Background: Electroacupuncture(EA) may reduce the severity of acute pancreatitis(AP) and provide additional pain relief in patients with chronic pancreatitis. However, the ability of EA to relieve pain in patients with AP has not been well ***: This study was undertaken to compare the pain-relieving effects of EA and conventional treatment in patients with ***, setting, participants and interventions: This study was conducted using a randomized, controlled,three-arm, parallel-group and multi-center design. Patients diagnosed with AP were randomly and equally assigned to EA1, EA2 or control groups. All participants received conventional standard-of-care therapy for AP. Local EA alone was administered in EA1, and local plus distal EA was given in *** EA included two abdominal acupoints, while distal EA included twelve peripheral acupoints. EA groups underwent one session of EA daily for 4 days(days 1–4), or until pain was resolved or *** outcome measures: The primary outcome measure was the change in the visual analogue scale(VAS;0–100) pain score between baseline and day ***: Eighty-nine participants were randomized into EA1, EA2 and control groups, and 88(EA1, 30;EA2, 29;control, 29) were included in the full-analysis set. VAS score change(median [interquartile range]) on day 5 was(12.3 ± 22.5) in the EA1 group,(10.3 ± 21.5) in the EA2 group, and(8.9 ± 15.2) in the control group. There were not significant differences in the change in VAS score among treatments(P = 0.983). However, time to food intake was significantly shorter in the EA group(EA1 + EA2) than in the control group(median 2.0 days vs 3.0 days), with a hazard ratio of 0.581(P = 0.022;95% CI,0.366–0.924). No significant adverse events ***: EA treatment did not significantly reduce pain after 4 days of treatment in patients with AP-associated abdominal pain but significantly reduced time to first food intake.

Carcinoma in situ arising in a tubulovillous adenoma of the distal common bile duct: A case report

Tubulovillous adenomas are common in the colon and rectum, but are rare in the common bile duct. Biliary adenomas may produce obstructive jaundice, which can be easily confused with a malignant neoplasm or stone. We report a case of a carcinoma in situ arising in a tubulovillous adenoma of the distal common bile duct causing obstructive jaundice. A 55-year-old male presented with a 10-d history of pruritus and progressive jaundice. Abdominal sonography and computed tomography showed a mass in the distal common bile duct. Endoscopic retrograde cholangiopancreatography showed luminal narrowing of the bile duct due to a polypoid mass. Positron emission tomography demonstrated no abnormal uptake. It was thought that this mass was a malignant tumor, thus a pylorus-preserving panceaticoduodenectomy was performed. The final pathology showed a tubulovillous adenoma with carcinoma in situ of the distal common bile duct. At follow-up 8 mo later, endoscopy showed multiple polyps in the rectum, colon and stomach. The polyps were removed by endoscopic mucosal resection and shown to be tubular adenomas with high grade dysplasia. Biliary adenomas require careful follow-up for early detection of recurrence and malignant transformation.

Proton pump inhibitor administration delays rebleeding after endoscopic gastric variceal obturation

AIM:To clarify the efficacy of proton pump inhibitors(PPIs)after endoscopic variceal obturation(EVO)with ***:A retrospective study was performed on16 liver cirrhosis patients with gastric variceal bleeding that received EVO with injections of N-butyl-2-cyanoacrylate at a single center(Kyung Hee University Hospital at Gangdong)from January 2008 to December *** records including patient characteristics and endoscopic findings were *** results,liver function,serum biochemistry and cirrhosis etiology were compared between patients receiving PPIs and those that did ***,the rebleeding interval was compared between patients that received PPI treatment after EVO and those who did ***:The patient group included nine males and seven females with a mean age of 61.8±*** the EVO procedure,eight of the 12patients that received PPIs and three of the four nonPPI patients experienced *** were no differences between the groups in serum biochemistry or patient *** rebleeding rate was not significantly different between the groups,however,patients receiving PPIs had a significantly longer rebleeding interval compared to non-PPI patients(22.2±11.2mo vs 8.5±5.5 mo;P=0.008).The duration of PPI use was not related to the rebleeding interval.A total of six patients,who had ulcers at the injection site,exhibited a shorter rebleeding interval(16.8±5.9 mo)than patients without ulcers(19.9±3.2 mo),though this difference was not statistically ***:PPI therapy can extend the rebleeding interval,and should therefore be considered after EVO treatment for gastric varices.

