Clinicopathologic characteristics of gastric carcinoma in elderly patients: A comparison with young patients

AIM:To examine the clinicopathologic features of elderly patients with gastric carcinoma and to investigate the relationship between prognosis and ***: We reviewed the hospital records of 2 014patients with gastric carcinoma retrospectively to compare the clinicopathologic findings in elderly (age ＞70 years) and young (age ＜36 years) patients during the period from 1986 to 2000 in a tertiary referral center in Gwangju, Korea. Overall survival was the main outcome ***: Of the 2 014 patients, 194 (9.6%) were in the elderly group and 137 (6.8%) were in the young *** elderly and young patients had similar distributions with respect to depth of invasion, nodal involvement, hepatic metastasis, peritoneal dissemination, tumor stage at the initial diagnosis, and type of surgery. Synchronous multiple carcinomas were found in 14/194 (7.2%) of the elderly group and 4/137 (2.9%) of the young group (P＜0.05). Using the Borrmann classification, type Ⅳ was more frequent in the young patients than in the elderly patients (P＜0.05).Significantly more elderly patients had a well or moderately differentiated histology, and more young patients had a poorly differentiated histology and signet ring cell carcinoma (P＜0.001). The 5-year survival rates of elderly and young patients did not differ statistically (52.8% vs 46.5%,P = 0.5290). Multivariate analysis showed that the histologic type, nodal involvement and operative curability were significant prognostic factors, and age itself was not an independent prognostic factor of survival for elderly gastric carcinoma ***: Elderly patients with gastric carcinoma do not have a worse prognosis than young patients. The important prognostic factor is whether the patients undergo a curative resection.

Effect of Water Stress Induced by Polyethylene Glycol 6000 on Somatic Embryogenesis in Cocoa (<i>Theobroma cacao</i>L.)

Climate change, coupled with insufficient and irregular rains, led to a decline in the productivity of Ivorian cocoa production. The present study aimed to screen three cocoa (Theobroma cacao L.) genotypes in order to evaluate their response to water stress at callus and somatic embryos induction and proliferation stages. Staminode and petal explants of the genotypes C1, C14 and C16 from the collection of National Center of Agronomic Research (CNRA) of Ivory Coast, were placed on medium DKW (Driver & Kuniyuki Walnut) in the presence of different concentrations of Polyethylene glycol (PEG) 6000 (0, 25, 50, 75, 100 and 125 g/l). This polymer was used as osmoticum to stimulate water stress. Data were recorded for callus induction frequency, callus fresh weight, embryogenic callus percentage and average number of somatic embryos. The results obtained showed that intensity of callus proliferation decreased with increasing concentration of PEG 6000. In all genotypes tested, only the petal explants underwent somatic embryogenesis. The induction rates and average number of somatic embryos per explant were reduced with the increase in the concentration of PEG 6000. Among the tested genotypes, C1 produced somatic embryos with all concentrations of PEG 6000 and expressed highest frequencies of induction (62%) and average number of somatic embryos per explant (6.22). This genotype would be the most tolerant to water stress. Somatic embryos obtained with high concentrations of PEG 6000 in this study provide an important basis for the selection and further production of water stress-tolerant varieties of cocoa.

Potential Probiotic Tiger Nut-Cashew Nut-Milk Production by Fermentation with Two Lactic Bacteria Isolated from Ivorian Staple Foods

Probiotification of plant milk can improve its sensory and health-promoting properties. As traditional fermented foods where lactic acid bacteria (LAB) are present have been associated with beneficial effects on human health, the beneficial effects of two LAB recently isolated from two current Ivorian staple foods (a pepper and a traditional beer) were screened. These two strains LAC 1 (Lactobacillus plantarum) and LAC 2 (Pediococcus acidilactici) which presented probiotic, exopolysaccharides, inflammatory and anti-oxidant activities, were used to ferment a composite plant milk of tiger-nut and cashew (80/20) compared to two starters of a commercial yogourt. The obtained plant milks SCT 2 and SCT 3 with a significant increase in their antioxidant and/or anti-inflammatory activities and lactic bacteria contents were more preferred by consumers than SCT 1 obtained by fermentation of the commercial yogourt starters. The mixing of LAC 1 and LAC 2 was not beneficial. SCT 2 (with an anti-inflammatory activity of 31.38% and an anti-oxidant activity of 17.30%) and SCT 3 (with an anti-oxidant activity of 22.28) could be further tested in animal models to verify their nutrition-health claims.

