Recent trends in bone metastasis treatments:A historical comparison using the new Katagiri score system

BACKGROUND Bone metastasis has various negative impacts.Activities of daily living(ADL)and quality of life(QOL)can be significantly decreased,survival may be impacted,and medical expenses may increase.It is estimated that at least 5%cancer patients might be suffering from bone metastases.In 2016,we published the Comprehensive Guidelines for the Diagnosis and Treatment of Bone Metastasis.Since then,the therapeutic outcomes for patients have gradually improved.As life expectancy is a major determinant of surgical intervention,the strategy should be modified if the prolongation of survival is to be achieved.AIM To monitor how bone metastasis treatment has changed before and after launch of our guidelines for bone metastasis.METHODS For advanced cancer patients with bone metastasis who visited the Department of Clinical Oncology at Akita University hospital between 2012 and 2023,parameters including the site and number of bone metastases,laboratory data,and survival time,were extracted from electronic medical records and the Katagiri score was calculated.The association with survival was determined for each factor.RESULTS Data from 136 patients were obtained.The 1-year survival rate for the poor prognosis group with a higher Katagiri score was 20.0%in this study,which was 6%and an apparent improvement from 2014 when the scoring system was developed.Other factors significantly affecting survival included five or more bone metastases than less(P=0.0080),and treatment with chemotherapy(P<0.001),bone modifying agents(P=0.0175)and immune checkpoint inhibitors(P=0.0128).In recent years,advances in various treatment methods have extended the survival period for patients with advanced cancer.It is necessary not only to simply extend survival time,but also to maintain ADL and improve QOL.CONCLUSION Various therapeutic interventions including surgical approach for bone metastasis,which is a disorder of locomotor organs,are increasingly required.Guidelines and scoring system for prognosis need to be revised promptly.

BACKGROUND Angiogenesis inhibitors(AIs)combination with cytotoxic chemotherapy is a promising treatment for patients with colorectal cancer(CRC).Aflibercept(AFL)is an option for second-line treatment of CRC,according to the‘VELOUR’trial.Currently,we can choose from three AIs,including bevacizumab,ramucirumab,and AFL.Different AIs can be used in subsequent treatment because of their distinctive mechanisms of action.We addressed the uncertainty regarding AFL efficacy and safety in heavily-treated patients by comparing outcomes of survival treatment with second-line treatment.AIM To determine and compare the efficacy and safety profiles of AFL in the secondline and salvage therapy settings.METHODS Clinical data of 41 patients with advanced CRC who received intravenous AFL combined with the folinic acid-fluorouracil-irinotecan(FOLFIRI)regimen were collected retrospectively from six institutions in Japan,for the period from May 2017 to March 2019.Patient characteristics collected included age,sex,tumor location,RAS and RAF status,metastatic sites,number of previous treatment cycles,therapeutic response,adverse events,duration of previous AI treatment,and survival time.The end points were time to AFL treatment failure(aTTF)and median survival time post-AFL(aMST).Statistical analyses were performed to compare the efficacy and safety in the second-line setting with those of the salvage therapy setting,which was defined as the days since the end of secondline therapy.RESULTS All 41 patients who received AFL+FOLFIRI for advanced CRC had metastatic or unresectable cancer.Twenty-two patients received AFL in the second-line setting and nineteen in the salvage therapy setting.The patient characteristics were similar in the two groups,except for two factors.The median duration of the previous AI administration was shorter in the second-line patients compared with that in the salvage therapy patients(144 d vs 323 d,P=0.006).In the second-line and salvage therapy groups,the objective response rates were 11%and 0%,respectively(P=0

High circulating tumor cell concentrations in a specific subtype of gastric cancer with diffuse bone metastasis at diagnosis

