Two-stage treatment of an unusual haemobilia caused by intrahepatic pseudoaneurysm

A 84-year-old man with a surgical history of subtotal gastrectomy for gastric cancer was transferred to our department because of a disorder of consciousness. Septic shock due to obstructive suppurative cholangitis secondary to choledocholithiasis was diagnosed. Anemia was also present,and upper gastrointestinal tract endoscopy revealed blood emerging from the Papilla of *** cause of the anemia was identified as *** showed a small aneurysm over the artery on segment 3(A3) .The cause of the haemobilia was suspected to be the bleeding into the biliary tree from this *** the patient's general condition was poor,minimally invasive therapy was *** arterial embolization(TAE) was selected ***,lateral sectionectomy was performed in order to remove the aneurysm on A3. No surgical complication occurred and,after surgery,no haemobilia was *** conclusion,a twostage treatment,namely,surgery following TAE,is recommended for patients in a physically poor condition who have haemobilia due to intrahepatic aneurysm.

Levovist ultrasonography imaging in intracystic hemorrhage of simple liver cyst

The differential diagnosis between intracystic hemorrhage and cystadenocarcinoma of the liver is often difficult even with the use of various imaging modalities. A 73-year-old woman was admitted to our hospital with the complaint of right upper quadrant pain. Ultrasonography (US) demonstrated a heterogeneous echogenic cyst measuring 11 cm × 8 cm in size in S2 of the liver, indicated intracystic hemorrhage of simple liver cyst or cystadenocarcinoma, but the differential diagnosis was considerably difficult. Levovist (Schering, Berlin, Germany) US revealed no enhancement of the intracystic structures, suggesting a clot in the case of intracystic hemorrhage. An operation was performed and the resected lesion showed a solitary benign liver cyst, measuring 5.5 cm × 4.7 cm × 8.5 cm containing a large blood clot. The patient had an uneventful recovery after the surgery. Levovist US may play an important role in discrimination between intracystic hemorrhage of simple hepatic cysts and cystadenocarcinoma of the liver.

Let K, 0 ±1, be the Kakeya maximal operator defined as the supremum of averages over tubes of the eccentricity . The (so-called) Fefferman-Stein-type inequality: $|| {K_\delta f} ||_{L^d({{\rm R}^d,w})} \le C_d ({1 \over \delta})^{(d - 2)/2d}(\log ({1 \over \delta } ))^{\alpha_d} || f ||_{L^d ({\rm R}^d,K_{\delta}w})}$is shown, where Cd and d are constants depending only on the dimension d and w is a weight. The result contains the exponent (d-2)/2d which is smaller than the exponent (d-2)(d-1)/d(2d-3) obtained in [7].

Attenuation of PKP（DF） in the Eastern Hemisphere is examined in terms of multiple scattering to simultaneously explain a puzzling relationship, a relatively fast velocity anomaly corre- sponding to strong attenuation. Reflectivity synthetics with one-dimensional random velocity fluctua- tions are compared with observations of PKP（DF）/PKP（Cdiff） amplitude ratios and differential travel times of PKP（Cdiff）-PKP（DF） for the equatorial paths. A Gaussian distribution of P-wave velocity fluctuations with the standard deviations of 5%, 6%, and 7% in the uppermost 200 km of the inner core is superimposed on the velocity structure that is slightly faster than the typical structure in the Eastern Hemisphere, which is likely to explain both the travel time and amplitude data as far as only the one-dimensional structure is considered. Further examinations of the statistic characteristic of scat- terer distribution in two and three-dimensions are required to obtain a realistic conclusion.

Photosynthetic recovery of desiccated intertidal seaweeds after rehydration

Intertidal seaweeds experience periodical desiccation and rehydration to different extents due to the tidal cycles and their vertical distributions. Their photosynthetic recovery process during the rehydration may show different patterns among the seaweeds from different zonations or depths at intertidal zone. In this study 12 species of seaweeds collected from the upper, middle, lower and sublittoral zones were examined. The relationship of the photosynthetic recovery to vertical distribution was assessed by comparing their patterns of photosynthetic and respiratory performances after rehydration following desiccation. Both the photosynthesis and dark respiration declined during emersion, showing certain degrees of recovery after re-immersion into seawater for most species, but the extents were markedly different from one species to the other. The species from upper intertidal zone after being rehydrated for 1 hour, following 2 hours of desiccation, achieved 100% recovery of their initial physiological activity, while most of the lower or sublittoral species did not achieve full recovery. It is the ability to withstand desiccation stress (fast recovery during rehydration), but not that to avoid desiccation (water retaining ability) that determines the distribution of intertidal seaweeds. Such physiological behavior during rehydration after desiccation reflects the adaptive strategy of intertidal seaweeds against desiccation and their capability of primary production in the process of rehydration.
