Peri-Operative Morbidity and Mortality of Elderly Patients in Traumatology-Orthopedics in Bouaké

Introduction: Elderly subjects have physiological and clinical characteristics that make surgical management more complex and have a major impact on their prognosis. The primary objective of this study was to assess peri-operative hospital morbidity and mortality in the elderly. Secondary objectives were to identify factors predictive of death. Methods: This was a retrospective observational descriptive and analytical study carried out over 02 years, based on hospitalized patient records. It concerned patients aged 65 or over who had undergone surgery or for whom an indication for surgery had been given, all pathologies included. A total of 138 files were collected. The mean age was 74.15 years. Diabetes (n = 52;37.68%) and arterial hypertension (n = 38;27.54%) were the most common comorbidities. The reasons for hospitalization were mostly degenerative (n = 56;40.58%) and traumatic (n = 49;35.51%) pathologies. Preoperative delay, intraoperative and postoperative complications, hospital stay, evolution (mode of discharge), time of occurrence and presumed cause of death were studied. Univariate (p Results: The mean preoperative delay was 5.53 days (1 - 23). Intraoperative complications were mainly hemodynamic instability (n = 26, 18.8%). Postoperatively, the main complications were anemia (n = 62;48.44%) and glycemic imbalance (n = 28;21.86%). Average hospital stay was 13.4 days. Age (>75 years), the existence of comorbidities, degenerative pathologies, ASA score > 2, delay in treatment > 3 days, the occurrence of complications and the blood transfusion requirement were the independent predictors of the occurrence of death in the multifactorial analysis. Conclusion: Peri-operative morbidity in the elderly was influenced by age (>75 years), the existence of comorbidities, degenerative pathologies, ASA score > 2, time to treatment > 3 days, the occurrence of complications and the blood transfusion requirement.

Background and aim: Hepatocellular carcinoma (HCC) is one of the leading causes of death in Africa. Many inflammatory scores have been used to predict the prognosis of this disease in various situations. The aim of this study was to determine the influence of NLR, PLR and SII on the prognosis of advanced HCC in hospitalized patients. Materials and methods: It was a cross-sectional analytical study carried out in the Internal Medicine Department of Bouake University Hospital over a period of 3 years. Results: Out of 96 patients included in the study, we found a significant association between SII and CHILD score ≥ B. We also identified NLR as an independent predictor of hepatic encephalopathy. Conclusion: This study shows the relationship between inflammatory scores and hepatic insufficiency and the possibility of using NLR to predict in-hospital hepatic encephalopathy in patients suffering from advanced HCC.

Optimization of Technical Parameters for Detecting Mycobacteria in Hospital Wastewater in Tropical Urban Areas: The Case of the City of Abidjan (Côte d’Ivoire)

The loads of organic matter, microorganisms, detergents and antibiotics in liquid hospital effluents make them complex environments, raising numerous health and ecological questions. Investigations of mycobacteria in water lack adequate techniques. This study is the first part of a pilot project aimed at developing an optimized protocol for the isolation of mycobacteria from hospital effluents, as a prelude to more in-depth investigation in this matrix. The aim was to compare the performance of two decontamination methods, three culture media and two incubation temperatures generally proposed in the literature, in order to identify the most effective methods in each case, as well as possible areas for improvement in the isolation of these germs from this environmental matrix. The results show that liquid hospital effluent can be decontaminated using both the NaOH method (4%;for 30 min.) and the CPC method (0.05%;for 30 min.), with the same mycobacteria recovery efficiency. Despite the low concentration, decontamination with CPC killed more mycobacteria and sufficiently eliminated contaminating germs. In contrast, decontamination with NaOH was less harmful to mycobacteria, but did not remove many contaminating germs. On the other hand, LJG medium performed better than LJGF medium and LJGP medium for the growth of mycobacteria in hospital waters. Finally, there was no difference in performance between the two incubation temperatures of 30℃ and 37℃. The results of this study show that further evaluation of existing protocols is required in order to optimize methods for the pre-treatment of hospital effluent for the isolation of mycobacteria.

