Giant Cervical Kystic Lymphangioma in Children: Surgical Management of a Case

Introduction: Cervical cystic lymphangiomas are rare benign dysembryoplastic tumors of lymphatic origin. Its severity in the child is due on one hand to their fast evolution and the compression of the way aerodigestive and on the other hand, by the classical difficulty of their excision. The purpose of this work was to report a case of giant cervical cystic lymphangioma in a child to discuss the difficulties associated with its management in our context. Observation: This was a 3-year-old female child who was admitted to the EnT department of CHU “Luxembourg” for right lateral cervical swelling. The clinical examination had noted a large anterior-latero cervical swelling of soft, renitent, painless palpation, movable in relation to the superficial and deep plane, measuring about 20 cm × 17 cm, the skin was healthy. It wasn’t particularity to the rest of the physical examination. The diagnosis of giant cervical cystic lymphangioma was discussed. Thorough excision of the swelling by right lateral cervicotomy was performed. Anatomopathological examination of the operative specimen confirmed the diagnosis. The postoperative course was simple and the evolution was favorable. Conclusion: Cervical cystic lymphangiomas are rare. Their management involves surgery, with short and long-term post-operative outcomes are often excellent.

Autoantibodies in Systemic Lupus Erythematosus, on Black African Subject, in Abidjan

Aim: To determine the clinical and immunological characteristics of patients with systemic erythematosus lupus in Abidjan. Patients and Method: We studied 117 patients’ files with systemic lupus erythematosus aged 12 to 73 years old, who fulfilled the American College of Rheumatology (ACR)’s criteria. Antinuclear autoantibodies (AnA) were searched by indirect immunofluorescence. Anti-DnA native autoantibodies, extractable nuclear anti-antigens autoantibodies (anti-Sm, anti-SSA, anti-SSB and anti-RnP) and anti-phospholipids autoantibodies have been searched by ELISA technic. Results: The most frequent clinical manifestations were: articular damages (86.32%), cutaneous and mucosal lesions (71.79%) and fever (76.67%). Kidney damages have been noticed in 40.17%. neurologic manifestations have been observed in 36.75%. Pericarditis and pleurisies have been noticed in 22.22% and 11.97% of cases, and anaemia in 86.32% of cases. AnA have been detected in 94.12% of cases, anti-DnA native’s autoantibodies in 73.53% and anti-Sm autoantibodies in 75% of cases. Anti-SSA and anti-SSB autoantibodies were respectively in 75% and 56.25% of cases. Anti-RnP autoantibodies were in all the patients, and anti-phospholipids autoantibodies were in 37.50% of cases. Conclusion: Systemic lupus erythematosus of Ivorian black subject is characterised by high prevalence of autoantibodies, mostly Anti-RnP.

Deafness: Epidemiological, Clinical and Therapeutic Aspects in 15 Year Olds and over at the University Hospital Center Gabriel Toure of Bamako

Objective: To study the epidemiological, clinical and therapeutic aspects of hearing loss in 15-year-olds and over at the CHU GT of Bamako. Materials and Method: This is a longitudinal prospective study carried out in the EnT department of the University hospital Gabriel Toure in Bamako over 13 months (from September 2018 to September 2019). We carried out an exhaustive sampling of all the patients who consulted in the department for hearing loss and whose deafness was confirmed at pure tone audiometry with age greater than or equal to 15 years. The exclusion criteria were all patients under the age of 15 as well as a hearing loss related to earwax or foreign bodies and refusals to participate in the study. Results: Two hundred and thirty (230) patients were collected during the study. Dominated by the male sex with a sex ratio of 1.5 or 3 men for 2 women, with an average age of 38.16 ± 19 years with extremes ranging from 15 to 86 years. Among our patients, 37.40% had unilateral deafness, progressive installation evolving more than 12 weeks with a rate of cases had headaches 41% and had a history of chronic otitis media (CMO) 27%. There were a few cases of mixed type deafness, 30% had mild deafness, 22% moderate deafness and 40% severe deafness, according to the BIAP classification. Among our patients, 22.16% had conductive hearing loss, and those with sensorineural hearing loss accounted for 15.24%. Conclusion: Pure tone audiometry remains essential in the diagnosis of deafness. It not only makes it possible to confirm or invalidate the deafness, but also to define the type of deafness and especially to classify the deafness according to the degree of the average hearing loss established by the BIAP.

