Efficacy of percutaneous radiofrequency ablation for hepatocellular carcinoma treatment in patients aged≥80 years

Background:Although radiofrequency ablation(RFA)is a minimally invasive treatment for early-stage hepatocellular carcinoma(HCC),it remains unclear whether RFA achieves favorable outcomes in pa-tients aged≥80 *** study aimed to determine the efficacy and safety of RFA for HCC in patients aged≥80 ***:A total of 512 naïve patients with HCC who had undergone RFA from January 2001 to December 2016 were *** were categorized into the≥80-year-old group and the control group(aged<80 years).The primary endpoint was overall survival(OS),and the secondary endpoints were recurrence-free survival,complications associated with RFA,and cause of *** score matching was performed to adjust for patients’sex,liver function,tumor number,tumor diameter,and hepatitis C virus ***,the data of 68 patients in the≥80-year-old group and 68 in the control group were analyzed;their baseline characteristics,primary endpoint,and secondary endpoints were ***:There were significant differences in the alanine aminotransferase level and prothrombin time between the *** cumulative OS rate was not significantly different between the groups(P=0.83):98.5%,87.9%,and 50.5%in the≥80-year-old group and 94.1%,72.8%,and 49.3%in the control group at 1,3,and 5 years,***≥80 years was not significantly associated with OS in multivariate ***-related death occurred in 17 patients in the≥80 year-old group and in 16 patients in the control group(P=1.00).Conclusions:RFA is safe and effective for the treatment of patients with HCC aged≥80 years.

AIM:To evaluate the effect of computed tomography(CT) attenuation values of ascites on gastrointestinal(GI) perforation site ***:The CT attenuation values of the ascites from 51 patients with GI perforations were measured by volume rendering to calculate the mean *** effect of the CT attenuation values of the ascites on perforation site prediction and postoperative complications was ***:Of 24 patients with colorectal perforations,the CT attenuation values of ascites were significantly higher than those in patients with perforations at other sites [22.5 Hounsfield units(HU) vs 16.5 HU,respectively,P = 0.006].Colorectal perforation was significantly associated with postoperative complications(P = 0.038).The prediction rate of colorectal perforation using attenuation values as an auxiliary diagnosis improved by 9.8% compared to that of CT findings alone(92.2% vs 82.4%).CONCLUSION:The CT attenuation values of ascites could facilitate the prediction of perforation sites and postoperative complications in GI perforations,particularly in cases in which the perforation sites are difficult to predict by CT findings alone.

Ecotourism has traditionally been viewed as a panacea concept for developing countries to stimulate the economy, as well as, directly provide support for conservation efforts. This study examined the concept of ecotourism for a relatively sustainable, single activity within an established tourism destination of Wasini Island in Kwale County. The purpose of the study was to assess the contribution of ecotourism initiatives to the livelihoods of the local community, guided by the following objective： to assess the attitude of the local community towards ecotourism development. Descriptive survey research was adopted for the study. The target population included all ecotourism entrepreneurs, conservationists, community based organizations （CBOs）, and tour operators based in Wasini Island. Systematic random sampling was used to select 320 respondents. Primary data were collected using structured questionnaires while secondary data were obtained through critical examination of public and private recorded documents related to the study. Data were analyzed using techniques which included frequencies, means, modes and percentages. Ecotourism initiatives in the study area have significantly contributed to job creation and financial empowerment to the local people. Ecotourism in the study area has contributed to the acquisition of both human （education） and physical assets owned by the local community. Environmental conservation has also been achieved through ecotourism initiatives. In the face of joblessness, residents of Wasini need to be capacitated to venture into ecotourism projects as it has the potential of improving livelihoods.

