The association of aromatase （CYP19） gene variants wil sperm concentration and motility

The irreversible transformation of androgens into oestrogens is catalysed by cytochrome P450 aromatase. In the present study, we explored the contribution of the （TTTA）n polymorphism in the aromatase gene （CYP19） to sperm concentration and motility. Ninety normozoospermic and 60 oligospermic men were examined during infertility examinations. DNA was extracted from spermatozoa, and the CYP19（TTTA）n polymorphism was genotyped by PCR. Genotype analysis revealed six CYP19（TTTA）n alleles with 7-12 repeats. The allelic distribution of the CYP19（TTTA）n polymorphism differed between normozoospermic and oligospermic men （P〈0.01）. Oligospermic men less frequently had long CYP19 alleles than did normozoospermic men （25 and 37.8%, respectively; P〈0.02）. The higher frequency of short CYP19alleles in oligospermic men compared to normozoospermic men （43.3 and 28.3%, respectively; P〈0.01） was primarily due to the distribution of the CYP19 （TTTA）7 allele. The CYP19 （TTTA）7 allele was associated with lower sperm concentration in normozoospermic men {P〈0.01） and in the total study population {P〈0.01）; it was also associated with lower sperm motility in normozoospermic men （P〈0.05） and in the total study population （P〈0.01）. In conclusion, the CYP19 （TTTA）7 allele probably impairs aromatase activity, which in turn alters aromatase and oestrogen levels in the testis, leading to decreased sperm concentration and motility. These findings support the significance of cytochrome P450 aromatase in human spermatogenesis and consequently in semen quality.

Choline is a crucial factor in the regulation of sperm membrane structure and fluidity, and this nutrient plays an important role in the maturation and fertilizing capacity of spermatozoa. Transcripts of phosphatidylethanolamine N-methyltransferase （PEMT） and choline dehydrogenase （CHDH）, two basic enzymes of choline metabolism, have been observed in the human testis, demonstrating their gene expression in this tissue. In the present study, we explored the contribution of the PEMTand CHDHgene variants to sperm parameters. Two hundred oligospermic and 250 normozoospermic men were recruited. DNA was extracted from the spermatozoa, and the PEMT -774G〉C and CHDH ＋432G〉T polymorphisms were genotyped. The genotype distribution of the PEMT-774G〉C polymorphism did not differ between oligospermic and normozoospermic men. In contrast, in the case of the CHDH ＋432G〉T polymorphism, oligospermic men presented the CHDH432GIG genotype more frequently than normozoospermic men （62% vs. 42%, P〈0.001）. The PEMT774GIG genotype was associated with a higher sperm concentration compared to the PEMT774GIC and 774C/C genotypes in oligospermic men （12.5±5.6× 10^6 spermatozoa m1-1 vs. 8.3±5.2×10^6 spermatozoa m1-1, P〈0.002） and normozoospermic men （81.5±55.6×10^6 vs. 68.1±44.5×10^6 spermatozoa m1-1, P〈0.006）. In addition, the CHDH432G/G genotype was associated with higher sperm concentration compared to CHDH432G.T and 432T/T genotypes in oligospermic （11.8±5.1×10^6 vs. 7.8±5.3×10^6 spermatozoa m1-1, P〈0.003） and normozoospermic men （98.6±62.2×10^6 vs. 58.8±＋33.6×10^6 spermatozoa m1-1, P〉0.001）. In our series, the PEMT-774G〈C and CHDH ＋432G〈T polymorphisms were associated with sperm concentration. This finding suggests a possible influence of these genes on sperm quality.

Uterine fibroids are the most common benign tumours in the reproductive system. They are proliferations of smooth muscle cells of the myometrium containing a large quantity of extracellular matrix and they are surrounded by a pseudo capsule of compressed areolar tissue and smooth muscle cells. They can cause various symptoms such as menorrhage, pain and infertility and therefore they can be a traumatic experience for several women. The treatment of choice is myomectomy. In the past, myomectomy was performed by relatively atraumatic techniques, which involved stretching the myoma from its pseudocapsule to extract the fibroid directly from the surrounding fibromuscular tissue, breaking up the fibrous bridge. Modern laparoscopic intracapsular myomectomy (LIM), however, leaves the fibrovascular network surrounding the myoma (namely the “fibroid neurovascular bundle”) intact which reduces the bleeding and/or uterine musculature trauma, and spares the neuropeptide fibers of the pseudocapsule. In this observational study, we compare the two techniques-laparoscopic intracapsular myomectomy (LIM) and conventional abdominal myomectomy (CAM) regarding the longterm uterine healing and a significantly faster healing process of the uterine incision was achieved by LIM compared to CAM.

