The histopathological diagnosis of gastric mucosal biopsy and endoscopic mucosal resection/endoscopic submucosal dissection specimens is important,but the diagnostic criteria,terminology,and grading system are not the same in the East and West.A structurally invasive focus is necessary to diagnose carcinoma for most Western pathologists,but Japanese pathologists make a diagnosis of cancer based on severe dysplastic cytologic atypia irrespective of the presence of *** the Vienna classification was introduced to reduce diagnostic discrepancies,it has been difficult to adopt due to different concepts for gastric epithelial neoplastic *** pathologists experience much difficulty making a diagnosis because we are influenced by Japanese pathologists as well as Western *** is geographically close to korea,and academic exchanges are ***,korean doctors are familiar with Western style medical *** a result,the terminology,definitions,and diagnostic criteria for gastric intraepithelial neoplasia are very heterogeneous in *** solve this problem,the Gastrointestinal Pathology Study Group of the korean Society of Pathologists has made an effort and has suggested guidelines for differential diagnosis:(1) a diagnosis of carcinoma is based on invasion;(2) the most important characteristic of low grade dysplasia is the architectural pattern such as regular distribution of crypts without severe branching,budding,or marked glandular crowding;(3) if nuclear pseudostratification occupies more than the basal half of the cryptal cells in three or more adjacent crypts,the lesion is considered high grade dysplasia;(4) if severe cytologic atypia is present,careful inspection for invasive foci is necessary,because the risk for invasion is very high;and(5) other structural or nuclear atypia should be evaluated to make a final decision such as cribriform pattern,papillae,ridges,vesicular nuclei,high nuclear/cytoplasmic ratio,loss

Objectives To evaluate the age-related one-year major adverse cardiocerebrovascular events （MACCE） after percutaneous coronary intervention （PCI） in acute myocardial infarction （AMI）. We analyzed the association between age and one-year MACCE after AMI. Methods A total of 13,104 AMI patients from korea Acute Myocardial Infarction Registry-National Institue of Health （KAMIR-NIH） between November 2011 and December 2015 were classified into four groups according to age （Group I, 〈 60 years, n = 4199; Group II, 60-70 years, n = 2577; Group III; 70-80 years, n = 2774; Group IV, ≥ 80 years, n = 1018）. Patients were analyzed for one-year composite of MACCE （cardiac death, myocardial infarction, target vessel revascularization, cerebrovascular events） after AMI. Results The one-year MACCE in AMI were 3.5% （Group I）, 6.3% （Group II）, 9.6% （Group III） and 17.6% （Group IV）. After adjustment for confounding para- meters, the analysis results showed that patients with AMI had incremental risk of one-year MACCE [Group II, adjusted hazard ratios （aHR） = 1.224, 95% CI： 0.965-1.525, P = 0.096; Group III, aHR = 1.316, 95% CI： 1.037-1.671, P= 0.024; Group IV, aHR = 1.975, 95% CI： 1.500-62.601, P〈 0.001） compared to Group I. Especially, cardiac death in the composite of primary end point played a major role in this effect （Group II, aHR = 1.335, 95% CI： 0.941-1.895, P= 0.106; Group III, aHR = 1.575, 95% CI： 1.122-2.210, P= 0.009; Group IV, aHR = 2.803, 95% CI： 1.937-4.054, P 〈 0.001）. Conclusions Despite advanced techniques and medications for PCI in AMI, age still exerts a powerful influence in clinical outcomes. Careful approaches, even in the modem era of developed cardiology are needed for aged-population in AMI intervention.

Does the antibody production ability affect the serum anti-Helicobacter pylori Ig G titer?

