The adaptation of cassava plantlets to natural conditions remains the main obstacle in the process of free virus plantlets production. In this study, cassava plantlets acclimatization was assessed by pre-acclimatization, height of acclimated plantlets, substrate, and nutrient watering solution. The results revealed that pre-acclimatization gave a high survival rate (61.9%) whereas the direct tunnel acclimatization was conducted to low rate (27.31%) of plantlets. Furthermore, the height of plantlets influenced significantly (p < 0.05) their survival rate with best survival rate observed on plantlet height from 1 cm to 5 cm. There was no significant difference between the substrates used and watering solutions. However, the Sawdust substrate gave a high survival rate (46.67%) and better phylogenesis (1.8) in the variety RB 89509. Likewise, plantlets treated with NPK nutrient watering solutions (20-10-10) had a better survival rate (48.57%), better phylogenesis (2.125) with the variety RB 89509 and better growth in height (1.18 cm) with the variety BF92/0267. These findings constitute a database for the establishment of the technical itinerary for the acclimatization of cassava plantlets.

Purpose: Primary malignant musculosqueletal tumors in adult are rare affections group and its treatment is still a real challenge today. The aim of this work was to describe the epidemiologic and treatment aspects of these tumors in the national reference center of our country. Materiel and Methods: It was a retrospective review of primary malignant musculosqueletal tumors treated in orthopedics and trauma unit of Sylvanus Olympio teaching hospital of Lomé, Togo from January 2000 to December 2014. Results: During the study period, 28 cases were selected and reviewed. There were 17 men (60.71%) and 11 women (39.29%). The average age was 32.7 years. There were 20 cases (71%) of primary malignant bone tumors (PMBT) and 8 cases (29%) of primary malignant soft tissues tumors (PMSTT). There were 6 (30%) of osteosarcoma. The tumor was located in the bones of the forearm and wrist/hand in 2 (10%) patients for each anatomical site;for the lower limb, around knee in 7 (35%) patients. Eight patients had PMSTT (28, 57%). There were three cases of rhabdomyosarcoma, 2 cases of fibrosarcoma. Tumors were located around knee in 4 cases and around ankle/foot in 3 cases. In the two groups of tumor, tumor resection was performed in 5 patients (17.86%) and limb amputation indicated in 23 patients (82.14%), was performed in 15 (53.57%) and 8 patients (37.78%), rejected it and left hospital against medical advice. Conclusion: Malignant musculoskeletal tumors are relatively rare in Togo. Their treatment is based on radical surgery which is often not supervised by adjuvant therapies. Ignorance, poverty of the population and embryonic state of diagnostic and treatment infrastructures are the obstacles to their care. Education and awareness must be integrated to care and fight against this group of diseases.

Air pollution is today a real concern across the world, responsible for around 7 million deaths per year around the world. It is currently involved in the appearance and worsening of several non-transmissible pathologies, making it an environmental, health, and societal problem. The objective of this work was to evaluate the effects of short-term exposure to air pollution on the level of sex hormones in mechanics. We conducted a cross-sectional, analytical, prospective case-control study in Brazzaville on 228 patients (76 cases and 152 controls) from June 2020 to December 2022, a period of 30 months. This study made it possible to evaluate, on the one hand, the level of sexual hormones in men exposed to automobile pollution according to the automated method (ELFA) on mini vidas and, on the other hand, the quality of the air by a colorimetric, punctual method on a Dräger tube coupled with a Dräger accuro pump. The results obtained made it possible to establish a statistically significant link between automobile pollutants (CO, CO2, NO2, SO2) and the increase in LH and FSH on the one hand and the decrease in testosterone on the other hand. We also noted that the concentrations of automotive pollutants during this study were all above the standards required by the WHO. Exposure to automobile pollutants affects the hypothalamic-pituitary axis and therefore disrupts the level of sex hormones.

Knowledge, Attitudes and Practices of Diabetic Patients on Ocular Complications of Diabetes in Lomé(Togo)

