Feasibility of progressive strength training shortly after hip fracture surgery

AIM: To investigate the feasibility of a 6-wk progressive strength-training programme commenced shortly after hip fracture surgery in community-dwelling ***: This prospective, single-blinded cohort study evaluated 31 community-dwelling patients from four outpatient geriatric health centres aged 60 years or older, who started a 6-wk programme at a mean of 17.5 ± 5.7 d after hip fracture surgery. The intervention consisted primarily of progressive fractured kneeextension and bilateral leg press strength training(twice weekly), with relative loads commencing at 15 and increasing to 10 repetitions maximum(RM), with three sets in each session. The main measurements included progression in weight loads, hip fracture-related pain during training, maximal isometric knee-extension strength, new mobility score, the timed up and go test, the 6-min walk test and the 10-meter fast speed walk test, assessed before and after the ***: Weight loads in kilograms in the fractured limb knee-extension strength training increased from 3.3 ± 1.5 to 5.7 ± 1.7 and from 6.8 ± 2.4 to 7.7 ± 2.6, respectively, in the first and last 2 wk(P < 0.001). Correspondingly, the weight loads increased from 50.3 ± 1.9 to 90.8 ± 40 kg and from 108.9 ± 47.7 to 121.9 ± 54 kg in the bilateral leg press exercise(P < 0.001). Hip fracture-related pain was reduced, and large improvements were observed in the functional outcome measurements, e.g., the 6-min walk test improved from 200.6 ± 79.5 to 322.8 ± 68.5 m(P < 0.001). The fractured limb knee-extension strength deficit was reduced from 40% to 17%, compared with the non-fractured limb. Ten patients reported knee pain as a minor restricting factor during the last 10 RM knee-extension strength-training sessions, but with no significant influences on performance. Estimation of Fatigue Life of Laser Welded AISI304 Stainless Steel T-Joint Based on Experiments and Recommendations in Design Codes

In this paper the fatigue behavior of laser welded T-joints of stainless steel AISI304 is investigated experimentally. In the fatigue experiments 36 specimens with a sheet thickness of 1 mm are exposed to one-dimensional cyclic loading. Three different types of specimens are adopted. Three groups of specimens are used, two of these are non-welded and the third is welded with a transverse welding (T-Joint). The 13 laser welded specimens are cut out with a milling cutter. The non-welded specimens are divided in 13 specimens cut out with a milling cutter and 10 specimens cut out by a plasma cutter. The non-welded specimens are used to study the influence of heat and surface effects on the fatigue life. The fatigue life from the experiments is compared to fatigue life calculated from the guidelines in the standards DNV-RP-C203 and EUROCODE 3 EN-1993-1-9. Insignificant differences in fatigue life of the welded and non-welded specimens are observed in the experiments and the largest difference is found in the High Cycle Fatigue (HCF) area. The specimens show a lower fatigue life compared to DNV-RP-C203 and EUROCODE 3 EN-1993-1-9 when the specimens are exposed to less than 4.0 1E06 cycles. Therefore, we conclude that the fatigue life assessment according to the mentioned standards is not satisfactory and reliable.

Cryptic Behaviour of Juvenile Turbot Psetta maxima *** European Flounder Platichthys flesus L.

The aim of this study was to examine the burying behaviour of hatchery-reared European flounder Platichthys flesus and turbot Psetta maxima, and whether conditioning on a sandy substrate would improve burying efficiency. Both species buried shortly after release on a sandy substrate. However, the study revealed interspecies differences;the flounder buried immediately after release, while the turbot buried gradually. No significant difference in burying efficiency was observed between naïve and conditioned flounder and turbot. An effect of size on burial efficiency was observed for both flounder and turbot with a tendency for larger fish to bury more efficiently than smaller fish, despite previous conditioning. Size at settlement was found to be >2 cm for flounder and >3 cm for turbot.

Restoration of a Temperate Reef:Effects on the Fish Community

The extraction of large boulders from coastal reefs for construction of harbours and coastal protection has led to habitat degradation for local fish populations through the destruction of cavernous reefs and changes in macroalgal cover resulting from a loss of substrate. The temperate reef at L?s? Trindel in Kattegat, Denmark, has now been re-established with the aim of restoring the reef’s historical structure and function. The effects of the restoration on the local fish community are reported here. Fishing surveys using gillnets and fyke nets were conducted before the restoration (2007) and four years after the restoration of the reef (2012). Species of the family Labridae, which have a high affinity for rocky reefs, dominated both before and after the restoration. Commercially important species such as cod Gadus morhua, and saithe Pollachius virens, occurred infrequently in the catches in 2007 but were significantly more abundant in the catches in 2012. Cods were especially attracted to the shallow part of the reef that was restored by adding stones. For some species, such as ballan wrasse Labrus bergylta, and cod, the proportion of larger individuals increased after the restoration. The findings highlight the importance of reef habitats for fish communities and the need for their protection.