Risk of sedation for diagnostic esophagogastroduodenoscopy in obstructive sleep apnea patients

AIM: To investigate whether patients with obstructive sleep apnea (OSA) are at risk of sedation-related complications during diagnostic esophagogastroduodenoscopy (EGD). METHODS: A prospective study was performed in consecutive patients with OSA, who were confirmed with full-night polysomnography between July 2010 and April 2011. The occurrence of cardiopulmonary complications related to sedation during diagnostic EGD was compared between OSA and control groups. RESULTS: During the study period, 31 patients with OSA and 65 controls were enrolled. Compared with the control group, a higher dosage of midazolam was administered (P = 0.000) and a higher proportion of deep sedation was performed (P = 0.024) in the OSA group. However, all adverse events, including sedation fail-ure, paradoxical responses, snoring or apnea, hypoxia, hypotension, oxygen or flumazenil administration, and other adverse events were not different between the two groups (all P > 0.1). Patients with OSA were not predisposed to hypoxia with multivariate logistic regression analysis (P = 0.068). CONCLUSION: In patients with OSA, this limited sized study did not disclose an increased risk of cardiopulmonary complications during diagnostic EGD under sedation.

Selecting suitable sites for mountain ginseng(Panax ginseng)cultivation by using geographically weighted logistic regression

With the well-being trends to pursue a healthy life, mountain ginseng(Panax ginseng) is rising as one of the most profitable forest products in South Korea. This study was aimed at evaluating a new methodology for identifying suitable sites for mountain ginseng cultivation in the country. Forest vegetation data were collected from 46 sites and the spatial distribution of all sites was analyzed using GIS data for topographic position, landform, solar radiation, and topographic wetness. The physical and chemical properties of the soil samples, including moisture content, p H, organic matter, total nitrogen, exchangeable cations, available phosphorous, and soil texture, were analyzed. The cultivation suitability at each site was assessed based on the environmental conditions using logistic regression(LR) and geographically weighted logistic regression(GWLR) and the results of both methods were compared. The results show that the areas with northern aspect and higher levels of solar radiation, moisture content, total nitrogen, and sand ratio are more likely to be identified as suitable sites for ginseng cultivation. In contrast to the LR, the spatial modeling with the GWLR results in an increase in the model fitness and indicates that a significant portion of spatialautocorrelation in the data decreases. A higher value of the area under the receiver operating characteristic(roC) curve presents a better prediction accuracy of site suitability by the GWLR. The geographically weighted coefficient estimates of the model are nonstationary, and reveal that different site suitability is associated with the geographical location of the forest stands. The GWLR increases the accuracy of selecting suitable sites by considering the geographical variations in the characteristics of the cultivation sites.

Observational Study of Surface Wind along a Sloping Surface over Mountainous Terrain during Winter

The 2018 Winter Olympic and Paralympic Games will be held in Pyeongchang, Korea, during February and March. We examined the near surface winds and wind gusts along the sloping surface at two outdoor venues in Pyeongchang during February and March using surface wind data. The outdoor venues are located in a complex, mountainous terrain, and hence the near-surface winds form intricate patterns due to the interplay between large-scale and locally forced winds. During February and March, the dominant wind at the ridge level is westerly; however, a significant wind direction change is ob- served along the sloping surface at the venues. The winds on the sloping surface are also influenced by thermal forcing, showing increased upslope flow during daytime. When neutral air flows over the hill, the windward and leeward flows show a significantly different behavior. A higher correlation of the wind speed between upper- and lower-level stations is shown in the windward region compared with the leeward region. The strong synoptic wind, small width of the ridge, and steep leeward ridge slope angle provide favorable conditions for flow separation at the leeward toot of the ridge. The gust factor increases with decreasing surface elevation and is larger during daytime than nighttime. A significantly large gust factor is also observed in the leeward region.

Relationship of plasminogen activator inhibitor 1 gene 4G/5G polymorphisms to hypertension in Korean women

Background Hypertension （HTN） is a major determinant of various cardiovascular events. Plasma levels of plasminogen activator inhibitor 1 （PAl-l） modulate this risk. A deletion/insertion polymorphism within the PAl-1 loci （4G/4G, 4G/5G, 5G/5G） affects the expression of this gene. The present study investigated the association between PAl-1 loci polymorphisms and HTN in Korean women. Methods Korean women （n=1312） were enrolled in this study to evaluate the association between PAl-1 4G/5G gene polymorphisms and HTN as well as other metabolic risk factors. PAl-1 loci polymorphisms were investigated using polymerase chain reaction amplification and single-strand conformation polymorphism analysis. Results The three genotype groups differed with respect to systolic blood pressure （P=0.043）, and diastolic blood pressure （P=0.009） but not with respect to age, body mass index, total cholesterol, low or high density lipoprotein cholesterol, triglycerides, or fasting blood glucose. Carriers of the PAl-1 4G allele had more hypertension significantly （PAl-1 4G/5G vs. PAl-1 5G/5G, P=0.032; PAl-1 4G/4G vs. PAl-1 5G/5G, P=0.034）. When stratified according to PAl-1 4G/5G polymorphism, there was no significant difference in all metabolic parameters among PAl-1 genotype groups in patients with HTN as well as subjects with normal blood pressure. The estimated odds ratio of the 4G/4G genotype and 4G/5G for HTN was 1.7 （P=0.005）, and 1.6 （P=0.015）, respectively. Conclusion These findings might indicate that PAl-1 loci polymorphisms independently contribute to HTN and that gene-environmental interaction may be not associated in Korean women.