A systematic study was carried out to assess the level of contamination with fluorides and heavy metals in the drinking water of the city of Daloa as well as the risks to the health of consumers. The waters of 11.11% of the sites sampled exceeded the fluoride limit for drinking water with a contamination index (CI) greater than 0. All the waters recorded concentrations of cadmium (Cd), copper (Cu), iron (Fe), manganese (Mn) and lead (Pb) above the recommended values with CI > 0. However, 22.22% of the sites recorded concentrations below the standard for zinc (Zn) with IC 1 for children in 22.22% of drinking water studied. HQs have an average of less than 1 for fluorine and greater than 1 for all metals. Moreover, the danger indices have values greater than 1. The incremental lifetime cancer risk (ILCR) and the total ILCR are above the recommended values. These results showed that the drinking water sampled is of poor quality due to higher levels of heavy metals, which can constitute a danger to human health. Long-term use of one of these poor quality waters can lead to cancer in consumers. It is therefore necessary to treat this water in order to eliminate the metals before using it for drinking. This study can help decision-makers and competent authorities in charge of water management.

Morphological Diversity and Distribution of <i>Garcinia kola</i>Heckel (Clusiaceae) in Two Agro-Ecological Areas of Côte d’Ivoire

The purpose of this study is to determine the morphological diversity and distribution of Garcinia kola Heckel (Clusiaceae) in two preferential agro-ecological growth areas in Cote d’Ivoire, for the sustainable management of the species. Ninety-four (94) trees of G. kola were sampled in Affery (south) and Biankouma (west) and characterized on the basis of 13 quantitative characteristics of the fruit, the general appearance of a plant, leaves and seeds. This study revealed the existence of very significant variability and differences within the trees sampled for most characteristics. Very strong correlations were found among the characteristics of the fruits. On the basis of these characteristics, it emerged that the fruits of the Biankouma area are larger than those of Affery. Multivariate analyses show that diversity is structured with tree distribution into four distinct groups based on the following discriminating characteristics: fruit height (Hfr), leaf width (largF), seed mass (Mgr) and trunk diameter (Dm). These parameters can be used as a basis for selecting and maintaining the high variability of G. kola. Analysis of the geographical distribution of trees, based on the nearest neighbour model, revealed an aggregate distribution in both areas.

Acute Intestinal Invagination of the Child at the Teaching Hospital of Bouaké: Ultrasound Diagnosis and Clinical-Echo-Surgical Correlation

Background: Acute intestinal invagination or intussusception is the most common abdominal surgical emergency in infants, but it can occur at any age. Performing an ultrasound scan at the slightest clinical suspicion contributes to early diagnosis and therapeutic management. Objective: To show the relevance of ultrasound in the therapeutic management of the child’s intussusception through a correlation between the ultrasound diagnosis and the clinical and/or per operative diagnosis. Patients and Methods: It was a retrospective study of 24 cases from July 2017 to September 2020 (30 months) in the Medical Imaging and Paediatric Surgery departments of Teaching Hospital of Bouaké. We included only patients from 0 to 15 years old who had digestive symptoms, an abdominal ultrasound scan. These patients were eligible for surgery. Data analysis was performed with Epi info 7 software. Results: Median age was 17.2 months [02 - 120]. Male gender predominated (83.3%). Clinico-biological data were dominated by abdominal pain (79.2%), vomiting (75%) and rectorragies (33.3%), with Ombredanne’s triad in 33.3% and hyperleukocytosis (70.8%). Ultrasonography showed invaginated coves (95.8%) sitting in the right angl iliac fossa, peri-umbilical and right flank in 73.9%. Cockade image coupled with the sandwich image accounted for 95.6%. Superficial adenopathies (45.8%) and peri-lesional fluid effusion (20.8%) were associated with it. An occlusive syndrome complicating intussusception was observed in 41.67% associated with intra-peritoneal effusion of fluid (25%). There was a significant diagnostic agreement between the clinic, ultrasound and surgery according to the Fisher’s exact test (p = 0.002). That between ultrasound and surgery was calculated at 95.4%. The stem barks of Prunus africana are used in the treatment of the benign prostate. Cameroon is one of the important exporters of the barks. Despite the important measures adopted in Cameroon for sustaining its harvesting, some many chalenges still remain. The objective of this work is to refine the forest management parameters in relation to P. africana in the regions of Adamaoua and the South-West by developing a volume rate which makes it possible to estimate the production for a new stem. The work took place in two phases: in the South-West in 2010 and in Adamaoua in 2011. Data collection used the semi-direct method, while the cubing equation was deduced by the multiple linear regression method. Two models for volume estimation and three models for mass prediction were developed. The predictive parameters retained are diameter, height of the bole and thickness of the bark. Results show that the average mass of the dry bark for a given P. africana tree species is 27.55 ± 14.44 kg and this varies according to the site. The strong adjusted coefficient of determination (adjusted R2) observed illustrates the reliability of the proposed models. These models provide a reliable tool that can be adopted as a standard in Cameroon for P. africana.

