Comparative Evaluation of Extra Capsular Extraction (ECE) with Suture and Small Incision Cataract Surgery (SICS) Manual without Suture in the University Hospital of Bouake (Ivory Coast)

Objective: Cataract is a partial or total opacification of the lens responsible for blindness. The treatment is surgical. The purpose of our study was to evaluate the results of the newly introduced manual small incision cataract surgery (SICS) and compare them with those of the conventional extra-capsular extraction method. Patients and Method: Analytical cross-sectional study was carried out at the ophthalmology department of the University Hospital of Bouaké for a period of 1 year. Results: The two surgical techniques were compared at equal proportions. The average duration of interventions was longer during the sutured Extra Capsular Extraction (ECE) compared to the SICS. Rupture of the posterior capsule with vitreous exit, which was the most observed intraoperative complication, was predominant with ECE with suture (16%) against 8% for SICS. Corneal edema observed in the early postoperative period, was predominant with sutured ECE (30%), against 20% for SICS. Corneal edema, which was more observed with EEC compared to SICS, dominated secondary and late postoperative complications. Post-operative astigmatism induced by ECE with suture was between ?2 and ?4 dioptre in 51.5% of patients and between ?0.5 and ?2 dioptre in 73.91% of SICS. Visual acuity without correction in the early days was better with SICS. Conclusion: In this comparative study of cataract surgery, we noted superimposable results for both techniques. However, the prevalence of pre- and post-operative complications was more frequent with sutured ECE. The technique without suture allows visual recovery and a quick return to normal life.

Production of Yam Tubers Using Seed Tubers from Vitroplants Regenerated from Aerial Stems in the Yam Species Dioscorea alata (L.) and Dioscorea cayenensis-rotundata (L. & P.) in Côte d’Ivoire

The aim of this study is to produce large quantities of yam tubers in the field from seed tubers derived from vitroplants regenerated from aerial stem cuttings and farmers’ tuber seeds. Seed tubers from ten yam cultivars, including five from Dioscorea alata (cDa083, cDa053, cDa115, cDa150 and cDa266) and five from Dioscorea cayenensis-rotundata (cDr015, cDr027, cDr150, cDr206 and cDr148) with distinct agro-morphological characteristics, were used as material. A completely randomized block design with 3 replications was used. In each block, all cultivars and seed types were represented. When put into cultivation, the tubers produced by the vitroplants all germinated and gave 100% healthy plants, compared with 86% to 100% healthy plants for the seed tubers produced by the farmers. Yields per hectare ranged from 10 to 25 tonnes for seed tubers produced by in vitro plants, and from 4 to 9 tons for seed tubers produced by farmers. Seed tubers from vitroplants can therefore be recommended to farmers as a solution to the problem of seed tuber unavailability.

Prevention of Cervix Cancer in a Low Resources Country: Operational and Clinical Aspects of a Screening’s Network in a Suburb of Abidjan (Côte d’Ivoire)

Background: Cancer of the cervix is the most common cancer and the leading cause of death by cancer in women in developing countries. According to WHO, there were in 2002 in the world more than 500,000 new cases of cervical cancer, with more than 90% in developing countries. Secondary prevention of cervical cancer is done through screening of precancerous lesions and early diagnosis of cancer followed by treatment. Objectives: Our experience was reported in the management of a network of screening in the prevention of cancer of the uterine cervix. Method: After a program of training and upgrade to a pool of specialist doctor of obstetrics and gynaecology and midwifery centers health on visual screening methods, a reference site that is also a trainer site was defined and equipped with video-colposcopy and cryotherapy device. In April 2014, four peripheral sites have been installed with an equipment of lamp, magnifying glasses, acid acetic 5% and Lugol’s iodine. The screening network has been implemented and works on the model of reference-cross-reference. Results: The assessment of a year’s activity of the peripheral centers reported 993 patients, 653 consultations at the reference site, 106 precancerous lesions (6%), and 86 cases of references. The age of attendance peak lies in the age group of 35 - 55 years. Conclusion: The improvement of women’s participation requires organized screening, mass communication strategies, and systematic proposal of screening in consultation.

