Description of Post-Stroke Cognitive Disorders at Abidjan University Hospitals, Côte d’Ivoire

Stroke patients often experience motor deficits and cognitive problems after a stroke. Objective: To improve our understanding of the cognitive consequences of stroke. Method: a descriptive and analytical cross-sectional study was conducted over 12-month period in the neurology departments of the Cocody and Treichville Hospitals in Côte d’Ivoire. Results: Out of 724 patients admitted to the neurology department, 415 (57.32%) were stroke patients, of which 145 (34.94%) were screened. The frequency of global cognitive functioning impairment was 86.21%, significantly higher than the frequency of patients without impairment, which was 13.79%. The study focused on detailing the cognitive status of stroke patients in neurology departments, assessing several cognitive functions during the subacute phase of stroke. These functions included global cognitive functioning, executive functions, language and memory. The frequency of post-stroke cognitive impairment is high among stroke patients in Abidjan. This frequency is comparable to figures found in Subsaharian stroke populations. Demographic and clinical characteristics studied included age, gender, education level, employment status, vascular diseases and cerebral affected area. Among these characteristics, only the education level and the cerebral affected area have been found significant. Conclusion: The incidence of cognitive impairment after a stroke is significantly high among stroke patients in Abidjan.

The aim of this study is to produce large quantities of yam tubers in the field from seed tubers derived from vitroplants regenerated from aerial stem cuttings and farmers’ tuber seeds. Seed tubers from ten yam cultivars, including five from Dioscorea alata (cDa083, cDa053, cDa115, cDa150 and cDa266) and five from Dioscorea cayenensis-rotundata (cDr015, cDr027, cDr150, cDr206 and cDr148) with distinct agro-morphological characteristics, were used as material. A completely randomized block design with 3 replications was used. In each block, all cultivars and seed types were represented. When put into cultivation, the tubers produced by the vitroplants all germinated and gave 100% healthy plants, compared with 86% to 100% healthy plants for the seed tubers produced by the farmers. Yields per hectare ranged from 10 to 25 tonnes for seed tubers produced by in vitro plants, and from 4 to 9 tons for seed tubers produced by farmers. Seed tubers from vitroplants can therefore be recommended to farmers as a solution to the problem of seed tuber unavailability.

In the tropics, lowland rice cultivation is often confronted with the problem of iron toxicity. The solution proposed by research in general is the use of industrial silicon. However, the high cost of industrial silicon limits its adoption by farmers. A study was carried out in Zakogbeu;Center-West of Côte d’Ivoire, to assess the potential of kaolin to mitigate the effect of this soil constraint on the root of the rice plant. Five kaolin-based treatments were analyzed (T0 = 0 kg kaolin ha−1, T1 = 366 kg kaolin ha−1, T2 = 736 kg kaolin ha−1, T3 = 1097 kg kaolin ha−1 and T4 = 1465 kg kaolin ha−1 are 0, 200, 400, 600 and 800 kg SiO2 ha−1) in a device in complete random blocks, with 5 repetitions. The results obtained show that kaolin supply increases the length of the root tissue as well as the number of branching of the root of the rice plant. Root tissue increased from 10 cm with T0 treatment to more than 15 cm with treatment T4. The microscopic observation of the roots shows that in the treatment T0, the roots present only primary ramifications and the tertiary and quaternary ramifications are observed with the treatments T3 and T4. The contribution of kaolin is an alternative to inhibit the effect of iron toxicity on the rice plant root development in iron toxicity condition.

Hippopotamus bites are rare in medical consultations, despite several studies exposing their aggressive character. A case of a hippopotamus bite on the shoulder of a fisherman has been reported. In addition to the osteoarticular aspect representing a therapeutic problem, the infectious aspect was marked by sepsis, with a death in less than 48 hours. Based on this observation, a protocol for the initial management of large animal bites was submitted.

The Ouladine lagoon has an irregular bottom lined with different substrates. The lithological characteristics make it possible to observe sediments ranging from silts to sands and mixed sediments of variable color. These different fractions have varying proportions. The coarse fraction (>63 μm) has percentages ranging from 0 to 77% and the fine fraction ( μm) from 22.46 to 100% in the different sediments collected. The minority fraction of fine sands occupies the northern banks where the greatest depths of the section parallel to the coastal strip are observed. On the other hand, the sandy fractions occupy the southern shore up to the vicinity of the Azuretti mangrove island. This sandy fraction is also present on almost the entire section from the Comoé-Ebrié lagoon confluence to the closed mouth. The grain size characters of the sediments, such as the diagram of the classification coefficient So and the mean Mz, show well-classified sands. These sediments are deposited in a slightly agitated environment coming mainly from dunes and rivers. The sands of the estuarine complex were emplaced by saltation for most samples and by rotation.