Estimation of the Actual Evapotranspiration by the SEBAL Method in the Irrigated Rice Perimeter of Zatta (Yamoussoukro—Côte d’Ivoire)

In this study, the SEBAL (Surface Energy Balance Algorithm for Land) model was used to map the spatio-temporal distribution of actual evapotranspiration in the Yamoussoukro department (Côte d’Ivoire). Like other regions of the country, the Yamoussoukro district is confronted with the phenomenon of evapotranspiration (ET). This is a very important component that comes into play in the water balance but also in the calculation of the water needs of agricultural crops. Consequently, its estimation is of paramount importance in research related to the rational management of water resources, particularly agricultural water. The objective of this study was to analyze the spatio-temporal distribution of actual evapotranspiration (AET) as a function of land cover and land use. The methodology used is based on the SEBAL model which uses remote sensing (Landsat 8_OLI/TIRS) and climatic data to estimate actual evapotranspiration and analyze the spatio-temporal distribution of AET. The results reveal that the AET varied from 0 to 5.44 mm/day over the period from December 2019 to February 2020 with an average value of 4.92 mm/day. The highest average values occurred for water bodies (4.90 mm/day) and flooded vegetation (4.88 mm/day) while the lowest values occurred in residential areas (2.04 mm/day). Furthermore, the results show that the difference between the SEBAL model and the FAO-Penman-Monteith method is minimal with an average RMSE of 0.36 mm/day for all the satellite images. This study demonstrates the considerable potential of remote sensing for the characterization and estimation of spatial evapotranspiration in the Zatta irrigated rice-growing area.

Uterine leiomyosarcoma (LMS) is a rare uterine malignancy that represents approximately 1% of uterine cancers. The nonspecific symptoms cause difficulties in the diagnostic, prognostic and therapeutic. We report an unusual progressive form, with a uterine inversion of LMS fundal externalized out of the vulva in a patient of 48 years old premenopausal.

Chronic Purulent Otorrhea in the South of the Sahara: Epidemiological, Clinical and Therapeutic Profiles about 135 Cases

Objective: To study the clinical and therapeutic epidemiological profile of chronic purulent otorrhea in the EnT and CCF departments of the Gabriel Touré university hospital. Materials and Method: This was a retrospective study from May 2019 to April 2021. We made an exhaustive sampling of all patients who consulted for chronic purulent otorrhea with a complete medical file. All ages were included (except from 0 to - 5 years), and all sexes combined. The data medium was the patient records and the department’s operating report register. The non-inclusion criteria were patients with incomplete files and patients who were less than 5 years old. Results: In total, we collected 135 cases. The average age of our patients was 29.27 years. The most represented age group was 16 to 25 years old. We noted a male predominance in 60.7%. The associated signs found were hearing loss, earache, tinnitus, ear pruritus, vertigo, headache and facial paralysis. The main pathology involved is chronic otitis media (CMO) in 55.5%, the most serious of which is dangerous chronic otitis (8.1%). All of our patients have received medical treatment. The treatment was surgical in 64% of patients after medical treatment. With the mean follow-up of 6 months, tinnitus and reperforation were the most common complications found in 6.2% and 3.1% respectively. Conclusion: Chronic purulent otorrhea is the most common ear discharge in EnT consultation. The main pathology involved is CMO, the most formidable of which is cholesteatoma. Medico-surgical treatment.

Hypofractionated Radiation Therapy for the Treatment of Breast Cancer: Experience of national Institute of Oncology, Rabat, Morocco

Hypofractionated radiation therapy has proven effective on locoregional control and tolerance in the adjuvant treatment of breast cancer. The aim of this study is to compare the results of hypofractionated radiation therapy versus conventional radiation therapy in terms of local control and tolerance. It was a retrospective study of patients observations collected from January 2007 to December 2008 in Department of Radiation Therapy in Institut national d’Oncologie de Rabat. The treatment results were evaluated by the rate of locoregional recurrence, distant recurrence and research of late toxicities. Radiotherapy was delivered using the same technique in both groups, by gamma photons of cobalt 60 with an energy of 1.25 MeV. They were 2 groups: the first group treated with standard dose rate and the second group treated by hypofractionated radiation therapy. The mean age of the patients was 42.8 ± 6.9 years old in the standard group and 43.22 ± 7.2 years old in the hypofractionation group. We noted a predominance of infiltrating ductal carcinoma. The majority of patients were pT2, pn0 and pn1. The majority of patients had radical surgery and chemotherapy with anthracyclines in both groups. We noted a statistically significant difference in the irradiation of chest wall between the standard (89.2%) and hypofractionated group (70.3%), with p = 0.043. The median duration of radiation therapy was statistically different in both groups: 39 days in the standard and 23 days in the hypofractionated group (p 0.001). The local recurrences were statistically identical to 12 and 24 months (p = 0.999). Concerning toxicities, the frequency of adverse event was similar in both groups. Hypofractionated radiation therapy with a total dose of 42 Gy at 2.8 Gy per fraction in 5 fractions weekly is comparable to standard radiotherapy in terms of local control and tolerance and is therefore a very good alternative to standard treatment.

