With the rapid increase in the number of COVID-19 patients in Japan,the number of patients receiving oxygen at home has also increased rapidly,and some of these patients have *** efficient approach to identify high-risk patients with slowly progressing and rapidly worsening COVID-19,and to avoid missing the timing of therapeutic intervention will improve patient prognosis and prevent medical *** admitted to medical institutions in Japan from November 14,2020 to April 11,2021 and registered in the COVID-19 Registry Japan were *** factors for patients with High Flow Nasal Cannula invasive respiratory management or higher were comprehensively explored using machine ***-specific cohorts were created,and severity prediction was performed for the patient surge *** were able to obtain a model that was able to predict severe disease with a sensitivity of 57%when the specificity was set at 90%for those aged 40e59 years,and with a specificity of 50%and 43%when the sensitivity was set at 90%for those aged 60e79 years and 80 years and older,*** were able to identify lactate dehydrogenase level(LDH)as an important factor in predicting the severity of illness in all age *** machine learning,we were able to identify risk factors with high accuracy,and predict the severity of the *** plan to develop a tool that will be useful in determining the indications for hospitalisation for patients undergoing home care and early hospitalisation.

Effects of Individual Explanations by Midwives about the Process of Delivery, Using 3D Animation Software, on Parturient Females’ Understanding of and Satisfaction with Delivery

Objectives: To clarify the effects on the delivery experience of midwives giving parturient females individual explanations about how the course of delivery, using 3D animation software (Delivery Animation). Method: 1) Subjects: Women admitted to Institution A for delivery;70 in the intervention group (34 primipara, 36 multipara), and 79 (41 primipara, 38 multipara) in the control group. 2) Data collection method: Intervention group;3) During phases 1 and 2 of delivery, after internal examinations, collaborative-researcher midwives gave parturient women explanations about the course of delivery, using tablets with the Delivery Animation software installed. This software was independently developed by the researchers. On postpartum day 1, the collaborative-researcher midwives distributed anonymous, self-administered questionnaires to the postpartum women. 4) During phases 1 and 2 of delivery, the course of delivery was explained using existing methods, and anonymous, self-administered questionnaires were distributed on postpartum day 1. 5) The intervention group and control group were compared using descriptive and inferential statistics, organized into primipara and multipara. Results: In the Understanding the Delivery Process category, primipara in the intervention group had significantly higher scores for “understood baby’s rotation” (p < 0.01) and “understood progress through delivery” (p < 0.05). In multipara, the intervention group scored significantly higher for “understood baby’s rotation” (p < 0.01). In the anonymous self-evaluation, primipara in the intervention group scored significantly higher in “midwife I could trust was by my side” (p < 0.01). Opinions on the Delivery Animation were classified into 7 categories. In their evaluations of the Delivery Animation, 89.0% of primipara and 92.0% of multipara said it helped them understand the course of labor, and 85.0% of primipara and 86.0% of multipara said

Pachinko and pachislot are unique forms of entertainment that dominate gambling activities in Japan. In this study, a web survey and interview were conducted in order to adjust three scales (DSM-5, SOGS, and PGSI) to pachinko/pachislot and explore an appropriate cutoff point for each of them. Using the assessment result in the interview as the outcome, the ROC curve of each scale was examined. The cutoff points were 4, 7, and 8 and the area under the curves (AUC) were 0.730, 0.783, and 0.724 for the DSM-5, SOGS, and PGSI, respectively. The study found that the type and situation of gambling influenced cutoff points in different ways depending on the questions asked in the scale.

The Expression of Surfactant Proteins A and D in the Intestines and Pancreas of Murine Fetuses

Purpose: Surfactant proteins exist in the digestive tract and may play an important role in the host defense. However, the expression of surfactant proteins in the premature digestive system remains unclear. The aim of this study was to investigate the expression of surfactant proteins in the intes-tines and pancreas of murine fetuses. Methods: Immunostaining for SP-A and SP-D was assessed in the small intestine and pancreas of ICR murine fetuses on days 15, 16, 17 and 18 of gestation (normal duration of pregnancy: 19 - 21 days). RT-PCR was performed to detect the expression of spa and spd mRNA in the small intestine and pancreas on day 16, 17 and 18 of gestation. Results: Immunoreactivity for SP-A and SP-D in the acinar cells of pancreas and intestinal mucosal surface were positive on day 16 of gestation onward. RT-PCR revealed that the expression of spa and spd mRNA was significant in the pancreas but weak in the small intestine. Conclusions: Our data revealed that surfactant proteins are present in the fetal intestines and pancreas and that a significant expression of spa and spd mRNA is detected in the fetal pancreas. Pancreas may be a possible organ involved in the synthesis and secretion of surfactant proteins into the intestinal lumen.

