Assessm.nt of the Efficacy of Rifaxim.n in the m.nagem.nt of Irritable Bowel Syndrom. (IBS)

The aim.of our study was to assess in our context, the efficacy of Rifaxim.in im.roving the sym.tom. of irritable bowel syndrom. particularly in its diarrheal (IBS-D) or m.xed (IBS-DC) com.onent and therefore assess its im.act on patients’ quality of life. Patients and m.thods: This was an uncontrolled, non-com.arative prospective cohort study of a single group of patients. Patients recruitm.nt was done in two University Hospitals for 6 m.nths (Septem.er 2015-February 2016). Were included am.ulatory patients, m.le or fem.le, aged 18 - 75 years, with diarrheic IBS (IBS-D) or m.xed IBS (IBS-DC) diagnosed according to Rom. III criteria and who agreed to participate in the study. Each patient received 400m. Rifaxim.n × 2/d for 15 days. The overall assessm.nt of the efficacy of treatm.nt at D15 (end of treatm.nt) and D30 (2 weeks post-treatm.nt) was the prim.ry criterion of judgm.nt. The statistical tests used were the Chi-square test and Fisher’s exact test for the qualitative variables and Student’s test for the quantitative variables. Results: A total num.er of 30 patients (16 wom.n) with an average age of 44.5 ± 13.9 years were included. The overall assessm.nt of sym.tom. by the patient with the Likert scale found 28 (93.3%) patients, 12 (40%) patients and 10 patients (33.3%) for a scale ≥2 at D0, D15 and D30 respectively. The assessm.nt of the intensity of pain or abdom.nal discom.ort found at D0, D15 and D30 respectively: 27(90%), 13(43.4%) and 6(20.1%) patients who had an EVA score > 2. The m.an score for Francis to assess the im.roved quality of life was 247.1 ± 97.4 at D0, 99.8 ± 63.1 at D15 and 128.8 ± 70.6 at D30. Conclusion: There is a good overall im.rovem.nt of sym.tom. in our patients suffering from.IBS-D or m.xed (IBS-DC) on Rifaxim.n with im.rovem.nt of the quality of life.

Virological profile of hepatitis B infection in Black African HBsAg chronic carriers

Aim.: 1) Describe virological profile of patients followed-up for chronic Hepatitis B virus (HBV);2) Search for a correlation between cirrhosis and virological profile of patients. Patients and m.thods: Retrospective study about 75 HBsAg positive patients followed-up for at least one year in two m.dical structures of Abidjan. Studied param.ters: clinical signs, biological check-up (serum.transam.nases every 3 m.nths for at least one year, platelets count and prothrom.in rate), abdom.nal echography, virological check-up (HBsAg, HBeAg, anti-HBe, total anti-HBc, anti-VHC and anti- HIV Ab, HBV DNA biannual quantification during at least one year). Histological or biochem.cal evaluation of hepatic activity and fibrosis were realized in case of transam.nases elevation or HBV DNA > 2000 IU/m.. Results: The m.an age of our 75 patients (54 m.n) was 42.1 ± 11.54 years. HBV was fortuitously discovered in m.st of our patients (74.7% of the cases). The HBV inactive chronic carriage was 50.7%;HBeAg-positive and HBeAg-negative chronic hepatitis represented respectively 9.3% and 40% of the cases. m.an B viral load was 327.5 IU/m. in HBV inactive chronic carriers, 44,047,663 IU/m. in HBeAg-positive chronic HBV and 20,231,822 IU/m. in HBeAg-negative chronic HBV. Cirrhosis prevalence was significantly high- er in positive or negative HBeAg chronic HBV than in HBV inactive chronic carriers (32.4% vs. 5.3%, p = 0.008;OR = 8.6). Conclusion: Our patients’ virological profile was dom.nated by HBeAg-negative chronic HBV and HBV inactive chronic carriage. The risk of having cirrhosis was m.ltiplied by 8.6 in case of active chronic hepatitis com.ared with HBV inactive chronic carriage.

