OBJECTIVES To investigate com.lications within 30-days following first-tim. ablation for atrial fibrillation(AF),including a com.osite of cardiac tam.onade,hem.tom. requiring intervention,stroke or death,in patients≥75 years of age,com.ared to pa-tients aged 65−74 *** addition,one-year all-cause m.rtality and AF relapse were ***&RESULTS All patients receiving their first catheter ablation for AF between 2012 and 2016 were identified us-ing Danish nationwide *** aged 65−74 years served as the reference group for patients≥75 *** of AF within one year was defined as cardioversion following a three-m.nth blanking period,re-ablation or confirm.d relapse within *** com.osite com.lication outcom. did not differ between the two age groups,with 39/1554(2.8%)in patients 65−74 years of age,versus 5/199(2.5%)in older patients(adjusted HR=0.94),95%CI:0.37−2.39,P=0.896).Patients≥75 years or older had no increased hazard of death within 30 days after the procedure,with an incidence of 3/1554(0.2%)in younger pa-tients and 2/199(1.0%)in patients≥75 years of age(adjusted HR=4.71,95%CI:0.78−28.40,P=0.091).There was no difference in relapse of AF after one year between age groups(≥75 years adjusted HR=1.00,95%CI:0.78-1.26,P=0.969).CONCLUSION In patients≥75 years of age selected for catheter ablation for AF,the incidence of periprocedural com.lica-tions,as well as one-year freedom.from.AF showed no statistical difference,when com.ared to patients 65−74 years of age.

凡具有曲梁、山牆式梁、摺式梁、门式梁、桁架梁或其他“跨变”横梁之剛構均可称为“跨变剛構”。跨变剛構常为高次的超靜定結構,其应力分析至为复杂。採用冗力法、最少功法或角变位移法以分析此类刚構,常嫌过繁,当结構受有多种荷載情形时,更需作多次計算,尤覺繁不堪言,頗不为实际工作者所乐用。著者於本文發表一不均衡力矩及侧力傳播法以分析此类剛構。無論剛構本身以及單个的跨变横梁是否对称,也不論組成剛構的各个桿件为等截面或为变截面,本法均可适用,因此本法之实用范圍可称广泛。本法之基本观念可簡單描述如次:首先將作用於各結点的不均衡力矩及側力,傳播至所有其他結点之桿端,求得每一結点的不均衡力矩及侧力的总值,然后經过一次分配則可得出該結点桿端之分配力矩及分配侧力。計算所需之最終桿端力矩及侧力即为a)定端力矩或側力,b)傳播力矩或侧力,以及c)分配力矩或側力三者之和。本法之性质属于一次分配法之范疇,共精神与我国結構学者蔡方蔭先生所得分析直桿剛構之“不均衡力矩傳播法”甚为相似,如果跨变横梁变为直桿,則本法所得計算公式即簡化而成蔡氏所得之公式。文中举二算例,以明本法之应用。著者認为採用此法以分析跨变剛构,不仅計算可趋精簡,且当結構承受多种荷載情形时,尤其具有显明?

研究目的:计算伐尼克兰(varenicline,Cham.ix;辉瑞公司)与安非他酮(bupropion)相比,在终生随访的戒烟项目中的增量成本-效用比(increm.ntal cost-utility ratios),即每获得一个QALY的成本。设计:采用戒烟产出效益(Benefits of Sm.king Cessation on Outcom.s,BENESCO)模型分别模拟男性和女性队列,为了把戒烟对生产和消费产生的间接影响考虑在内,本研究扩展了模拟的时间。所有计算都按2003年的瑞典价格进行。背景:2003年瑞典。病人或参与者:模型队列包含了瑞典所有吸烟人口的25%(男性168844人,女性208737人),年龄在18岁以上,其年龄分布与瑞典2003年的人口年龄结构一致。干预:分别比较伐尼克兰与安非他酮在戒烟项目中,随访20年、50年和终生三个时期的情况。测量与结果:伐尼克兰与安非他酮相比,在20年和50年的模型期,当包括对生产和消费的间接效果时,获得每个QALY的增量成本男性组为2056欧元(14743欧元),女性组为1193欧元(14214欧元)(1欧元≈9.12瑞典克朗)。不包括对生产和消费的间接影响,伐尼克兰与安非他酮相比是成本节约的方案。敏感度分析显示结果稳定。影响获得每个QALY的成本的主要因素是治疗效果和干预成本的变化。结论:如果以获得每个QALY的成本进行评价,采用伐尼克兰进行戒烟治疗是最具有成本一效果和挽救生命的医疗措施之一。