AIM: To clarify the biological feature contributing to gastric cancer with diffuse bone metastases at diagnosis.METHODS: The participants visited the Department of Clinical Oncology, Akita University Hospital, from January 2014 to August 2015. The selection criterion for gastric cancer with diffuse bone metastases at diagnosis includes over 29 hot spots of bone scintigraphy. Circulating tumor cell were collected from 20 m L of peripheral venous blood drawn using a Cell Search kit and a Cell Tracks Auto Prep system by SRL, a clinical laboratory. The endpoints of this study were correlations between circulating tumor cells(CTC) count and therapeutic outcomes. RESULTS: Among 39 patients with gastric cancer, 5 patients met the criterion. The incidence of this subtype was 12.8%. CTC counts ranged from 235 to 6440 cells/7.5 m L of peripheral blood(median of 1724). These values were much higher than common gastric cancers(2 cells). In chemo-sensitive cases, CTC counts decreased within 14 d(median) from 275, 235 and 1724 to 2, 7 and 66, respectively. On the other hand, CTC counts increased after treatment failure or insensitive case from 2, 7 and 6440 to 787, 513 and 7885, respectively. The correlation between CTC count and survival time showed a trend, but did not reach significance(Y = 234.6- 0.03 X, P = 0.085).CONCLUSION: High CTC count is a biological hallmark of this subtype, and can be used as a direct and definitive indicator of therapeutic outcome.

Acute and delayed psychiatric sequelae among patients hospitalised with COVID-19:a cohort study using LIFE study data

Background Characterising the psychiatric sequelae of coronavirus disease 2019(COVID-19)can inform the development of long-term treatment strategies.However,few studies have examined these sequelae at different time points after COVID-19 infection.Aims The study aimed to investigate the incidences and risks of acute and delayed psychiatric sequelae in patients hospitalised with COVID-19 in Japan.Methods This retrospective cohort study was conducted using a database comprising healthcare claims data from public health insurance enrollees residing in a Japanese city.We analysed a primary cohort comprising patients hospitalised with COVID-19 between March 2020 and July 2021 and two control cohorts comprising patients hospitalised with influenza or other respiratory tract infections(RTI)during the same period.We calculated the incidences of acute(1–3 months after infection)and delayed(4–6 months after infection)psychiatric sequelae.These sequelae were identified using diagnosis codes and categorised as mood/anxiety/psychotic disorder,mood disorder,anxiety disorder,psychotic disorder or insomnia.Multivariable logistic regression models were used to estimate the odds ratios(ORs)of psychiatric sequelae occurrence after COVID-19 infection compared with influenza and other RTI.Results The study population with acute psychiatric sequela consisted of 662 patients with COVID-19,644 patients with influenza,and 7369 patients with RTI who could be followed for 3 months;the study population with delayed psychiatric sequelae consisted of 371 patients with COVID-19,546 patients with influenza,and 5397 patients with RTI who could be followed for 6 months.In the analysis of acute psychiatric sequelae,COVID-19 had significantly higher odds of mood/anxiety/psychotic disorder(OR:1.39,p=0.026),psychotic disorder(OR:2.13,p<0.001),and insomnia(OR:2.59,p<0.001)than influenza,and significantly higher odds of insomnia(OR:1.44,p=0.002)and significantly lower odds of anxiety disorder(OR:0.56,p<0.001)than other RTI.In the analysis of delayed ps

Vertical impedance functions of pile groups under low-to-high loading amplitudes:numerical simulations and experimental validation

Piles in a group experience additional displacements in soil due to pile-to-pile interactions apart from those resulting from the external loading.The effect of these interactions determined assuming soil as an elastic and/or viscoelastic material on pile head impedance functions of the pile group is studied by relating the group stiffness to the static stiffness of a single pile.However,the prevailing elastic solutions may misestimate the resulting pile group response due to the lack of consideration for either soil(material)and/or soil-pile interface nonlinearities.It is well established that soil behaves nonlinearly under moderate-to-high loading amplitudes,and besides,the soil-pile interface nonlinearity can exist even at small loading amplitudes.This study addresses the effects of these nonlinearities on the vertical impedance functions of a 3×3-pile group using numerical methods by direct analyses and superposition using pile-to-pile interaction factors.The numerical results are validated using scaled model tests under 1 g conditions.The results highlight the overestimation of pile-to-pile interactions in the pile group when assuming elastic soil conditions.The cases either by direct analyses or superposition approach involving soil and soil-pile interface nonlinearities agree well with the experimental pile group responses under close-to-elastic and nonlinear conditions.