Knowledge and Skills for Management of Sexually Transmitted Infections by Rural Medical Practitioners in Bangladesh

Sexually transmitted infection (STI) management is considered rudimentary among rural medical practitioners (RMPs) in Bangladesh. We sought to understand the level of knowledge and skills in STI management and to assess the impact of a two-day training orientation among RMPs in Tangail district. Data were collected through a baseline survey of 225 practicing RMPs in the study area and a three-month follow-up survey of 99 RMPs who participated in a two-day STI/HIV orientation training. The level of formal training among RMPs ranged from none (22.7%), to paramedical training (14.7%) and local medical assistant training (62.6%). The baseline survey revealed a low level of STI/HIV knowledge and misconceptions about the transmission of STI/HIV among RMPs. RMPs mostly prescribed first line antibiotics for treatment of common reproductive tract infections (RTIs) including STIs, but they rarely prescribed the correct dosages according to the national RTI/STI management guidelines. Only 3% of RMPs were able to correctly answer all four HIV transmission (unprotected sexual intercourse, blood transfusion, needle sharing and mother to child transmission) questions at baseline, while 94.9% of RMPs answered all four correctly at three months following the training (p = 0.001). Only 10% of RMPs reported suggesting the recommended drug (azithromycin) and only 2% mentioned about the recommended dosage (2 gm single dose) for the treatment of urethritis/cervicitis;compared to 49.5% suggested azithromycin at follow-up with 39.4% mentioned the recommended 2 gm single dose (p = 0.001). Our study found low level of knowledge and poor practices related RTI/STI management among RMPs. Short orientation training and education intervention shown promise to increase knowledge and management skills for RTIs/STIs.

目的　利用地理信息系统（ＧＩＳ）数据库分析中国大陆钉螺的空间区域分布。方法　收集中国九省３４个种群钉螺等位基因实验所得的种群遗传变异资料以及钉螺易感性实验资料，建立数据库。以世界数字地图为背景，在ＡｒｃＶｉｅｗ３．０ａ软件上与钉螺的遗传变异和易感性数据库相联。采用ＡｒｃＶｉｅｗ空间分析和ＧＩＳ重叠分析功能进行区域划分。收集１９９５年ＡＶＨＲＲＮＤＶＩ卫星图像组合成流行季节的卫星图像，并重叠各种群钉螺的杂合指数、多态位点百分率和血吸虫感染率分布图，进行图像分类。结果　空间分析结果，将钉螺遗传杂合性指数、多态位点百分率和钉螺感染率的分布图，划分成西部和东部二大区域，各区域内有各种类型。三层分布图与ＡＶＨＲＲＮＤＶＩ卫星图像重叠分析后，显示出西部四川、西部云南、中部江湖洲滩和东南部沿海４个明显不同的区域。结论　首次运用ＧＩＳ分析种群遗传学资料，佐证了中国大陆钉螺种群结构为分裂亚群结构，并可区分出多个亚种的理论。

Subjective and Objective Results of Native Anterior Vaginal Wall Repair in Local Anesthesia:A 10-Year Follow-Up

Introduction and Hypothesis: This follow-up study evaluates long-term subjective and objective outcome of native tissue anterior vaginal wall repair using local anesthesia. Methods: 72 women were operated. At 10-year follow-up anatomical results were evaluated by clinical examination. Furthermore, the women filled in a validated symptom and quality of life questionnaire. Results: Forty women (56%) completed the 10-year follow-up. Eighteen women (25%) had died within the ten-year follow-up period and fourteen women (19%) were lost to follow-up. Six (15%) of the women who came for follow-up had been reoperated for anterior vaginal wall prolapse within the 10-year follow-up period and were analysed as a separate group. Of the 40 women who came for the 10-year clinical examination none had stage 0 pelvic organ prolapse. Twenty (50%) women had stage 1, whereas, 13 (32%) had stage 2 and 1 (3%) had stage 3 pelvic organ prolapse. Ten years after surgery, 28 women (70%) had no bulge symptoms. Six (15%) of the women experienced bulge symptoms at the time of follow-up. Ten years postoperatively, 30 (75%) of the women in an overall quality of life assessment considered their condition improved after surgery. Conclusion: At 10-year follow-up 70% of women were relieved from their bulge symptoms and 75% still considered themselves better or much better than before the operation. However, 15% of women had been reoperated and 15% still experienced bulge symptoms.
