged ventilation severe bilateral pneumonia, not related to the endoscopic procedure. No procedure related complications were observed. transluminal endoscopic necrosectomy with temporary application of a self-expanding metal stent and a high-flow water-jet system shows promise for enhancing the potential of this endoscopic app