Effects of phosphodiesterase 5 inhibitors on sperm parameters and fertilizing capacity

The aim of this review study is to elucidate the effects that phosphodiesterase 5 （PDE5） inhibitors exert on spermatozoa motility, capacitation process and on their ability to fertilize the oocyte. Second messenger systems such as the cAMP/adenylate cyclase （AC） system and the cGMP/guanylate cyclase （GC） system appear to regulate sperm functions. Increased levels of intracytosolic cAMP result in an enhancement of sperm motility and viability. The stimulation of GC by low doses of nitric oxide （NO） leads to an improvement or maintenance of sperm motility, whereas higher concentrations have an adverse effect on sperm parameters. Several in vivo and in vitro studies have been carried out in order to examine whether PDE5 inhibitors affect positively or negatively sperm parameters and sperm fertilizing capacity. The results of these studies are controversial. Some of these studies demonstrate no significant effects of PDE5 inhibitors on the motility, viability, and morphology of spermatozoa collected from men that have been treated with PDE5 inhibitors. On the other hand, several studies demonstrate a positive effect of PDE5 inhibitors on sperm motility both in vivo and in vitro. In vitro studies of sildenafil citrate demonstrate a stimulatory effect on sperm motility with an increase in intracellular cAMP suggesting an inhibitory action of sildenafil citrate on a PDE isoform other than the PDE5. On the other hand, tadalafil＇s actions appear to be associated with the inhibitory effect of this compound on PDE11. In vivo studies in men treated with vardenafil in a daily basis demonstrated a significantly larger total number of spermatozoa per ejaculate, quantitative sperm motility, and qualitative sperm motility; it has been suggested that vardenafil administration enhances the secretory function of the prostate and subsequently increases the qualitative and quantitative motility of spermatozoa. Objective: To compare the efficacy of GnRH antagonist vs. GnRH agonist adminis tration for controlled ovarian hyperstimulation (COH) in assisted reproduction. Design: A prospective, randomized trial. Setting: Clinical research unit at a te rtiary care medical center. Patient(s): Sixty-five patients with unexplained in fertility or mild male subfertility undergoing COH for IUI. Intervention(s): Twe nty-nine women (group A) were randomized to receive 600 μg of busereline aceta te per day starting in the midluteal phase of the cycle (long protocol), whereas 36 women (group B) were treated with 0.25 mg/d of the GnRH antagonist Cetroreli x starting from day 6 of the cycle. The starting dose of recombinant FSH was 150 IU in women of both groups. Insemination was performed 34 hours after hCG injec tion. Main Outcome Measure(s): Clinical and successful ongoing pregnancy rate (P R), measurements of serum FSH, LH, E2, and P, number of recruited follicles, dur ation of stimulation period, and amount of gonadotropins used. Result(s): Women in group A required significantly more days of treatment (median: 12.0 vs. 9.0) and significantly more total units of recombinant FSH (median 1,800 vs. 1,550) a s compared with the corresponding values of the antagonist group (group B). Seru m FSH, LH, E2, and P were significantly higher on the antagonist group on days 2 and 6 of stimulation. However, these differences regress until the day of hCG a dministration. Conclusion(s): The GnRH antagonists have facilitated short and si mple treatment, and are particularly attractive for administration in women unde rgoing COH, achieving comparable PR with the long protocol regimen.

After administration of GnRH antago nist from day 1of stimulation for IVF,12.1cumulus oo cyte complexes were retrieved(95%confidence intervalCI,10.6-13.6),1.9embryos were transferred(95%CI,1.7-2.0),and2.4embryos were cryopreserved(95%CI,1.7-3.1).Stimulation for 8.8days(95%CI,8.4-9.2)with re-combinant FSH resulted in an ongoing implantation rate of26.5%(95%CI,17.1-35.9)and an ongoing preg-nancy rate of 39.7%per started cycle(95%CI,30.1-50.8)-and of 42.4%per ET(95%CI,32.3-53.1).
