Kawasaki disease without changes in inflammatory biomarkers:A case report

BACKGROUND Kawasaki disease(KD)is diagnosed based on clinical *** tests and other tests are auxiliary diagnostic *** KD is a disease caused by arterial inflammation,many patients with KD have elevated levels of inflammatory biomarkers,such as C-reactive protein(CRP),erythrocyte sedimentation rate(ESR),and serum amyloid A protein(SAA)in blood *** report our experience of a patient with KD who did not have elevated levels of inflammatory *** SUMMARY A 1-year-old boy presented with a 3-day history of *** of the six symptoms of KD were observed,except for changes in the lips and oral *** tests revealed no elevation in CRP,ESR,or SAA *** the blood test results were atypical,the patient was diagnosed with KD based on clinical symptoms and was admitted to the hospital for *** patient was administered intravenous immunoglobulin(IVIG)and *** commencing treatment,the fever persisted;therefore,additional IVIG was administered,the dosage of aspirin was increased,and ulinastatin was *** doses of IVIG were administered and the fever resolved on day 11 of KD symptoms *** tests performed during hospitalization showed normal levels of inflammatory *** examined leucine-rich alpha-2-glycoprotein 1-a protein that is elevated during the acute phase of *** protein levels did not increase during *** This case suggests the need to identify criteria and biomarkers for detecting KD conditions that do not require KD treatment.

Recording the direct surgeon’s view with an operating microscopic view improves microscopic ophthalmic surgery training

Isolation, Identification and Tyrosinase Inhibitory Activities of the Extractives from Allamanda cathartica

Tyrosinase inhibitory activity of the extractives from A. cathartica was examined and their new bioactivity and potent active compounds were identified. Five compounds, glabridin, new lignan, kaempferol, naringenin, and allamandicin, were isolated by a series of chromatography, and identified by NMR and LC-MS. Among them, glabridin had the high-est tyrosinase inhibitory activity (IC50:2.93 μM) which is 15 times stronger than that of kojic acid used as positive con-trol (IC50:43.7 μM). Moreover the lignan was indentified as 1-[3-(4-allyl-2,6-dimethoxyphenoxy)-4-methoxyphenyl] propane-1,2,diol which was a novel lignan.

Spatial evolution of the near-field distribution on planar gold nanoparticles with the excitation wavelength across dipole and quadrupole modes

We investigate the superposition properties of the dipole and quadrupole plasmon modes in the near field both experimentally, by using photoemission electron microscopy(PEEM), and theoretically. In particular, the asymmetric near-field distributions on gold(Au) nanodisks and nanoblocks under oblique incidence with different polarizations are investigated in detail. The results of PEEM measurements show that the evolutions of the asymmetric near-field distributions are different between the excitation with s-polarized and p-polarized *** experimental results can be reproduced very well by numerical simulations and interpreted as the superposition of the dipole and quadrupole modes with the help of analytic calculations. Moreover, we hypothesize that the electrons collected by PEEM are mainly from the plasmonic hot spots located at the plane in the interface between the Au particles and the substrate in the PEEM experiments.

Revealing the plasmon coupling in gold nanochains directly from the near field

We studied the near-field properties of localized surface plasmon resonances in finite linear gold nanochains using photoemission electron microscopy(PEEM).The localization of the electromagnetic field in the near-field region was mapped at high spatial *** tuning the excitation laser wavelength,we can obtain the near-field spectra,from which the energy splitting between longitudinal(L)and transverse(T)plasmon modes can be *** particular,the L-mode red shifts and the T-mode blue shifts with increasing chain *** red shift of the L-mode is highly dependent on the gap *** contrast,the T-mode almost remains constant within the range of gap distance we *** energy splitting between the L-mode and the T-mode of metallic chains is in agreement with previous far-field measurements,where it was explained by dipole-dipole near-field ***,we provide direct proof of this near-field plasmon coupling in nanochains via the above-described near-field measurements using *** addition,we explore the energy transport along the gold nanochains under excitation at oblique illumination via PEEM measurements together with numerical simulations.