Preparation of human decellularized peripheral nerve allograft using amphoteric detergent and nuclease

Animal studies have shown that amphoteric detergent and nuclease(DNase I and ribonuclease A) is the most reliable decellularization method of the peripheral nerve. However, the optimal combination of chemical reagents for decellularization of human nerve allograft needs further investigation. To find the optimal protocol to remove the immunogenic cellular components of the nerve tissue and preserve the basal lamina and extracellular matrix and whether the optimal protocol can be applied to larger-diameter human peripheral nerves, in this study, we decellularized the median and sural nerves from the cadavers with two different methods: nonionic and anionic detergents(Triton X-100 and sodium deoxycholate) and amphoteric detergent and nuclease(3-[(3-cholamidopropyl)dimethylammonio]-1-propanesulfonate(CHAPS), deoxyribonuclease I, and ribonuclease A). All cellular components were successfully removed from the median and sural nerves by amphoteric detergent and nuclease. Not all cellular components were removed from the median nerve by nonionic and anionic detergent. Both median and sural nerves treated with amphoteric detergent and nuclease maintained a completely intact extracellular matrix. Treatment with nonionic and anionic detergent decreased collagen content in both median and sural nerves, while the amphoteric detergent and nuclease treatment did not reduce collagen content. In addition, a contact cytotoxicity assay revealed that the nerves decellularized by amphoteric detergent and nuclease was biocompatible. Strength failure testing demonstrated that the biomechanical properties of nerves decellularized with amphoteric detergent and nuclease were comparable to those of fresh controls. Decellularization with amphoteric detergent and nuclease better remove cellular components and better preserve extracellular matrix than decellularization with nonionic and anionic detergents, even in large-diameter human peripheral nerves. Abstinence from prolonged psychostimulant use prompts stimulant withdrawal *** adaptations within the dorsal striatum have been considered the main hallmark of stimulant abstinence. Here we explored striatal miRNA-target interaction and its impact on circulating miRNA marker as well as behavioral dysfunctions in methamphetamine(MA) abstinence. We conducted miRNA sequencing and profiling in the nonhuman primate model of MA abstinence, followed by miRNA qPCR,LC-MS/MS proteomics, immunoassays, and behavior tests in mice. In nonhuman primates, MA abstinence triggered a lasting upregulation of miR-137 in the dorsal striatum but a simultaneous downregulation of circulating miR-137. In mice, aberrant increase in striatal miR-137-dependent inhibition of SYNCRIP essentially mediated the MA abstinence-induced reduction of circulating miR-137. Pathway modeling through experimental deduction illustrated that the MA abstinence-mediated downregulation of circulating miR-137 was caused by reduction of SYNCRIP-dependent miRNA sorting into the exosomes in the dorsal striatum. Furthermore, diminished SYNCRIP in the dorsal striatum was necessary for MA abstinence-induced behavioral bias towards egocentric spatial learning. Taken together, our data revealed circulating miR-137 as a potential blood-based marker that could reflect MA abstinence-dependent changes in striatal miR-137/SYNCRIP axis, and striatal SYNCRIP as a potential therapeutic target for striatum-associated cognitive dysfunction by MA withdrawal syndrome.

Enhanced biometric encryption algorithm for private key protection in BioPKI system

Protection of private key is the most critical part in public key infrastructure(PKI) system since it depends on the confidentiality of private key. Thread of password detection has been known as the vulnerability in this PKI system. Recently, studies have been conducted on Bio PKI system that uses the biometric information of users in order to replace the password type of private key protection in PKI system. However, Bio PKI system also has vulnerability in that biometric information used for protection of private key cannot be reused once it is stolen or lost. So, we propose the method to protect the private key using FIDSEQi which binds sequence to biometric information. The proposed method enhances reusability of biometric information and presents higher attack complexity than the method of authentication by cross matching single biometric information.