Land use monitoring occupies a very important place in the analysis of the dynamics of the earth system. It helps to understand the organization and helps to provide relevant elements for the establishment of diagnoses and the development of environmental forecasts. The objective of this study is to follow the evolution of the agricultural landscape in the department of Séguéla from 1988 to 2020 and to make a prediction for 2050, in order to manage the spaces reasonably. The methodology adopted is based on the one hand on the processing of satellite images for the analysis of land cover and on the other hand on predictive modeling (LCM model) by 2050. The results obtained show that the land use maps produced after processing the satellite images made it possible to highlight the dynamics of the agricultural landscape in this part of the Worodougou region. During the period 1988 to 2020, we witness an increase in the area of cultivated territory as well as a slight reduction in wooded savannas which are largely made up of perennial crops (cashew trees, cocoa trees, coffee trees, etc.). These two aforementioned classes have respective annual rates of change of 2.42% and −0.44%. A scenario modeling land cover changes in 2050 with an overall accuracy of 80.35% revealed a continued growth of crops and fallows to the detriment of natural forests and wooded savannas.

Marahoué watershed, located in the Center West of Côte d’Ivoire, has experienced significant population growth in recent decades. And a major economic boom linked to intense agricultural activity and the presence of certain industries. This population growth is also accompanied by seasonal water shortages. Hence it needs to better manage the basin’s groundwater, which is a permanent resource and more resistant than surface water to climatic hazards. The objective of this study is therefore to propose a conceptual model of hydrogeological flow for the sustainable exploitation of groundwater resources in the Marahoué watershed. The establishment of the conceptual model was carried out in two stages. The first step consisted in defining the stratigraphic units. For this purpose, three units have been defined. These are the layer of alterite, the useful fissured horizon and the sound basement. The thickness of the layer of alterite varies from 0 to 80 m with an average of 26 m. As for the useful fissured horizon, its thickness is between 43 and 46.5 with an average of 45 m. In addition, the roof of the basement presents a slightly uneven morphology with a North-West, South-East dip and the altitudes are between 150 and 390 m. The second step corresponds to the phase of determining the hydrodynamic parameters. During this phase, the crack porosity, the transmissivity, the conductivity, the storage coefficient, the hydrological balance and the piezometric map were determined. Indeed, these parameters (the crack porosity, the transmissivity, the conductivity and the storage coefficient) confirm not only the heterogeneity of the medium but that the cracked horizon is sufficiently porous to be assimilated to an equivalent continuous medium during the simulation.

Prevalence and Factors Associated with Intestinal Metaplasia in Chronic Helicobacter pylori Gastritis in a Country with High Endemicity: Ivory Coast Case

Context/Objective: Few studies have been carried out in a country with high endemicity for Helicobacter pylori (H. pylori) infection in Sub-Saharan Africa looking for the association of intestinal metaplasia (IM) with chronic gastritis. We hypothesize that IM is correlated with the intensity of H. pylori infection in a country with high endemicity, Ivory Coast. The objective of this study was to determine the prevalence of intestinal metaplasia in chronic H. pylori gastritis in Ivory Coast. Methods: This was a prospective, cross-sectional, multicenter study, carried out over a period of 5 months, in the reference hospital centers of Abidjan, specialized in Gastroenterology. All patients who had undergone Gastroscopy with biopsies according to the criteria of the Sydney System for the anatomopathological study, those with chronic gastritis and/or H. pylori intestinal metaplasia on histology and in whom all the parameters of the Sydney system classification had been well informed. The quantitative variables were expressed by their means accompanied by their standard deviations and the qualitative variables by their numbers and percentages. Chi-square and Fischer tests were used to look for associations between variables. The significance level was set at 5%. Results: 152 patients were retained. The mean age was 44.9 ± 12.9 years. The prevalence of intestinal metaplasia was 11.8%. In univariate analysis, no significant association was found between clinical and pathological sociodemographic factors (age, sex, ethnicity, educational level, profession) and intestinal metaplasia in chronic Helicobacter pylori gastric cases. In multivariate analysis we found that prolonged use of Proton Pump Inhibitors (PPIs) and a history of Gastroesophageal Reflux Disease (GERD) were significantly associated with the absence of IM. Introduction: Neglected ankle fractures are common in our regions. Patients first consult traditional healers and it is when the signs persist that they consult the surgeon for a swollen and painful ankle. Initially we always performed anatomical repairs with osteosynthesis, but the majority of patients still complained of pain and disability. We wondered whether a primary arthrodesis would not be indicated in old ankle fractures to avoid disabling pain for patients. We therefore performed a series of arthrodesis on neglected post-traumatic ankles. The objective of our study was to describe the results of arthrodesis on an old post-traumatic ankle and to propose a therapeutic indication in the face of any neglected ankle fracture. Material and Method: This was a prospective study over 8 years between December 2012 and November 2020, involving 36 cases of neglected ankle fractures treated by arthrodesis using Meary’s technique. We used AOFAS (American Orthopedic Foot and Ankle Score) preoperatively and postoperatively for an objective assessment. Results: The average follow-up was 4.2 years and our results were satisfactory (94.4%) with an average AOFAS score ranging from 27.8 preoperatively to 76.7 postoperatively. The fusion rate was 97.2%. Discussion: Arthrodesis appears here as an important alternative in the treatment of neglected ankle fractures with significant trophic disorders.