AIM: To investigate the relationship between serum titers of anti-Helicobacter pylori(***) immunoglobulin G(IgG) and hepatitis B virus surface antibody(HBsA b).METHODS: korean adults were included whose samples had positive Giemsa staining on endoscopic biopsy and were studied in the hepatitis B virus surface antigen(HBsA g)/HBsA b serologic assay,pepsinogen(PG) assay,and *** serologic test on the same *** were excluded if they were positive for HBs Ag,had a recent history of medication,or had other medical condition(s).We analyzed the effects of the following factors on serum titers of HBsA b and the *** IgG : Age,density of *** infiltration in biopsy samples,serum concentrations of PG Ⅰ and PG Ⅱ,PG Ⅰ/Ⅱ ratio,and white blood cell ***: Of 111 included subjects,74(66.7%) exhibited a positive HBsA b *** serum *** IgG titer did not correlate with the serum HBsA b titer(P = 0.185); however,it correlated with the degree of *** infiltration on gastric biopsy(P < 0.001) and serum PG Ⅱ concentration(P = 0.042).According to the density of *** infiltration on gastric biopsy,subjects could be subdivided into those with a marked(median: 3.95,range 0.82-4.00)(P = 0.458),moderate(median: 3.37,range 1.86-4.00),and mild *** infiltrations(median: 2.39,range 0.36-4.00)(P < 0.001).Subjects with a marked *** infiltration on gastric biopsy had the highest serological titer,whereas in subjects with moderate and mild *** infiltrations titers were correspondingly lower(P < 0.001).After the successful eradication,significant decreases of the degree of *** infiltration(P < 0.001),serum *** IgG titer(P < 0.001),and serum concentrations of PG I(P = 0.028) and PG Ⅱ(P = 0.028) were ***: The *** IgG assay can be used to estimate the burden of bacteria in immunocompetent hosts with *** infection,regardless of the HBsA b titer after HBV vaccination.

Background There are numerous but conflicting data regarding gender differences in outcomes following percutaneous coronary intervention(PCI). Furthermore, gender differences in clinical outcomes with acute myocardial infarction(AMI) following PCI in Asian population remain uncertain because of the under-representation of Asian in previous trials. Methods A total of 13,104 AMI patients from korea Acute Myocardial Infarction Registry-National Institute of Health(KAMIR-NIH) between November 2011 and December 2015 were classified into male(n = 8021, 75.9%) and female(n = 2547, 24.1%). We compared the demographic, clinical and angiographic characteristics, 30-days and 1-year major adverse cardiac and cerebrovascular events(MACCE) in women with those in men after AMI by using propensity score(PS) matching. Results Compared with men, women were older, had more comorbidities and more often presented with non-ST segment elevation myocardial infarction(NSTEMI) and reduced left ventricular systolic function. Over the median follow-up of 363 days, gender differences in both 30-days and 1-year MACCE as well as thrombolysis in myocardial infarction minor bleeding risk were not observed in the PS matched population(30-days MACCE: 5.3% vs. 4.7%, log-rank P = 0.494, HR = 1.126, 95% CI: 0.800-1.585;1-year MACCE: 9.3% vs. 9.0%, log-rank P = 0.803, HR = 1.032, 95% CI: 0.802-1.328;TIMI minor bleeding: 4.9% vs. 3.9%, log-rank P = 0.215, HR = 1.255, 95% CI: 0.869-1.814). Conclusions Among korean AMI population undergoing contemporary PCI, women, as compared with men, had different clinical and angiographic characteristics but showed similar 30-days and 1-year clinical outcomes. The risk of bleeding after PCI was comparable between men and women during one-year follow up.

Vitamin D Is Associated with Severity and Mortality of Non-alcoholic Fatty Liver Disease: A US Population-based Study

