Model combining pre-transplant tumor biomarkers and tumor size shows more utility in predicting hepatocellular carcinoma recurrence and survival than the BALAD models

AIM To assess the performance of BALAD, BALAD-2 and their component biomarkers in predicting outcome of hepatocellular carcinoma(Hcc) patients after liver *** BALAD score and BALAD-2 class are derived from bilirubin, albumin, alpha-fetoprotein(AFP), Lens culinaris agglutinin-reactive AFP(AFP-L3), and des-gammacarboxyprothrombin(DcP). Pre-transplant AFP, AFP-L3 and DcP were measured in 113 patients transplanted for Hcc from 2000 to 2008. Hazard ratios(HR) for recurrence and death were calculated. Univariate and multivariate regression analyses were conducted. c-statistics were used to compare biomarker-based to predictive models. RESULTS During a median follow-up of 12.2 years, 38 patients recurred and 87 died. The HRs for recurrence in patients with elevated AFP, AFP-L3, and DcP defined by BALAD cut-off values were 2.42(1.18-5.00), 1.86(0.98-3.52), and 2.83(1.42-5.61), respectively. For BALAD, the HRs for recurrence and death per unit increased score were 1.48(1.15-1.91) and 1.59(1.28-1.97). For BALAD-2, the HRs for recurrence and death per unit increased class were 1.45(1.06-1.98) and 1.38(1.09-1.76). For recurrence prediction, the combination of three biomarkers had the highest c-statistic of 0.66 vs. 0.64, 0.61, 0.53, and 0.53 for BALAD, BALAD-2, Milan, and UcSF, respectively. Similarly, for death prediction, the combination of three biomarkers had the highest c-statistic of 0.66 vs 0.65,0.61, 0.52, and 0.50 for BALAD, BALAD-2, Milan, and UcSF. A new model combining biomarkers with tumor size at the time of transplant(S-LAD) demonstrated the highest predictive capability with c-statistics of 0.71 and 0.69 for recurrence and death. cONcLUSION BALAD and BALAD-2 are valid in transplant Hcc patients, but less predictive than the three biomarkers in combination or the three biomarkers in combination with maximal tumor diameter(S-LAD).

HDPairFinder:A data processing platform for hydrogen/deuterium isotopic labeling-based nontargeted analysis of trace-level amino-containing chemicals in environmental water

The combination of hydrogen/deuterium(H/D)formaldehyde-based isotopic methyl labeling with solid-phase extraction and high-performance liquid chromatography–high resolution mass spectrometry(HPLc-HRMS)is a powerful analytical solution for nontargeted analysis of trace-level amino-containing chemicals in water *** the huge amount of chemical information generated in HPLc-HRMS analysis,identifying all possible H/Dlabeled amino chemicals presents a significant challenge in data *** address this,we designed a streamlined data processing pipeline that can automatically extract H/D-labeled amino chemicals from the raw HPLc-HRMS data with high accuracy and ***,we developed a cross-correlation algorithm to correct the retention time shift resulting from deuterium isotopic effects,which enables reliable pairing of H-and D-labeled ***,we implemented several bioinformatic solutions to remove false chemical features generated by in-source fragmentation,salt adduction,and natural13c ***,we used a data mining strategy to construct the AMINES library that consists of over 38,000 structure-disjointed primary and secondary amines to facilitate putative compound ***,we integrated these modules into a freely available R program,*** rationale of each module was justified and its performance tested using experimental H/D-labeled chemical standards and authentic water *** further demonstrated the application of HDPairFinder to effectively extract N-containing contaminants,thus enabling the monitoring of changes of primary and secondary N-compounds in authentic water *** is a reliable bioinformatic tool for rapid processing of H/D isotopic methyl labeling-based nontargeted analysis of water samples,and will facilitate a better understanding of N-containing chemical compounds in water.

Metabolic syndrome and the urinary microbiome of patients undergoing percutaneous nephrolithotomy

Objective:To identify possible stone-promoting microbes,we compared the profiles of microbes grown from stones of patients with and without metabolic syndrome(MetS).The association between MetS and urinary stone disease is well established,but the exact pathophysiologic relationship remains *** evidence suggests urinary tract dysbiosis may lead to increased nephrolithiasis ***:At the time of percutaneous nephrolithotomy,bladder urine and stone fragments were collected from patients with and without *** sample types were subjected to expanded quantitative urine culture(EQUc)and 16 S ribosomal RNA gene ***:Fifty-seven patients included 12 controls(21.1%)and 45 MetS patients(78.9%).Both cohorts were similar with respect to demographics and non-MetS *** controls had uric acid stone *** EQUc,bacteria were detected more frequently in MetS stones(42.2%)compared to controls(8.3%)(p=0.041).Bacteria also were more abundant in stones of MetS patients compared to *** validate our EQUc results,we performed 16 S ribosomal RNA gene *** 12/16(75.0%)sequence-positive stones,EQUc reliably isolated at least one species of the sequenced *** were detected in both“infectious”and“non-infectious”stone ***:Bacteria are more common and more abundant in MetS stones than control *** findings support a role for bacteria in urinary stone disease for patients with MetS regardless of stone composition.