Dissemination of Resistance Integrons and Genes Coding for Blse and Cabapenemases in the Urban Drainage Network in Cote d’Ivoire

Antibiotic resistance has become a major threat to human health worldwide. Environment, particularly the water environment, has long been overlooked as a player in the antibiotic resistance cycle, although its role remains unclear. These can provide an ideal setting for the acquisition and dissemination of antibiotic resistance, as they are frequently affected by anthropogenic activities. The objective of this study was to establish a diffusion map of resistance integrons used as genetic markers of resistance associated with antibiotic resistance conferring genes (ARGs). Total DNA extracts from non-cultivable bacterial communities were used for the analyses. These communities were obtained from wastewater samples from 14 sites upstream and downstream of drainage channels or effluents in the cities of Abidjan, Bouaké, and Yamoussoukro. The results obtained correspond to the number of positives among the treated samples (n = 39). Among the genetic markers of dissemination, class 1 integrons were the most evident in 94.8% of samples in Abidjan (93.3%), Bouaké (100%) and Yamoussoukro (91.6%). Class 2 integrons and class 3 integrons were found respectively in 41% and 51% of all samples. Genes coding for β-lactamases and blaTEM was identified in almost all samples at a rate of 97.4%. A co-presence of the three genes blaTEM, blaSHV and blaCTX-M is also remarkable in the sites of the city of Yamoussoukro. Among the genes coding for carbapenemases, only blaKPC 17.94%, blaNDM 30.76% and blaOXA48 38.46% were detected in the samples.

Measurement of Atmospheric Black Carbon Concentration in Rural and Urban Environments: Cases of Lamto and Abidjan

Black carbon is one of the primary aerosols directly emitted from biomass known to have strong absorbing properties. The INDAAF and PASMU observational field campaigns which took place (2018) in Abidjan (urban area) and Lamto (rural area) allow the analysis of Black carbon concentration at different time scales through real-time measurements using an analyzer named Aethalometer AE-33. Results presented here show at Lamto: 1) for the diurnal scale an average of 1.71 ± 0.3 μg⋅m-3 (0.34 ± 0.09 μg⋅m-3) in the dry (wet) season;2) for the monthly scale an average of 1.14 ± 0.84 μg⋅m-3;3) on the seasonal scale, an average of 2.2 ± 0.02 μg⋅m-3 (0.6 ± 0.19 μg⋅m-3) in the dry (wet) season. The black carbon variation at Lamto is seasonal with an amplification factor of 85.6. Regarding the urban area of Abidjan, due to sampling issues, our analyses were limited to daily, diurnal and weekly time scales. We observed: a) at a daily scale an average of 5.31.± 2.5 μg⋅m-3, b) diurnal scale, an average ranging from 6.87 to 13.92 μg⋅m-3. The analysis indicated that emissions from urban areas are more related to social and economic activities, with weekday concentrations (7.24 μg⋅m-3) higher than concentrations over the weekend (e.g. Saturday 6.59 μg⋅m-3 and Sunday 6.00 μg⋅m-3). Moreover, BC concentration in Abidjan is quite noticeable compared to that of rural areas (Lamto). The ratio between the maximum values of the two areas is of the order of 5.86. In addition, concentrations in some urban areas are slightly above the daily threshold set by the WHO (10 μg⋅m-3). Therefore, the levels of urban BC concentrations are alarming whilst rural BC concentrations remain below daily WHO thresholds and are of the same magnitude as those of West African megacities.