Doublecortin and CaM kinase-like-1 as an independent prognostic factor in patients with resected pancreatic carcinoma

To elucidate the effect of expression of doublecortin and CaM kinase-like-1 (DCLK1) in patients with pancreatic ductal adenocarcinoma (PDAC). METHODSTumor specimens were obtained from 136 patients with pancreatic cancer who had undergone resection without preoperative therapy between January 2000 and December 2013 at the Department of Surgical Oncology, Osaka City University. The resected specimens were analyzed for associations with clinicopathological data, including DCLK1 expression, epithelial mesenchymal transition (EMT) marker expression, and cancer stem cell (CSC) marker expression. Univariate and multivariate survival analyses were performed and we assessed the association between DCLK1 expression and clinicopathological factors, including the EMT marker and CSC marker. RESULTSIn total, 48.5% (66/136) of the pancreatic cancer samples were positive for DCLK1. Patients with DCLK1-positive tumors had significantly shorter survival times than those with DCLK1-negative tumors (median, 18.7 mo vs 49.5 mo, respectively; P < 0.0001). Positive DCLK1 expression correlated with histological grade (P = 0.0290), preoperative CA19-9 level (P = 0.0060), epithelial cell adhesion molecule (EpCAM) expression (P = 0.0235), and the triple-positive expression of CD44/CD24/EpCAM (P = 0.0139). On univariate survival analysis, five factors were significantly associated with worse overall survival: histological grade of G2 to G4 (P = 0.0091), high preoperative serum SPan-1 level (P = 0.0034), R1/2 (P < 0.0001), positive expression of DCLK1 (P < 0.0001) or CD44 (P = 0.0245). On multivariate survival analysis, R1/2 [odds ratio (OR) = 2.019, 95% confidence interval (CI): 1.380-2.933; P = 0.0004] and positive DCLK1 expression (OR = 1.848, 95%CI: 1.2854-2.661; P = 0.0009) were independent prognostic factors. CONCLUSIONDCLK1 expression was found to be an independent prognostic factor and it may play a crucial prognostic role by promoting acquisition of stemness.

Effect of Grains of Paradise (GP) Extract Intake on Obesity and Sympathetic Nerve Activity

The methanol extract of grains of paradise (GP), the seed of Aframomum melegueta, which is distributed throughout West Africa, was administered during an animal breeding test. The extract suppressed body weight gain and decreased the weight of adipose tissues in breeding mice, with a greater effect on mice fed a high-fat diet (HFD) than on those fed a normal diet (ND). Other significant effects of GP intake included increased serum triglyceride (TG) concentration and reduced hepatic total cholesterol (TC) and TG concentrations. GP intake markedly prevented fat accumulation and improved hepatic lipid metabolism in HFD-fed mice. In addition, GP extract at a dosage of 5 mg/kg body weight decreased sympathetic nerve activity (SNA) in brown adipose tissue (BAT), while capsaicin, a major component of chili pepper, activated BAT SNA. This suggested that GP exerts a potential anti-obesity effect by a different mechanism from that of capsaicin.

Computer-Aided Design and Fabrication of Finger Prosthesis

Custom-made esthetic finger prostheses, which are used for rehabilitation of patients with missing or impaired fingers, have been fabricated manually. However, such fabrication is time-consuming and requires manual skill. Here we propose a computer-aided method for fabricating finger pros-theses to save time and allow fabrications that do not require considerable manual skill. In this method, the dimensions of a patient’s healthy finger on the contralateral hand are first measured using a caliper. Using these dimensions, a three-dimensional model is constructed for fabricating a prosthesis for the patient’s impaired finger. Using the 3D model, a mold is designed using 3D modeling tools and a computer-aided design system. The resulting mold is then fabricated using a 3D printer. A finger prosthesis is fabricated by pouring silicone resin into the mold. A finger prosthesis for a volunteer was experimentally fabricated according to the proposed method. To evaluate the size and shape of the finger prosthesis, the difference between the finger prosthesis and the original finger of the volunteer was analyzed. Because the average difference between them was 0.25 mm, it was concluded that the proposed method could be used to fabricate a finger prosthesis of adequate size and shape.

Near Infrared Fluorescence Enhancement by Local Surface Plasmon Resonance from Arrayed Gold Nanoblocks

The near infrared (NIR) fluorescence enhancement by local surface plasmon resonanoce from arrayed gold (Au) nanoblocks was investigated by NIR fluorescent dyes, IR780, immobilized in hydrophobic DNA thin film on glass substrates, to clarify the gap mode effect on the fluorescence enhancement. In the substrate with Dimer type Au nanoblock arrangement, average total fluorescence intensity was larger by 10.0, 2.4, and 12.4 times for non-polarized, P- and S- polarization as compared with that on a glass substrate alone, respectively. These findings suggested that enhancement of excitation light intensity at nanogap in the Dimer type Au nanoblock arrangement affected the fluorescence intensity. Average total fluorescence intensity, on the other hand, was smaller by 0.63 times as compared with that on a glass substrate alone in the checkerboard type Au array. It is suggested that the fluorescence quenching was caused by the energy transfer from the excited state of IR780 to Au nanoblocks or by the increased deactivation of excited dye molecules induced by resonance with Au nanoblocks at the checkerboard arrangement. We have firstly achieved the NIR fluorescence enhancement by LSPR due to the gap mode.
