BACKGROUND Electromagnetic interference(EMI),means disturbance to the operation of implanted electrical devices caused by external *** cardiac pacemaker is implanted into the body,the risk of EMI should be considered when performing neuromuscular electrical stimulation(NMES).So far,no case has been reported that clinical magnets are used to safely manage the EMI risk of patients with cardiac pacemaker in *** SUMMARY A 72-year-old male with swallowing disorder due to pure motor lacunar syndrome was transferred to rehabilitation department six days after the symptom *** risk needed be considered when implementing NMES on pharyngeal muscles,since cardiac pacemaker was implanted on his left chest due to the sick sinus *** the first NMES,the function of the pacemaker was directly monitored using telemetric *** the second day,by a simple method of placing a magnet on the pacemaker,we chose to move the pacemaker into a mode that the device was not influenced by external *** magnet method has been used repeatedly for a year for the safe NMES *** could remove Levin tube four months after the initial symptom and dysphagia related symptoms had not been noted during two-year follow-up *** This report is the first case of dysphagia rehabilitation that EMI risk was handled using mode change of pacemaker with *** method is unfamiliar to doctors,but safe and easy *** paper could be guidance for clinicians who need to treat patients with EMI risk.

Microforming of superplastic 5083 aluminum alloy

The mechanical behavior of superplastic 5083 aluminum alloy during microforming process was investigated by finite element analysis.A micro V-groove die was modeled to analyze the effects of forming time,load and temperature on the microformability of the 5083 aluminum ***,the microformability of the 5083 aluminum alloy was estimated using a microformability *** simulation results show that the microformability increases with the forming load,time and temperature *** of the 5083 aluminum alloy during microforming using the V-groove die was also investigated in terms of the effective strain *** results show that the superplasticity of the 5083 aluminum alloy occurs in a specific part of the material for a specific period during the microforming process depending on the forming conditions and the microformability index.

Metastatic penile carcinoma is rare and usually originates from genitourinary tumors. The presenting symptoms or signs have been described as nonspecific except for priapism. Rectal adenocarcinoma is a very unusual source of metastatic penile carcinoma. We report a case of metastatic penile carcinoma that originated from the rectum. Symptomatic improvement occurred with palliative radiotherapy.

Interaction of Streamwise and Wall-Normal Velocities in Combined Wave-Current Motion

The aim of this paper is to present an analytical expression for the streamwise velocity distribution in a non-uniform flow in the presence of waves; the correlation between the horizontal and vertical velocity components has been compreheusively examined. Different from previous researches which attributed the deviation of velocity from the classical log-law to the wave Reynolds stress, i.e. - ρ uv^- only, this study demonstrates that the momentum flux caused by mean velocities, i.e., u^- and v^-, is also responsible for the velocity deviation, and it is found that the streamwise velocity for a flow in the presence of non-zero wall-normal velocity does not follow the classical log-law, but the modified log-law proposed in this study based on simplified mixing-length theorem. The validity of the modified log-law has been verified by use of available experimental data from published sources for combined wave-current flows, and good agreement between the predicted and observed velocity profiles has been achieved.

With the advancement of video recording devices and network infrastructure,we use surveillance cameras to protect our valuable *** paper proposes a novel system for encrypting personal information within recorded surveillance videos to enhance efficiency and *** proposed method leverages Dlib’s CNN-based facial recognition technology to identify Regions of Interest(ROIs)within the video,linking these ROIs to generate unique *** IDs are then combined with a master key to create entity-specific keys,which are used to encrypt the ROIs within the *** system supports selective decryption,effectively protecting personal information using surveillance ***,the system overcomes the limitations of existing ROI recognition technologies by predicting unrecognized frames through *** research validates the proposed technology through experimental evaluations of execution time and post-processing techniques,ensuring comprehensive personal information *** for setting the thresholds used in this process are also *** the proposed method could serve as an effective solution to security vulnerabilities that traditional approaches fail to address.

Fabrication of Near Net Shape Metallic Foam via Plaster Mould

Fabrication characteristics are unstable in direct foaming method. Therefore, most of near net-shape metallic foams are produced, and investigated by powder metallurgy. Direct foaming method, however, has many benefits （i.e. reduce the unit cost of goods and fabrication process etc.） to fabrication of metallic foams. In this article, the fabrication characteristic of near net-shape metallic foams by direct foaming method was evaluated. Al and Plaster was used for base material and mould material respectively. Ca and TiH2 were added to molten Al as thickening and blowing agent for stable condition of bubbles. Thickening time was about 10 min with a stirring speed of 600 r/min. Foaming time was 30-120 s for evaluation of the optimum foaming condition. Amount of agent was selected by pre-experimental data. Porosity of near net-shape goods was measured by Archimedes method. On the other hand, it seems that increasing poring time and thickening agent make the poor porosity