Introduction: The frequency and severity of eye complications from diabetes make patient education essential, which is the basis for adequate management. What is the level of education of diabetic patients in Lomé? The aim of this study was to assess the knowledge, attitudes and practices of diabetic patients on the ocular complications of diabetes in Lomé. Methods: We conducted a cross-sectional study by surveys on diabetic patients from April 1 to June 31, 2020, in 6 diabetes management centers in Lomé. The correct response rate was established for each component. Knowledge: poor (≤ 50%), low (50% - 65%), medium (65% - 85%) and good (≥ 85%). Attitudes: harmful (≤ 50%), erroneous (50% - 65%), approximate (65% - 85%) and fair (≥ 85%). Practice: harmful (≤ 50%), inadequate (50% - 65%) and adequate (>65%). Results: Over the study period, 150 patients were enrolled. The mean age was 56.98 ± 13.37 years with a sex ratio of 1.1 (79 men/71 women). The overall level of knowledge was insufficient (50.6%) with 65.5% for general knowledge about diabetes and 35.8% for ocular complications of diabetes. Attitudes were approximate (68.7%). In the case of hyperglycemia, 31.3% of patients would confide in a diabetologist and 22.0% in a general practitioner. If decrease in vision, 84.3% of patients would contact the ophthalmologist. The level of practice was harmful (20.4%). For diabetic follow-up, 36.7% of patients have already seen an ophthalmologist, 41.3% have performed a fundus examination, 17.3% retinal angiography, 4% laser retinal photocoagulation and 2.7% intravitreal injection (IVT). An awareness session on the ocular complications of diabetes was followed by 52.7% of patients and 50.7% of patients followed a therapeutic education session. Conclusion: This study has shown that the level of knowledge was poor, attitudes approximate and practices harmful. It is therefore important to raise awareness about diabetes and its ocular complications with the aim to change behavior.

Epidemiology of Open Limb Fractures in a Country with Low-Income

Introduction: Open limb fractures are frequent and severe. Very often they result from high velocity traffic accidents. The aim of this study was to identify the epidemiologic aspects of open limb fractures in order to improve prevention and care. Material and Methods: It was a retrospective study carried out in the Department of Orthopaedic and traumatology Surgery of Afagnan hospital. It concerned the files of patients admitted from January 1st to December 31st 2016. Patients of 15 years old and older who presented open limb fractures were included in the study. The study variables were: age, sex, occupation, trauma aetiology, injury mechanism, location and type of lesions. An X-ray (antero-posterior and lateral view) of the injured limb segment was done in all patients. Results: On 87 patients, 148 cases of fractures were collected. The mean age was 38.08 years with extremes of 15 and 82. There were 23 women (26.4%) and 64 men (73.6%). The most common etiology was traffic accidents (73.5%). Motorcycle accidents (58.3%);accidents involving motorcycle and car (19.4%). Accidents at work represented (16.1%). Lower limbs were more affected (77.1%). Open fractures classified Gustillo-Anderson type III were predominant with type IIIa (51.0%), IIIb (32.6%) and IIIc (16.3%). Conclusion: Open limb fractures are severe. They are mainly due to the increase of high-velocity traffic accidents. This explains the predominance of the young male population. Road safety and all actors’ behaviour change are essential for the reduction of traffic accident morbidity.

Outpatient Management of Sickle Cell Disease: Assessment of 13 Years Follow-Up of Pediatric Patients at the Sylvanus Olympio University Hospital

Objective: To evaluate thirteen years of ambulatory follow-up of patients with major forms of sickle cell disease. Methods: This was a study of 1055 records of sickle cell patients aged 6 months to 45 years followed between 2006 and 2018. Results: Six hundred and eighty-one (64.5%) homozygous SS, 283 (26.8%) SC, 86 (8.2%) SF, 4 (0.4%) S? thalassemia and 1 (0.1%) SD sickle cell patients were followed. The majority of the patients (97.9%) were living in the capital city of Lomé and its surrounding suburbs. Most children (67.3%) were less than 5-year-old when they were diagnosed with sickle cell disease, and only 21.3% of cases were discovered before the first year birthday of the patients. Osteo-articular vaso occlusive crisis (VOC) was the principal symptom at the onset of the diagnosis, approximately 28.9% of the cases, followed by severe anemia (19%), then hand-foot syndrome in 15.6%. The systematic screening was performed in 5.4%. Twenty-six percent were G6PD deficient, 4 were HIV immunocompromised, and 29.7% were transfused. Among the degenerative complications, 2.4% had retinopathy, 1.9% osteonecrosis of the femoral head, 1.4% leg ulcers, and 0.8% cardiomyopathy. Salmonella typhi was the main cause of infection, 51.4% of the cases. The mean baseline hemoglobin level (n = 27) was 7.67 ± 1.64 g/dl. Four (0.4%) patients had a splenectomy. The mortality rate was 2.4%. The primary cause of death was anemia (28%), followed by sepsis in 20% of the cases. One case of suicide was documented as the cause of death. Conclusion: Outpatient medical follow-up of sickle cell patients is essential in sub-Saharan African countries where neonatal screening does not exist, and diagnosis is often late. The improvement in quality of life observed should be studied.