To understand the reality of gambling disorder caused by playing pachinko/pachislot, a national pachinko/pachislot survey was conducted. Based on the survey result, we examined the appearance ratio and the characteristics of individuals with a suspected pachinko-pachislot playing disorder. The analysis result indicated that approximately 0.4% of survey respondents were suspected to have the disorder. This appearance ratio was comparable to the prevalence of gambling disorder in the general population. Those who had a suspected disorder were likely to be divorced, and have no savings. Meanwhile, no distinctive characteristics were found in their gender, annual household income, or the highest educational attainment.

The Association between Sentinel Lymph Node Metastasis and Ki-67 Labeling Index

Background: The purpose of this study was to elucidate the association between sentinel lymph node (SLN) metastasis and Ki67 labeling index and to elucidate whether Ki-67 was useful or not for prediction of SLN metastasis in breast cancer. Methods: We identified 343 invasive breast cancer patients with sentinel lymph node biopsy (SLNB) from 2003 to 2012. The association between SLN status and clinicopathological features, molecular subtypes and Ki-67 labeling index were evaluated. Results: SLN metastasis was detected in 79 patients (23.0%). SLN metastasis was significantly associated with clinical T-stage (p = 0.0003), lymphovascular involvement (LVI) (p 0.0001). Ki-67 labeling index of primary tumor was significantly lower in SLN positive patients (p = 0.0331), and Ki-67 cut-off point of 7.5% was useful for dividing SLN positive from negative (p = 0.0197). Conclusion: Low value of Ki-67 labeling index, in addition to progression of clinical T-stage and presence of LVI, is significantly associated with SLN metastasis, and it seems to be useful to consider Ki-67 labeling index for SLN metastasis prediction.

Salivary Biomarkers May Be Useful to Assess Stress State in Patients with Lung Cancer Undergoing Chemotherapy

Objective: As it is important for medical staff to understand the stress state of patients with cancer, this study investigated the stress state in patients with lung cancer undergoing chemotherapy using salivary biomarkers and questionnaires. Materials and Methods: The subjects included 19 men and 9 women, with a mean age of 66.1 ± 7.1 years who received chemotherapy for lung cancer. Salivary samples of the participants were collected before and the 7 day after chemotherapy. Simultaneously, they completed the POMS (profile of mood states) and QOL (quality of life) questionnaires. The salivary cortisol and secretory IgA as stress markers were measured by enzyme immunoassay. Results: The salivary secretory IgA level was significantly higher at the 7th day after chemotherapy than before chemotherapy (p < 0.05), although the salivary cortisol level did not change. The tension-anxiety score, which is one of POMS subclasses, and total mood disturbance score were lower at day 7 of chemotherapy than before chemotherapy (p < 0.05), although no correlations were observed between POMS scores and salivary markers. However, a negative correlation was observed between the salivary cortisol level and the QOL full score (r= -0.411, p < 0.05). Conclusion: Measurement of salivary stress markers is noninvasive and may be useful for assessing the stress state of the patients.

Background: It is important for medical staffs to understand the state of stress of patients with cancer. The authors investigated stress assessment in patients with lung cancer undergoing chemotherapy. Materials and Method: Salivary cortisol and α-amylase were measured as stress markers before and after chemotherapy in 10 patients. Emotional status was assessed using the Profile of Mood States (POMS) questionnaire, and Quality of Life (QOL) was assessed using the Functional Assessment of Cancer Therapy-Lung (FACT-L) questionnaire. Results: No significant differences were found between the before and after chemotherapy in salivary levels of cortisol or α-amylase, the POMS and QOL scores. The salivary cortisol levels correlated with the POMS subscales of anger-hostility, and confusion. A negative correlation was observed between the salivary cortisol level and the QOL full score. The FACT-L full score correlated with the POMS subscales of depression-dejection, fatigue, and confusion. Intravenous immunoglobulin suppresses IL-10 production by activated B cells in vitro

A therapeutic preparation of polyclonal human IgG, i.e., intravenous immunoglobulin (IVIg), has been employed to treat several inflammatory and autoimmune disorders. B cells are supposed to be a target of IVIg, but the molecular mechanism is elusive because of the lack of a suitable experimental system. To gain an insight into the beneficial effect of IVIg on B cells, we first established an experimental setting in which IVIg modulates a murine B cell function in vitro, and then aimed at identifying the mechanistic features at the molecular level. Here we show that IVIg down-regulates IL-10 production by CpG-activated B cells in vitro. The responsible component of IVIg was identified as the F(ab’)2 portion, whose polyclonality is mandatory for the suppressive effect. IVIg, bound to the surface of activated B cells, was found to be co-localized with intracellular SHP-1 on confocal laser microscopy, suggesting that B cell-surface immunoreceptor tyrosine-based inhibitory motif-harboring receptors that recruit SHP-1 are target molecules for IVIg in our experimental setting. Overall, we postulate a scenario in which IVIg attenuates B cells by suppressing IL-10 production, a B cell growth factor, and thus down-regulates the production of pathogenic antibodies.
