＜正＞Aims: Osteocytes play an important role in the proposed ’ Mechanostat ’ in bone physiology. Osteonecrosis is characterized histopathologically as death of a significant amount of osteocytes. We accordingly hypothesized that there might be a shift of ’ Mechanostat ’ as well in steroid -associated osteonecrosis.

Introduction Bone-tendon junction (BTJ) is characterized with a transitional fibrocartilage zone and its healing is difficult. In this study, we explored the application of low intensity pulsed ultrasound (LIPUS) to accelerate the bone to tendon junction healing, as LIPUS has demonstrated certain effectiveness in accelerating fracture healing, delayed union, non-union, and soft tissue repair process by stimulating some endogenous biological growth factors.

Importance:131I-metaiodobenzylguanidine(131I-mIBG)has a significant targeted antitumor effect for ***,currently there is a paucity of data for the use of 131I-mIBG as a"front-line"therapeutic agent in those patients with newly diagnosed high-risk neuroblastoma as part of the conditioning regimen for myeloablative chemotherapy(MAC).Objective:To evaluate the feasibility of upfront consolidation treatment with 131I-mIBG plus MAC and hematopoietic stem cell transplantation(HSCT)in high-risk neuroblastoma ***:A retrospective,single-center study was conducted from 2003-2019 on newly diagnosed high-risk neuroblastoma patients without progressive disease(PD)after the completion of induction *** received 131I-mIBG infusion and MAC followed by ***:A total of 24 high-risk neuroblastoma patients were enrolled with a median age of 3.0 years at *** receiving this sequential consolidation treatment,3 of 13 patients who were in partial response(PR)before 131I-mIBG treatment achieved either complete response(CR)(n=1)or very good partial response(VGPR)(n=2)after *** a median follow-up duration of 13.0 months after 131I-mIBG therapy,the 5-year event-free survival and overall survival rates estimated were 29%and 38%for the entire cohort,and 53%and 67%for the patients who were in CR/VGPR at the time of 131I-mIBG ***:Upfront consolidation treatment with 131I-mIBG plus MAC and HSCT is feasible and tolerable in high-risk neuroblastoma patients,however the survival benefit of this 131I-mIBG regimen is only observed in the patients who were in CR/VGPR at the time of 131I-mIBG treatment.

背景鼻咽癌(Nasopharyngeal cancer,NPC)患者的主要治疗方法是放疗。但放疗后的中耳并发症及颞骨放射性骨坏死(ORN)所造成的听力损伤是摆在 NPC 患者面前的大难题。而传统的助听器可能加重耳漏和感染,并且耳模会对 ORN 患者的外耳道溃疡造成直接的创伤。目的探讨 Baha 对 NPC 患者放疗后听觉损伤的治疗是否有益。病例对13例 NPC 放疗后伴听力损伤的患者分两个阶段,观察两年。疗效评定一期手术时的骨钻标本做组织切片。同时进行听力学评估、植入固定度观察、邻近皮肤分级及满意度问卷调查分析。结果尽管组织切片显示53．8％(n=7)植入部位未建立骨性连接,但到目前所有 BaHa 植入均在位(平均 FU=14．8月;跨度=12-26月),80％的 BaHa 使用者每天开机时间＞8小时,平均满意主为85％。所有病例反映转用 Baha 后耳漏减少。讨论对所选择 NPC 病要而言,BaHa 是一种理想的声音处理器。我们的结果显示所有入选病人的骨整合成功,主观听觉清晰度增高、耳漏减少,使用 BaHa 时间越长,患者的满意度越高。结论 Baha 对 NPC 伴有听力损伤的患者是一种非常有效的助听器。骨整合成功率与未放疗病人无明显差异。

目的扩髓是髓内钉内固定手术的常用步骤, 由于可能引起肺部脂肪栓塞综合征而受到争议。尽管已知扩髓可使髓内压升高, 但髓内压升高和肺部脂肪栓塞之间的关系以及肺部脂肪栓子的来源仍不清楚。作者用山羊制作动物模型,研究扩髓对髓内压和肺部脂肪栓塞的影响。方法在实验组进行扩髓手术,而对照组只进行软组织切开术,然后测量髓内压及用薄层层析和气相色谱检测法分析骨髓和肺部组织的脂肪成分。结果在第一次扩髓时,髓内压升高最明显(532 m mHg)。对照组的肺组织没有脂肪栓塞,但全部实验组均有脂肪栓塞,而且栓塞的程度与髓内压的累积量成正比(r= 0-87,P=0-005)。骨髓脂肪的主要成分是甘油三酯,其含量最高的脂肪酸是油酸(18∶1,n- 9) 。在肺部脂肪检测中,实验组的甘油三酯明显升高(对照组0-10 mg/g,实验组0-45 mg/g,P=0-036),其中包括硬脂酸(18∶0)(对照组0-03 mg/g,实验组0-13 mg/g,P= 0-021)和油酸(18∶1,n- 9)( 对照组0-03 mg/g,实验组0-18 mg/g,P= 0-020)。结论本研究显示:肺部的脂肪栓子来源于骨髓脂肪,而且过度急速的扩髓可使髓内压升高,从而引起严重的肺部脂肪栓塞。