Results of Chronic Neck Pain Rehabilitation at Cotonou

Background: Neck pain is a real public health problem. Its treatment uses different techniques, such as functional rehabilitation, whose results are not widely popularized, from developing countries in Black Africa. Objective: To assess the result of rehabilitation of chronic common neck pain (CCNP) at the CNHU-HKM in Cotonou. Methods: Cross-sectional, retrospective, descriptive and analytical study. It was carried out from May to September 2020, based on the files of former patients followed in functional rehabilitation department at CNHU-HKM in Cotonou, from 2015 to 2019, for CCNP. The result of rehabilitation was assessed based on the progress, between the start and the end of the rehabilitation sessions, of pain intensity, mobility of the cervical spine, muscle strength and functional capacity of the patient. Anova and chi-square tests were used for the analysis of factors associated with the result of rehabilitation. Results: The sample consisted to 73 patients. They were predominantly female (58.90%), with a mean age of 53.60 ± 14.08 years. The neck pain, at least of moderate intensity (97.26%), was present since 12.54 ± 8.54 months, on average. Patients have done 10 to 20 functional rehabilitation sessions, the result of which was satisfactory in 67.12%. This result was mainly associated with patient’s age, sports practice and the seniority of pain progression. Discussion-Conclusion: Rehabilitation results in the management of CCNP are interesting. They prove the need to sensitize patients and prescribers on the importance of early treatment of CCNP.

Introduction: In December 2021, Beninese soldiers were victims of armed terrorist attacks. This study aims to determine the psychological impact of the events experienced on the victims in order to better prevent post-traumatic stress disorder. Materials and methods: This is a descriptive cross-sectional study conducted from December 13 to 14, 2021, i.e. 12 days after the oldest event and 4 days after the most recent. The experience of the event was assessed with the Peritraumatic Distress Inventory and the Peritraumatic Dissociation Experiences Questionnaire. Symptoms experienced in the hours and days after the event were assessed with the Revised Event Impact Scale. Results: 36 soldiers were included out of the 38 survivors, i.e. 94.73%. They were 51.43% victims of the night assault, 37.14% of the ambush on patrol and 11.43% of the explosion of vehicle on mine, all male. The average age was 31 years with extremes of 25 and 49 years. The 25 - 30 year olds predominated, 52.78%. 100% of soldiers had significant peritraumatic dissociation, of which 94.44% also had significant peritraumatic distress. 100% of the soldiers had manifestations of intrusion, 89% of manifestations of avoidance and 83% of manifestations of hyperexcitation. 88.89% had an index for acute stress including 100% of victims of night assault and vehicle explosion on mine. Conclusion: This study shows a high prevalence of peritraumatic reactions, recognized factors favoring post-traumatic stress disorder, and the interest of their evaluation for medical and psychological help adapted to the victims.

Background: Although proptosis is rare, it is nevertheless the main sign of orbital disease. Its discovery usually indicates a serious eye disease especially in children. Objective: To determine epidemiological and diagnostic aspects of childhood proptosis in Lome (Togo). Methods: Retrospective cross-sectional study conducted from June 2010 to May 2020, which is 10 years, in the ophthalmology department of CHU-campus of Lome. Files of patients between the age of 0 and 15 who presented a proptosis during the study period were taken into account in the study. Patients with a false proptosis or incomplete records during the study period were not taken into account in the study. Results: The study considered 42 children with an age average of 7.63 years ± 4.96 [4 days;15 years] and a sex-ratio of 1.33. The frequency of proptosis was 0.5% with an average progression of 383.7 days before the first consultation. The proptosis was unilateral in 80.95% of cases. The orbito-cerebral CT scan was done in 47.62% of cases. The proptosis in these patients was predominantly of grade 3 in 45% of cases. The tumor pathologies were at the forefront in 35.71%, dominated by retinoblastoma, and followed by infectious and inflammatory diseases in 14.29%. Conclusion: Childhood proptosis is rare and often indicative of infectious and tumor pathologies in our context. Its diagnosis is delayed, which shows the importance of an early and effective diagnosis. .

Introduction: Kawasaki disease (KD) is a severe febrile vasculitis that affects children under 5 years of age. The severity of KD is related to coronary involvement. Few cases have been reported in sub-Saharan Africa. Objective: We present a clinical observation of a Togolese infant to illustrate the typical clinical picture that every pediatrician should know. Observation: The patient was a 20-month-old infant from Togo who presented with conjunctivitis, cheilitis, stomatitis, and pharyngitis in a febrile setting. There was a satellite sub-maxillary adenopathy. There was a persistent biological inflammatory syndrome with thrombocytosis and a doppler ultrasound confirmed the diagnosis of a coronary aneurysm. Treatment was based on corticosteroid therapy and acetylsalicylic acid with success. Conclusion: KD involves cardiac complications that require rigorous and sometimes long-term monitoring to prevent death.