Prevention of Venous Thromboembolism in General Surgery at Gabriel TouréUniversity Hospital in Mali

Through its impact on morbidity and mortality and the cost of medical care, Venous Thromboembolism (VTE) has a significant influence on the quality of care in a surgical environment. The objectives were to determine the risk of postoperative venous thromboembolism and to assess its prophylactic management in the general surgery department of Gabriel Toure University Hospital. This was a prospective observational study from May 01, 2018 to December 31, 2018. It included all patients of 18 years old and over, operated in regulated surgery, under general anesthesia in the department. For all of these patients, Caprini score was used to assess the risk of VTE. The ninth American College of Chest Physicians’ Consensus was used as guidelines to assess Thromboprophylaxis practices in the ward. The Caprini score was evaluated in 80 patients for 8 months. The average age was 50.2 years. The sex ratio was 0.48. The risk level of VTE was low in three patients (3.8%), moderate in 13 patients (16.3%), high in 34 patients (42.5%) and highest in 30 patients (37.5%). The main risk factors found were major surgery (87.5%), age > 40 years (72.5%), cancer (33.8%), bed rest (31.2%), obesity (27.5%) and minor surgery (12.5%). Early mobilization was performed in all patients. Pharmacologic prophylaxis was required in 80% of our patients but only 38.8% had received low molecular weight heparin. VTE complicated 5 procedures, including 3 cases of deep vein thrombosis of the limbs and 2 cases of pulmonary embolism who died. Venous thromboembolism is serious and common in surgical settings. Its prevention remains insufficient in our context.

Otological Injuries Observed in Head and Neck Trauma in a Second-Line Hospital

Otological injuries are common during cervicofacial trauma. The aim of this study is to describe the epidemiological characteristics and to map otological injuries during head and neck trauma. This was an observational, descriptive, prospective study conducted at the ENT unit of the Referral Health Center Hospital in Mali from January 2017 to September 2018. Patients with middle and/or inner ear injuries due to trauma were included. The prevalence of otologic injuries was 34.54% of cases. The mean age was 25 years and the sex ratio was 1.11. Students accounted for 42.10%. The mechanism of occurrence was an intentional assault in 68.42% of the cases followed by road traffic accidents (15.79%). The most common functional signs were hearing loss (68.42%), tinnitus (57.89%), and facial paralysis in 5.26%. Otoscopy showed tympanic perforation (47.83%), otorrhagia (21.05%) and otoliquorrhea (5.26%). Rock fractures with osteodural breach (10.53%) and pneumencephaly were found on a CT scan of the rock. The evolution was marked by the regression of the facial paralysis and the healing of the tympanum in 45.45% of the cases. Sequelae persisted in the form of sensorineural deafness (17%). The otological lesions observed during cervicofacial trauma are varied. The challenges in our context are felt at three levels: the improvement of the technical platform, the long-term follow-up of patients, and the cost of care in terms of equipment. These lesions are responsible for disabling deafness, a source of difficulty in social and school integration.

A new enolate derivative of furostanol glycoside, named asparagusin A, was isolated from the roots of Asparagus filicinus and established as 3-O-beta-D-xylopyranosyl(1-->4)-beta-D-glucopyranosyl (25S)-furost-20(22)-ene-3 beta, 26-diol 26-O-beta-D-glucopyranoside (1) by spectroscopic and chemical methods. Asparagusin A (1) exhibited a cytotoxic activity effect on PC12 cells.

