Qualitative Assessment and Typology of the Water Resource Used for the Production of Drinking Water in Duékoué, Western Côte d’Ivoire

The objective of this study was to evaluate the typology of water through physicochemical and bacteriological characterization with the aim of highlighting its vulnerability as a drinking water resource. The methodology adopted was to evaluate the level of pollution by monitoring the spatiotemporal variation of the water pollution indicator levels of Lake Dohou. The sampling frequency is monthly for one year, from November 2017 to October 2018, in order to obtain a fairly representative image of water quality and its seasonal evolution in eight (8) well-defined stations. The parameters were determined using the standard methods defined by the French Association for Standardization (AFNOR). This study shows that the waters are acidic, with an average pH of 5.81 and a low electrical conductivity of between 42.67 ± 4.30 and 59.62 ± 21.84 μ***-1. At all stations, seasonal mean water transparencies are low (<1 m). It is also noted that 99.7% of the water samples collected had total nitrogen (TN) levels above the limit of 4 mg.L-1. All of the lake’s waters have non-compliant BOD5 (2.L-1) for raw water intended for the production of drinking water. Sites D6 and D7 have COD/BOD5 ratio greater than 3, which indicates the presence of non-biodegradable organic matter in these areas. Total coliforms, Escherichia coli and Enterococci were present in 100% (28/28) water samples at concentrations ranging from 2300 to 173,000 CFU/100 mL, from 100 to 1650 CFU/100 mL and from 20 to 1140 CFU 1/100 mL respectively. For Salmonella pathogens, they were detected in 50% of the dry season samples and in 100% of the rainy season samples. This almost permanent presence of this pathogenic germ denotes a poor quality of water with reference to this parameter. Assessment of Sediment and Pollutants in Buyo Lake, Ivory Coast, Using SWAT （Soil and Water Assessment Tool） Model

The changes in land use in the last 30 years in the territory of agro-forest watershed of Lake Buyo resulted in significant sediment into the lake. Sediments are a preferred means of transportation for certain pollutants, like phosphorus in excess. By mapping the source areas of erosion, the authors can determine the risk areas and help to prioritize interventions in the territory. This mapping is done using the SWAT （soil and water assessment tool） model. Several types of data, including topography, land use, soil and climate data are needed to run the model. In this paper, all different steps are presented, from the designing of HRU （hydrological response units）, basic units to run the SWAT model until the simulations. The establishment of HRU has three main stages： space discretization, land use and soil data integration and HRU distribution：（1） space discretization which consist in extracting the limits and the water network of the watershed from the DEM （digital elevation model） and in subdividing them into sub-basins; （2） land use and soil data integration： it consists in digitizing the physical maps of land use and of soils under Mapinfo 7.5 and in keeping them in ＂shape＂ format; （3） HRU distribution： it leads to subdivide the sub-watersheds in small units that combine a single soil type and one type of land use. It appears from this study to obtain 23 sub-watersheds and 71 HRU. Once the HRU designed, it is necessary to integrate climate data, data on physico-chemical characteristics of soils and agricultural practices, before starting the simulations. This will allow the model to assess the risk of sedimentation and eutrophication of the lake using the MUSLE （modified universal soil loss equation） and phosphorus cycle.

Endometriosis Revealed by Recurrent Hemorrhagic Ascites

Ascites is a rare manifestation of endometriosis. The case reported is that of a 26-year-old woman with recurrent massive hemorrhagic ascites. The analysis of biopsy specimens made at laparoscopy confirmed the diagnosis. The report noted pelvic endometriosis associated but there was no umbilical or pleural involvement. The evolution was favorable under a long-term hormone treatment (by GnRH analogues) with relay through continuous estrogen-progestogen. The diagnostic and therapeutic difficulties of this rare form of endometriosis are presented through a review of the literature.

Initial Treatment of Knee Dislocation at Yopougon Teaching Hospital/Abidjan, Ivory Coast