Objective:In recent years,minimally invasive surgery has been emphasized in gynecological surgery,and total laparoscopic hysterectomy has been increasingly *** this retrospective single-center study,the main objective was to identify risk factors for the development of surgical-site infection(SSI)after total laparoscopic *** secondary objective was to investigate the efficacy of transvaginal drainage as a treatment for ***:This retrospective study investigated 377 patients who underwent total laparoscopic hysterectomy in the Department of Obstetrics and Gynecology at the Saitama Medical Center,Saitama Medical University,Japan between January 1,2015 and December 31,*** were divided into the SSI group and non-SSI group based on whether they suffered from *** data of preoperative,intraoperative,and postoperative risk factors for SSI were collected and ***:Of the 377 patients who underwent total laparoscopic hysterectomy,21 patients were in the SSI group and 356 patients were in the non-SSI *** the comparison between the two groups and the multivariate analysis,only the C-reactive protein level on postoperative day 3(OR¼1.556,95%CI:1.233 e1.964,p7.6 mg/dL on postoperative day 3 could detect the SSI onset *** the SSI group,the hospital stay was longer for patients with transvaginal drainage than for patients without(17.40±3.21 d vs.10.90±2.39 d,p¼0.0027).However,none required ***:Patients with a high postoperative C-reactive protein level may be experiencing SSI,and ultrasonography and other imaging procedures should be performed *** a vaginal abscess is confirmed on imaging,transvaginal drainage should be performed early,which may prevent reoperation.

BACKGROUND Amebic colitis is an infection caused by Entamoeba histolytica and most commonly observed in regions with poor *** is also seen as a sexually transmitted disease in developed *** amebic colitis usually has a chronic course with repeated exacerbations and remissions,it may also manifest as a fulminant form that rapidly progresses and leads to severe,life-threatening complications,such as intestinal perforation,peritonitis,and sepsis,that have a high mortality *** SUMMARY A 68-year-old man was admitted to our hospital with chest pain and acute *** was diagnosed with acute coronary syndrome,acute heart failure,and bacterial *** respiratory condition worsened despite receiving intensive care and intravenous *** the fifth day of hospitalization,he was diagnosed with acute respiratory distress syndrome and was started on steroid *** subsequently developed bloody stools and was diagnosed with cytomegalovirus(CMV)enterocolitis based on biopsy results and a peripheral blood CMV pp65 antigenemia test *** we started antiviral therapy with ganciclovir,which was successful in reducing his antigen titers,he continued to have bloody *** weeks after initiation of ganciclovir therapy and six weeks after his admission,the patient died from intestinal *** only posthumously diagnosed him with amebic colitis and CMV enterocolitis based on autopsy findings of transmural necrosis of the entire colon with massive ameba *** We urge clinicians to consider Entamoeba histolytica infection if severe colitis progresses after steroid *** treatment is recommended then.

Detecting Bank Conflict of GPU Programs Using Symbolic Execution—Case Study

GPU (Graphics Processing Unit) is used in various areas. Therefore, the demand for the verification of GPU programs is increasing. In this paper, we suggest the method to detect bank conflict by using symbolic execution. Bank conflict is one of the bugs happening in GPU and it leads the performance of programs lower. Bank conflict happens when some processing units in GPU access the same shared memory. Symbolic execution is the method to analysis programs with symbolic values. By using it, we can detect bank conflict on GPU programs which use many threads. We implement a prototype of the detector for bank conflict and evaluate it with some GPU programs. The result states that we can detect bank conflict on the programs with no loop regardless of the number of threads.

Most of the researches on profit and cost evaluation of redundant system focus on the effect of failure and repair on revenue generated. However, as these systems continue to work, their strength gradually deteriorates. Where such deterioration occurs, minor and major maintenance is employed to remedy the deterioration. Little or no attention is paid on the effect of deterioration on the impact of deterioration and their maintenance on the revenue generated. In this paper, we study the profit generated of two-stage deteriorating linear consecutive 2-out-of-3 system. Failure, repair and deterioration time are assumed exponential. The explicit expressions of availability, busy period of a repairman and profit function are derived using Kolmogorov’s forward equations method. Various cases are analyzed graphically to investigate the effect of deterioration parameters such as slow deterioration, fast deterioration, and their maintenance such as minor and major minimal maintenance on profit generated.

In this work, the reaction cross-section for neutron-induced reactions on 127I isotope was calculated using EXIFON code in the energy range of incident particle from 0 MeV to 30 MeV. The code is based on an analytical model for statistical multistep direct and multistep compound reactions (SMD/SMC model). In order to see the effect of nuclear structure on cross sections, the calculation was done using nuclear shell structure effect and without considering shell structure effect. Obtained results and statistical analysis showed that shell structure effect does not give significant changes to the cross-section at considered energy ranges. This shows that EXIFON code is a good tool for investigation of nuclear reaction cross section and is useful in the production of the radioisotopes of Iodine, Antimony and tellurium of high purity and in an efficient manner using cyclotron or nuclear reactors, these isotopes have potential application for field of medical science especially for diagnostics and therapeutic purposes.