Characteristics of inflammatory bowel diseases in patients with concurrent immune-mediated inflammatory diseases

Patients with inflammatory bowel disease(IBD)are more likely to have concurrent immune-mediated inflammatory diseases(IMIDs)than those without IBD.IMIDs have been observed to alter the phenotype and outcomes of IBD in recent studies.Several studies have found that IBD patients with concurrent IMIDs may have more extensive or severe disease phenotypes,and are considered to be at increased risk of requiring biologics and IBD-related surgeries,suggesting that having multiple IMIDs is a poor prognostic factor for IBD.Furthermore,IBD patients with primary sclerosing cholangitis and Takayasu arteritis are reported to have unique endoscopic phenotypes,suggesting concurrent IMIDs can influence IBD phenotype with specific intestinal inflammatory distributions.In this review,we discuss the pathogenesis,disease phenotypes,and clinical outcomes in IBD patients with concomitant IMIDs.

Nano-bubble hydrogen water:An effective therapeutic agent against inflammation related disease caused by viral infection in zebrafish model

Since the anti-inflammatory effect of hydrogen has been widely known,it was supposed that hydrogen could suppress tissue damage by inhibiting virus-related inflammatory reactions.However,hydrogen is slightly soluble in water,which leads to poor effect of oral hydrogen-rich water therapy.In this study,the nano-bubble hydrogen water(nano-HW)(about 0.7 ppm)was prepared and its therapeutic effect against viral infection was investigated by utilizing spring viraemia of carp virus(SVCV)-infected zebrafish as model.Three-month-old zebrafish were divided into nano-HW treatment-treated group and aquaculture water treated group(control group).The results revealed that the cumulative mortality rate of SVCV-infected zebrafish was reduced by 40%after treatment with nano-bubble hydrogen water,and q RT-PCR results showed that SVCV replication was significantly inhibited.Histopathological examination staining showed that SVCV infection caused tissue damage was greatly alleviated after treatment with nano-bubble hydrogen water.Futhermore,SVCV infection caused reactive oxygen species(ROS)accumulation was significantly reduced upon nano-HW treatment.The level of proinflammatory cytokines IL-1β,IL-8,and TNF-αwas remarkably reduced in the nano-HW-treated group in vivo and in vitro.Taken together,our data demonstrated for the first time that nano-HW could inhibit the inflammatory response caused by viral infection in zebrafish,which suggests that nano-HW can be applied to antiviral research,and provides a novel therapeutic strategy for virus-caused inflammation related disease.

Law Education in Japan and China:Comparative Analysis Focusing on Law-Related Materials

Purpose:This study examines law-related education in Japan and China to reveal the current state of research and identify the roles,possibilities,and challenges facing such teaching at the elementary,junior high,and senior high school levels.This study conducts a comparative review of research on perspectives toward law education in both countries,as well as the characteristics of and issues facing law education in Japan.Design/Approach/Methods:This study examines trends in research and educational reform related to law education in Japan.In doing so,it evaluates how the characteristics of and issues facing law education in Japan are related to law education in China.Findings:Results show no evidence of a systematic study of law in Japan.Reforming subjects to engage in collaborative learning is a more realistic strategy than attempting to design wholly new subjects.Accordingly,exploring what kind of learning activities are linked to law-abiding education in China offers insights and references for Japan.Adapting these strategies to the Japanese context and law material can help create a more systematic form of learning.Originality/Value:Through joint research by researchers and educators from East Asian countries,we intend to conduct further research on the development of curricula,textbooks,and class models suited to specific subjects.Employing the joint research learning approach discussed in this study in Japan and China may result in further learning possibilities.

Learning Rat-Like Behavior for a Small-Scale Biomimetic Robot

Existing biomimetic robots can perform some basic rat-like movement primitives(MPs)and simple behavior with stiff combinations of these MPs.To mimic typical rat behavior with high similarity,we propose parameterizing the behavior using a probabilistic model and movement characteristics.First,an analysis of fifteen 10 min video sequences revealed that an actual rat has six typical behaviors in the open field,and each kind of behavior contains different bio-inspired combinations of eight MPs.We used the softmax classifier to obtain the behavior-movement hierarchical probability model.Secondly,we specified the MPs using movement parameters that are static and dynamic.We obtained the predominant values of the static and dynamic movement parameters using hierarchical clustering and fuzzy C-means clustering,respectively.These predominant parameters were used for fitting the rat spinal joint trajectory using a second-order Fourier series,and the joint trajectory was generalized using a back propagation neural network with two hidden layers.Finally,the hierarchical probability model and the generalized joint trajectory were mapped to the robot as control policy and commands,respectively.We implemented the six typical behaviors on the robot,and the results show high similarity when compared with the behaviors of actual rats.