Knee dislocation is a serious and rare injury. Its rarity and the variety of injuries that result from it mean that there is no adequate management that is universally accepted. The aim of this study was to evaluate our emergency care strategy for these injuries. Materials and methodology: This was a retrospective study conducted from January 1992 to December 2004 on nine cases of knee dislocation. It consisted of six men and three women. The average age of these subjects was 35, ranging from 15 to 50 years. The causes for these injuries included: public road accidents (n = 4), household accidents (n = 3), a sporting accident and a dislocation that occurred following a fight. The dislocations were anteromedial (n = 4), posterolateral (n = 2), posterior (n = 2) and anterior (n = 1). Associated injuries were sore joint (n = 2), contusion of the common peroneal nerve (n = 1) and vascular injury (n = 1). An angiography was performed on one patient for a vascular injury and an Elecmyography (EMG) for a common peroneal nerve injury. Closed reduction was used for eight patients and open reduction for one patient. Results: Five knees were stable with normal range of motion and some residual pain. Four patients had knee instability;two anterior, one anteroposterior and one lateral. In three of these patients, the CT arthrography/arthro-scan confirmed central pivot damage, with meniscal damage in two of them. The four patients were referred to a knee surgeon in a private practice. The post-operational effects were minimal in vascular injury. The EMG performed for the common peroneal nerve detected a nerve contusion. Conclusion: Our emergency care strategy remains closed reduction. This therapeutic management is consistent, at least in the first 15 days, with the literature.

First Emergency Hemodialysis Session at the Nephrology Department of the Teaching Hospital of Yopougon: About 146 Cases

Context: Due to the late referral of the pads to the nephrologist and the lack of medical follow-up, many patients are admitted with complications from kidney disease requiring the urgent start of hemodialysis sessions. Purpose: Study the profile of emergency hemodialysis patients in order to ease their management. Methods: This was a retrospective, descriptive and analytical study carried out at the Nephrology Department of the Teaching Hospital of Yopougon from January 1st to December 31st, 2016. This study included all patients who had received a first session of hemodialysis in an emergency context. Results: We collected 146 patients with an average age of 39.80 ± 14.55 years and a sex ratio of 0.6 for men. Before admission, patients were known as hypertensives (63.9%), followed by CKD (23.9%) and HIV-infected (8.2%). The main clinical signs were hypertension (64.3%), edema (44.5%) and coma (30.1%). Anemia was observed in 97.2% of cases and it was less than 8 g/dl in 57.5%. Kidney failure was chronic in 75.3% and acute in 24.6%. Chronic nephropathies were glomerular (54.1%), vascular (20.5%). The main indications for hemodialysis were encephalopathy (33.5%), severe uremia (28%), acute pulmonary edema (19.8%), persistent anuria (11.6%) and hyperkalemia (5.4%). The vascular approach was a catheter in 97.2% (femoral site in 53.4% and jugular in 43.8% and arteriovenous fistula in 2.7%). Death was observed in 17.8%. In univariate analysis, age > 65 years (p = 0.04), coma (p = 0.004) and acute kidney failure (p = 0.02) were associated with the risk of death, and in multivariate analysis, only coma (p = 0.024, OR = 5.88) seemed to be associated with mortality risk for our patients. Conclusion: Hemodialysis in an emergency situation is a common practice in the Teaching Hospital of Yopougon and mainly concerns patients with CKD.

Results of the Management of Urogenital Fistulae from Community Caravans

Objective: To report the experience of surgical caravans for urogenital fistulas care. Methodology: This was a retrospective study covering the period from January 2014 to December 2014 and which took place on 7 sites of fistula care. The epidemiological, anatomoclinical, therapeutic and evolutionary aspects were studied. Results: 346 patients were operated during 14 caravans. Their average age was 33.11 years (range: 12 to 70 years). Most of these patients were without remunerative activities (80%) and without education (63.3%). The average duration of fistula progression was 6.08 years (range: 0 to 42 years). Obstetrical etiology was predominant (87.9%). According to the classification of Kees Waaldijk, fistulas were divided into type I (67.4%), type II (21.1%), and type III (11.5%). The most common surgical approach was the transvaginal route (82.1%). The vesicovaginal splitting with separated suture of the bladder and the vagina was the basic technique (94.7%). The therapeutic results were judged after a follow-up of 1 month and 3 months. Across the cohort, 80 patients (23.1%) were lost of sight for follow-up at 1 month and 245 (70%) at 3 months. The success rates evaluated in patients reviewed at 1 month and 3 months were respectively 70% and 64%. Conclusion: The incidence of urogenital fistulas is still high in Cote d'Ivoire. Various anatomoclinical varieties have been identified and treated with satisfactory results in poorly equipped local structures.