Synergistic immunotherapy of immune checkpoint blockade (ICB) and immunogenic cell death (ICD) has shown remarkable therapeutic efficacy in various cancers. However, patients show low response rates and undesirable outcomes to these combination therapies owing to the recycling mechanism of programmed death-ligand 1 (PD-L1) and the systemic toxicity of ICD-inducing chemotherapeutic drugs. Herein, we propose all-in-one glycol chitosan nanoparticles (CNPs) that can deliver anti-PD-L1 peptide (PP) and doxorubicin (DOX) to targeted tumor tissues for a safe and more effective synergistic immunotherapy. The PP-CNPs, which are prepared by conjugating ᴅ-form PP (NYSKPTDRQYHF) to CNPs, form stable nanoparticles that promote multivalent binding with PD-L1 proteins on the targeted tumor cell surface, resulting in effective lysosomal PD-L1 degradation in contrast with anti-PD-L1 antibody, which induces recycling of endocytosed PD-L1. Consequently, PP-CNPs prevent subcellular PD-L1 recycling and eventually destruct immune escape mechanism in CT26 colon tumor-bearing mice. Moreover, the ICD inducer, DOX is loaded into PP-CNPs (DOX-PP-CNPs) for synergistic ICD and ICB therapy, inducing a large number of damage-associated molecular patterns (DAMPs) in targeted tumor tissues with minimal toxicity in normal tissues. When the DOX-PP-CNPs are intravenously injected into CT26 colon tumor-bearing mice, PP and DOX are efficiently delivered to the tumor tissues via nanoparticle-derived passive and active targeting, which eventually induce both lysosomal PD-L1 degradation and substantial ICD, resulting in a high rate of complete tumor regression (CR: 60%) by a strong antitumor immune response. Collectively, this study demonstrates the superior efficacy of synergistic immunotherapy using all-in-one nanoparticles to deliver PP and DOX to targeted tumor tissues.

Clinicopathologic characteristics of gastric carcinoma in elderly patients: A comparison with young patients

AIM:To examine the clinicopathologic features of elderly patients with gastric carcinoma and to investigate the relationship between prognosis and ***: We reviewed the hospital records of 2 014patients with gastric carcinoma retrospectively to compare the clinicopathologic findings in elderly (age ＞70 years) and young (age ＜36 years) patients during the period from 1986 to 2000 in a tertiary referral center in Gwangju, Korea. Overall survival was the main outcome ***: Of the 2 014 patients, 194 (9.6%) were in the elderly group and 137 (6.8%) were in the young *** elderly and young patients had similar distributions with respect to depth of invasion, nodal involvement, hepatic metastasis, peritoneal dissemination, tumor stage at the initial diagnosis, and type of surgery. Synchronous multiple carcinomas were found in 14/194 (7.2%) of the elderly group and 4/137 (2.9%) of the young group (P＜0.05). Using the Borrmann classification, type Ⅳ was more frequent in the young patients than in the elderly patients (P＜0.05).Significantly more elderly patients had a well or moderately differentiated histology, and more young patients had a poorly differentiated histology and signet ring cell carcinoma (P＜0.001). The 5-year survival rates of elderly and young patients did not differ statistically (52.8% vs 46.5%,P = 0.5290). Multivariate analysis showed that the histologic type, nodal involvement and operative curability were significant prognostic factors, and age itself was not an independent prognostic factor of survival for elderly gastric carcinoma ***: Elderly patients with gastric carcinoma do not have a worse prognosis than young patients. The important prognostic factor is whether the patients undergo a curative resection.

Bispectral index monitoring as an adjunct to nurse-administered combined sedation during endoscopic retrograde cholangiopancreatography

AIM:To determine whether bispectral index (BIS) monitoring is useful for propofol administration for deep sedation during endoscopic retrograde cholangiopancreatography (ERCP). METHODS:Fifty-nine consecutive patients with a variety of reasons for ERCP who underwent the procedure at least twice between 1 July 2010 and 30 November 2010. This was a randomized cross-over study, in which each patient underwent ERCP twice, once with BIS monitoring and once with control monitoring. Whether BIS monitoring was done during the first or second ERCP procedure was random. Patients were intermittently administered a mixed regimen including midazolam, pethidine, and propofol by trained nurses. The nurse used a routine practice to monitor sedation using the Modified Observer's Assessment of Alertness/Sedation (MOAA/S) scale or the BIS monitoring. The total amount of midazolam and propofol used and serious side effects were compared between the BIS and control groups. RESULTS:The mean total propofol dose administered was 53.1 ± 32.2 mg in the BIS group and 54.9 ± 30.8 mg in the control group (P = 0.673). The individual propofol dose received per minute during the ERCP procedure was 2.90 ± 1.83 mg/min in the BIS group and 3.44 ± 2.04 mg in the control group (P = 0.103). The median value of the MOAA/S score during the maintenance phase of sedation was comparable for the two groups. The mean BIS values throughout the procedure (from insertion to removal of the endoscope) were 76.5 ± 8.7 for all 59 patients in using the BIS monitor. No significant differences in the frequency of < 80% oxygen saturation, hypotension (< 80 mmHg), or bradycardia (< 50 beats/min) were observed between the two study groups. Four cases of poor cooperation occurred, in which the procedure should be stopped to add the propofol dose.