Hepatocellular carcinoma and food contamination: Ochratoxin A as a great prompter

Ochratoxin A (OTA) is a secondary metabolite of Asper-gillus and Penicillium, microorganisms that can be haz-ardous to health when present as food contaminants. OTA is a potent member of a group of mycotoxins. Prolonged exposure to mycotoxins in the diet is related to cancer, among other diseases. Hepatocellular car-cinoma (Hcc) accounts for 70%-90% of primary liver cancers and is the third leading cause of cancer-related deaths worldwide. In a recent study, Ibrahim et al proposed a correlation between the incidence of Hcc and contamination with OTA. Analysis of OTA in serum samples showed that Hcc patients had the highest incidence of OTA of the subjects examined (5-fold higher than that of the control group). OTA levels were significantly increased in Hcc patients. This study demonstrates that chronic exposure to high levels of OTA may be associated with a high risk of liver cancer development. Future epidemiologic studies of Hcc should focus on good practices in food preparation, food storage and the consumption of OTA-containing foods.

The trend towards miniaturization has increased dramatically over the last decade, especially within the fields concerned with bioengineering, microelectronics, and aerospace. Micromillin8 is among the principal manu- facturing processes which have allowed the development of components possessing micrometric dimensions, being used to the manufacture of both forming tools and the final product. The aim of this work is to present the principal aspects related to this technology, with emphasis on the work material requirements, tool ma- terials and geometry, cutting forces and temperature, quality of the finished product, process modelling and monitoring and machine tool requirements. It can be noticed that size effect possesses a relevant role with regard to the selection of both work material （grain size） and tooling （edge radius）. Low forces and temper- ature are recorded during micromillin8, however, the specific cutting force may reach high values because of the ploughing effect observed as the uncut chip thickness is reduced. Finally, burr formation is the principal concern with regard to the quality of the finished part.

Determination of particulate polycyclic aromatic hydrocarbons in ambient air by gas chromatography-mass spectrometry after molecularly imprinted polymer extraction

A solid phase extraction procedure(SPE)is described for the quantitative analysis of polycyclic aromatic hydrocarbons(PAHs)in atmospheric particulate matter(PM),as ubiquitous environmental pollutants routinely measured in air qualitymonitoring.A SPE cartridge was used based on a molecular imprinted polymer(MIP-SPE)properly tailored for selective retention of PAHs with 4 and more benzene fused *** performance of the clean-up procedure was evaluated with the specific concern of selective purification towards saturated hydrocarbons,which are the PM components mostly interfering Gc analysis of target *** optimized operative conditions,the MIP-SPE provided analyte recovery close to 95%for heavier PAHs,from benzo(α)pyrene to benzo(ghi)perylene,and close to 90%for four benzene rings PAHs,with good reproducibility(RSDs:2.5%-5.9%).Otherwise,c_(17)-c_(32) n-alkanes were nearly completely *** proposed method was critically compared with Solid Phase Micro Extraction(SPME)using a polyacrylate *** methods were successfully applied to the analysis of ambient PM2.5 samples collected at an urban polluted *** the two procedures,the MIP-SPE provided the highest recovery(R%≥93%)for PAHs with 5 and more benzene rings,but lower for lighter *** contrast,SPME showed a mean acceptable R%value(∼80%)for all the investigated PAHs,except for the heaviest PAHs in the most polluted samples(R%:110%-138%),suggesting an incomplete purification from the interfering n-hydrocarbons.

