Systemic Scleroderma at University Teaching Hospital (UTH) of Cocody (Abidjan—Cote d’Ivoire): A 19 Cases Report

Objective: The aim of this study was to describe the epidemiological, clinical and therapeutic features of systemic scleroderma at Cocody UTH. Methodology: We conducted a retrospective and descriptive study over a period of 10 years (September 15, 2008 to April 15, 2019) on the files of patients hospitalized for systemic scleroderma in the rheumatology unit of the UTH of Cocody. We used the classification criteria of the American Society of Rheumatology (1980) to retain the diagnosis. Results: Nineteen patients’ files had been collected, representing a hospital frequency of 0.32%. The average age was 37.25 ± 13.82 years old. There were 15 women and 4 men. The average consultation time was 26.44 months. The mode of revelation of the disease was mostly cutaneous and articular. All patients had cutaneous sclerosis (average Rodnan score = 27.63/11.61 (min = 4, max = 49).) Scleroderma was diffuse in 70.59% of cases;a Raynaud’s phenomenon was seen in 47.37%. The main clinical manifestations were: cutaneous (100%), articular (89.47%), pulmonary (57.89%) and digestive (63.16%). No renal damage was found. Pulmonary fibrosis (5 cases), pulmonary arterial hypertension (3 cases) and pericardial effusion (2 cases) were sometimes founded in explorations. The positivity of antinuclear antibodies (ANA) was seen in 72% of patients and anti scl70 antibodies in 42.85%. The treatment included corticosteroids and immunosuppressants, respectively used in 84.2% and 63.16% of cases. The outcome was marked by 5 cases of death attributed to respiratory distress. Conclusion: Systemic scleroderma seems to be a very rare condition in Ivorian rheumatology milieu. The main systemic manifestations were digestive and pulmonary. Treatment was very often symptomatic sometimes associated with D-penicillamine.

Objective: To describe paraclinical, therapeutic and evolutionary aspects of osteoarticular tuberculosis in Abidjan. Materials and Method: We realized a descriptive retrospective study carried out in the Rheumatology Department of the University Hospital Center of Cocody in Abidjan about 550 patients over a period of 10 years from January 2004 to December 2013. The paraclinical, therapeutic and evolutionary data had interested us. Results: Osteoarticular tuberculosis represented 12.13% of 4531 rheumatic diseases identified during the study period. The tuberculin skin test was positive in 78.49% of cases. An inflammatory syndrome was present with erythrocyte sedimentation rate and C-reactive protein respectively into 69.45% and 76.54% of cases. The research of Mycobacterium tuberculosis was realized into 176 of cases which 50% was positive. Polymerase chain reaction and biopsy of tissue were performed respectively in 8 and 6 cases. Plain radiography (89.82%) and computed tomography (83.63%) were the most realized imaging. Computed tomography has contributed for the diagnostic in 99.78% of cases. The majority of our patients (82.91%) received anti-tuberculous treatment which consisted of two months of combination of rifampicin (R), isoniazid (H), pyrazinamide (Z) and Ethambutol (E) (2RHZE), followed by 10 months of rifampicin (R) and isoniazid (H) (10 RH). The outcome in patients on this treatment was favorable in 528 cases (96%). Conclusion: This study revealed that the research of Mycobacterium tuberculosis was often realized and positive. On the contrary, polymerase chain reaction and biopsy of tissue were rarely performed. The main imaging is computed tomography. Our protocol of treatment is effective.

Objective: To describe the epidemiological, clinical, paraclinical and therapeutic aspects of systemic juvenile idiopathic arthritis observed in Abidjan. Materials and Method: This retrospective and descriptive study covered 13 children suffering from systemic juvenile idiopathic arthritis selected in the Rheumatology Department of University Hospital Center of Cocody in Abidjan (Cote d’Ivoire) from January 2005 to December 2015. We were interested to the sociodemographical, clinical, paraclinical and therapeutic aspects. Results: The systemic form of the juvenile idiopathic arthritis represented 0.2% of the 4608 rheumatologic diseases and 70.58% of the JIA. We selected 6 boys and 7 girls, with an average age of 10.8 years and mostly going to school (84.61%). The diagnostic delay was 18 months. The main clinical signs were fever and joint damage observed each in 100% of cases, impaired general condition (92.30%) and tumor syndrome (83.33%). Biological signs were characterized by hyperleukocytosis (69.20%) and the presence of a biologic inflammatory syndrome (on average, erythrocyte sedimentation rate 59.6 mm and C Reactive Protein 56.4 mg/l). The cervical damage was the essential functional complication (38.46%). The major treatment has been a therapeutic combination based on corticotherapy and methotrexate (100%) with 1 death case by macrophage activation syndrome. Conclusion: Systemic juvenile idiopathic arthritis is rarely diagnosed in the rheumatologic practice in Abidjan. It concerns children relatively big, and is characterized by a febrile polyarthritis with impaired general condition and tumor syndrome. This systemic form is treated by corticotherapy and methotrexate.

