Prognosis following transcatheter arterial embolization for 121 patients with unresectable hepatocellular carcinoma with or without a history of treatment

AIM： To retrospectively evaluate the prognosis of patients with hepatocellular carcinoma （HCC） with or without a history of therapy for HCC following transcatheter arterial embolization （TAE）. METHODS： One hundred and twenty-one patients with HCC treated with TAE from 1992 to 2004 in our hospital were enrolled in this study. Eighty-four patients had a history of treatment for HCC, while 37 did not. At the time of entry, patients with extra-hepatic metastasis, portal vein tumor thrombosis, or Child-Pugh class C were excluded. TAE was repeated when recurrence of HCC was diagnosed by elevated tumor markers, or ultrasonography or dynamic computed tomography findings. RESULTS： Tumor size was larger and the number of tumors was fewer in patients without past treatment （P〈0.01）. However, there were no differences in tumor node metastasis （TNM） stage or survival rate between the 2 groups. A bilobular tumor and high level of α-fetoprotein （AFP）（〉100 ng/mL） were factors related to a poor prognosis in patients with a history of HCC. CONCLUSION： The prognosis following TAE is similar between HCC patients with and without past treatment. Early diagnosis of HCC or recurrent HCC and obtaining good local control against HCC before entry to a repeated TAE course can improve prognosis.

Background:Lenvatinib is used for unresectable hepatocellular carcinoma(u-HCC)as first-line,as well as second-and third-line therapy in *** evaluated the therapeutic efficacy of newly developed ramucirumab when given after lenvatinib for post-progression ***:Of 385 patients with u-HCC and treated with lenvatinib at 16 different institutions in Japan between May 2018 and January 2020,28 who received ramucirumab as the next treatment were enrolled and therapeutic responses were evaluated in a retrospective ***:The median age of the 28 patients given ramucirumab was 70 years and the median albumin-bilirubin score *** the 28 patients,23 were male,21 were classified as Child-Pugh A and 7 as Child-Pugh B,and 25 were Barcelona Clinic Liver Cancer Stage *** was given as second-line therapy in 14,third-line in 9,and fourth-line in *** response was obtained in only 26 patients;the objective response rate was 3.8%(1/26)and the disease-control rate was 42.3%(11/26),with a median period to progression of 2.0 *** reasons for discontinuation of ramucirumab were progression of disease in 16 and Grade 3 adverse events(gastrointestinal bleeding,ascites)in ***:The anticipated therapeutic efficacy of ramucirumab for post-progression treatment following lenvatinib was not seen in our early experience.

BACKGROUND Postoperative aortobronchial fistula(ABF)is a rare complication that can occur in 0.3%-5.0%of patients over an extended period of time after thoracic aortic *** visualization of the fistula via imaging is *** To investigate the relationship between computed tomography(CT)findings and the clinical signs/symptoms of ABF after thoracic aortic *** Six patients(mean age 71 years,including 4 men and 2 women)with suspected ABF on CT(air around the graft)at our hospital were included in this retrospective study between January 2004 and September *** CT findings included direct confirmation of ABF,peri-graft fluid,ring enhancement,dirty fat sign,atelectasis,pulmonary hemorrhage,and bronchodilation,and the clinical course were retrospectively *** proportion of each type of CT finding was *** ABF detection after surgery was found to have a mean and median of 14 and 13 years,*** signs and symptoms were asymptomatic in 4 patients,bloody sputum was found in 1 patient,and fever was present in 1 *** complications of ABF included graft infection in 2 patients and graft infection with hemoptysis in 2 *** the 6 patients,3 survived,2 died,and 1 was lost to *** locations of the ABFs were as follows:1 in the ascending aorta;1 in the aortic arch;2 in the aortic arch leading to the descending aorta;and 2 in the descending *** were directly confirmed by CT in 4/6(67%)***-graft dirty fat(4/6,67%)and peri-graft ring enhancement(3/6,50%)were associated with graft infection,endoleaks and pseudoaneurysms were associated with hemoptysis(2/6,33%).CONCLUSION Asymptomatic ABF after thoracic aortic surgery can be confirmed on chest *** is useful for the diagnosis of ABF and its complications.

用近震源波形资料拟合反演地震的震源破裂过程 ,所包含的一些不确定因素将对反演结果的精度及可靠性产生影响 .文中的数值实验分析了所假定的反演断层模型参数的某些不确定性对反演结果的影响程度 ,并对观测波形的截取长度对反演精度的影响进行了讨论 .结果表明 :(1)近震源地震波形资料能较好地分辨断层浅部的破裂过程 .然而对断层深部的位错分布的约束和反演能力较差 .联合使用近、远场地震波资料进行反演 ,能反演出一个更为完全的整个断层破裂过程的图像 .(2 )用近震源地震波资料反演时 ,反演结果对所假定的反演断层的走向和倾角非常敏感 .断层走向偏离真实值 2°或倾角偏离真实值 5°都会导致一个虚假的反演结果 .(3)反演中所使用的介质速度结构模型的不确定性 ,也会对反演结果产生影响 .

A new method for analysis of lichen triterpenoids was established using high performance liquid chromatography with the combination of a differential refractive index detector(RID) and a photodiode array detector(PDA).It is proved that this method was convenient to detect and identify aromatic and aliphatic lichen substances;it enabled quantitative analysis of substances having no or less absorption of ultraviolet rays such as *** addition,they can be measured in high accuracy compared with the TLC method.

Gain-of-function mutations in the STING-encoding gene TMEM173 are central to the pathology of the autoinflammatory disorder STING-associated vasculopathy with onset in infancy(SAVI).Furthermore,excessive activity of the STING signaling pathway is associated with autoinflammatory diseases,including systemic lupus erythematosus and Aicardi–Goutières syndrome(AGS).Two independent studies recently identified pharmacological inhibitors of ***,both types of compounds are reactive nitrocontaining electrophiles that target STING palmitoylation,a posttranslational modification necessary for STING *** a consequence,the activation of downstream signaling molecules and the induction of type I interferons were *** compounds were effective at ameliorating inflammation in a mouse model of AGS and in blocking the production of type I interferons in primary fibroblasts from SAVI *** mini-review focuses on the roles of palmitoylation in STING activation and signaling and as a pharmaceutical target for drug development.