Chronic and potential non-healing wounds are a great challenge for patients, physicians, and wound care professionals and the health system. A balanced nutrition intake is essential for health as well as a speedy recovery of such wounds. The study objective was to compare chronic wound inpatients supplemented with food nutrient content with outpatients with their usual intake in a cross-sectional study. Patients’ food samples were taken for chemical analyses. The protein, beta-carotene, sodium, magnesium, manganese, and potassium content prove statistically significant differences. Outpatients’ diets are more diversified than inpatients’ diets, and supplementation diet richest in beta-carotene and in protein. Outpatients have better dietary diversity than those who were hospitalized. The result provides insights of supplementary food as critical issues pertaining to chronic wounds management. However, analyzing bioavailability of nutrient on patients’ blood may provide more knowledge in the appropriate integrated wound management.

Anesthesiological Management of Eclampsia in Tropical Africa: Experience of the University Hospital Center of Bouake

Introduction: Eclampsia is a common cause of maternal mortality in developing countries. The objective of this study is to evaluate the practice of anesthesia for eclampsia in tropical Africa. Patients and Methods: Retrospective descriptive study on the parturients who benefited from eclampsia anesthesia in the operating theaters of the gynecology and obstetrics department, the University Hospital Center (CHU) of Bouake over a two-year period (January 2015 to December 2016). The parameters studied were: anesthetic risk assessment, anesthetic management, immediate anesthetic and post-anesthetic accidents and incidents. Results: Out of a total of 3831 emergency cesarean sections, 65 were for eclampsia, or 1.69% of obstetrical surgical activity. The mean age of the patients was 20.30 ± 3.34 years (range: 14 years and 39 years). Prenatal consultation was not performed in 70% of cases and 85% of patients were primiparous. Patients classified as ASA IIIU accounted for 75% of the population and in 62% of cases, the Glasgow score was between 9 and 12. Induction was achieved with thiopental in 98% of patients and vecuronium was the only muscle relaxant used. The most common fetal complications were hypotrophy (50%), prematurity (30%) and respiratory distress (40%). Postoperative anesthetic complications were agitation (70%) and wake delay (15%). Maternal mortality was 5% and the neonatal death rate was around 7.69%. The elements of poor maternal prognosis were the ASA IVU class (P = 0.015) and the Aldrete awakening score between 3 and 5 awakening (P = 0.001). Conclusion: Anesthesia for eclampsia at CHU of Bouake is difficult. It needs to be improved thanks to better equipment of the health structures.

Background: Herbal medicinal preparations are used as dietary supplements for disease prevention and as alternative/complementary medicine. The growing interest in herbal medicine raises the question of its safety and efficacy. Numerous recorded cases of intoxication and toxicological studies reveal that medicinal plants can be toxic, which hinders their safe use. Plant intoxications related to a variety of factors include plant organs and many extraction solvents. Little toxicological data on medicinal plants is available. The need to investigate is important for safe use. Imperata cylindrica, a poaceae, is one of the medicinal plants for which few toxicological data are available. Materials & Methods: To expand toxicological data, water, 70% ethanol, and 30% acetone extracts of the leaves and roots, obtained by maceration and reflux methods, were used for phytochemicals molecules qualitative detection and toxicity test by the brine shrimp lethality assay. Results: The qualitative analysis of the different extracts revealed the presence of alkaloids, polyphenolic compounds, saponins, and polyterpenoids. The toxicity endpoint was lethal concentration 50 (LC50). The leaves’ extracts LC50 was between 489.78 μg/mL and 1066.6 μg/mL. As for the root extracts, the LC50 was between 341.98 μg/mL and 1530 μg/mL. Discussion: The different compounds’ presence justifies the use of Imperata cylindrica as a medicinal plant. According to Clarkson classification the root extracts are moderately toxic (LC50: 168.47 μg/mL), and leaf extracts are weakly toxic (LC50: 527.25 μg/mL). The extrapolation made in relation to the Gosselin, Smith, and Hodge scale, allows us to characterize the Imperata cylindrica root and leaf extracts as non-toxic to humans by oral route. Conclusion: This result can be a base for more precise toxicological studies.