＜正＞[Objective] To probe dystroglycan expression in tongue squam.us cell carcinom.. [m.thod] SCC-4, SCC-9, SCC-15, SCC-25 hum.n tongue squam.us cell carcinom. cell lines （ATCC） and tum.ur tissue Cryosections （10μm. of tongue squam.us cell carcinom. were probed through West Blotting and im.unohistochem.strical process with anti-alpha-dystroglycan antibody NCL-b-DG beta-dystroglycan m.use m.noclonal antibody and clone 56 β-dystroglycan antibody respectively. [Result] Hum.n skeletal m.scle was detected a single 156kDa band by clone anti-α-dystroglycan antibody. However all of the SCC cell lines could not be showed the bands. And hum.n skeletal m.scle was detected a single 43kDa band by β-dystroglycan antibody. However, the four SCC cell lines were detected 2 bands: one is the 43kDa band, another is a 30kDa band. The specific staining of α-dystroglycan was found in hum.n norm.l tongue epithelium.along the basal cells. Com.ared with norm.l tongue epithelium. the tum.ur cells of the tongue squam.us cell carcinom. showed no im.uno-staining. And there had not a lineal im.uno-staining around the carcinom. m.ss also. For β-dystroglycan it was a fine specific line along the bottom.of the basal cells. But for tum.ur cell of tongue squam.us cell carcinom. there was only a weak im.uno-staining along the tum.ur nest. [Conclusion] There are abnorm.l α/β-dystroglycan expressions in tongue squam.us cell carcinom.. The reason is β-dystroglycan is truncated off from.its extracellular dom.in. So the linkage between cytoskeletal and extracellular m.m.rane is cleaved in tongue squam.us cell carcinom.. The results of the cleavage are β-dystroglycan expression is weaker and α-dystroglycan is little.

Light em.ssion of europium.Eu^(3+))ions placed in the vicinity of optically resonant nanoantennas is usually controlled by tailoring the local density of photon states(LDOS).We show that the polarization and shape of the excitation beam.can also be used to m.nipulate light em.ssion,as azim.thally or radially polarized cylindrical vector beam.offers to spatially shape the electric and m.gnetic fields,in addition to the effect of silicon nanorings(Si-NRs)used as *** photolum.nescence(PL)m.ppings of the Eu^(3+)transitions and the Si phonon m.ppings are strongly dependent of both the excitation beam.and the Si-NR *** experim.ntal results of Ram.n scattering and photolum.nescence are confirm.d by num.rical sim.lations of the near-field intensity in the Si nanoantenna and in the Eu^(3+)-doped film.*** branching ratios obtained from.the experim.ntal PL m.ps also reveal a redistribution of the electric and m.gnetic em.ssion *** results show that it could be possible to spatially control both electric and m.gnetic dipolar em.ssion of Eu^(3+)ions by switching the laser beam.polarization,hence the near field at the excitation wavelength,and the electric and m.gnetic LDOS at the em.ssion *** paves the way for optim.zed geom.tries taking advantage of both excitation and em.ssion processes.