Etiologies of Bone Metastases at the Rheumatology Department (UTH) of Abidjan

Objectives: The aim of this study was to describe the diagnostic and etiological aspects of bone metastases in the Rheumatology Department of Cocody’s University Teaching Hospital (UTH). Methodology: This was a descriptive, 11-year retrospective study (January 1, 2006 to December 31, 2016) of inpatient records of bone metastases. The diagnosis was made on clinical (bone signs), radiological (osteolysis, bone condensation) and sometimes histological basis. Result: Eighty out of 6, 1111 inpatients suffered from bone metastasis with a hospital frequency of 1.30%. The average age was 60.74 years (range 26 to 81 years). Men were predominant (53 men for 27 women) with a sex ratio of 1.96. The main complaints were pain (97.6%). chronic (90%), severe (73.8%), inflammatory (93.8%). There was sometimes a neurological complication: a motor deficit (21.3%), sensitive (13.8%). These symptoms were associated with fever (56.3%) and altered general state (85%). Bone metastases have been revealing in the vast majority of cases (93.75%);the primary cancer was known only in 5 patients (prostate = 2, breast = 2 and cervix = 1). Bone condensation (61.3%), osteolysis (50%) and mixed lesions (7.5%) where the main radiological lesions observed. The primary tumors were: prostatic (50%), pulmonary (18.8%), mammary (11.3%), uterine (5%), renal (2.5%), hepatic (2.5%), bladder (1.3%) and adrenal (1.3%). Conclusion: Bone metastases affect mostly the elderly;inflammatory spinal pain is the main symptom. Bone condensation is the most common radiologic lesion. The prostate, breast and lungs are the main primary tumors.

Detection of Cocoa Leaf Diseases Using the CNN-Based Feature Extractor and XGBOOST Classifier

Among all the plagues threatening cocoa cultivation in general, and particularly in West Africa, the swollen shoot viral disease is currently the most dangerous. The greatest challenge in the fight to eradicate this pandemic remains its early detection. Traditional methods of swollen shoot detection are mostly based on visual observations, leading to late detection and/or diagnostic errors. The use of machine learning algorithms is now an alternative for effective plant disease detection. It is therefore crucial to provide efficient solutions to farmers’ cooperatives. In our study, we built a database of healthy and diseased cocoa leaves. We then explored the power of feature extractors based on convolutional neural networks such as VGG 19, Inception V3, DenseNet 201, and a custom CNN, combining their strengths with the XGBOOST classifier. The results of our experiments showed that this fusion of methods with XGBOOST yielded highly promising scores, outperforming the results of algorithms using the sigmoid function. These results were further consolidated by the use of evaluation metrics such as accuracy, mean squared error, F score, recall, and Matthews’s correlation coefficient. The proposed approach, combining state of the art feature extractors and the XGBOOST classifier, offers an efficient and reliable solution for the early detection of swollen shoot. Its implementation could significantly assist West African cocoa farmers in combating this devastating disease and preserving their crops.

Effect of the seeds provenance and treatment on the germination rate and plants growth of four forest trees species of Cote d’Ivoire

The rapid rate of deforestation in Cote d’Ivoire has led to loss of plant species diversity and also threatens some commercial tree species with *** reforestation and aff orestation has been ***,for some species,the numbers of surviving transplants has ***,it is necessary to develop properly adapted and resilient genotypes that can eff ectively support the programs of forest *** this context we evaluated the eff ects of the provenances of seeds and of various treatments on the germination rates and on plant growth of four commercial trees species(Pycnanthus angolensis,Terminalia superba,Mansonia altissima and Pterygota macrocarpa).We analyzed these parameters for seeds of three provenances(Daloa,Akoupéand Daoukro)of four species and for thirty plants aged 6 months per *** to begin germination and time to reach maximum germination were similar among species,and were not infl uenced by either treatment or *** seeds and those soaked for 24 and 48 h in water at room temperature yielded the highest germination rates,irrespective of species or *** one exception was ***,for which no germination was *** species,some provenances produced higher germination percentages than did *** clear relationship was computed between the germination rate and the morphometric characteristics of the seeds which varied signifi cantly between *** growth and morphological variability also depended greatly on *** factors might be implicated,therefore provenance trials should be undertaken and ***,molecular analysis should be undertaken to confi rm the implications of genetic factors and allow for genetic selection.