Background and Aims:There has been increasing evidence that vitamin D deficiency may increase the risk of metabolic *** metabolic syndrome is a major risk factor for non-alcoholic fatty liver disease (NAFLD),we aimed to investigate the association between vitamin D and the severity and mortality of ***:Data was obtained from the United States Third National Health and Nutrition Examination Survey conducted in 1988-1994,with followup mortality data through *** was defined by ultrasonographic detection of hepatic steatosis in the absence of other liver diseases and categorized as normal,mild,moderate or *** severity of hepatic fibrosis was determined by NAFLD fibrosis score (NFS).ANOVA (F-test) was used to evaluate the association between vitamin D level and degree of NAFLD,and Cox proportional hazards regression analysis was used for survival ***:Vitamin D levels for normal,mild,moderate and severe steatosis were 25.1 ± 0.29 ng/mL,24.7 ± 0.42 ng/mL,23.7 ± 0.37 ng/mL and 23.6 ± 0.60 ng/mL,respectively (trend p < 0.001).Likewise,vitamin D levels for low,intermediate and high NFS categories were 24.7 ± 0.38 ng/mL,23.4 ± 0.42 ng/mL and 21.5 ± 0.57 ng/mL,respectively (trend p < 0.001).After median-follow up over 19 years,vitamin D deficiency was significantly associated with diabetes-and Alzheimer's diseaserelated mortality (hazard ratio (HR):3.64,95%CI:1.51-8.82 and HR:4.80,95%CI:1.53-15.1,respectively),with a borderline significance in overall mortality (HR:1.16,95%CI:0.99-1.36,p =0.06).Conclusion:Vitamin D level was inversely related to the degree of liver steatosis and ***,vitamin D deficiency was associated with diabetes-and Alzheimer's disease-related mortality in NAFLD patients.

Pravastatin activates PPARα/PPARγexpression in the liver and gallbladder epithelium of hamsters

BACKGROUND:Our earlier study with cultured gallbladder epithelial cells demonstrated that statins(HMG-CoA reductase inhibitors)activate the expression of PPARαand PPARγ, consequently blocking the production of pro-inflmmatory *** present study used hamsters to investigate the effects of pavastatin on PPARα/PPARγexpression in the liver and gallbladder epithelium,and to determine whether pravastatin suppresses cholesterol crystal formation in the gallbladder. METHODS:A total of 40 Golden Syrian male hamsters(4 weeks old)were randomly assigned to four groups(basal diet control; basal diet+pavastatin;high cholesterol diet;high cholesterol diet+pravastatin).All hamsters were 11 weeks old at the end of the *** liver,gallbladder and bile were harvested. Immunohistochemical staining and Western blotting for PPARαand PPARγwere performed in the liver and gallbladder. A drop of fresh bile was examined for cholesterol crystals under a microscope. RESULTS:In the gallbladder and liver of the hamsters, pravastatin activated the PPARαand PPARγexpression of gallbladder epithelial cells and hepatocytes,and particularly the response of PPARγwas much stronger than that of PPARα. Pravastatin suppressed the formation of cholesterol gallstones or crystals in the gallbladder. CONCLUSION:Pravastatin is an effective medication to activate PPARs(especially PPARγ)in the liver and the gallbladder epithelium of hamsters,and contributes to the prevention of gallstone formation.

AIM： Recently, germ-line mutation in the base excision repair gene MYH has been identified to cause a novel autosomal recessive form of familial adenomatous polyposis （FAP）. Interestingly, a striking evidence for MYH mutations within different ethnic groups has been demonstrated. In this study, we screened 30 patients with multiple adenomatous polyps for MYH mutations to assess its prevalence and ethnic specificity in korea. METHODS： Thirty patients （21 men and 9 women; mean age 62.3 years） with multiple adenomatous polyps were examined for MYH mutations. The mean number of adenomas per patient was 10.0. Sixteen exonic regions and their intronic sequences were amplified by PCR and subjected to SSCP and DNA sequencing analyses. RESULTS： None of the patients was identified to carry any truncating or sequence alterations in MYH. Our screening for the mutational regions, which were recognized from Caucasian patients or affected Indian families, also failed to detect sequence substitutions. CONCLUSION： Mutation in MYHmay be rarely involved in the pathogenesis of multiple sporadic colorectal adenomas in korean population, although a large-scale analysis will be required to clarify the presence of specific MYH variants in a subset of patients and their role in the predisposition of multiple colorectal adenomas in korean population.