Diagnostic accuracy of biochem.cal m.rkers of fibrosis in black African patients with chronic hepatitis B

Contradictory results of the accuracy of bio-chem.cal m.rkers to predict the stage of fibrosis in black African patients with chronic hepatitis B (CHB) were previously published. We con-ducted a prospective cohort study to determ.ne the diagnostic accuracy of aspartate am.-notransferase to platelet ratio (APRI), aspartate am.notransferase to alanine am.notransferase ratio (AAR), platelet count, age-platelet (AP) in-dex, and FIB-4 index for the prediction of sig-nificant fibrosis or cirrhosis in 117 black African patients (m.dian age: 38 years, m.les: 73 %) with CHB not previously treated. Am.ng them. 45 had significant fibrosis and 18 had cirrhosis using the m.TAVIR score system. Factors as-sociated either with significant fibrosis or cir-rhosis were determ.ned in logistic m.ltivariate analysis. Areas under receiver operating curve were assessed and com.ared for APRI, AAR, AP index, FIB-4 index and platelet count. Sensitivity, specificity, positive and negative predictive values were determ.ned for each biochem.cal m.rkers. m.ltivariate analysis showed that as-partate am.notransferase (p 1.1) and FIB-4 index (cut-off > 2.1) ruled out significant fibrosis with high specific-ity of 84.7 % and 86.1 % respectively and nega-tive predictive values of 78.2 % and 72.9 % re-spectively. m.re accurately, APRI (cut-off > 0.63) or FIB-4 index (cut-off > 1.26) ruled out cirrhosis with high sensitivity of 94.4% and 88.9% and high negative predictive values of 98.1% and 96.3% respectively. In conclusion, APRI and FIB-4 index are sim.le readily available m.rkers to exclude significant fibrosis or m.re accu-rately cirrhosis in black African patients with CHB.

Ultrasound Diam.ter of the Portal Vein to Healthy Adult in Parakou (Benin)

Introduction: The portal vein (PVD) diam.ter is one of the predictive variables of portal hypertension. This diam.ter can differ according to the population. The objective of this study was to determ.ne the PVD in a healthy adult population in Parakou using ultrasonography. m.thods: A cross-sectional descriptive study was conducted in the m.dical im.ging departm.nt of the Borgou and Alibori University-affiliated Hospitals in Parakou from.July 1st to October 1st, 2019. The study population consisted of 201 healthy adults. The ultrasound diam.ter of the portal vein was m.asured at the level of the hepatic hilum.on a cross-sectional epigastric view and on an oblique intercostal view. An association was sought between the PVD and sociodem.graphic and anthropom.tric factors. Results: The m.an age was 28.97 ± 10.56 years and the sex ratio (m.F) was 0.79. The m.an PVD in regular breathing (FB) was 9.83 ± 0.95 m. with a 95% confidence interval of [9.70;9.97], a 95th percentile of 11.49 m. with extrem.s of 7.50 m. and 12.23 m.. PVD was correlated with age, sex, weight, height and abdom.nal girth. After m.ltiple linear regressions (r = 0.19), there was correlation between the PVD in norm.l breathing with age (p = 0.0090), weight (p = 0.0026), body m.ss index (p = 0.0171) and body surface (p = 0.004). Conclusion: A PVD greater in norm.l adults is less than 12 m. and seem. to be correlated to the subject age, weight, body m.ss index and body surface. A m.re than 13 m. PVD should lead to the suspicion of portal vein hypertension in adults in our populations.

Background: Hp infection is the m.st com.on chronic bacterial infection in developing countries and is contracted especially in childhood where it rem.ins silent. Because of its involvem.nt in the genesis of certain cancers, the WHO has classified Hp in the category of carcinogen class I. The aim.of this study was to determ.ne the prevalence of Hp infection am.ng children from.a District in Northern Benin, and to identify the factors associated with it. Patients and m.thods: This scientific investigation is a cross-sectional, descriptive, and analytical study based on a prospective collection of data carried out from.July to Septem.er 2018. Recruitm.nt was probabilistic;it was based on the WHO cluster sam.ling technique im.lem.nted am.ng children aged 3 to 10 years without recent history of antibiotic treatm.nt and proton pum. inhibitors. Hp infection was diagnosed during the search for bacteria antigen in the feces through a quick Elisa test which proved positive. Results: We included 250 children in the study. Am.ng them. 151 (60.4%) had Hp infection. 79 (52.3%) out of the 151 infected children were fem.le, i.e. a sex ratio of 0.91. m.an age for those infected children was 6 ± 2.3 years. Am.ng the 151 children infected with Hp, 98 (64.9%) were asym.tom.tic. Factors associated with that infection were: children’s age > 5 years (p = 0.0461), use of contam.nated drinking water (p = 0.0001), m.als away from.hom. (p = 0.0039), m.thers’ low educational status (p = 0.0137) and low m.nthly incom. (p = 0.0116) as well as household size > 3 (p = 0.0002). Conclusion: Hp infection is com.on am.ng children aged 3 to 10 years in Northern Benin. Often asym.tom.tic, it is facilitated and exacerbated by unsanitary conditions and low socioeconom.c status.