Atmospheric dynamics and climatology in West Africa are strongly dominated by the latitudinal migration of the ITCZ which imposes directly or indirectly determinism on coastal ecosystems. Thus, the Grand-Lahou lagoon system and its watershed are influenced by the ITCZ, whose seasonal study made it possible to understand its mode of action first on climatic factors, then hydroclimatic, and finally hydrological in the context of global changes. The study of these factors showed a differentiated impact of its migration on the hydrological regime defining a new configuration of the lagoon system and beyond;the coastal ecosystems of the northern coast of the Gulf of Guinea irrigated by numerous fluvial inputs. Salinity, a hydrological parameter of critical ecosystem importance, combined with transparency and depth, has enabled the spatio-temporal description of hydrology. To do this, a series of monthly measurements (in-situ) for spatial coverage of 25 stations was tested. Long before that, the climatic and hydroclimatic parameters were obtained respectively at SODEXAM, the meteorological site *** and ONADE. This study brought together the dynamics of the ITCZ and the hydrological system of the lagoon.

Initial Treatment of Knee Dislocation at Yopougon Teaching Hospital/Abidjan, Ivory Coast

Knee dislocation is a serious and rare injury. Its rarity and the variety of injuries that result from it mean that there is no adequate management that is universally accepted. The aim of this study was to evaluate our emergency care strategy for these injuries. Materials and methodology: This was a retrospective study conducted from January 1992 to December 2004 on nine cases of knee dislocation. It consisted of six men and three women. The average age of these subjects was 35, ranging from 15 to 50 years. The causes for these injuries included: public road accidents (n = 4), household accidents (n = 3), a sporting accident and a dislocation that occurred following a fight. The dislocations were anteromedial (n = 4), posterolateral (n = 2), posterior (n = 2) and anterior (n = 1). Associated injuries were sore joint (n = 2), contusion of the common peroneal nerve (n = 1) and vascular injury (n = 1). An angiography was performed on one patient for a vascular injury and an Elecmyography (EMG) for a common peroneal nerve injury. Closed reduction was used for eight patients and open reduction for one patient. Results: Five knees were stable with normal range of motion and some residual pain. Four patients had knee instability;two anterior, one anteroposterior and one lateral. In three of these patients, the CT arthrography/arthro-scan confirmed central pivot damage, with meniscal damage in two of them. The four patients were referred to a knee surgeon in a private practice. The post-operational effects were minimal in vascular injury. The EMG performed for the common peroneal nerve detected a nerve contusion. Conclusion: Our emergency care strategy remains closed reduction. This therapeutic management is consistent, at least in the first 15 days, with the literature.

This study aims to examine the quality and quantity of the groundwater resources from hand-dug wells, within two of these slums—Anoumabo (Marcory) and Adjouffou (Port-Bouet), both located in the southern part of the city. Twenty-eight representative groundwater samples were collected from different domestic wells within the study area. In addition, water samples were collected from the adjoining surface water bodies—the ébrié lagoon and the Atlantic Ocean. The water samples were also tested for microbial indicators of fecal contamination using the conventional membrane filtration method. The groundwater samples are alkaline to acidic with pH ranging between 4.4 and 8.1. They are slightly mineralized with electrical conductivity, EC values ranging between 388 μS/cm and 1494 μS/cm. The dominant hydrochemical facies are Na-Cl, Na-SO4, Ca-Cl and Ca-SO4. Although, majority of the water samples have anions and cations concentrations conforming to the World Health Organization, alerting levels of nitrate contamination was recorded in the area. About 67 percent of the tested samples have nitrate values greater than the recommended WHO limit for drinking water (NO3 > 50 mg/ι). Exceeding high nitrate concentrations in drinking water have been medically proven to be detrimental to infant health. Microbial analyses reveal bacterial contamination at varying degrees in all of the water wells. The presence of these microbial organisms in the samples is also indicative of the presence of some other disease causing pathogens, responsible for sicknesses like cholera, diarrhea, typhoid, etc. The water wells located within Anoumabo have relatively higher levels of groundwater contaminants in comparison to those located within Adjouffou. This is obviously due to the poor well designs and prevalent unhygienic and poor sanitary habits of its inhabitants.
