Improving detection of cerebral small vessel disease aetiology in patients with isolated lobar intracerebral haemorrhage

Background and purpose We evaluate whether non-haemorrhagic imaging markers(NHIm)(white matter hyperintensity patterns,lacunes and enlarged perivascular spaces(EPVS))can discriminate cerebral amyloid angiopathy(CAA)from hypertensive cerebral small vessel disease(HTN-cSVD)among patients with isolated lobar intracerebral haemorrhage(isolated-LICH).methods In patients with isolated-LICH,four cSVD aetiologic groups were created by incorporating the presence/distribution of NHIm:HTN-cSVD pattern,CAA pattern,mixed NHIm and no *** pattern consisted of patients with any combination of severe centrum semiovale EPVS,lobar lacunes or multiple subcortical spots ***-cSVD pattern consisted of any HTN-cSVD markers:severe basal ganglia PVS,deep lacunes or peribasal ganglia white matter hyperintensity *** NHIm consisted of at least one imaging marker from either *** hypothesis was that patients with HTN-cSVD pattern/mixed NHIm would have a higher frequency of left ventricular hypertrophy(LVH),which is associated with *** In 261 patients with isolated-LICH,CAA pattern was diagnosed in 93 patients,HTN-cSVD pattern in 53 patients,mixed NHIm in 19 patients and no NHIm in 96 *** frequency of LVH was similar among those with HTN-cSVD pattern and mixed NHIm(50%vs 39%,p=0.418)but was more frequent in HTN-cSVD pattern compared with CAA pattern(50%vs 20%,pmodel,HTN-cSVD pattern(OR:7.38;95%CI 2.84 to 19.20)and mixed NHIm(OR:4.45;95%CI 1.25 to 15.90)were found to be independently associated with *** Among patients with isolated-LICH,NHIm may help differentiate HTN-cSVD from CAA,using LVH as a marker for HTN-cSVD.

Satellites have provided high-resolution(m)water color(i.e.,remote sensing reflectance)and thermal emission imagery of aquatic environments since the early 1980s;however,global operational water quality products based on these data are not readily available(e.g.,temperature,chlorophyll-a,turbidity,and suspended particle matter).Currently,because of the postprocessing required,only users with expressive experience can exploit these data,limiting their ***,we provide paths(recipes)for the nonspecialist to access and derive water quality products,along with examples of applications,from sensors on board Landsat-5,Landsat-7,Landsat-8,Landsat-9,Sentinel-2A,and *** emphasize that the only assured metric for success in product derivation and the assigning of uncertainties to them is via validation with in situ *** hope that this contribution will motivate nonspecialists to use publicly available high-resolution satellite data to study new processes and monitor a variety of novel environments that have received little attention to date.

Characteristics of Children Referred for Drainage of Sub-cutaneous Soft Tissue Infections

Purpose: Severe skin and soft tissue infections in children are a common cause of hospital admission. Surgical drainage is often required for definitive therapy. Our aim was to review common characteristics of children requiring surgical incision and drainage (I&D) for subcutaneous abscesses. methods: IRB approval was obtained for a retrospective chart review of children requiring I&D of subcutaneous abscesses between July 2006 and June 2011. Data points included patient demographics, abscess location, surgical procedure, microbiology cultures, and hospital length of stay. Results: Surgical I&D were performed on 1042 children, and 781 patients (75%) were admitted to the surgical service. The average age was 3.4 years. 605 patients (58%) were female. 637 abscesses (61%) were located in the groin/buttock/perineum area. methicillin-resistant Staphylococcus aureus (mRSA) was identified in 70% of the culture specimens. The average length of stay was 2.8 days. The rate of recidivism was 10.9%. Conclusions: Soft tissue infections requiring hospital admission and I&D are common in the pediatric population. The vast majority of these are due to mRSA infections. Infections requiring drainage most frequently occurred in the diaper area of girls less than 3 years old. A significant number of children have recurrent skin infections and represent an important patient subpopulation.