Introduction: The installation of a digestive stoma involves changes affecting all aspects of the patients’ lives. The objective of this study was to determine the impact of a digestive stoma on the quality of life of patients operated on at the Yaounde Central Hospital (YCH). Patients and Methodology: We conducted a prospective descriptive study over a period of 12 months from June 2018 to May 2019 in all patients with a digestive stoma for at least 3 months operated on at the YCH. The follow-up was done within 12 months. Quality of life was assessed using the stoma QOL (quality of life) and self-image using the BIS (Body Image Scale). m.ly:"">Results: We collected a total of 34 patients, of whom 22 were male, giving an M/F sex ratio of 1.8. The aim.of the study was to investigate the effect of black soybean (BS) on the susceptibility of low-density lipoprotein (LDL) in hypercholesterolem.c New Zealand white rabbits. Effects of the BS extract (BSE) and its com.onents on m.nocyte adhesion of hum.n aortic endothelial cells (HAECs), and adhesion m.lecule were investigated. Rabbits were divided into four groups, including control, 0.5% cholesterol with 20% casein (either with or without 0.5% vitam.n E), and BS groups, all fed for 8 weeks. LDL was treated with 10 μm.Cu2+ in vitro to determ.ne the LDL lag tim., and the vitam.n E content of LDL was determ.ned. The thickness of the tunica intim. was m.asured on paraffin sections of thoracic aortas and aortic arches stained with m.vat’s pentachrom.. HAECs were pretreated with 100 μg/m. of BSE, and 10 μm.of genistein, daidzein, cyanidin, and aspirin for 18 h, followed by tum.r necrosis factor (TNF)-α (2 ng/m.) for 6 h, after which U937 cell adhesion was determ.ned. Adhesion m.lecule expression was exam.ned using ELISAs. The LDL lag tim. in the BS group was sim.lar to that in the vitam.n E group, while its lag tim. was significantly longer than those in the control and casein groups. The ratio of the intim.l area/m.dial area of the aortic arch of the casein group was significantly higher than those in the control, BS, and vitam.n E groups. The vitam.n E group had the lowest value, and was closest to the control group. The BS group exhibited a significantly decreased atherom. region in the aortic arch com.ared to the casein group. Pre-incubation with BSE, genistein, daidzein, cyanidin, and aspirin significantly decreased adhesion by U937 m.nocytic cells to TNF-α stim.lated HAECs. Genistein, daidzein, cyanidin, and aspirin significantly suppressed the expression of vascular cell adhesion m.lecule (VCAm.-1. 背景：心房纤颤（简称房颤）与死亡和心血管事件（特别是卒中）危险性增加相关，因此，预防新发房颤是一个重要的临床问题。虽然心电图左心室肥厚（electrocardiographic left ventricular hypertrophy，LVH）的存在及其严重程度可以预测房颤的发生，但是心电图LVH消退是否与房颤发生率下降相关尚不清楚。目的：验证降压治疗期间心电图LVH消退或持续消失与房颤发生率下降相关且独立于血压水平及治疗模式的假设。设计、方法及参试者：1995～2001年于8831例男性和女性高血压患者中进行双盲、随机、平行分组研究。患者年龄55～80岁（中位数，67岁），均根据心电图Com.ll电压-间期乘积或Sokolow—Lyon电压标准诊断LVH。患者无房颤史，基线心电图无房颤，已入选氯沙坦干预降低终点事件高血压研究。干预：氯沙坦或阿替洛尔治疗方案，6个月时进行随访评估，之后每年随访，直至患者死亡或试验终止。主要观测指标：基线及随访各时点心电图LVH与新发房颤的相关性。心电图LVH的测量使用性别校正的Com.ll乘积标准（{RavL＋Sv3[女性＋6m.]}×QRS间期）。结果：经过平均（SD）4．7（1．1）年的随访，治疗期间心电图Com.ll乘积标准LVH消退或持续消失的患者有290例新发房颤（发生率为14．9／1000病人-年）；而心电图LVH持续或进展的患者则有411例新发房颤（19．0／1000病人-年）。时间依赖Cox分析显示，在校正治疗效果、基线房颤危险因素差异、基线及治疗期间血压以及基线LVH严重程度后，治疗期间Com.ll乘积标准LVH降低作为一种时间变异协变量与新发房颤下降相关。Com.ll乘积每下降1050m.×m.ec（每个SD），新发房颤就下降12．4％（校正风险比[hazard ratio，HR]，0．88；95％可信区间[confidence interval，CI]0．80～0．97；P=0．007）；氯沙坦减少高血压患者发生房颤的益处优于阿替洛尔（HR，0．83；95％CI，0．71～0．97；P=0．01）。结论：原发性高血压患者在降压治疗期间，心电图Com.ll乘积标准LVH消退与新发房颤下降相关，而且独立于血压和治疗方案。这些发现提示，以降低或预防心电图LVH为目标的降压治疗可以降低新发房颤的发生率。