Predictors of Direct-Acting Antivirals Failure in Patients with Chronic Hepatitis C Virus in Ivory Coast: About 5 Cases

Context and Aim. Direct-acting antivirals are the therapeutic revolution in the m.nagem.nt of viral hepatitis C, but often with a few cases of failure. The aim.of this study was to identify factors that m.y be im.licated in the failure of direct-acting antiviral therapy in patients with hepatitis C virus in Ivory Coast. m.thodology: This was a cross-sectional and descriptive study with a retrospective com.endium.of data from.the year 2016 to the year 2018. This study included all patients with chronic viral hepatitis C, patients naive to antiviral treatm.nt, or pre-treated patients in whom.a failure to treatm.nt with direct-acting antiviral has been diagnosed. Results: During the study period, 5 patients out of 14 cases of therapeutic failure were included, i.e. 35.71%. m.st of the patients were over 50 years (4 out of 5) and predom.nantly fem.le (3 out of 5). High blood pressure was the m.st com.on com.rbidity (3 out of 5 patients). Genotype 1 was found in 4 patients versus 1 case of genotype 2. None of the patients had hepatitis B or HIV coinfection. A viral load > 6log was found in 4 patients at baseline. All our patients had hepatic cytolysis. Two of them.were cirrhotic. All 4 cases of genotype 1 benefited from.the com.ination sofosbuvir + ledipasvir for 12 weeks and the case of genotype 2 from.the com.ination sofosbuvir + ribavirin for 24 weeks. The resistance m.tations were observed m.inly on the sequencing of NS5A at the Y93H, L31m. 28L, 30T, 31V, 58S and 93H genes and a case of m.tation on the N3 gene sequencing. Conclusion: Age > 50 years, com.rbidities (high blood pressure), polym.dication, fem.le gender, genotype 1b, virem.a > 6log, and cytolysis were the profile of patients with treatm.nt of HCV by direct-acting antiviral failure.

In Western countries, the current trend is to use sequential quadruple therapy or bism.th-based instead of triple therapy for the eradication of Helicobacter pylori (H. pylori). In sub-Saharan Africa, high prevalence area of the H. pylori infection, the effectiveness of these triple therapies widely used in routine has been little evaluated. The purpose of this study was to evaluate and com.are the effectiveness of three patterns of first-line triple therapy based on com.ining a proton pum. inhibitors (PPI), and 3 types of antibiotics: om.prazole (O), am.xicillin (A), clarythrom.cin (C) and m.tronidazole (m.. Patients and m.thods: This is a random.zed clinical trial opened on 3 parallel arm.: OAm.(group 1 or G1), OAC (group 2 or G2) or OCm.(group 3 or G3). The prim.ry endpoint was H. pylori eradication rate after seven days triple therapy. H. pylori diagnosis infection was based on bacterium.detection on the histological exam.nation of the gastric biopsies. Histological control was perform.d 4 weeks after the end of treatm.nt to assess H. pylori eradication rate. Results: The average age of our 153 patients included in the study (86 m.n) was 44.33 ± 11.72 years. The m.in reason of the endoscopy was the dyspeptic syndrom. (75.16%). The gastroscopy was norm.l in 28.76%. A Gastric or duodenal peptic ulcer was found in 17% of cases and gastropathy in 45.75%. Histologically, the GC was active in 90.9% of cases, follicular in 35.3% of cases, atrophic in 22.5% of cases and was associated with intestinal m.taplasia (Im. in 5.2% of cases. Patients of these three groups (n = 64 for G1, n = 56 for G2 and n = 33 for G3) were com.arable for age, gender, endoscopy indications, alcohol consum.tion history or sm.king, and anti-inflam.atory drugs taking. Approxim.tely 23% of patients experienced adverse reactions. The overall H. pylori eradication rate was 22.3%.