Risk calculators have offered a viable tool for clinicians to stratify patients at risk of prostate cancer(PCa)and to mitigate the low sensitivity and specificity of screening prostate specific antigen(PSA).While initially based on clinical and demographic data,incorporation of multiparametric magnetic resonance imaging(mRI)and the validated prostate imaging reporting and data system suspicion scoring system has standardized and improved risk stratification beyond the use of PSA and patient parameters ***-naïve patients with lower risk profiles for harboring clinically significant PCa are often subjected to uncomfortable,invasive,and potentially unnecessary prostate biopsy *** risk calculator data into prostate mRI reports can broaden the role of radiologists,improve communication with clinicians primarily managing these patients,and help guide clinical care in directing the screening,detection,and risk stratification of PCa.

Biological Removal of Nitrogen Compounds at a Coke-Oven Effluent Stream

The steel company SSAB in Oxel?sund, Sweden operates a coke-oven plant and has since a long time operated an activated sludge plant for treatment of effluent water. Along with more stringent requirements on discharge quality, especially focusing on nitrogen compounds and a new consent value for total nitrogen discharge (m of total N) the company decided to operate a pilot plant facility to investigate two major issues: 1) To define the conditions and restrictions for nitrification of ammonia nitrogen in the water;2) To find out how efficiently a denitrification would perform. In order to find answers to these questions SSAB hired a pilot plant for testing. The test facility is based on a single sludge activated sludge reactor system for biological nutrient removal, with a reactor volume of 3.8 m3. After a test period of 5 months it was possible to draw reliable conclusions regarding the performance. The untreated wastewater has a high content of total nitrogen, around 240 ppm. The major nitrogen part is ammonia nitrogen, but an important fraction is found as thiocyanate nitrogen. The following main conclusions were drawn from the test operation: · It was found to be crucial that the solids retention time (SRT) was kept at a sufficiently high level. During the successful operation the SRT was in the range of 40 - 50 days;· It is desirable to have an equalization basin upstream the main biological reactor to meet short time peak loads, defined as both flow and pollution;· The major toxic risks for the biological process were high thiocyanate and ammonia concentrations in the raw wastewater;· The system showed however a good microbiological capacity to acclimatize to the prevailing conditions after the needed time;· The tests did not include an optimization of the oxygen supply with respect to nitrification;however it was evident that the oxygen level was sufficient to maintain a complete nitrification at normal operating conditions;· Once the nitrification was

AIm To assess the performance of BALAD, BALAD-2 and their component biomarkers in predicting outcome of hepatocellular carcinoma(HCC) patients after liver *** BALAD score and BALAD-2 class are derived from bilirubin, albumin, alpha-fetoprotein(AFP), Lens culinaris agglutinin-reactive AFP(AFP-L3), and des-gammacarboxyprothrombin(DCP). Pre-transplant AFP, AFP-L3 and DCP were measured in 113 patients transplanted for HCC from 2000 to 2008. Hazard ratios(HR) for recurrence and death were calculated. Univariate and multivariate regression analyses were conducted. C-statistics were used to compare biomarker-based to predictive models. RESULTS During a median follow-up of 12.2 years, 38 patients recurred and 87 died. The HRs for recurrence in patients with elevated AFP, AFP-L3, and DCP defined by BALAD cut-off values were 2.42(1.18-5.00), 1.86(0.98-3.52), and 2.83(1.42-5.61), respectively. For BALAD, the HRs for recurrence and death per unit increased score were 1.48(1.15-1.91) and 1.59(1.28-1.97). For BALAD-2, the HRs for recurrence and death per unit increased class were 1.45(1.06-1.98) and 1.38(1.09-1.76). For recurrence prediction, the combination of three biomarkers had the highest c-statistic of 0.66 vs. 0.64, 0.61, 0.53, and 0.53 for BALAD, BALAD-2, milan, and UCSF, respectively. Similarly, for death prediction, the combination of three biomarkers had the highest c-statistic of 0.66 vs 0.65,0.61, 0.52, and 0.50 for BALAD, BALAD-2, milan, and UCSF. A new model combining biomarkers with tumor size at the time of transplant(S-LAD) demonstrated the highest predictive capability with c-statistics of 0.71 and 0.69 for recurrence and death. CONCLUSION BALAD and BALAD-2 are valid in transplant HCC patients, but less predictive than the three biomarkers in combination or the three biomarkers in combination with maximal tumor diameter(S-LAD).