Major Limb Amputation Secondary to Locally Advanced Cutaneous Squamous Cell Carcinoma in Tropical Environments: Determinants

Objective: To identify the determinants of secondary limb amputation in advanced squamous cell carcinoma. Material and Methods: This was a retrospective study carried out in the Orthopedic Traumatology Department of the Bouaké University Hospital in Côte d’Ivoire from January 2013 to December 2016. It involved ten patients with locally advanced skin squamous cell carcinoma, confirmed by histology and having amputated limbs. The parameters studied were: demographic data (gender, age), socio-economic conditions (occupation, place of residence), the risk factors involved, the use of topical self-medication and the use of traditional medicine, antecedents and comorbidities factors, lifestyle, clinical aspects (seat, size), extension assessment (X-ray), anatomo-pathological examination data, duration of evolution of squamous cell carcinoma, the function of the affected limb, the treatment performed, the evolutionary modalities and the equipment. Results: Between 2013 and 2016, ten patients underwent either upper or lower limb amputations following locally advanced squamous cell carcinoma. The average age was 34.1 years (19 - 64 years). There were 7 men (70%) and 3 women (30%) and all our patients were black (sub-Saharan Africa). The majority of our patients were farmers (70%). The risk factors for squamous cell carcinoma were neglected chronic wounds. All our patients recognized during the interrogation a large use of the topicals of traditional medicine for the treatment of the initial cutaneous lesions. In terms of lifestyle, five (50%) were smokers and alcoholics. The mean time to progression of squamous cell carcinoma was 5.9 years. Lesions were localized preferentially to the pelvic limbs six (60%) cases and four (40%) to the limbs chest. The lesion was diagnosed late in all our patients. The size of the tumor in all our patients was greater than 5 cm and had a deep invasion (nerves, vessels and bone). The proximal ipsilateral ganglionic invasion was constant.

COVID-19 in Bouake (Côte D’Ivoire): Comparative Study of the Epidemiological, Clinical, Therapeutic, Evolutionary and Prognostic Aspects in Younger and Elderly Patients

Introduction: The clinical manifestations of many diseases vary with age, and older people often do not show typical symptoms of the disease. The present study aims to compare the epidemiological, clinical, therapeutic and evolutionary aspects of elderly patients with COVID-19 compared to young adult patients and to identify risk factors for mortality. Patients and Methods: This was a retrospective single-center analytical study conducted from January 27, 2021 to January 27, 2022 at the COVID-19 Care Center of the Infectious and Tropical Diseases Department of the Bouaké University Teaching Hospital. The study population consisted of all patients aged at least 18 years seen in consultation and/or hospitalized with a positive COVID-19 RT-PCR. The patients were divided into two groups: younger patients (Results: Of a total of 779 patients, 644 (82.7%) were young. The median age of all patients was 41 years (IQR 32 - 54, extreme 18 and 96). Of all the patients 38.5% had a comorbidity. Compared to younger patients, chronic heart disease (61.7% vs 21.5%;p Conclusion: Elderly people with COVID-19 have a different clinical presentation from younger, characterized by more atypical symptoms. Mortality risk factors are age, dyspnoea, impaired consciousness and fever. It is therefore necessary to act on its various factors to improve the prognosis of COVID-19 in this age group.

Looking of finding an alternative to the use of chemical fertilizers to increase yields of cassava (Manihot esculenta), trials were carried out in the Lamto zone in central Côte d’Ivoire. Thus, the effects of compost and ash from cocoa shell and NPK were tested on the agronomic parameters of cassava. The trial was conducted for two years with four varieties of cassava: Yacé, Alleda agba, Six mois and Bonoua, grown on elementary plots treated with one of these fertilizers. The experimental design was complete randomized blocks with three replicates. Analysis of the results showed that the Six mois variety gave the longest stems (131, 67 cm) with the cocoa shell compost. The Bonoua variety, on the other hand, gave the largest diameter of the stem base (21.56 mm), a higher number of leaves (77.30) and a large wingspan (136.

Introduction: The prevalence of Non-communicable diseases (NCDs) has increased so it’s becoming a global public health problem. This problem is also threatening in Sub-Sahara Africa (SSA) country including Ivory Coast. Ivory Coast is a country of 22.6 million people experiencing rapid economic development and social change. All development is typically associated with an increase in non-communicable disease (NCD) risk factors. Our study aimed to determine the prevalence and associated factors of the major risk factors of NCDs among the population of Cocody, Abobo and Yocoboué in the Ivory Coast. Methods: A descriptive and analytical cross-sectional study was conducted and involved 1146 adults of 19 to 60 years from general population in Cocody (urban), Abob (peri-urban) and Yocoboué (rural). The WHO STEPS risk factor survey has been changed a bit and was administered. it contains anthropometric and biochemical measures. Results: The prevalence of NCD risk factors was markedly different across the three sites particularly between urban (Cocody) and rural (Yocoboué) areas. The overall prevalence estimates of the risk factors were 15.2% for current smoking, 57.71% for harmful use of alcohol, 58.84% for low physical activity, 24.71% for sedentariness (sitting at least 7 h), 69.45% for skipping breakfast, 61% for having late dinner and 36% for snacking.