The effects of temperature and resource availability on denitrification and relative N_(2)O production in boreal lake sediments

Anthropogenic environmental stressors（like atmospheric deposition,land use change,and climate warming） are predicted to increase inorganic nitrogen and organic carbon loading to northern boreal lakes,with potential consequences for denitrification in ***,our ability to predict effects of these changes is currently limited as northern boreal lakes have been largely neglected in denitrification *** aim of this study was therefore to assess how maximum potential denitrification and N_2O production rates,and the relationship between the two（relative N_2O production）,is controlled by availability of nitrate（NO_3^-）,carbon（c）,phosphorus（P）,and *** were performed using the acetylene inhibition technique on sediments from a small,nutrient poor boreal lake in northern Sweden in *** potential denitrification and N_2O production rates at 4°c were reached already at NO_3^-additions of 106–120 μg NO_3^-–N/L,and remained unchanged with higher NO_3 *** incubation temperatures increased maximum potential denitrification and N_2O production rates,and Q10 was somewhat higher for N_2O production（1.77） than for denitrification（1.69）.The relative N_2O production ranged between 13% and 64%,and was not related to NO_3^-concentration,but the ratio increased when incubations were amended with c and P（from a median of 16% to 27%）.combined,our results suggests that unproductive northern boreal lakes currently have low potential for denitrification but are susceptible to small changes in NO_3 loading especially if these are accompanied by enhanced c and P availability,likely promoting higher N_2O production relative to N_2.

Uncovering the uncertainty: Risk factors and clinical relevance of P1 lesions on small bowel capsule endoscopy of anemic patients

AIM To identify risk factors for P1 lesions on small bowel capsule endoscopy(SBcE) and to describe the natural history of anemic patients with such type of *** One hundred patients were consecutively selected for a case-control analysis performed between 37 cases with P1 lesions and 63 controls with negative SBcE. Age, gender, comorbidities and regular medication were collected. Rebleeding, further investigational studies and death were also analyzed during the *** No significant differences on gender, median age or charlson index were found between groups. Although no differences were found on the use of proton pump inhibitors, acetylsalicylic acid, anticoagulants or antiplatelet agents, the use of non-steroidal antiinflammatory drugs(NSAID) was associated with a higher risk of P1 lesions(OR = 12.00, 95%cI: 1.38-104.1). From the 87 patients followed at our center, 39 were submitted to additional studies for investigation of iron-deficiency anemia(IDA), and this was significantly more common in those patients with no findings on SBcE(53.7% vs 30.3%, P = 0.033). A total of 29 patients had at least one rebleeding or IDA recurrence episode and 9 patients died of non-anemia related causes but no differences were found between cases and *** P1 lesions are commonly found in patients with IDA submitted to SBcE. The use of NSAID seems to be a risk factor for P1 lesions. The outcomes of patients with P1 lesions do not differ significantly from those with P0 lesions or normal SBcE.

Metal matrix nanocomposites(MMncs)comprise a metal matrix filled with nanosized reinforcements with physical and mechanical properties that are very different from those of the *** ZA-27 alloy-based nanocomposites,the metal matrix provides ductility and toughness,while usually used ceramic reinforcements give high strength and *** ZA-27 alloy-based nanocomposites,reinforced with different types(Si c and Al2O3),amounts(0.2 wt.%,0.3 wt.%,and 0.5 wt.%)and sizes(25 nm,50 nm,and 100 nm)of nanoparticles were produced through the compocasting process with mechanical alloying pre-processing(ball milling).It was previously shown that the presence of nanoparticles in ZA-27 alloy-based nanocomposites led to the formation of a finer structure in the nanocomposites matrix and an improvement in the basic mechanical properties(hardness and compressive yield strength)through the enhanced dislocation density strengthening *** particle erosive wear testing demonstrated that these improvements were followed with an increase in the erosive wear resistance of tested nanocomposites,as ***,by analyzing the influences of type,amount,and size of nanoparticles on the erosive wear resistance of nanocomposites,it was demonstrated that there is an optimal amount of nanoparticles,which in our case is 0.3 wt.%,and that the presence of Sic nanoparticles and smaller nanoparticles in nanocomposites had more beneficial influence on erosive wear resistance.

The multi-organ failure syndrome associated with acute and acute-on-chronic liver failure(AcLF)is thought to be mediated by overwhelming systemic inflammation triggered by both microbial and non-microbial *** plasma exchange(TPE)has been proven to be an efficacious therapy in autoimmune conditions and altered immunity,with more recent data supporting its use in the management of liver *** therapies have been shown to improve survival in critically ill patients with liver failure who are not expected to survive until liver transplantation(LT),who are ineligible for LT or who have no access to *** has been shown to reduce the levels of inflammatory cytokines,modulate adaptive immunity with the potential to lessen the susceptibility to infections,and reduce the levels of albumin-bound and water-bound toxins in liver *** patients with acute liver failure,high volume TPE has been shown to reduce the vasopressor requirement and improve survival,particularly in patients not eligible for *** volume TPE has also been shown to reduce mortality in certain sub-populations of patients with *** may be most favorably employed as a bridge to LT in patients with *** this review,we discuss the efficacy and technical considerations of TPE in both acute and acute-on-chronic liver failure.
