A successful birth of severe secondary recurrent miscarriage case after a decline of phosphatidylserine-dependent anti-prothrombin antibody by intravenous immunoglobulin administration

A 33 years old woman was referred to our hospital since her sixth pregnancy had been revealed. In fact, at 19 years of age she had diagnosed as having systemic lupus erythematosus without organ failure. In addition, she had a past history of uncontrollable severe pregnancy-induced hypertension occurred during the second pregnancy, resulting in extremely premature delivery and following postpartum HELLP syndrome. It was so severe that we employed administration of dexamethasone and plasma exchange to ameliorate a life-threatening situation. In the course of her recovery it was revealed that she had been complicated with antiphospholipid antibodies, and at the same time we observed that phosphatidylserine-dependent anti-prothrombin antibody IgG levels were declining as her condition was getting better. There-after, she became pregnant three times, but all pregnancies ended in miscarriage despite administration of prednisolone and anticoagulant therapy. Therefore, we realized that her recurrent miscarriages could not be prevented with generally acceptable therapies, so we tried intravenous immunoglobulin shortly after fetal heart beats were detected. In fact, her sixth pregnancy was going well, but we had to terminate it at the 35th week of gestation due to the onset of HELLP syndrome-like condition. However, she could achieve an almost intact pregnancy outcome without neonatal complications or persistently worsening postpartum HELLP syndrome-like condition. Considering the etiologic relation overlapping between systemic lupus erythematosus, antiphospholipid syndrome and recurrent miscarriage, intravenous immunoglobulin can be one of the treatment options for severe secondary recurrent miscarriage, although the evidence of the treatment is always certain. In addition, a decline of phosphatidylserine-dependent anti-prothrombin antibody IgG levels we observed in this case may represent its therapeutic immunomodulatory effects.

Dynamic robust optimal reorder point with uncertain lead time and changeable demand distribution

In fixed order quantity systems,uncertainty in lead time is expressed as a set of scenarios with occurrence probabilities,and the mean and variance in demand distribution are supposed to be changeable according to a known pattern.A new concept of "dynamic robust optimal reorder point" is proposed in this paper and its value is calculated as a "robust optimal reorder point function with respect to reorder time".Two approaches were employed in determining the dynamic optimal reorder *** first is a shortage rate satisfaction approach and the second is a backorder cost minimization *** former aims at finding the minimum value of reorder point at each reorder time which satisfies the condition that the cumulative distribution function (CDF) of shortage rate under a given set of scenarios in lead time is greater than or equal to a basic CDF of shortage rate predetermined by a *** the latter approach,the CDF of closeness of reorder point is defined at each reorder time to express how close to the optimal reorder points under the set of scenarios,and the dynamic optimal reorder point is defined according to stochastic *** numerical examples demonstrate the features of these dynamic robust optimal reorder points.

Pathophysiology of acute graft-versus-host disease from the perspective of hemodynamics determined by dielectric analysis

Predictive factors for efficacy of testosterone replacement therapy for late-onset hypogonadism in Japanese men:a preliminary report

Although testosterone replacement therapy(TRT)is the first-choice method used worldwide for late-onset hypogonadism(LOH),clinical benefits are not seen in all *** study was conducted to determine the predictors of TRT efficacy for ***-six patients who visited our Men’s Health Clinic(Kawanishi City Medical Center,Kawanishi and Hyogo Medical University,Nishinomiya,Hyogo,Japan)between November 2003 and June 2021 with data available before and after TRT were *** were divided into responders(Group 1;n=45,accounting for 80.4%)and nonresponders(Group 2;n=11,accounting for 19.6%)based on the clinical response to TRT,including patient *** noted before TRT included age,body mass index,aging males’symptoms score,sexual health inventory for men,luteinizing hormone,follicular-stimulating hormone,testosterone,free testosterone,prolactin(PRL),estradiol(E2),and testosterone/estradiol(T/E2)ratio in *** statistical analysis,a multivariable logistic regression model was *** analysis revealed PRL(odds ratio TORI:0.9624;95%confidence interval[Cl]:0.9316-0.9943,P<0.05),E2(OR:0.8692;95%Cl:0.7745-0.9754,P<0.05),and T/E2 ratio(OR:1.1312;95%Cl:1.0106-1.2661,P<0.05)to be predictive *** analyses showed that T/E2 ratio was an independent predictive factor(OR:1.1593;95%Cl:1.0438-1.2875,P<0.01).The present results suggest that a low value for T/E2 ratio may predict a reduced response to *** T/E2 ratio threshold to predict nonresponders based on receiver-operating characteristics(ROC)curve analysis was shown to be *** additional studies with larger number of patients are necessary,we propose the determination of serum E2 level and testosterone level prior to performing TRT.

Food Preference of Pecky Rice Bugs Is Influenced by Experience

This study investigated whether prior experience influences the plant food preference of Trigonotylus caelestialium and Stenotus rubrovittatus which cause pecky rice grain, by using rice plants and 2 poaceous weeds. In a choice experiment between Digitaria ciliaris and the rice plants, both T. caelestialium and S. rubrovittatus adults that were reared on D. ciliaris plants showed significant initial preference for D. ciliaris over rice. In a choice experiment between Echinochloa crus-galli var. aristata and rice plants, T. caelestialium adults reared on E. crus-galli var. aristata strongly preferred E. crus-galli var. aristata over rice throughout the experiment. However, at and after 24 h, T. caelestialium adults reared on rice showed no food preference, although T. caelestialium initially preferred E. crus-galli var. aristata. In contrast, S. rubrovittatus adults reared on rice showed no preference between E. crus-galli var. aristata and rice. However, S. rubrovittatus reared on E. crus-galli var. aristata initially preferred E. crus-galli var. aristata to rice, with this preference waning with time. The same results were obtained for both sexes. Although the effect of experience differed with food source and the species of mirid bug, prior experience initially had a noticeable effect, which disappeared with time (1 d).

Seasonal Variation in a Hymenopterous Parasitoid, Holcotetrastichus rhosaces (Walker) (Hymenoptera: Tetrastichinae), on Its Hosts, Cassida nebulosa L. and C. piperata Hope (Coleoptera: Chrysomelidae), in Sendai, Northeastern Area of Japan

To investigate the seasonal variation of parasitism on Cassida nebulosa L. and C. piperata Hope, this research was conducted at the experimental field of Tohoku University in Sendai, Japan, where these tortoise beetles were parasitized by hymenopterous parasitoid Holcotetrastichus rhosaces (Walker). Percentage parasitism started to increase from the middle of July and reached nearly 100% in August. Adults of H. rhosaces emerged from the pre-pupae of the tortoise beetles, with 6 to 30 adults emerging per pre-pupae. The second generation of C. nebulosa and C. piperata was heavily parasitized by H. rhosaces. The adults of these tortoise beetles were rarely found in the field during late summer due to the high rates of parasitism.