Post-Traumatic Palatine Odontogenic Abscess: About 2 Cases

Introduction: Palatal abscess or cellulitis of dental origin is a rare clinical form of circumscribed cellulitis that can sometimes pose a diagnostic problem. It is the consequence of poor or non-treatment of dental caries or trauma. The aim of this study is to describe the clinical and therapeutic aspects of these two cases of palatal cellulitis of post-traumatic dental origin. Observation: The patients consulted for painful palatal swelling secondary to untreated dental trauma of the 21s. The diagnosis of palatal cellulitis was based on the inflammatory and fluctuating nature of the swelling. A probabilistic bi-antibiotic treatment and an incision and drainage associated with treatment of the portal of entry were carried out. Progression was favourable in both cases. Conclusion: Palatal cellulitis is a rare condition and can be prevented by systematic stomatological consultation after dental trauma. Diagnosis is clinical. However, CT scans are sometimes useful. The course is generally favorable with appropriate treatment.

Rain-flow Modeling Using a Multi-Layer Artificial Neural Network on the Watershed of the Cavally River(Cote d’Ivoire)

Water resources management is nowadays a significant stake for the world. However, missing or bad quality of the hydro-climatic historical data available of the studied area makes sometimes hydrological studies difficult. Generally, conceptual rain-flow models are designed to bring an appropriate answer with the correction of gaps and prediction of the flows. Historical hydro-climatic data of the Ity station, located on Cavally River, contain gaps which must be bridged. This study aims to establish a rainfall-runoff model through artificial neural networks for filling the gaps into the flow data series of the hydrometric station of Ity on the watershed of Cavally River. A multi-layer perceptron of feed forwards with two entries (monthly average rain and evapotranspiration) and an exit (flows) was established with flow evapotranspiration data. Comparison of the criteria of performance of the various architectures of the neural network model showed that architecture 2-3-1 gives best results. This architecture provides Nash coefficients of 75.79% and correlation linear coefficient of 95.64% for the calibration and Nash coefficients of 73.32% and correlation linear coefficient of 98.33% for the validation. The correlations between simulated flows and observed flows are strong. The correlation coefficients are 83.89% and 83.08% respectively for the calibration and validation.

This study focused on the transfer of experimental results of anaerobic digestion of liquid waste from an attiéké (steamed cassava semolina) factory to a 6 m3 pilot digester. The experimental digester and the pilot were powered as follows: Lw + U + C (liquid waste + urine + cow dung). To the results, the experimental digester mesophilic with a progressive elimination of COD. Also, the nitrogen concentrations in the experimental reactor had little removal with alkaline pH. As for the biogas product in this digester, a volume of 3.6 m3 was obtained with a positive flammability test. The transition from the laboratory scale to the semi-industrial scale retains the results of purification and fuel biogas production of the experimental digester.

A Comparison of Hydrothermal Aging, SO₂ and Propene Poisoning Effects on NH₃-SCR over Cu-ZSM-5 and Cu-SAPO-34 Catalysts

This study was aimed to investigate the effects of hydrothermal aging, propene and SO2 poisoning on the ammonia-selective catalytic reduction (NH3-SCR) performance of both Cu-SAPO-34 and Cu-ZSM-5. The catalytic activities of fresh, aged and poisoned samples were tested in ammonia-selective catalytic reduction (NH3-SCR) of NOx conditions. The XRD, TG and N2-desorption results showed that the structures of the Cu-SAPO-34 and Cu-ZSM-5 remained intact after 750˚C hydrothermally aged, SO2 and propene poisoned. After hydrothermal aging at 750˚C for 12 h, the NO reduction performance of Cu-ZSM-5 was significantly reduced at lower temperatures, while that of Cu-SAPO-34 was less affected. Moreover, Cu-SAPO-34 catalyst showed high NO conversion with SO2 or propene compared to Cu-ZSM-5. However, Cu-ZSM-5 showed a larger drop in catalytic activity with SO2 or propene compared to Cu-SAPO-34 catalyst. The H2-TPR results showed that Cu2 ions could be reduced to Cu and Cu0 for Cu-ZSM-5, while no significant transformation of copper species was observed for Cu-SAPO-34. Meanwhile, the UV-vis DRS results showed that CuO species were formed in Cu-ZSM-5, while little changes were observed for the Cu-SAPO-34. Cu-SAPO-34 showed high sulfur and hydrocarbon poison resistance compared to Cu-ZSM-5. In summary, Cu-SAPO-34 with small-pore zeolite showed higher hydrothermal stability and better hydrocarbon and sulfur poison resistant than Cu-ZSM-5 with medium-pore.