Application of the Dempster-Shafer Theory to the Classification of Pixels from Aster satellite Images and Spectral Indices

In this paper, it is proposed to apply the Dempster-Shafer Theory (DST) or the theory of evidence to map vegetation, aquatic and mineral surfaces with a view to detecting potential areas of observation of outcrops of geological formations (rocks, breastplates, regolith, etc.). The proposed approach consists in aggregating information by using the DST. From pretreated Aster satellite images (geo-referencing, geometric correction and resampling at 15 m), new channels were produced by determining the spectral indices NDVI, MNDWI and NDBaI. Then, the DST formalism was modeled and generated under the MATLAB software, an image segmented into six classes including three absolute classes (E,V,M) and three classes of confusion ({E,V}, {M,V}, {E,M}). The control on the land, based on geographic coordinates of pixels of different classes on said image, has made it possible to make a concordant interpretation thereof. Our contribution lies in taking into account imperfections (inaccuracies and uncertainties) related to source information by using mass functions based on a simple support model (two focal elements: the discernment framework and the potential set of belonging of the pixel to be classified) with a normal law for the good management of these.

Quantitative Structure Anti-Cancer Activity Relationship (QsaR) of a Series of Ruthenium Complex Azopyridine by the Density Functional Theory (DFT) Method

A series of ruthenium azopyridine complexes have recently been investigated due to their potential cytotoxic activities against renal cancer (A498), lung cancer (H226), ovarian cancer (IGROV), breast cancer (MCF-7) and colon cancer (WIDR). Thus, in order to predict the cytotoxic potentials of these compounds, quantitative structure-activity relationship studies were carried out using the methods of quantum chemistry. Five Quantitative Structure Activity Relationship (QsaR) models were obtained from the determined quantum descriptors and the different activities. The models present the following statistical indicators: regression correlation coefficient R2 = 0.986 - 0.905, standard deviation S = 0.516 - 0.153, Fischer test F = 106.718 - 14.220, correlation coefficient of cross-validation = 0.985- 0.895 and = 0.010 - 0.001. The statistical characteristics of the established QsaR models satisfy the acceptance and external validation criteria, thereby accrediting their good performance. The models developed show that the variation of the free enthalpy of reaction , the dipole moment μ and the charge of the ligand in the complex Ql, are the explanatory and predictive quantum descriptors correlated with the values of the anti-cancer activity of the studied complexes. Moreover, the charge of the ligand is the priority descriptor for the prediction of the cytotoxicity of the compounds studied. Furthermore, QsaR models developed are statistically significant and predictive, and could be used for the design and synthesis of new anti-cancer molecules.

Primary Naso-Sinus Tuberculosis (NST) at the ORL and Cervico facial Surgery Department of the University Hospital of Cocody: About a Case

Objective: To report a case of naso-sinus tuberculosis and discuss its diagnosis and management in our context. Clinical Case: A 37-year-old patient, with no previous history, visited us for a swelling under left orbital including a nasal obstruction and intermittent frontal headaches, which has been developed since about one year. Clinical Examination: The anamnesis founds no notion of epistaxis, visual disability, nor notion of fever. It has been noted a swelling under left angulo orbital during physical examination, a burgeoning mass filling the left nasal including a Lymphadenopathy under homolateral mandibular. Para clinical investigations in particular CT scan of the sinuses have highlighted a tissue process under orbital with a left naso-ethmoido-maxillary extension. The anatomical pathologic examination of the biopsy carried out through endoscopy permitted to highlight epithelioid giganto-cell granulomas with caseous necrosis evoking tuberculosis. The patient has been prescribed an anti-tuberculosis treatment. The course of illness is marked by the healing of the patient.

Primary Cerebral Lymphoma: About 22 Cases with Literature Review

Primitive cerebral lymphoma (PCL) is a rare entity often poorly known. They are defined as extra nodal tumor interesting the CNS, brain and eye, and this in the absence of systemic lymphoma disease and brain metastases. The favoring factor is established immunosuppression which is explained by an increase in the incidence related to HIV. The basis of treatment is chemotherapy with consolidation radiotherapy. We report the experience of the National Institute of Oncology in Rabat in management of this disease.