Multilevel Cervicothoracic and Lumbar Pott’s Disease: About Three new and Unusual Cases in the Rheumatology Department of the Teaching Hospital of Cocody in Abidjan (Côte d’Ivoire)

Pott’s disease usually affects two contiguous vertebrae or two spinal levels. The involvement of the three cervicothoracic and lumbar levels is rarely described. Only three cases have been described in the literature to our knowledge. The authors report three new cases of tuberculous spondylodiscitis of cervicothoracic and lumbar site. Our three cases were characterized by symptoms similar to the other topographical forms of Pott’s disease. A predisposing factor is often identified like the infection with human immunodeficiency virus. The definitive diagnosis was made in two cases by identifying acid and alcohol-fast bacilli for one case and by histology for the other case. For the third case, there was a diagnostic presumption reinforced by the satisfactory outcome on tuberculosis chemotherapy for a period of 12 months. The multilevel nature of diseases does not influence the effectiveness of treatment of this form of spinal tuberculosis.

Quantitative Structure-Activity Relationship Study of a Benzimidazole-Derived Series Inhibiting Mycobacterium tuberculosis H37Rv

This work was carried out on a series of twenty-two (22) benzimidazole derivatives with inhibitory activities against Mycobacterium tuberculosis H37Rv by applying the Quantitative Structure-Activity Relationship (QSAR) method. The molecules were optimized at the level DFT/B3LYP/6-31 + G (d, p), to obtain the molecular descriptors. We used three statistical learning tools namely, the linear multiple regression (LMR) method, the nonlinear regression (nLMR) and the artificial neural network (Ann) method. These methods allowed us to obtain three (3) quantitative models from the quantum descriptors that are, chemical potential (μ), polarizability (α), bond length l (C = n), and lipophilicity. These models showed good statistical performance. Among these, the Ann has a significantly better predictive ability R2 = 0.9995;RMSE = 0.0149;F = 31879.0548. The external validation tests verify all the criteria of Tropsha et al. and Roy et al. Also, the internal validation tests show that the model has a very satisfactory internal predictive character and can be considered as robust. Moreover, the applicability range of this model determined from the levers shows that a prediction of the pMIC of the new benzimidazole derivatives is acceptable when its lever value is lower than 1.

Caudal Anesthesia: Experience in the Post-Operative Analgesia in Pediatric Ambulatory Surgery at the University Hospital of Treichville

Introduction: The caudal anesthesia is used by many authors for postoperative analgesia. The purpose of this study was to report our experience in the practice of caudal block as post operative analgesia method in ambulatory surgery in a context of limited technical equipment. Patients and Method: Over a period of 5 months, a prospective study was conducted on 39 children aged 3 to 5 years weighing on average 15.12 kg. Children classified ASA I and II were selected. After premedication with midazolam (0.3 mg/kg) by intra rectal route, the inhalation induction was made with sevoflurane 8%, conveyed by fresh gas (50% O2 and 50% air). The caudal block was obtained with the levobupivacaine 0.25% at a dose of 1 ml/kg. The hemodynamic parameters (systolic and diastolic blood pressure, heart rates) and respiratory parameters (respiratory frequency) pre-, per- and post-operative were measured. Post-operative pain was assessed with the Objective Pain Scale (OPS). The date of first use of analgesia was noted. The adverse effects of caudal block (meningitis, respiratory disorders, acute urinary retention, cardiac disorders) have been assessed. Results: The average duration of the procedure was 55.2 minutes. The use of analgesia was made 4 hours after skin closure, when the OPS Broadmann score was greater than 3. An agitation was observed in 6 children. Haemodynamic parameters have not significantly varied from the pre- to the post-operative. no infectious complications or intolerance to local anesthetics were observed. Allthe children were able to drink 4 hours after the end of the intervention and issued their first urine later than 3 hours after surgery. Conclusion: This type of anesthesia has been found very suitable for ambulatory surgery of the child, and is always helpful. It assured a post operative analgesia of good quality, and a reduction in consumption of morphine intraoperatively.
